Introduction.Multiple studies on immune response in patients with various clinical forms of tick-borne encephalitis (TBE) have shown conflicting results. The study aim was to estimate the changes in peripheral blood lymphocyte subset counts and activity of spontaneous and lipopolysaccharide (LPS)-stimulated cytokine production in the mononuclear leukocyte cultures in patients with febrile and meningeal acute TBE.Materials and methods.Groups 1 and 2 included 16 and 12 patients with febrile and meningeal acute TBE, respectively. The control group included 13 healthy donors. Hemogram and T-lymphocyte, T-helper cell, T-cytotoxic and NK cell counts were analyzed by flow cytometry at week 1 of the disease as well as the spontaneous and LPS-stimulated secretion levels of TNFα, IL-1β, IL-6, IL-10, IL-8, and MCP-1 were assessed by ELISA in the supernatants of mononuclear cell cultures twice: during hospitalization and two weeks later. Statistical analysis was performed by the Mann–Whitney U-test, and Wilcoxon test.Results.Group 2 demonstrated significant decrease in spontaneous and/or LPS-stimulated levels of proinflammatory cytokines IL-1β, IL-6, MCP-1, and TNFα, but showed higher IL-8 level as compared with Group 1. In addition, spontaneous and/or LPS-induced levels of IL-6 and TNFαin Group 2 did not significantly differ from the controls, which presumably also indicated the suppression of their production. In contrast, spontaneous and/or LPS-induced levels of IL-1β, IL-6, MCP-1, and TNFαin Group 1 were higher than in the controls. The spontaneous IL-10 level in the patients from both groups were higher than in the controls. Peripheral blood lymphocyte and T-cytotoxic T-lymphocyte counts in both groups of TBE patients were lower than in the controls. There was significant increase in neutrophil counts, decrease in NK cell count in Group 2 as compared to the patients with a milder febrile form.Conclusion.Meningeal acute TBE patients was presumably associated with inadequate immune response with NK cell deficiency, T-cell dysfunction, increased neutrophil count in the peripheral blood and impaired pro-inflammatory cytokine production related to innate immune response.

Objective: is to assess parameters of cytokine profile, and some biomarkers of hemogram, cytogenetic damage, and oxidative stress to determine their associations with clinical symptoms and identify early prognostic factors for tick-borne encephalitis febrile form compared with Lyme borreliosis non-erythemal form at the onset of the disease.Materials and methods. In 25 patients with febrile tickborne encephalitis (mean age: 49.51 years), 24 patients with Lyme borreliosis non-erythemal form (mean age: 46.70 years) and 14 healthy donors (mean age: 47.20 years), the following parameters were analyzed: hemogram, blood leukocyte indices, cytokine profile, the levels of micronucleated T-lymphocytes, malondialdehyde and total immunoglobulin (Ig) G. For statistical analysis, the Mann–Whitney U-test, Pearson’s goodness-of-fit test, Spearman’s correlation coefficient, and ROC assay with AUC assessment were used.Results. In the tick-borne encephalitis patient group compared to the controls, the following parameters were increased: neutrophil count, blood leukocyte indices, the levels of micronucleated T-lymphocytes, malondialdehyde and proinflammatory cytokines in the blood, which directly correlated with maximal pyrexia levels. Prognostic value for early preliminary diagnosis of tick-borne encephalitis compared with borreliosis showed the following blood parameters: increases in neutrophil count and the levels of micronucleated T-cells, neutrophils to monocytes ratio, neutrophils to lymphocytes ratio, concentrations of malondialdehyde, tumor necrosis factor (TNF)-α, interleukins (IL)-8, IL-6, IL-10, and TNF-α/IL-4 ratio as well as decreases in the levels of IL-4 and total serum IgG. The parameters showed from average to excellent predictive ability as well as high positive and negative predictive values.Conclusion. In patients with tick-borne encephalitis, we detected correlations between the pyrexia levels and the biomarkers of inflammation, oxidative stress, and cytogenetic damage as well as it were found some differences in these parameters from the group of borreliosis patients, which probably had prognostic significance.

Introduction. Features of cytokine production in mononuclear cell cultures from Lyme borreliosis patients based on clinical data remained poorly studied. The study aim was to estimate the patterns of baseline and lipopolysaccharide-induced cytokine-secretory activity of peripheral blood mononuclear leukocytes from patients with erythema migrans form of acute Lyme borreliosis based on clinical parameters. Materials and methods. Groups of 22 and 12 patients with the diagnoses of mild or moderate severity of monoinfection and co-infection with tick-borne encephalitis of Lyme borreliosis with erythema migrans were examined twice: on week 1 after disease onset and day 14. The control group included 17 healthy donors. Basal and lipopolysaccharide-induced IL-6, IL-10, and TNFα secretion levels were assessed in mononuclear leukocyte culture supernatants applying enzyme immunoassay. Statistical analysis was performed by using the Mann–Whitney U-test, Wilcoxon test, and Spearman’s rank correlation. Results. The group of moderate severity patients was clinically distinguished by severer fever and intoxication manifestations. At the disease onset, the basal TNFα, IL-6 and IL-10 secretion levels in the moderate severity patient cultures were significantly higher than in those of the other groups. After antibiotics treatment, the baseline TNFα and IL-10 levels tended to decrease. At the onset, lipopolysaccharide-induced cultures from the moderate severity patients showed significantly suppressed TNFα production and increased IL-10 secretion as compared to the other groups. Lipopolysaccharide-induced IL-6 secretion in the moderate vs. mild severity group supernatants was significantly lower. In dynamics, the induced TNFα levels in the moderate severity patients were increased to the magnitude exceeding that in the controls. Positive correlations between the IL-6 and TNFα basal levels and maximum body temperature or the C-reactive protein serum concentrations were revealed in the patients. Induced TNFα levels showed negative correlations with fever levels or with IL-10 secretion. Conclusions. It was demonstrated that basal TNFα, IL-6 and IL-10 secretion levels in the mononuclear cell cultures of acute Lyme borreliosis patients increased with the increasing disease severity. Suppression of lipopolysaccharide-induced TNFα production in the moderate severity patient cultures was presumably associated with the regulatory cytokine IL-10 effects.

The aim of the study was to characterize mono- and mixed infection with causative agents of vector-borne infections in Dermacentor reticulatus ticks inhabiting one of the anthropourgic foci of the Tomsk Region. Materials and methods. D. reticulatus ticks were collected from vegetation “on the flag” in the forest park zones of the southern part of Tomsk in August and September, 2020–2021. To isolate nucleic acids from homogenates of mites and subsequent PCR diagnostics, kits of the RealBest series (“Vector-Best”, Novosibirsk) were used. 184 individual samples of nucleic acids were examined for the presence of molecular-genetic markers of tick-borne encephalitis virus (TBEV), pathogenic borrelia, babesia, rickettsia, anaplasma, and ehrlichia. Results and discussion. The spectrum of pathogens detected through PCR assay of ticks collected in one of the anthropurgic foci of the Tomsk Region in 2020 and 2021 includes TBEV, pathogenic borrelia, rickettsia, and babesia. The highest rate of D. reticulatus infection has been established in relation to candidate pathogens of tick-borne rickettsioses (Rickettsia sp.) and babesioses (Babesia sp.). The overall frequency of co-infection in D. reticulatus was 12.88 %. Seven different associations of pathogens infecting D. reticulatus have been identified. The majority (88.24 %) of the detected cases of mixed infection were rickettsia associations. Double infection has been registered in 94 % of the total number of mixed variants. Markers of four tick-borne infection pathogens simultaneously (Rickettsia species DNA, Babesia species DNA, Borrelia burgdorferi s.l. DNA and TBEV RNA) have been found in one sample. Changes in areal of the D. reticulatus against the background of an increase in their numbers, determines the need to analyze the epidemiological significance of the ticks of this species in combined foci of tick-borne infections, especially in the zone of sympatry with ixodic ticks of other species.

Objective: is to assess clinical and laboratory prognostic factors to develop a differential diagnostic model between the monoinfection of tick-borne encephalitis febrile form and the mixed infection of tick-borne encephalitis with Lyme borreliosis non-erythemal form at the onset of the disease.Materials and methods. The clinical examination involving 56 patients with tick-borne encephalitis febrile form (mean age: 46.1±3.1 years) and 27 patients with the mixed infection of tick-borne encephalitis with Lyme borreliosis non-erythemal form (mean age: 47.2±3.2 years) has resulted in the assessment of 65 clinical and laboratory parameters in the first week of the disease including 14 indicators of standard and extended hemogram profiles and 6 blood leukocyte indices. Pearson’s goodness-of-fit test was used for statistical analysis. The predictive values of the parameters were determined by the odds ratio and ROC analysis with AUC. The logistic regression model was developed using STATISTICA 12.0.Results. To make differential diagnosis between mono- and mixed infection at the onset of the disease the following hematological parameters with “average” or “good” predictive values can be used: band neutrophil count (AUC=0.65), the index of leukocytes and erythrocyte sedimentation rate ratio (AUC=0.66), erythrocyte sedimentation rate (AUC=0.70), neutrophil granularity intensity (AUC=0.66), neutrophil reactivity intensity (AUC=0.72) and reactive lymphocytes count (AUC= 0.72). A logistic regression model with a “very good” predictive value (AUC=0.83) is developed which includes the following four predictors: band neutrophil count, erythrocyte sedimentation rate, NEUT-RI and NEUT-GI in peripheral blood.Conclusion. The model is allowed to make a differential diagnosis between the mono- and the mixed infection of tick-borne encephalitis with good sensitivity and specificity values in the first week of disease.

Background. Overweight and obesity are key factors for the occurrence of many morphofunctional disorders in organs and tissues, including bronchopulmonary system.The aim. To study the influence of metabolic disorders that occur against the background of obesity on the state of the airways tone in rats.Materials and methods. Obesity in male Wistar rats was induced using a high-fat and high-carbohydrate diet. In animals, body weight and fat mass were measured, and the heart-lung complex was extracted. In blood serum, the levels of glucose, insulin, leptin, triglycerides, and cholesterol were assessed. Bronchoalveolar lavage fluid was obtained by an open method, in which the concentration of protein, interleukin (IL) 6 and IL-10 was determined. The contractile activity of the isolated bronchial smooth muscle segments was studied using mechanographic method. The effect of acetylcholine (10–7–10–4 M), indomethacin (10–5 M), and forskolin (10–7–10–5 M) on the changes in the tone of airway smooth muscles was assessed.Results. High-fat and high-carbohydrate diet caused an increase in body weight, visceral obesity, hyperglycemia, insulin resistance, leptinemia, dyslipidemia in rats of the experimental group. In the bronchoalveolar lavage fluid of experimental animals, an increase in the content of protein and IL-6 was found, which positively correlated with the level of leptin and the fat mass. In obese rats, the contractile responses of bronchial smooth muscle segments increased in response to the effect of the cholinergic agent acetylcholine. The bronchoconstrictor effect of acetylcholine was reduced by the cyclooxygenase inhibitor indomethacin. In turn, the adenylate cyclase activator forskolin caused relaxation of the airway segments smooth muscles in rats of both groups, which was more pronounced in the experimental group.Conclusion. The obtained results indicate that the change in the reactivity of the respiratory tract can be the cause of bronchospastic conditions in obesity and of the inflammatory reaction in the respiratory system induced by obesity.

The aim of the study was to identify the features of the cellular composition and cytokine profile of bronchoalveolar lavage fluid in rats in a model of diet-induced metabolic syndrome.Materials and methods. In an experiment on animals (rats), a model of metabolic syndrome (MS) induced by a high-fat and high-carbohydrate diet was reproduced. To assess the viability of the reproduced model, biochemical and morphometric methods were used, such as measurement of body weight, specific gravity of liver and visceral fat, and blood pressure, determination of glucose concentration in the blood (including a glucose tolerance test), as well as determination of blood lipid parameters. To assess the intensity of the inflammatory response in the blood, the concentration of total protein, the total number of leukocytes, and the levels of immunocytokines (interleukin (IL)-6, IL-10, tumor necrosis factor (TNF)α, monocyte chemoattractant protein (MCP)-1) were determined. Open bronchoalveolar lavage was performed on the isolated heart – lung complex. The concentration of protein, immunocytokines (IL-6, IL-10, TNFα, MCP-1), the total number of leukocytes, and the ratio of their morphological types were determined in the bronchoalveolar lavage fluid (BALF).Results. In animals with MS, an increase in the total number of leukocytes in the blood due to granulocytes and a rise in the concentration of protein, TNFα, and IL-10 were revealed compared with the parameters in the controls. BALF analysis revealed an increase in the concentration of protein, the total number of leukocytes, and the absolute number of alveolar macrophages, neutrophil granulocytes, and lymphocytes. The levels of IL-6 and MCP-1 were more than 1.5 times higher.Conclusion. Changes in the qualitative and quantitative parameters of BALF are inflammatory in nature and are formed during a systemic inflammatory response accompanying metabolic disorders in modeling MS in rats in the experiment.

Background. There is no current generally accepted classification and clinical approaches to assess the clinical course severity of tick-borne encephalitis. The aim is to develop models for the quantitative severity scores of the febrile form of tick-borne encephalitis using the most informative predictors. Materials and methods. In accordance with clinical criteria of the height and duration of fever, two groups of 9 and 30 patients (52.2 ± 4.3 and 49.5 ± 3.9 years old) with severe and non-severe febrile form of tick-borne encephalitis were formed. The study design was a retrospective case-control study. Statistical significance assessment of variables was carried out using the chi-square test. The odds ratios were also calculated. The development of logistic regression models was carried out using Statistica 12.0 modules. Results. A questionnaire of febrile form of tick-borne encephalitis severity score was developed using the height of fever and 11 most informative clinical predictors including arthralgia, chills, sleep disturbance, dizziness, orbital pain, nausea, photophobia, myalgia, severe headache, vomiting and meningism. To predict the severe clinical course of febrile form of tick-borne encephalitis, two logistic regression models were developed with or without severity score assessment of fever syndrome (coefficients α and β were –12.13 and 0.94 and –5.78 and 0.75 for models 1 and 2, respectively). Models 1 and 2 demonstrated a high predicted probability for cut-off values equal to 14 and 9 points and excellent quality of prediction test (AUC are equal to 0.99 and 0.98, respectively). Conclusions. The models of severity score demonstrated high predicted probabilities to predict severe clinical course of febrile form of tick-borne encephalitis, which can be used in clinical practice and for scientific purposes.

Objective: is to develop a model for early diagnosis of mixed infection of the non-erythema migrans form of Lyme borreliosis and tick-borne encephalitis using the assessment of the complete blood count and the blood leukocyte indices of patients in the first week of the disease.Materials and methods. The retrospective clinical study involved Group 1 of 27 patients with the mixed infection of the non-erythema migrans form of Lyme borreliosis and the febrile form of tick-borne encephalitis and Group 2 of 29 patients with the monoinfection of the non-erythema migrans form of Lyme borreliosis, who were hospitalized no later than in the 7th day of the disease. The average age of patients in Groups 1 and 2 was 50.6±3.4 and 49.9±2.3. We analyzed 14 parameters of the complete blood count as well as calculated the leukocyte intoxication index and the body resistance index. Statistical significance assessment was carried out using the chi-square test and ROC analysis. Logistic regression model was developed using STATISTICA 12.0 modules.Results. Relevance. According to official data, in 2020 in Russia, Ixodic tick-borne borreliosis took the leading position in the prevalence and registration frequency among tick-borne infections, which amounted to 38.5% of all cases of natural focal transmissible infections. In the Tomsk region for many years, the average long-term incidence of Ixodic tick-borne borreliosis exceeds the Russian one by more than 3 times. Aim. Identify features the features of the clinical and epidemiological manifestations of the Ixodic tick borne borreliosis foci in the Tomsk region; to identify priority areas for carrying out problem-oriented research in the field of ecology, epidemiology and the clinic of borreliosis infection in the Tomsk region. Materials and methods. The material for the study was the data of the federal statistical observation form «Information on infectious and parasitic morbidity» for the 2015-2020. The materials of 713 cases of Ixodic tick-borne borreliosis disease and more than 125 thousand appeals to medical organizations of the region regarding the tick bites, registered in official registration forms, were studied. As part of the annual epidemiological monitoring for a six-year period, 1200 specimens of Ixodid ticks collected in natural biotopes of the Tomsk region were examined for the presence of Borrelia DNA, depending on their genus (Ixodes and Dermacentor). DNA detection of Borrelia burgdorferi sensu lato complex (B. burgdorferi, B. garinii, B. afzelii) and B. miyamotoi was performed by real-time PCR. Results. The high number of ixodid ticks in local areas of natural biotopes (up to 200.0 specimens/km at the height of the epidemiological season), as well as the failure to comply with personal prevention measures, determines a consistently high rate of referral of the population of the Tomsk region to medical organizations due to tick bites. The number of requests annually averages 20.8 thousand. As a result of an annual study using PCR tests of ticks taken from people who applied to emergency prevention centers, Borrelia DNA was detected on average in 40% of samples; blood samples from individuals affected by tick bites were positive on average in 3.7% of cases. In the period 2015-2019 spirochetes B. garinii and B. afzelii were most often found in the tissues of ixodid ticks. Since 2020, the species Borrelia miyamotoi has been included in the list of pathogenic borrelia species to be monitored in the Tomsk region – spontaneous infection of ticks (Ixodes) was 3%. The maximum values both in the number of patients with ixodic tick-borne borreliosis and in the density of infections per 10,000 hectares (90-110 cases) are observed in the south of the Tomsk region (Asinovskyi, Kozhevnikovskyi, Krivosheinskyi, Tomskyi, Shegarskyi districts, the city of Tomsk). In the epidemic season of 2020, the number of laboratory-confirmed cases of non-erythema ixodic tick-borne borreliosis was more than 2.5 times higher than the number of diseases with skin manifestations. Conclusion. The large number of ixode mites in natural biotopes, the high degree of their infection with pathogenic spirochetes indicate the presence of active natural foci of ixodic tick-borne borreliosis in the Tomsk region. The study of the genotypic diversity of pathogenic borrelias in relation to the species diversity of vectors, the analysis of the clinical manifestations of different etiological variants of tick-borne borreliosis (mono- and mixed infections), as well as the development of an algorithm for differential diagnostic search and a model for predicting the outcomes of the infectious process in tick-borne borreliosis and mixed infections are priority directions of problem-oriented scientific research in Tomsk region.

The aim of this study was to identify the features of CD4+ lymphocytes apoptosis in children with cryptosporidiosis. Feces for the detection of cryptosporidium and venous blood for the study of lymphocytes apoptosis were the material of the study. Mononuclear leukocytes were isolated from venous whole blood and cultured in a complete culture medium. Cells were incubated for 24 hours with inducers of the receptor (TNFα) and mitochondrial pathways of apoptosis (dexamethasone).Cryptosporidia have been found in 35% of acute intestinal infections in children. The number of lymphocytes with cytoflurimetric signs of apoptosis in the group of cryptosporidiosis-positive patients did not differ from that in patients without cryptosporidiosis (p=0.421). There were no intergroup differences in the number of CD4+ lymphocytes expressing the Fas receptor on their surface (p=0.462). In cultures incubated in the presence of dexamethasone, a decrease in the number of apoptotically altered CD4+ lymphocytes was registered only in the group of cryptosporidiosispositive patients (p=0.028).The study showed that in cryptosporidiosis, the sensitivity of CD4+ cells to the induction of the mitochondrial pathway of apoptosis changes in favor of slowing down this variant of cell death.

Abstract. Parameters of peripheral blood lymphocytes subpopulations have been studies in the patients with extensive destructive pulmonary tuberculosis caused by drug-sensitive or drug-resistant Mycobacterium tuberculosis before and in the course of specific anti-tuberculosis chemotherapy. It was revealed that in the patients before treatment, a marked immunosuppression takes place, being characterized by decrease of total T-lymphocytes and their basic regulatory and effector subpopulations. E.e., an increase in CD20+-, CD25+- and CD95+cells was observed. The completion of intensive anti-tuberculosis chemotherapy is not accompanied by correction of lymphocyte subpopulations. By contrary, in the patients with drug-sensitive pulmonary tuberculosis M. tuberculosis a sharp increase in CD95+-lymphocytes is registered. After undergoing a full course of therapy a well-noted tendency to normalization of T-lymphocytes (CD3+) quantity, as well as is regulatory populations (CD4+ and CD8+) were found. The level of CD95+-lymphocytes remained increased in the patients with pulmonary tuberculosis caused by drug-resistant M. tuberculosis.

Evaluation of proliferative and IL-2-producing activity of peripheral blood lymphocytes wasperformed, using cultural methods, in patients with drug-sensitive and drug-resistant infiltrative pulmonary tuberculosis. The cell testing was performed at basal level and following in vitro stimulation with recombinant IL-2 and M. tuberculosis antigens. It was established that clinical course of infiltrative pulmonary tuberculosis, independently on drug sensitivity/resistance of the infectious pathogen, is accompanied by suppression of spontaneous lymphoproliferation. The levels of induced IL-2 production in drug-sensitive tuberculosis proved to be increased, whereas a reserve of IL-2-secreting reactivity of blood lymphocytes was lower than in drugresistant infection. Also, it was revealed that the level of lymphoproliferative response induced by IL-2, does not depend on clinical variant of tuberculosis, whereas stimulation of IL-2 production in blood lymphocytes is attained only in cases of drug-resistant tuberculosis variant.

In present work, some immunogenetic aspects of pulmonary tuberculosis were studied, using modern techniques from molecular genetics and immunology. It is shown that carriage of Т allele and homozygous TT genotype in +874А/Т IFNγ gene polymorphism comprise a immunogenetic factor which correlated with a protective effect, regarding a susceptibility to pulmonary tuberculosis. Predisposition for tuberculosis infection is associated with A allele of this gene, as well as with АА and АТ genotypes of +874A/T IFNγ gene. Clinical evolution of pulmonary tuberculosis is accompanied by increase in IFNγ production.

Subpopulation indices of peripheral blood and pleural exudate lymphocytes have been studied in the patients with various clinical variants of a tuberculous pleurisy. It has been shown that, in patients with both MBT-negative and MBT-positive variants of tubercular pleurisy, decreased quantity of CD3- and CD4-positive lymphocytes was observed, whereas an increase in numbers of CD8+ and CD16+ lymphocytes was more pronounced in the persons with MBT-negative pleurisy. In pleural exudates from the patients with tubercular pleurisy, the numbers of CD3+ lymphocytes proved to be significantly higher than their contents in peripheral blood. It may be assumed that lymphocytosis in pleural exudates results from migration of the cells from peripheral blood into pleural cavity. It is revealed that, during exudative pleurisy of tuberculosis origin, T-cells with CD4+ and CD8+ phenotype are more likely subject to migration into pleural cavity, accomplished by additional migration of CD16+ lymphocytes in MBT-positive cases of pleurisy.

Abstract. The article deals with results of studying combined effects of ethambutol and protein antigen of Мycobacterium tuberculosis upon proliferative response and cytokine-producing ability of peripheral blood mononuclear cells in patients with infiltrative lung tuberculosis prior to beginning of specific antibacterial therapy. The mononuclear blood cells were cultivated with complex protein antigen of Мycobacterium tuberculosis and ethambutol. In the patients with lung tuberculosis, a decrease in proliferative reserve of lymphocytes was detected, in absence of increased IL-2 and IL-10 secretion. The combined effect of ethambutol and protein antigen of Мycobacterium tuberculosis upon peripheral blood mononuclear cells in the patients with pulmonary tuberculosis and healthy donors was similar, and it was expressed as antiproliferative effect and selective suppression of IFNγ production.

Abstract. A distribution mode for allelic variants of cytokine genes was evaluated in 184 patients with slow viral infections, including 97 patients with chronic herpetic infection and 87 HIV-infected patients. Using modern methods of immunogenetics, we have found that relative risks of recurrent course and poor outcome of infection are positively associated with AA promoter region genotype and AA promoter genotype of +874 A/T polymorphism in the IFNG gene. Immunogenetic factors associated with protective effect in slow virus infections, include G allele of TNFA gene (G-308A SNP), and T allele/TT genotype of promoter region in the IFNG gene (+874 A/T SNP). Abstract. Using modern immunological methods of studies, we investigated some immunopathogenetic aspects of rheumatoid arthritis complicated by chronic herpesvirus infection. It was established that both rheumatoid arthritis, and chronic herpesvirus infection are accompanied by pronounced imbalance in production of immunoregulatory Th1/Th2-cytokines produced in immunocompetent cells of peripheral blood. In case of rheumatoid arthritis combined with herpes simplex infection, IL-2 and IFNγγ levels were decreased, whereas IL-4 и IL-6 levels proved to be increased, as compared with isolated diseases under study.

Abstract. The study considers some aspects of pathogenetic mechanisms underlying blood eosinophilic reactions developing in pulmonary tuberculosis, taking into account some novel data obtained by means of modern immunological techniques. It was revealed that IL-5 is among key factors determining eosinophilic reactions of blood following infection with M. tuberculosis. Blood serum IL-5 concentrations proved to be significantly higher in patients with tuberculosis accompanied by eosinophilia. In these cases, eotaxin is a chemokine, which selectively acts upon eosinophilic cells. Its effects include prolonged persistence of eosinophils in bloodstream, along with initiation of eosinophil adherence to vascular endothelium, thus enabling subsequent migration of eosinophilic leukocytes to the foci of specific granulomatous inflammation. Abstract. The study dealt with subpopulation analysis of peripheral blood T-regulatory cells (Treg), and in vitro testing of immunosuppressive cytokine (IL-10, TGF-β) production in the patients with fibrous/cavernous pulmonary tuberculosis, depending on results of intracutaneous tuberculin tests (Mantoux test). We have shown that Trn, natural T-regulatory lymphocytes (CD4+CD25+FoxP3+), and TGF-β cytokine, produced by these cells, play a leading role for immune suppression developing in fibrosis-cavernous pulmonary tuberculosis. Moreover, an imbalance of Treg cell subsets has been revealed in patients with fibrous/ cavernous pulmonary tuberculosis, with either positive, or negative Mantoux reaction, as shown by increased content of Treg cells with CD4+CD25+FoxP3+ phenotype, along with higher amounts of CD4+CD25-FoxP3+ subpopulations, and deficiency of CD4+CD25+FoxP3- Treg lymphocytes in blood samples (in cases of positive Mantoux tests). Increased amounts of CD4+CD25+FoxP3+ Treg cells in peripheral blood of patients with fibrous/cavernous pulmonary tuberculosis is associated with increase in basal and BCG-induced TGF-β production, and decreased in vitro IL-10 secretory potential. Subpopulation structure of regulatory T-cells of peripheral blood in patients with newly diagnosed pulmonary tuberculosis depending on the clinical form of disease and sensitivity of Mycobacterium tuberculosis to anti-tuberculosis drugs have been analyzed in this work. It has been shown that in formation immune suppression at infiltrative, dissemination and fibrosis-cavity pulmonary tuberculosis the leading part play natural regulatory CD4+ CD25+ Foxр3+ -T-lymphocytes. Thus increase of their number in blood at drug-resistance and drugsusceptible patients. It has been demonstrated that in patients with fibro-cavernous and infiltrative form of the disease and drug-resistance pulmonary tuberculosis the number of CD4+ CD25- Foxр3+ -regulatory T-cells were increasing.

The results of 77 M. tuberculosis clinical isolates genetic typing, allocated from patients with extensive destructive infiltrative and disseminated pulmonary tuberculosis are presented in the article. It is established that among genetically non-uniform Mycobacterium tuberculosis strains, circulating in Tomsk and the Tomsk Region, and causing acute progressive destructive pulmonary tuberculosis, 19% show multi drag resistance. Beijing family strains make 27% from the general population, have a high clusterization index (74%), level of their multi drag resistance in 3 times exceeds that at «not Beijing» strains for which the share of clustering strains makes 11%.

The paper presents the research results of monetary income differentiation of the population with use of the author's methodical approach with regard to different purchasing power of ruble in the Russian regions. All the population of Russia was rearranged from regional quintile groups into the all-Russian groups that resulted in significant changes of inequality parameters at the interregional level of incomes in comparison with the Rosstat parameters. For the first time an influence of latent incomes to inequality and poverty level in the Russian regions is analyzed. Division of the population of the country into needy and wealthy groups is offered. Calculations and recommendations regarding redistributive overcoming of poverty at the expense of increase of the rate of surtax on incomes of the wealthy group are developed. The models by Pen, Lorenz and Ravallion-Huppi, modified by the authors of the article, were applied in the research. Calculations were carried out for all subjects of the Russian Federation (without the Chechen republic) according to the Russian State Statistics Service figures for 2000–2008.

The results of research show the impact of ofloxacin, para-aminosalicylic acid (PASA) and capreomycin in conjunction with the BCG vaccine strain to the cytokine-production ability of mononuclear peripheral blood leukocytes for patients with infiltrative pulmonary tuberculosis prior to specific antituberculosis therapy. The results show that ofloxacin has inducing effect on the secretion of interleukin (IL) 4 and transforming growth factor (TGF) β; PASA and capreomycin have such effect for the products of IL10 and TGFβ. This influence has a negative effect during the active development of the pathological process and during specific treatment.

The article presents up-to-date literature data on the role of γδТ-cells and different NK-cells subpopulations in the immune response. Their origin, immunophenotypes, range of secreted cytokines as well as effector functions and factors having stimulating and inhibitory effect on γδТ- and NK-cells are described. Mechanisms of antibacterial action of this type of cells are analyzed.

In article the data, concerning features of cellular composition, proliferative activity and cytokines production of peripheral blood lymphocytes is presented in patients with various clinico-pathogenetic variants of pulmonary tuberculosis: drag-sensitive and drug-resistant infiltrative, disseminated, fibrous-cavernous. It is established that the extensive destructive tuberculosis without dependence from the clinical form of disease is accompanied by the expressed insufficiency of the cellular-mediated mechanisms of immunological resistance both to drag-sensitive, and to drug-resistant M. tuberculosis against quantitative prevalence of B-lymphocytes and intensity of their functional activity.

In the article is consider reasons of the second immunodeficiency, attendants flow the infectious diseases of viral kind. It is established, that molecular machinery make conditional viral-induce immunosuppression, in those number role in her forming CD4+ CD25+ T-lymphocytes, witch known as regulatory T-cells or T-reg, able expressed Foxp3 factor control transcription genes responsible for differentiation T-cells and secretion cytokins, participated in the suppression of immunity response.

The intake of Simvastatin in a dosage of 40 mg daily within two months in patients with ischemia heart disease has caused increase in the general percentage of ω6 fat acids (FA) at a constant level of ω3 FA, that was not accompanied by change of a ratio ω3/ω6 FA. Despite of progressing increase in total concentration of ω7 FA (at women) and increase of a level ω9 FA (at men) during simvastatin treatment at women and at men led to decrease in the total percentage of nonsaturated fat acids concerning corresponding parameters at healthy donors. The Simvastatin treatment was accompanied by increase in blood plasma in patients with ischemia heart disease (and first of all at women) percentage of the separate saturated fat acids, and also their total level. The index of nonsaturation of fat acids of blood plasma is decreased.

In article the results of the analysis of changes of the contents macro- and microelements (К+, Ca2+, Fe2+, Cu2+, Mn2+, Zn2+, Mg2+, Al3+) in lymphocytes and monocytes of peripheral blood in patients with infiltrative drug-sensitive and drug-resistant pulmonary tuberculosis before and on a background of realization standard antituberculosis therapy are discussed

Aimed at non-specific reactivity of the organism, cytochemical activity and microbicidal properties of blood neutrophils and peripheral blood monocytes were studied in 102 patients with drug-sensitive pulmonary tuberculosis before the treatment, after in-tensive phase of therapy and after stopping anti-tuberculosis therapy. Increased hydrolytic enzymes' activity of leucocytes, levels of non- cationic proteins and decreased levels of cellular glycogen and induced NBT-test were observed without their dependence on drug-sensitivity of mycobacterial tuberculosis before and during the treatment.. At the same time, content of lipids in neutrophils and monocytes undergoes different changes.

The authors of the article are trying to generalize the literary data that characterizing proinflammatory and anti-inflammatory cytokines production of peripheral blood immune cells during tick-borne neuroinfections: Lyme borreliosis, associated with tick- borne encephalitis. The immune response development to antigens of a tick-borne encephalitis virus and Borrelia burgdorferi in pa- tients with a mixed-infection essentially differs from those during monoinfections.

Nitric oxide NO• production by blood monocytes was studied in 53 patients with drug-sensitive pulmonary tuberculosis (TB) and 50 patients with multidrug resistance (MDR) TB before and during the anti-tuberculosis therapy. Levels of NO• production were determined using the Griess's reactant. Increased levels of NO• secretion were observed before the treatment and after the intensive phase of therapy of drug-sensitive TB. However, it decreased after the full course of the anti-tuberculosis therapy. At the MDR TB the NO• production by monocytes was reduced before and after the intensive phase of treatment and increased after the end of the full course of therapy.

Actual issues of modern phthisiology concerning tubercular infection immune pathology and nature of immune imbalance during primary and secondary tuberculosis are considered in the article. Literature and original data which are basing mechanisms of Т-cellular deficiency development in patients with drug-sensitive and drug-resistant pulmonary tuberculosis are analyzed.

Among patients with acute and chronical clinical course of Ixodidea mite borreliosis there was studied a subpopulation structure and chromosome of peripheral blood lymphocytes, blasttransformation of cells under influence of T- and B-mitogens, production by IL-12 and IL-10 mononuclear leucocytes. Secretion of IL-12 at acute clinical course was increased, IL-10 was depressed, and under chronic clinical course the excretion of both cytoxicants was increased. The level of chromosomal abnormalities in T-lymphocytes was higher in debut of acute period of illness. There was registered the instability of B-lymphocytes genome at the end of acute phase of borreliosis. Deficit of cytotoxic lymphocytes was accompanied by accumulation of cells with caryo-pathology.

The aim of this study was to evaluate the level of secretion of IL-12β by mononuclear leukocytes of peripheral blood under infiltrative and disseminated pulmonary tuberculosis. Blood mononuclear cells were isolated by gradient centrifugation, then cultured in complete culture medium; the level of IL-12β production was determined by the ELISA (ELISA). Peripheral blood served as the material of the research. It has been established, that under infiltrative pulmonary tuberculosis the levels of basal, protein and lipid-induced IL-12β production were significantly lower than in healthy donors and patients with disseminated pulmonary tuberculosis. Under disseminated pulmonary tuberculosis basal IL-12β production varies within normal limits; when stimulated by protein antigen, it increases, when influenced by lipid antigen — decreases.

Some biochemical indicators of peripheral blood and pleural fluid (level of the total protein, glucose, lactate dehydrogenase activity) at patients with various variants of tubercular exudative pleurisy (MBT-positive and MBT-negative) for the purpose of definition of possibility of use this indicators as criteria for differential diagnostics of a pleurisy, have been studied. It has been established that tubercular exudative pleurisy is characterized by the expressed changes of the biochemical status of blood and pleural fluid in comparison with that at healthy donors and patients with not tubercular exudative pleurisy, however, this changes in most cases are nonspecific and don't depend on a variant of tubercular exudative pleurisy.
