Myositis ossificans occurs extremely rare in the wrist and hand. Its clinical manifestations and prognosis differ significantly in comparison to the classical localizations on the thigh or shoulder. It is difficult to diagnose this kind of disease during the early stages of the pathological process. The article presents a clinical case of successful treatment of a patient with myositis ossificans of the lumbricals of the hand. Myositis ossificans in our case is shown not just through local symptoms, but also by clinical neuropathy in the median nerve. A review of the available literature on this topic over the past 40 years is presented.

Introduction. Organotin compounds are promising candidates for antitumor drugs. Identification of pathogenetic features of the general toxic effect of hybrid organotin compounds during the period of the greatest severity of the intoxication clinical picture will allow to estimate the risk of hepatotoxic and nephrotoxic complications with the administration of bis (3,5–di– tert–butyl–4–hydroxyphenylthiolate) dimethyltin (Me-3) and ((3,5–di–tert–butyl–4–hydroxyphenylthiolate) triphenyltin (M-e5) as chemotherapeutic agents.Аim: To conduct a comparative analysis of morphological and biochemical changes with a single intragastric administration of hybrid organotin compounds Me-3 and Me-5 in the maximum tolerated dose (MTD) on the 7th day of the toxic process development.Material and Methods. Hybrid organotin compounds Me-3 and Me-5 were administered once intragastrically to Wistar rats (females) at a MTD of 2000 mg/kg and 750 mg/kg, respectively. Biochemical and morphological studies were carried out on the 7th day of the development of intoxication symptoms according to standard methods.Results. With the introduction of Me-3 and Me-5 in the liver, signs of fatty dystrophy of varying severity were revealed, with a predominant lesion of centrolobular hepatocytes, an increase in the size of portal tracts due to edema and fibrosis, and scant lymphocytic infiltration. With the introduction of Me-5, morphological changes were more severe, with the involvement of the vascular bed of the organ in the process. When the tested compounds were administered in the kidneys, the same type of damage to the glomerular apparatus and renal tubules was recorded, characteristic of toxic nephropathy. Unidirectional changes in the blood of experimental animals were revealed in the group of nonspecific biochemical markers of cytolysis: a moderate decrease in transaminase activity and an increase in the activity of creatine kinase (CC), lactate dehydrogenase (LDH) and creatinine levels. The process of formation of urea and protein synthesis was functionally preserved.Conclusion. On the 7th day of the development of intoxication with a single intragastric administration of hybrid organotin compounds Me-3 and Me-5 in the maximum tolerated doses, biochemical and morphological changes in the body of animals could be attributed to a moderate degree of severity.

Aim. The presented study aims to develop and describe a contract management system for FMCG chain retailing companies in the context of the digital transformation of the economy.Tasks. The authors examine the specific aspects of developing a contract management system for chain retailing companies; develop a classification of contracts by sector of activity and outline the landscape of the contract management process; create and describe a system for managing contracts.Methods. The methodological basis of this study includes conceptual representations of the contract management system, which make it possible to apply a systems approach, generalization, grouping, methods of formal system representation, and socio-economic experimentation.Results. The study examines the specific aspects of developing a contract management system for chain retailing companies in the context of the digital transformation of the economy, describing the major types of contracts used by these companies. The contracts are classified by sector of activity, and the landscape of the contract management process is outlined. Its individual units correspond to the Deming cycle and represent a cyclically repeating decision-making process.Conclusions. As a result, a contract management system for chain retailing companies is proposed, and the relationship between the management subsystem and the system of requirements for the architecture of business services in the field of contract management is determined. It is shown that successful implementation of an efficient contract management system requires preliminary work to identify requirements for the architecture of business services. Taking into account these requirements in the modeling of architectural solutions and integrating them into the business architecture will ensure high-quality contract management through the optimization of the company’s resources and highly efficient regulation of the relationship between the stakeholders and counterparties of chain retailing companies.

Introduction. The high prevalence of renal diseases caused by urinary tract obstruction led to the need for experimental research of compensatory and pathological processes with kidney injury. It is also of relevance to study key mechanisms providing a compensatory function of the contralateral kidney for early diagnosis, treatment, and prognosis of obstructive renal diseases.Purpose of the study. To examine epithelial nephron cells phenotype dynamics changes in contralateral kidney using unilateral ureteral obstruction experimental model.Materials and methods. Model of unilateral ureteral obstruction was established using adult rabbits. The studies were carried out on days 7, 14 and 21 of complete obstruction of the left ureter. Immunophenotyping was performed on contralateral kidney tissue samples using epithelial (cytokeratin 7, E-cadherin) and mesenchymal (vimentin, α-smooth muscle actin) markers.Results. The contralateral kidney under additional load can maintain the morphological and functional characteristics of the nephron for a long time. The first transmogrify signs in the nephron epithelium phenotype were detected by day 21 as the diffuse appearance of mesenchymal marker vimentin with unaltered visualization of epithelial phenotype markers.Conclusion. The results obtained allow us to assume that the compensatory reserve of the contralateral kidney is gradually decreasing when the duration of the obstruction increases. Thus, the likelihood of developing negative disorders increases.

BACKGROUND. Determining the pathogenetic mechanisms of small-caliber renal artery remodeling in chronic glomerulone­phritis (GN) is an urgent task of nephrology, the implementation of which will allow establishing new diagnostic and therapeutic approaches to patients management. THE AIM: To evaluate the influence of hemodynamic, tubulointerstitial, and endothe- liotropic risk factors on the probability of remodeling of interlobular arteries in an experimental model of glomerulonephritis. MATERIALS AND METHODS. The experiment included 45 individuals of white mongrel rats, 3 of which were used to prepare an antigen suspension (AS), and 42 rats were divided into 4 groups: 9 individuals in the main groups (with the introduction of only AS, with the introduction of an incomplete Freund adjuvant, with AS and sodium chloride, with AS and perindopril) and 6 ratsin the control group. Glomerulonephritis was formed in the main groups of the experiment. Systolic blood pressure (SAD), the protein level in the urine, and the presence of edematous syndrome were monitored initially, on the 15th, 30th, and 60th day of the experiment. The size of the interlobular artery (MA) was determined by the morphological study, and the expression of VEGF and TGFp in the kidneys. RESULTS. Morphological signs of glomerulonephritis were obtained in all the main groups of the experiment as early as day 15. The greatest increase in SAD, protein in the urine, the presence of edematous syndrome in groups with the introduction of AS and AS with sodium chloride was found. The highest expression of VEGF and TGFp was found in these groups of rats. In the group with AS with perindopril, normotension was formed, the protein level was lower than in rats with AS with or without the use of sodium chloride, and there was no edematous syndrome. The expression of VEGF and TGFp was minimal. Interlobular artery remodeling in established groups of AS an AC with sodium chloride. In the remain­ing groups of rats, the size of the interlobular arteries was comparable to the control group. CONCLUSION. The leading role of systemic blood pressure in the remodeling of small-diameter kidney arteries in glomerulonephritis has been established. Despite the presence of active glomerulonephritis in rats, the structure of small arteries does not change during the formation of normotension.

At the current time economy digitalization of the Russian Federation significantly affects hospitality industry, in particular, hotel automation associated with the use of modern technologies that improves the quality of customer service. This research focuses on such technologies as artificial intelligence, blockchain, augmented reality technologies, cloud technologies, analysis of speech scripts, face recognition technologies, voice assistants, and many others.The results of the study show that despite the fact that hotel business is successfully mastering modern digital technologies, only fulfillment of continuous data collection, processing, analysis and aggregation will allow the hospitality industry management to achieve one of their main goals, which is to better understand their customers and interact with them.

Despite the fact that recent decades have witnessed significant advances in treatment of patients with DeBakey type I acute aortic dissection, it still remains difficult to restore the aortic root when the dissection extends to the Valsalva's sinuses. Thinned aortic walls are vulnerable to traumatization on applying a vascular suture. We used in patients with this pathology the Florida sleeve technique in order to reinforce the weakened aortic root. After mobilization of the aortic root and coronary arteries, the transplant 'wraps' the sinuses from the outside, like the neoadventitia, in order to strengthen the weakened aortic wall. The graft's diameter should be selected with due regard for tight enwrapment of coronary sinuses in their maximal dilatation and, if necessary, preserving the possibility of narrowing the fibrous ring. This technique is simple and effective. First, it allows strengthening the weakened root, secondly helping to prevent haemorrhage. Besides, it may prove alternative, should preservation or correction of the geometry of the aortic root be required.Несмотря на то, что за последние десятилетия были достигнуты значительные успехи в лечении пациентов с острым расслоением аорты I типа по ДеБейки, по-прежнему представляет сложность восстановление корня аорты при распространении расслоения на синусы Вальсальвы. Истонченные стенки аорты подвержены травматизации при накладывании сосудистого шва. Мы использовали у больных с данной патологией технику Florida Sleeve для укрепления ослабленного корня аорты. После мобилизации корня и коронарных артерий трансплантат 'оборачивает' снаружи синусы, как неоадвентиция, чтобы укрепить ослабленную стенку аорты. Подбор диаметра протеза должен быть выполнен с учетом плотного облегания коронарных синусов в их максимальном расширении и в случае необходимости сохраняется возможность сузить фиброзное кольцо аортального клапана. Эта операционная техника проста и эффективна. Во-первых, она позволяет укрепить ослабленный корень, во-вторых, помогает избежать кровотечения. Кроме того, она может быть альтернативой, если требуется сохранение или коррекция геометрии корня аорты.

The presented study improves the model of top-level documentation workflow in FMCG chain retailing companies by building a special data architecture.Aim. The study aims to develop a model of top-level documentation workflow for building architectural solutions of FMCG chain retailing companies based on data architecture modeling.Tasks. First, it is necessary to examine the general documentation workflow system of FMCG chain retailing companies, identifying the major interested parties — participants of external, internal, and supporting workflow. It is also advisable to analyze certain types of internal workflow and to determine the relationships between them. Based on the conducted analysis, it is necessary to develop a conceptual model of data architecture and a documentation workflow model for FMCG chain retailing companies.Methods. This study represents the general documentation workflow system of FMCG chain retailing companies as three subsystems (external, internal, and supporting workflow). The authors examine the major channels of interaction between chain retailing companies and such interested parties as government authorities, suppliers, consumers, and third-party organizations. Based on the analysis of the elements of the workflow system, this study builds a top-level documentation workflow model and presents a conceptual model of data architecture for FMCG chain retailing companies with allowance for the above-mentioned entities. The model uses UML notation, which makes it possible to reflect key classes and relationships and to identify relationships that need to be simplified.Results. The study examines the process of improving documentation workflow in FMCG chain retailing companies in the context of active implementation of new IT solutions and automation of business processes to reduce costs and increase sales. The authors show the general documentation workflow system of FMCG chain retailing companies, identifying the major interested parties — participants of external workflow — and analyzing certain types of internal workflow. According to the results of the study, a top-level documentation workflow model for FMCG chain retailing companies is developed based on data architecture design.Conclusions. Large FMCG chain retailing companies are actively implementing new IT solutions and automate business processes to reduce costs and increase sales. Optimal documentation support for FMCG chain retailing companies is impossible without generating huge amounts of data. Methods of data storage and processing have a direct impact on the speed and quality of business processes. In order to improve documentation workflow, it is becoming increasingly important to develop and optimize data architecture. Data architecture is one of the fundamental models of company operation in terms of comprehensive digitalization. The obtained data model can be considered universal for FMCG chain retailing companies since it makes it possible to organize management in such a way as to ensure the development and efficient functioning of the architecture of chain retailing companies.

The high prevalence of kidney diseases caused by urinary tract obstruction has led to the need for experimental studies of the dynamics of pathological processes in their lesions. Despite the fact that the general patterns of development of obstructive uropathy are known, the features of renal tissue damage, in particular structural and molecular biological changes in this pathology, remain insufficiently studied.to study the dynamics of changes in the phenotype of epithelial cells of the nephron of an obstructive kidney with unilateral ureteral obstruction using an experimental model.The experimental study was carried out on the basis of the Rostov State Medical University. The model of unilateral ureteral obstruction was reproduced in adult rabbits. The studies were carried out on the 7th, 14th and 21st days of complete obstruction of the left ureter. Immunophenotyping of obstructive kidney tissue samples was performed for markers of epithelial phenotype (cytokeratin 7, E-cadherin) and mesenchymal phenotype (vimentin, - smooth muscle actin).The sequence of changes in the phenotype of nephron epithelial cells during ureteral obstruction has been established. The first signs of an epithelial-mesenchymal transition (EMT) appear by day 7 in the form of a decrease in visualization of markers of the epithelial phenotype. On the 14th day, the expression of both epithelial and mesenchymal markers is noted. Significant changes in the phenotype of nephron epithelial cells: loss of epithelial markers (cytokeratin 7, E-cadherin) and the acquisition of mesenchymal markers (vimentin, - smooth muscle actin), are noted by the 21st day of the experiment.An experimental model of unilateral ureteral obstruction revealed the transformation of the nephron tubule cell phenotype from epithelial to mesenchymal.

A total of 1419 children aged 5-14 years living in 4 agricultural regions of the Central Russia (in Southern Belgorod and Central Western Voronezh regions) were examined in August 1997 within the framework of Program of the UN International Childhood Foundation, WHO, and its International Committee for Iodine Deficiency Diseases Control “Thyroid: Standardized Analysis of Iodine Supply in Europe”. Standard sampling in each region consisted of 10 subgroups for each year of life, 35-36 subjects per subgroup, with equal number of boys and girls. Iodine was measured in the morning urine by the cerium arsenite method. The parents were interviewed about nutrition using universal questionnaires. Slight iodine deficiency was detected in the region, the median of ioduria varying from 69 to 86 mcg/liter. Renal iodine excretion below the threshold normal value (

The processability of production waste of Yaroslavsky Mining Company is studied. The features of processing characteristics of secondary raw materials are estimated. The methods to eliminate the negative effects caused by residues of primary processing reagents and new formations on mineral particles are considered. The efficiency of ultrasound pulp treatment aimed at renewal of mineral particle surface and desorption of surface coatings is shown. Experimental data are presented, which indicate an increase in the selectivity of fluorite and calcite separation from the primary operations of a flow chart. A comparative analysis of the flotation kinetics using the standard model and the flow chart with ultrasound pulp pretreatment indicates an increase in the process speed with a simultaneous selectivity growth. It is found that fluorite extraction to concentrates with 93.48% of CaF2 and the use of ultrasound treatment of flotation feed reaches 60.27–64.43%, and with 95.67% of CaF2—49.82%.

Introduction The study of remodeling of small-diameter renal arteries in hypertension (AH) and primary chronic glomerulonephritis (CGN) is promising. This is due to the high prevalence of CGN and the likelihood of developing chronic renal failure, as well as insufficient information about the process of remodeling the small arterial wall of the kidneys. Goal To evaluate the influence of clinical and morphological parameters on the risk of small-diameter renal artery fibroelastosis in patients with AH and CGN. Materials and methods The study included 105 patients with AH and CGN. One of the main criteria for inclusion in the study was the presence of indications for performing nephrobiopsy. All subjects underwent a standard clinical and laboratory study, with determination of serum creatinine, of glomerular filtration rate (GFR) (CKD-EPI), detection of daily proteinuria, systolic (SAD) and diastolic blood pressure (DAP), and the degree and stage of AH. That 66 patients had no signs of fibroelastosis of arteries (the first group), and 39 patients had fibroelastosis (the second group). The presence of tubulointerstitial fibrosis (TF) and inflammation (TI), the prevalence of fibrosis were recorded. Results In the first group of patients, the level of SAD, established at the stage of inclusion in the study, was 124.33±21.5mmHg, in the second group – 131.15±17.3mmHg. (p=0.09). DAP values were in the range of 81.97±11.2 and 83.08±9.15mmHg. respectively (p=0.59). When analyzing the degree and stage of AH, statistically significant differences were obtained - 1.29±1.05 and 1.82±0.9 degrees (p=0.009), 1.41±1.08 and 2.08±0.9 stages (p=0.002), respectively. The value of creatinine in the first group was 98.33±39.8mmol/l (1.11 mg/dl), in the second group – 131.49±54.4mmol/l (1.49 mg/dl) (p=0.03), GFR - 97.6±40.4 and 69.3±32.5 ml/min/1.73m2 (p=0.0001), respectively. There were no significant differences in the proteinuria of patients in the study groups (3.64±6.2 and 4.1±5.1 g/l, respectively, p=0.42). Analysis of data obtained during morphological examination allowed us to establish more pronounced changes in TF and TI in the study groups (44 and 35 cases of TF (p=0.009), 35 and 29 cases of TI (p=0.025), respectively, groups). In the first group of patients, the prevalence of TF fever was set at 14.1±19.4%, in the second group – 27.95±21.9% (p=0.001). Conclusions The leading reasons for remodeling the vascular bed of small-diameter kidneys in AH and CGN are hemodynamic factor and tissue changes in the kidneys. The presence of fibrosis and interstitial inflammation is associated not only with the effects of the main CGN process, but also with the presence of AH. Confirmation of this conclusion is obtained differences in the stages of AH. An result of the study is data indicating that there are no differences in the clinical sign of progression of CGN-proteinuria, that is, the main pathogenetic factor of CGN does not affect of remodeling of the arteries. The authors describe studies into dressability of mill tailings from tailings pond of processing plant of Yaroslavsky Mining Company. It is found that the mill tailings contain 14.92% of CaF2, 13.16% of CaCO3 and 0.49% of Zn. The use of new compositions of modifiers in flotation of fluorite allows production of standard grade concentrates with CaF2 content of 93.19–95.54% at the recovery of 52.75–30.15%. Zinc recovery is 38.84% with the yield of 42.11% Zn concentrate.

The studies into the fine-ingrained high-carbonate fluorite ore concentration show that increasing of pulp dispersion uniformity results in higher selectivity of flotation. The researchers find out availability of quality fluorite concentrate in processing of low-grade ore with carbonate index less than 1 at CaF2 recovery up to 70%.

AIM: to evaluate the significance of high-resolution anoscopy (HRA) in diagnostics of inflammatory and thrombotic changes in hemorrhoid piles and to assess the efficacy of micronized purified favonoid fraction (MPFF) in patients with inflammatory thrombotic changes of hemorrhoids according to HRA results in the preoperative conservative treatment of hemorrhoids. PATIENTS AND METHODS: a prospective cohort study included 77 patients with grade III chronic hemorrhoids. Patients were divided into 3 groups depending on the degree of development of inflammatory and thrombotic changes according to the HRA. The 3 group included patients with preoperative conservative treatment by MPFF. All patients underwent Milligan-Morgan procedure with further histological study of removed piles. RESULTS: it was found that the diagnostic sensitivity of the HRA in detection of inflammatory thrombotic changes was 91.3 % (CI=83.6-96.2 %), and diagnostic specificity - 40 % (CI=19-64 %). HRA diagnostic accuracy was 82.1 % (p=0.001). A 30 % decrease in the number of removed piles with moderate inflammatory thrombotic changes after MOFF therapy has also been revealed. CONCLUSION: HRA permits to determine the severity of thrombotic inflammatory changes, which is extremely importantfor the management of patients with acute hemorrhoids.

The article presents the research results aimed at improving the comprehensiveness of zinc-fluorite ore processing in the Primorye Territory, and discusses two variants of the technology of saleable sphalerite and fluorite concentrates production. Besides, the authors offer model engineering solutions for the efficient change of flotation regime or sequence using sulfhydryl and oxihydryl collecting agents.

This work presents the results of the study of a small technological sample taken from the borehole core drilled in the ore body of the Skrytoe deposit. Data on the mineral composition and the textural-structural features of the ores are reported. It was found that the studied ores are characterized by low concentrations of tungsten (WO3 less than 0.5%), high contents of phosphorus (P2O5 > 0.5%), and a content of sulfide minerals of no more than 10%. Based on these results, it was proposed to modify the available dressing scheme developed by the ZAO Mekhanobr-Inzhiniring company. This will permit one to optimize the efficiency of the scheelite extraction during mining.

Influence of virus load, the beginning and the regimen of specific antiretroviral (AVR) prophylaxis on expression of destructive and adaptive changes in placental bed structures of the uterus during HIV-associated pregnancy is studied. 61 HIV-infected pregnant women from 17 till 41 years were surveyed. It is proved, that in the placental bed specimens collected during SC in HIV-infected women, the focal chronic deciduitis took place which morphologically correlated with prolonged pre-conceptional persistency of infection. Increasing of the virus load led to elevation of inflammatory cells and destructive changes. With increasing duration of AVR prophylaxis and administration of highly active antiretroviral therapy (HAART) the areas of necrosis and fibrosis were enlarged, atrophic changes in the decidual tissue were observed. Statistical analysis has allowed to find out that virus load, duration and the therapeutic regimen determine severity of destructive processes in uterine placental bad and realization of adaptive reactions that promote the development of placental insufficiency during HIV-associated pregnancy.

The authors provide insight into the problem of selective flotation of fluorite from carbonate-fluorite ores. It is found that oxyhydrate collectors used in combination with sodium fluoride provide rather high level of selectivity in fluoride flotation from poor high-carbonate ores and make it possible to eliminate high-temperature pulp treatment with acceptable reduction in the flotation temperature to 15°C and below.

The trends in development of aortic and coronary atherosclerosis in healthy male population were studied by WHO methods in three trials: in 1963-1965, 1985-1987, 2001-2003. It was found that atherosclerosis and coronary arteries stenosis were less frequent in the native population of Yakutia than in migrants.

We have analyzed the development of aortic and coronary atherosclerosis over 40 years in native and non-native males of Yakutsk by comparing the data obtained in 1963-1965, 1985-1987 and 2001-2003. The research was carried out in compliance with the WHO program for the atherosclerosis epidemiology studies using autopsy material. Subjects (361 natives and 692 non-natives) included in the research died of external causes at the age of 20-59 years and had no history of chronic diseases, which was confirmed by post-mortem examination. The prevalence and the extent of raised atherosclerotic lesions (fibrous plaques, complicated lesions and calcinosis) in the aorta and coronary arteries were determined by visual planimetry after staining with Sudan IV. An accelerated development of aortic and coronary atherosclerosis was revealed in both populations throughout the entire period of 40 years with more severe manifestations in non-native males. The severity of atherosclerosis increased considerably in native and non-native males aged over 40 years. The rate of the increase in the extent of raised atherosclerotic lesions tended to be greater in native than in non-native males.
