Prevention and treatment of long-term symptoms of coronavirus infection in children with bronchial asthma (BA) is currently a significant challenge due to the scale of COVID-19 pandemic. Objective. To evaluate the efficacy and safety controlled hypoxic stimulation in children after coronavirus infection, including those with BA. Patients and methods. This study included 72 children aged 9–12 years who had had COVID-19 at least 1.5–2 months before the enrollment;of them, 24 patients had mild to moderate BA. Study participants were randomized into two groups: ‘intermittent hypoxic training’ (IHT, n = 31) and ‘intermittent hypoxic-hyperoxic training’ (IHHT, n = 41). Before the procedures, we assessed hypoxic resistance of children using a hypoxic test. The following parameters were measured before and after the course, which included 10 procedures: hypoxic resistance (hypoxic index, time to desaturation, and reoxygenation time), spirometry parameters (forced expiratory volume in 1 second, peak expiratory flow, and airway resistance), changes in systolic/diastolic pressure, heart rate variability, tolerability of procedures (using a visual analog scale), and anxiety level (CMAS test). Results. We found no significant differences between BA and non-BA children in their spirometry parameters and hypoxic resistance. We observed good tolerability of the procedures in children from different groups. After the course of IHT/IHHT, patients demonstrated a significant increase in their resistance to hypoxic stimulation. IHHT procedures were more effective and safer than IHT according to spirometry parameters (RINTexp), tolerability, and anxiety level. Conclusion. Hypoxic conditioning procedures can be recommended as a part of comprehensive rehabilitation in BA children who had had COVID-19. © 2021, Dynasty Publishing House. All rights reserved.

Introduction. Bronchial asthma (BA) is one of the most common chronic lung diseases in children. At the beginning of the COVID-19 pandemic, BA, like other lung diseases, was considered a risk factor for the severe course of COVID-19. Aim of the study – to analyze the main clinical manifestations of COVID-19 in children with BA. Material and methods. During 2020, 500 children with BA from the pulmonary department of the Sechenov University Children’s hospital were questioned. 3 % of them (15 children) were with COVID-19. Moreover, clinical picture of COVID-19 was analyzed in other 75 children with BA and 53 children without BA, who were observed in outpatient department and in Moscow pulmonary sanatorium N 15. Results. It has been shown that the symptoms of COVID-19 can be similar to those of an exacerbation of asthma and manifest itself as dry cough and shortness of breath, and fever, which can also be observed with exacerbations of asthma against the background of a respiratory infection of any genesis. Conclusion. Based on the factual clinical material, it was shown that the new coronavirus infection is easier in children, and in asthmatics among patients with less pronounced clinical symptoms.

In order to substantiate new approaches to maintain the quality of life of elderly patients with cardiac pathology, the current study is made to evaluate the effectiveness and safety of technology, based on the principles of adaptive medicine, the method of interval hypoxic-hyperoxic training (IHHT). It was proved the use of IHHT technology in elderly people with HIBS in the prescribed regime leads to a substantial increase in the subjective perception of the quality of life, their psycho-emotional status, the number of attacks of angina pectoris, which was accompanied by an increase in the level of physical working capacity, exercise tolerance, normalization of lipid and carbohydrate metabolism. Treatments by interval hypoxic-hyperoxic training are well tolerated, do not cause side effects; when developing optimal individualized regimens' principles IHHT can be used in the complex rehabilitation of elderly patients with cardiac and co-morbid pathology.Для обоснования новых подходов поддержания качества жизни пожилых пациентов с кардиальной патологией проведено исследование эффективности и безопасности внедрения технологии, базирующейся на принципах адаптационной медицины — метода интервальных гипоксически-гипероксических тренировок (ИГГТ). Установлено, что применение технологии ИГГТ у лиц пожилого возраста с хронической ИБС в курсовом режиме приводит к существенному повышению субъективного восприятия качества жизни, их психоэмоционального статуса, снижению числа приступов стенокардии. Это сопровождалось повышением уровня физической работоспособности, переносимости нагрузок, нормализацией показателей липидного и углеводного обмена. Процедуры ИГГТ хорошо переносятся, не вызывают побочных эффектов. При отработке оптимальных индивидуализированных режимов они могут быть использованы в комплексной реабилитации пожилых пациентов с патологией сердца и сосудов.

to assess effect of interval hypoxic-hyperoxic training (IT) on exercise tolerance and quality of life of patients with ischemic heart disease (IHD) receiving optimal medical therapy, as well as the safety of IHHT use.Patients with stable IHD with functional class II and III angina (n=46) were randomized into two groups: IHHT (n=27, 15 treatments in 3 weeks), and IHHT imitation (n=19). Cardiopulmonary stress test was performed to evaluate the following parameters of exercise tolerance: peak oxygen consumption (VO2peak, VO2peak/kg), % of predicted peak oxygen consumption (%VO2 peak) and anaerobic threshold (VO2AT). MOS SF-36, SAQ, HADS questionnaires were used for assessment of quality of life (QL).Exercise tolerance (VO2peak/kg) after course of IHHT significantly increased (p=0.03) and remained significantly elevated during subsequent month (p=0.036). Marked improvement was also observed in patients subjective perception of QL. This was evidenced by dynamics of characteristics of physical functioning as well as of psychological state, significant increase of values on all scales of disease-specific questionnaire SAQ, reduction of depression and anxiety according to dynamics of HADS scores. These effects persisted in 1 month after IHHT. IHHT was safe and well tolerated. Side effects were minimal (transient slight dizziness, feeling of shortage of air) and did not require IHHT termination.We received clinical confirmation of safety and effectiveness in of IHHT in medically treated patients with stable angina. IHHT was associated with significant improvement of exercise tolerance, subjective perception of QL, reduction of number of angina attacks. Thus, IHHT has significant potential as component of complex treatment and rehabilitation of patients with stable angina.

For improvements in exercise tolerance and cognitive function in geriatric patients Multimodal training programs (MTP) are used as combination of physiotherapy, occupational therapy and cardiovascular training. Intermittent Hypoxic-Hyperoxic Training (IHHT), a modified type of intermittent hypoxic training (IHT) is proposed to be included in MTP to elicit more pronounced beneficial effects in exercise tolerance and cognitive functions of geriatric patients likely by an additional pathway than a single MTP. Thirty four patients of the Geriatric Day Clinic aged between 64 and 92 years participated in the placebo controlled clinical trial. They were randomly assigned to receive MTP plus IHHT (experimental group - EG) or MTP plus placebo-breathing through a machine face mask (control group - CG) in a double blind fashion. Before and after the interventions course cognitive performance was assessed by the Dementia-Detection-Test (DemTect) and the Clock-Drawing-Test (CDT), and functional exercise capacity - by the total distance of 6-Minute-Walk-Test (6MWT). After IHHT combined with MTP cognitive performance (DemTect) increased significantly when compared to NG (+16,7 % vs. +0,39 %, p0,001). The CDT indicated similar results with a significant increase in the EG while the score of the CG even decreased (+10,7 % vs. -8%, p=0,031). Concerning the functional exercise capacity, both groups improved the total distance in the 6MWT but with a significantly larger increase in the EG compared to the CG (+24,1 % vs. +10,8 %, p=0,021). In addition, there was a significant relationship between the changes of the 6MWT and the DemTect Scores and the CDT. IHHT contributed significantly to improvements in cognitive performance and exercise capacity in elderly performing MTP. IHHT sessions are considered to be easily applicable to and well tolerated by geriatric patients up to 92 years.Для поддержания физической работоспособности и когнитивных функций у пожилых пациентов применяются мультимодальные реабилитационные программы (МРП) на основе физических тренировок, физиотерапевтических процедур, психологического тренинга и пр. Для повышения эффективности МРП у пожилых предложено применять новый вариант адаптации к интервальной нормобарической гипоксии — метод интервальных гипоксически-гипероксических тренировок (ИГГТ). В плацебо-контролируемом клиническом исследовании приняли участие 34 пациента 64-92 лет дневного гериатрического стационара Клиники Клагенфурта (Каринтия, Австрия), рандомизированных на две группы: опытная (ОГ) — получившие МРП и курс ИГГТ; контрольная (КГ) — прошедшие курс МРП при имитации процедур ИГГТ. До и после курса реабилитации когнитивные функции и нагрузочная выносливость пациентов были оценены с применением тестов определения деменции (DemTect), рисования часов (Clock-drawing Test), а также проведением 6-минутного теста ходьбы (6MWT). При проведении курса ИГГТ когнитивные возможности пациентов ОГ значимо улучшились по сравнению с пациентами КГ: прирост значений в тесте деменции составил +16,7 % (в КГ +0,39 %, p0,001), в тесте рисования часов +10,7 % (в КГ −8 %, p=0,031). Дистанция, пройденная в 6-минутном тесте, увеличилась в обеих группах, однако достоверно больше в ОГ — +24,1 % (в КГ +10,8 %, p=0,021). Выявлены прямые значимые корреляции между приростом в нагрузочной толерантности и когнитивных тестах. Таким образом, включение процедур адаптации к интервальной гипоксии-гипероксии в МРП у пожилых приводит к значимому повышению их эффективности, что проявляется в улучшении когнитивных функций и физической выносливости. Процедуры ИГГТ хорошо переносятся, не вызывают побочных эффектов.

Purpose - to identify the specificity in objective autonomic manifestations of different signs emotions of healthy persons in relation to subjective emotional phenomenology. When benchmarking subjective self-reports of experienced emotions and values autonomic in the process of laboratory simulation in 30 subjects negative and positive emotions found that the degree and sign of subjectively evaluated emotional experiences did not always correspond to characteristics of emotion-generating stimulus, and "autonomic portrait" did not associated with the severity and the "sign"of emotional subjective experiences. Students, subjectively experienced vividly expressed strong negative emotions, did not display adequate autonomic activation, but in modeling of positive emotions they retain the negative emotional background and prolonged sympathetic overactivation in cardiovascular functions. In subjects with dominance of positive subjective assessments of their emotional status in modeling of psychological stress and in relaxation minimum shifts to parasympathetic autonomic regulation were observed.

The dynamics of the microvascular circulation and tissue oxygenation in the hypoxic test and subsequent hyperoxia are considered. In 30 healthy young men indicators of microcirculation and tissue oxygenation (laser Doppler flowmetry, and the optical tissue oximetry) during the hypoxic test - HT (10 min breathing by gas mixture with 10% O2 followed by hyperoxia (30% O2) were registered. It is found that the HT does not entail changes in the relative level of tissue oxygen saturation (SO2), but leads to a relative degree of tissue oxygen extraction significant reduction (δSaO2-SO2) with a rapid recovery in the hyperoxic phase. This is accompanied by activation of neurogenic sympathetic-related vasomotor mechanisms, as well as the endothelium-dependent microvascular tone component with a reduction in blood shunting (BSh) mostly in the hyperoxic phase and not during hypoxia. The nature of the microcirculatory response to hypoxia depends on the initial individual resistance--sensitivity level to hypoxia. Among the subjects sensitive to hypoxia HT led to a significant reduction in SaO2 and δSaO2-SO2 in the absence of changes in the microcirculation regulation. Among the subjects resistant to hypoxia HT leads to the nutritive blood flow activation by increasing the initially decreased endothelium-dependent and neurogenic sympathetic components in regulation of microhaemodynamics activity, reduction of blood shunting. At the same time δSaO2-SO2 has not significantly changed, and the activation of micro blood circulation remains in the hyperoxic recovery stage. Identified functional criteria for contribution of endothelial and neurogenic vasomotor component in the regulation of microhemodynamics in HT substantiate the contribution of the microcirculatory part and confirm the important role of hyperoxic phase in the body' hypoxia-hyperoxia adaptive response.

A comprehensive comparative analysis of hemodynamics, microcirculation (the method of laser Doppler flowmetry with occlusal probe and the optical tissue oxymetry), blood circulation neuro-humoral regulation (analysis of heart rate variability - HRV) between almost healthy young people with different levels of subjectively experienced emotional stress is given. Depending on the degree of everyday stress (acute and/or chronic) the character of neuro-autonomic regulation of blood circulation, as well as the state of the microcirculatory blood flow and its regulation are substantially different. Moderate stress is accompanied sympathetic-parasympathetic regulatory mechanisms co-activation with augmented HRV baroreflex regulation circuits that compensate hemodynamic changes and is not accompanied by hypertensive reactions. An increase in the activity of neurogenic and myogenic tone microhemodynamics in the examinees with moderate stress is discovered, which determines a high probability of blood shunting in tested tissue and decrease of relative oxygen extraction. Moderate levels of stress are also associated with increase in a latency of post-occlusive vasoreactive hyperemia, which is considered to be an initial sign of the endothelium-mediated dysfunction of microcirculation.

It is shown in the paper, polymodal rhythmic sensory influences (light, sound, vibrating-tactile etc), in combination being measures of neuro-correction, can be used for correction of a human psychophysiological condition. Efficiency of a method is shown in positive effects on mental sphere of a person, causing decrease in level of uneasiness and improvement of subjective self-assessment, in influence on cognitive and autonomic functions. Polymodal rhythmic sensory influences are means of increase in human intellectual activity efficiency: attention' concentration is increased in proportion with the increase of parasympathetic activity induced by sensory polymodal stimuli.

In the first part of this paper the modern concepts of stress and possibility of diagnostics of early stress manifestations are considered. Though significant advances of researches of this problem many methodological and methodical aspects are continued to be discussed. The categorical proff are stage development of adaptation processes and there classification [H. Selye]. A long-term studies of adaptation processes of students in semesters and exams have been conducted by authors with specially development and to accumulate new data. The second part of this paper is the survey of these results published in different time. The polyparametric technology including some electrophysiological methods with the system analysis using artificial intelligence algorithms and methods of pattern recognition permitted to show some regular dependences in relationships of electrophysiological parameters and there variations under stress development. It was shown for diagnostics of stress stages in necessary to assess not only absolute values of parameters but also there relationships, which are new diagnostics signs considering as information connections between physiological systems. The main characteristic of functional state of an organism is balance of parameter's relationships in totality, representing invariant for healthy persons. In condition of disharmony of parameter relationships there are aroused the strain, overstrain functional state or failure of adaptation in depends on a degree of misbalance. The represented new data don't contradict to known reasons and mechanisms of stress and accumulated data of its variety manifestations. As the work-hypothesis the information-wave model of appearance of psychosomatic reactions of stress is introduced.

В реабилитации и вторичной профилактике кардиоваскулярной патологии значительное место отводится рациональной комбинации медикаментозных и немедикаментозных методов, направленных на коррекцию факторов риска, полиморбидности. В этом плане определенный потенциал открывает гипоксическое прекондиционирование или интервальные гипоксические тренировки (ИГТ), значимость которых показана в комплексной реабилитации пациентов с сердечно-сосудистой патологией, бронхиальной астмой, алиментарным ожирением и пр. В экспериментальных работах показано, что эффективность процедур ИГТ можно повысить чередованием коротких гипоксических экспозиций гипероксическими «импульсами», что более эффективно предупреждает развитие свободнорадикально-индуцированных стрессорных нарушений, повышает физическую выносливость по сравнению с «традиционным» режимом ИГТ. Нами разработан новый способ гипокситренировки человека, в котором для потенцирования ее эффектов используется дыхание гипоксическими газовыми смесями, чередующееся с дыханием гипероксическими (30% О2) газовыми смесями - метод интервальной гипоксически-гипероксической тренировки - ИГГТ и устройство для его реализации с дозированием гипоксического воздействия по принципу биологической обратной связи (Патент РФ № 2365384 от 27.08.2009). Выполнены исследования эффективности ИГГТ в коррекции индивидуальных компонентов метаболического синдрома (МС). 35 пациентов случайным порядком были разделены на опытную (23 чел., на фоне базовой терапии без использования фармпрепаратов коррекции пищевого поведения и массы тела, прошедших курс из 14 процедур ИГГТ) и контрольную (12 чел., базовая терапия с учетом сопутствующей патологии + дыхание через маску обычным воздухом) группы. Процедуры ИГГТ (5 раз в нед.) проводили с применением опытного образца установки Reoxy (AI Mediq, Люксембург). Индивидуальные режимы подбирались на основе проведения 10-минутного гипоксического теста с ежеминутным мониторингом ЧСС и сатурации крови кислородом - SaО2. Применение ИГГТ (в сравнении с группой плацебо) приводило к значимому снижению массы тела пациентов за счет уменьшения жировой массы и сопровождалось нормализацией липидемического «профиля» - достоверным снижением уровня ОХС, ЛПНП, глюкозы плазмы натощак, оптимизацией исходно повышенных значений АД, ЧСС покоя, повышением гипоксиче-ской устойчивости, физической выносливости - значимым приростом пройденной дистанции в 6-минутном тесте, снижением показателей ситуативной тревожности, депрессии. Результаты исследований доказывают эффективность применения новой методики гипоксиченских тренировок-технологии ИГГТ в коррекции модифицируемых метаболических и кардиоваскулярных факторов риска, для восстановления физической работоспособности и аэробной выносливости пациентов с МС. Дополнительные перспективы метод имеет у пациентов с ожирением 2-3, ортопедической коморбидностью - для достижения первых значимых результатов редукции массы тела и повышения их вовлеченности в долгосрочные программы физической реабилитации.

Неблагоприятные современные тенденции в кардиоваскулярной заболеваемости населения актуализируют расширение средств и технологий нелекарственной кардиореабилитации и профилактики. Одним из перспективных подходов является применение повторных эпизодов гипоксического прекондиционирования - интервальные гипоксические тренировки (ИГТ) или гипокситерапия. В последнее время предпринимаются попытки повысить эффективность ИГТ, применяя «дозный» подход, комбинацию с гиперкапнией, индивидуализируя режимы тренировки и пр.Цель. Оценить эффективность и безопасность нового метода - интервальных гипоксически-гипероксических тренировок (ИГГТ) в реабилитации пациентов с кардиоваскулярной патологией.Методы. В пилотном исследовании приняли участие 40 пациентов с ишемической болезнью сердца (ИБС), ФН 2-3 (31 мужчина, 9 женщин, средний возраст 61,7±7,7 года, 14 - с перенесенным инфарктом миокарда): группы ИГГТ (n=30, прошли 20 процедур по 45-50 мин 5 раз в неделю) и контроля (n=10, 20 имитирующих процедур). Для создания газовых смесей с содержанием O2 от 10 до 35-40% использовали опытный образец прибора Reoxy («AI Mediq S.A.», Люксембург). Перед началом и в конце курса определяли индивидуальную чувствительность к гипоксии путем проведения 10 мин гипоксического теста (ГТ). Процедуры ИГГТ проводили в интервальном режиме - начинали с подачи через маску 12-11% О2 (5-7 мин), затем 2-3 мин - с 30% О2 (вместо традиционной нормоксической паузы). Переключение газовых смесей осуществлялось автоматически по специальным алгоритмам (биообратная связь), за 1 процедуру повторялось 6-8 циклов «гипоксия-гипероксия». До и после курса все пациенты прошли обследование: клинический и биохимический анализ крови, ЭКГ, ЭхоКГ покоя, субмаксимальное нагрузочное тестирование на беговой дорожке с определением времени выполнения нагрузки до отказа, мощности выполненной нагрузки; толерантности к нагрузке в метаболических единицах (МЕ).Результаты. После курса ИГГТ отмечено значимое повышение толерантности к физическим нагрузкам - прирост времени выполнения нагрузки до отказа на 34,1% (в контроле - 2,7%), увеличение МЕ на 15,8% против 5,4% в контроле, значимое снижение количества ангинальных приступов в качестве причины прекращения нагрузки. Повышение физической выносливости сопровождалось достоверным снижением значений общего холестерина, триглицеридов плазмы крови, исходно повышенных значений артериального давления, частоты сердечных сокращений покоя, повышением порога толерантности к гипоксии в ГТ. Субъективно режим ИГГТ легко переносился всеми пациентами, не сопровождался побочными эффектами.Выводы. Курс процедур ИГГТ повышает толерантность пациентов с ИБС к физическим нагрузкам, что сопровождается нормализацией липидного профиля, снижением артериального давления, количества ангинальных приступов, повышением гипоксической устойчивости. Метод может применяться для коррекции модифицируемых факторов риска, в долгосрочных программах кардиореабилитации, до и после чрескожных коронарных вмешательств, в случаях ограничений использования физических тренировок, а также для потенцирования их эффектов.

In the article theoretical and application development of one of postulates of the Anokhin's theory of functional systems--the principle of multiparametric interaction is attempted in study of singularities of intersystem relationships of a number of leading functional homeostatic systems in a developing adolescent's organism living in radioecological unfavorable conditions and during rehabilitational procedures with application of interval dosed normobaric hypoxia. On the basis of a dynamic research of parameters cardiorespiratory and vegetative-humoral homeostasis is established, that the acclimatization of a children's organism to the factors of ecological and social risk in regions under small doses of radionuclides contamination is exhibited in reorganization of multiparametric relations of functional systems: a) increase of "rigidity" of intrasystem links separate cardiovascular effectors, b) of maximum activation sympathetic, pituitary-adrenal and pituitary-thyroid axes of a system stress-response, c) lack of intersystem consolidation of cardiorespiratory functional systems at a level of useful adaptive results. Thus character of interaction of homeostatic functional systems, their stability depend on personal combination of typological singularities in "the integral constitution" of the child-types of a vegetative regulation, somatic constitution, versions of emotional uneasiness. Principally important that it is revealed the capability of a correction in intersystem relations of homeostatic parameters, in particular, with the use of interval normobaric hypoxia of training (IHT). Hypoxic indorsements rendering the influence first of all through an exterior link of a functional system of breathing is carry on to recovery of integration of functional systems defining a homeostasis for children as at a level useful adaptive results. Thus the role initially high neurohumoral activity in achievement of best values cerebral, peripheral blood flow, lung ventilation is reduced, the contribution of separate effectors to security of physical functionality and aerobic capabilities of a children's organism "is equilibrated".
