Abstract

Purpose: To determine efficacy and toxicity of two pemetrexed-based regimens in chemonaive patients with locally advanced or metastatic non–small cell lung cancer. Experimental Design: Patients were randomly assigned to receive pemetrexed 500 mg/m2 plus oxaliplatin 120 mg/m2 (PemOx) or pemetrexed plus carboplatin AUC6 (PemCb). All drugs were given on day 1 of a 21-day cycle for up to six cycles. Folic acid and vitamin B12 were given to all patients to minimize pemetrexed-related toxicities. Results: Forty-one patients received PemOx and 39 received PemCb. Objective tumor response rates were 26.8% for PemOx patients (95% confidence interval, 14.2-42.9) and 31.6% for PemCb patients (95% confidence interval, 17.5-48.7). Median time to progression was 5.5 and 5.7 months, respectively, for PemOx and PemCb. Median overall survival times were 10.5 months for both treatment groups (range, 20 months). The 1-year survival rate was 49.9% for PemOx patients and 43.9% for PemCb patients. Common toxicity criteria grade 3 or 4 hematologic toxicities among PemOx patients were grade 3 or 4 neutropenia (7.3%), grade 3 thrombocytopenia (2.4%), and grade 3 anemia (2.4%). PemCb patients experienced grade 3 or 4 neutropenia (25.6%), grade 3 or 4 thrombocytopenia (17.9%), and grade 3 anemia (7.7%). Grade 3 vomiting occurred in three PemOx patients and grade 3 fatigue occurred in three PemCb patients. One grade 3 neurosensory toxicity occurred in the PemOx group. Three patients (PemOx 1 and PemCb 2) experienced febrile neutropenia. Conclusions: Efficacy measures for both regimens seem similar to the most effective chemotherapies for advanced non–small cell lung cancer (platinum combinations) with less hematologic and nonhematologic toxicity. Comparing either of these two regimens to platinum-based therapies in a large randomized trial is warranted.

12. C-reactive protein in the diagnosis of melioidosis

Abstract

Previous work suggested that C-reactive protein (CRP) may be a useful test in the diagnosis of melioidosis, the infection caused by Burkholderia pseudomallei. We reviewed patients with culture-confirmed melioidosis to define the role of this inflammatory marker in the diagnosis of melioidosis. In 175 patients, we found that the admission CRP level may be normal or only mildly elevated, including patients with severe sepsis, fatal cases, and in relapsed melioidosis. In a multivariate analysis, sepsis and bacteremia were more strongly associated with mortality than CRP. Admission levels of CRP are not a sensitive marker for the presence of melioidosis and a normal level cannot be used to exclude acute, chronic, or relapsed melioidosis in febrile patients in or from endemic regions.

17. Development of spiral-groove self-acting seals for helicopter engines

Abstract

A spiral-groove, self-acting face seal was rig tested at advanced gas turbine operating conditions to determine wear and leakage rates. The spiral-groove, self-acting geometry was located in the rotating seal seat. Seal component wear induced by start-stop operation was measured after subjecting the test seal to 176 start-stop cycles. Wear occurring during normal operation was documented throughout a 75-hour endurance test. Seal air leakage was also measured. During endurance operation, the seal was subjected to operating conditions bounded by the values surface speed - 244 m/s (800 ft/sec), air pressure - 148 N/sq cm abs (215 psia), and air temperature - 622 K (660 F). The post-test condition of the seal components was documented. Wear data is presented in tabular form, while seal air leakage is presented graphically, as a function of pressure and speed.

18. Development of spiral-groove self-acting face seals

Abstract

An experimental evaluation and a 100-hour endurance test were performed on a spiral groove geometry, self-acting face seal. The seal was tested and operated successfully at maximum conditions of 243.8 m/s surface speed, 199.9 N/sq cm air pressure, and 645.4K (702 F) air temperature. The maximum speed condition of 243.8 m/s was obtained at a shaft speed of 72,500 rpm. Seal wear, gas leakage, and sealing element temperature were monitored during the test. Condition of the seal at the completion of the test was documented and found acceptable for further use. The spiral groove wear rate measured during the endurance test indicates a minimum potential seal life of over 2700 hours. Seal air leakage measured during the test program is within the range considered acceptable for consideration for use in a small gas turbine engine.

20. Kennedy Space Center ocean beach erosion

Abstract

Dune barrier erosion and possible breakthrough due to storm and hurricane wave activity is studied near Mosquito Lagoon, in Kennedy Space Center property. The results of a geological as well as hydrodynamic appraisal of the problem area indicate that no inlet has existed across the dune barrier since 500 A.D., and that there is little likelihood of a possible breakthrough inlet remaining open permanently, primarily because the relatively shallow lagoon does not contain enough volume of water to maintain an inlet between the ocean and the lagoon. It is therefore recommended that only minimal measures, such as closing up the man-made passes across the dunes, be carried out to ensure continuation of the action of natural beach maintaining processes.

45. Certification of a Pilot with Charcot-Marie-Tooth Disease.

Abstract

BACKGROUND: Charcot-Marie-Tooth disease (CMT) is a rare hereditary motor and sensory neuropathy. This is a report of a pilot with this condition with a discussion of the challenges for the regulator in the assessment for medical certification of pilots with a neurological disability. CASE REPORT: A pilot with CMTX1 declared his condition to the United Kingdom Civil Aviation Authority when his brother was diagnosed with the same condition. Apart from high arched feet and some difficulty playing sports, he had no problems until his mid-forties, when he very slowly developed increasing weakness with foot dorsiflexion and later wasting and weakness of the small hand muscles. He reported no problems with any flying activity. On clinical examination, it seemed likely that the disability would have an impact on his ability to undertake all the flying tasks of a commercial pilot, including those required in emergencies. DISCUSSION: A modified Medical Flight Test (MFT) specifically tailored by the regulator to test areas of functional impairment allowed the successful certificatory assessment of a pilot with this condition; an approach which could apply to any pilot with a rare neurological disability. Abstract

BACKGROUND: Cavernomas, cavernous angiomas, or cerebral cavernous malformations are clusters of endothelium-lined blood vessels usually found in the brain. With the increasing use of radiological imaging, these are being detected incidentally in asymptomatic aircrew. The UK Civil Aviation Authority (CAA) experience of cavernomas is described and the aeromedical concerns, that is, the risk of epilepsy, hemorrhage, and the development of a neurological deficit, are considered. METHODS: A search of the CAA database between 1990 and 2020 was performed for the term cavernoma. The gender, age at diagnosis, class of certification held, clinical presentation, location, and size of the lesion were noted. A PubMed literature review for papers with complications of cavernoma was performed. RESULTS: Six cases of cavernoma have been declared to the CAA: five professional pilots and one private pilot. Five were men and one was a woman. The age range was between 38 and 60 yr, with a mean of 48 yr. Two cases presented with clinical symptoms and four were asymptomatic. Complication rates for seizure and hemorrhage were extracted from the published literature together with the significance of other factors such as cavernoma size, family history, multiplicity, and the development of new lesions. DISCUSSION: A policy for the medical certification of aircrew with cavernomas that have presented with clinical symptoms and those that are detected incidentally is proposed. Jagathesan T, OBrien M. Abstract

Current medical practice requires us to be able to educate and alter an individual's approach to health maintenance. The objective is to prevent illness--not only to treat illness. This approach should result in keeping people out of hospitals and clinics. Diet should be thought of as a noun, not a verb. It is not synonymous with the corrective measures it supposedly sill insure, i.e., correct obesity, reduce hypertension, control blood lipid abnormalities. In keeping with this approach, USAF medical team members should be able to do dietary counseling. This paper will examine each of these topics, emphasizing dietary prejudice at the level of the counselor, the subject and his family, and the USAF medical team.

Abstract

After completing a screening for hypertension among patients aged between 45 and 54 in a group practice, a sample of both responders and non-responders to screening were surveyed to determine their attitude to screening and knowledge of hypertension.The reasons for non-response were various, and 18 per cent felt screening to be unnecessary. There was little difference between responders (both hypertensive and non-hypertensive) and non-responders in their knowledge of hypertension, and they were well informed about related conditions and illnesses caused by hypertension. The majority were aware of some likelihood of the disease being symptomless, and 38 per cent thought no symptoms were likely to be caused by hypotensive drugs. They seemed aware that the treatment was long term, but only 14 per cent thought it would be life-long.
