1. Change in the karyotype of the embryo as a cause of spontaneous abortion in the first trimester

Abstract

Currently, one of the most actual health problems in the world is the problem of a spontaneous abortion. Chromosomal abnormalities are one of the main reasons of early abortion, stillbirth or the birth of a child with multiple congenital malformations. The results of karyotyping of chorionic villi from 149 patients with diagnoses of "undeveloped pregnancy" or "anembryonia" were presented in the article. Metaphase chromosomes were used for cytogenetic research of the embryos. The samples were obtained from cytotrophoblast cells of chorionic villi by the "direct" method, without culture, according to standard technique. A GTG-method was used for staining the chromosome slides. The cytogenetic study of chorionic villi makes it possible to identify the karyotype abnormalities, which caused the fading of pregnancy, since the chorion karyotype corresponds to the embryo karyotype The slide analysis was carried out in accordance with the international system of the human cytogenetic nomenclature. As a result of the cytogenetic study, changes in the karyotype were revealed in 53.39 % of the samples, namely, genomic mutations – aneuploidy and polyploidy. Among the aneuploidies, we found the embryos with trisomy in the autosomes 5, 13, 16, 18, 20, 21, and 22. In addition, the karyotypes with trisomy and monosomy of the X chromosome, as well as the male karyotypes with an additional copy of the X or Y chromosome were present The polyploids in the study group were represented by triploid and tetraploid karyotypes of embryos. All the above mentioned karyotype disorders were found in both full and mosaic forms. Further, some non-developing embryos contained a chimeric karyotype - "chi46,XX/46,XY". In the study group, the prevailing were the embryos with a tetraploid karyotype in a mosaic form (6.71 %), Klinefelter syndrome in a mosaic form (6.04 %), and a triploid karyotype in a mosaic form (6.04 %). A statistically significant increase in the incidence of stillbirths with mosaic tetraploid karyotype was found in 2020. The share of this pathology was 25. %. Currently, there is no reliable information on the effect of the SARS-CoV-2 virus on the embryonic genome. Nevertheless, it is known that members of the coronavirus family are responsible for serious complications during pregnancy - pregnancy fading, fetal growth retardation, premature birth, death of the mother or fetal death in the neonatal period. In this connection, we can hypothesize that mitotic disorders and, as a consequence, appearance of embryos with a tetraploid karyotype seem to be associated with a mild SARS-CoV-2 infection that occurs in pregnant women in an inconspicuous form.

Aim. To synthesize cis- and trans-isomers of 3-(4-hydroxyphenyl)cyclobutanecarboxylic acid and evaluate the biological activity of their derivatives against GPR-40. Results and discussion. Cis- and trans-isomers of 3-(4-hydroxyphenyl)cyclobutanecarboxylic acid were synthesized. The derivatives of this compound were tested as GPR-40 agonists and exhibited the micromolar activity. Experimental part. The methyl ester of 3-(4-hydroxyphenyl)cyclobutanecarboxylic acid was obtained as a mixture of cis/trans-isomers in 3 steps starting from a commercially available 3-oxocyclobutanecarboxylic acid. Further transformation of this compound into isomerically pure 3-(4-hydroxyphenyl)cyclobutanecarboxylic acids was achieved in five steps based on the chromatographic separation of diastereomeric amide derivatives. New GPR-40 ligands were obtained by O-alkylation of a phenolic oxygen atom of the corresponding carboxylic acid methyl ester. The biological activity of the agonists synthesized was studied using a fluorometric bioassay and the engineered Chinese hamster ovary (CHO) stable cell line expressing the human GPR-40. Conclusions. An effective synthetic approach to 3-(4-hydroxyphenyl)cyclobutanecarboxylic acid allowing to isolate two single cis/trans-stereoisomers of this compound has been developed. In order to demonstrate the possibility for the bioisosteric replacement of the ethylene moiety in the structures of free fatty acid receptor (FFAR) agonists by the cyclobutane ring, four new GPR-40 ligands possessing the micromolar activity have been synthesized. Abstract

Study question Does the intrauterus application of platelet-rich plasma (PRP) allow improving the thickness of endometrium in patients with adenomyosis: comparative characteristics of the PRP-techniques? Summary answer The endometrial thickness in poor-endometrium-patients is increased after subendometrial PRP- injection comparing with the endometrium size after PRP- irrigation of the uterine cavity (P What is known already The endometrium plays an important role in achieving optimal outcomes of assisted reproductive technologies. PRP is a novel method that is used in reproductive medicine to improve the IVF outcome. The mechanisms of PRP have not been completely elucidated, but laboratory studies have shown that the high concentration of growth factors in PRP can potentially speed up the healing process. Recently, the intrauterine PRP-infusion and the subendometrial PRP-injection during the hysteroscopy have been described as a way to promote endometrial growth and receptivity. Study design, size, duration Totally 64 patients with adenomyosis and poor endometrium (≤ 6 mm) were divided into three experimental groups depending on PRP administration: intrauterine PRP-infusion, subendometrial PRP-injection and standard hormonal therapy of adenomyosis without PRP application. The period of study – December 2019 – November 2020. The study’s protocol was approved by the Center’s IRB. Participants/materials, setting, methods Intrauterine PRP-infusion was performed for 23 patients with age 34.2±3.6 y.o. (Group 1). Subendometrial PRP-injection was applied for 16 patients with the middle age 32,6±2,4 y.o. (Group 2). Additional hormonal therapy was carried out for patients from Groups 1, 2. The third control Group 3 consisted of 25 adenomyosis women with the middle age 35.7±4.1 y.o. Only the hormonal therapy was performed for patients of the control group. Main results and the role of chance The procedure of intrauterine PRP-infusion was done using the intrauterine catheter on the 8th and 12th days of the f hormone replacement therapy. Subendometrial PRP-injection was performed during the hysteroscopy on the 8th–10th day of the previous menstrual cycle. The control of the endometrium size was done by ultrasound examination. Study question Does the intrauterine administration of peripheral blood mononuclear cells (PBMCs) effect the outcome of IVF for patients with advanced maternal age when the euploid embryos after PGT-A are transferred? Summary answer The implantation rates were significantly higher after the intrauterine application of PBMCs in patients with advanced maternal age (AMA) before transfer of the euploid embryo. What is known already The aneuploidy rates of blastocysts in IVF is in range 45–70% depending on different factors. Besides that, the endometrium plays an important role in achieving optimal outcomes of assisted reproductive technologies. It has been proposed that intrauterine administration of peripheral blood mononuclear cells modulates maternal immune response to favor implantation. Study design, size, duration The effect of the intrauterine application of PBMCs to improve the implantation rates in the group of patients with advanced maternal age was studied. Two group of patients (PBMCs-group and non-PBMCs-group_ were formed. Single euploid embryo was transferred for each patient. Participants/materials, setting, methods: The ploidy status of 373 blastocysts from 82 AMA-patients was analyzed by the method of next generation sequencing (NGS). PBMCs were applied for 39 women with the mean age 39.2±3.2 y.o. before embryo transfer (Group 1). For 43 patients with the mean age 38.2±2.1 y.o. single euploid embryo transfers were performed without PBMCs administration (Group 2). Chi-squared test was used for data analysis. The study’s protocol was approved by the Center’s IRB. Main results and the role of chance Totally the rate of euploid embryos was 27.1% (101 blastocysts). In the mentioned study 55.0% of examined blastocysts were aneuploid (205 embryos) and 17.9% of blastocysts were detected as mosaic (67 embryos). Single euploid embryo was transferred in each case in the patients of both experimental groups. The implantation rate was significantly higher in Group 1 with PBMCs application comparing with non-PBMCs experimental Group 2 (38.5% (15 pregnancies) vs. 23.3% (10 pregnancies), df = 1, χ2 = 5.487, χ2critical = 3.841, P = 0.020). Limitations, reasons for caution The embryo biopsy was performed only for blastocyst with top-quality morphology. Wider implications of the findings: The implantation rates were significantly higher when the intrauterine application of PBMCs in patients with advanced maternal age before the transfer of the euploid embryo (P = 0.020). Abstract

Four 3-((hetera)cyclobutyl)azetidine-based isosteres of piperidine, piperazine, and morpholine were designed and synthesized on up to gram scale. The key step of the synthetic sequence included cyclization of N-protected 2-(azetidin-3-yl)propane-1,3-diol or the corresponding 1,3-dibromide. X-ray diffraction studies of the products obtained, followed by exit vector plot analysis of their molecular geometry, demonstrated their larger size and increased conformational flexibility as compared to the parent heterocycles and confirmed their potential utility as building blocks for lead optimization programs.

Abstract

Hitherto unknown cis- and trans-1-amino-3-fluoro-3-methylcyclobutanecarboxylic acids were synthesized as pure diastereomers starting from diisopropyl 3-oxocyclobutane-1,1-dicarboxylate. While pKa values of the carboxylic acid functions are the same for both stereoisomers (pKa = 2.80), the values for the amino groups are slightly different (8.46 or 8.77, respectively) presumably because of different interactions with the fluorine atoms.

Abstract

The synthesis of a set of conformationally restricted fluorinated analogues of piperidine, based on a 2-azaspiro[3.3]heptane scaffold, is reported. Different pattern of fluorine substitution within the rigid skeleton make the analogues excellent candidates for use in drug design. The overall simplicity of the experimental procedures and the availability of inexpensive starting materials allow for multigram-scale syntheses of the described compounds.

Abstract

New non-flattened amino group-containing building blocks and fluorinated analogs based on the spiro[3.3]heptane motif were synthesized. The syntheses included a challenging deoxofluorination of sterically hindered carbonyl groups via an intermediate carbocation. The synthesized compounds could be useful in medicinal chemistry due to their three-dimensional shape, and a different pattern of the fluorine substitution.

Abstract

Prior studies suggested sperm with damaged DNA permits fertilization but may lead to failure of embryo implantation following blastocyst formation. Quantitative correlations between DNA damage and risk of implantation failure have, however, so far not been performed. The aim of this study was to investigate two FSHR gene polymorphisms G919A (Ala307Thr) and A2039G (Asn680Ser) in Eastern Ukrainian Caucasian men with abnormally low fertility. The molecular genetic analysis was performed by real-time PCR, with the level of DNA fragmentation measured by the sperm chromatin dispersion (SCD) method. The relationship between DNA fragmentation in sperm and these genetic polymorphisms was estimated. Compared to homozygotes, the risk of high-level DNA fragmentation (>20%) increased in men up to age 35 years 16-fold for heterozygotes GA of polymorphic variant G919A, 28-fold for homozygotes AA of polymorphic variant G919A; and 16-fold for heterozygotes GG of polymorphic variant A2039G. A statistically significant positive correlation between number of alternative alleles of the FSHR gene in genotype and degree of DNA fragmentation is proved (rs = 0.70, P < 0.01).

Abstract

Introduction Embryo quality has always been considered an important predictor of successful implantation and pregnancy. However, knowledge of the relative impact of each morphological parameter needs to be increased, and this evaluation method has some limitations. Usually the morphologically high quality blastocysts are chosen for embryo transfer in IVF treatment. But the results of different authors about the correlation of embryo morphology with its aneuploidy are ambiguous. The aim of our study was to evaluate possible relationships among blastocyst ploidy status and standard morphology in PGS with next generation sequencing (NGS) in IVF cycles. Materials and Methods In this study, the morphology of 183 blastocysts, obtained in 53 PGS cycles performed in 45 infertile couples from April 2016 to December 2017, with trophectoderm (TE) biopsy and NGS, were retrospectively analyzed. Totally 183 blastocysts with different morphological quality were biopsied on the 5th and 6th days of culture and outsourced to commercial reference laboratory for next generation sequencing (NGS) test. Correlation of blastocyst morphology and embryo aneuploidy was examined with Spearman coefficient. Results In the mentioned study60.1% of examined blastocysts were aneuploid or mosaic (110 blastocysts). We evaluated the possible relationship between the morphology of blastocysts and PGS results taking into account the quality of trophectoderm and intracellular mass (ICM). There was no correlation between the quality of ICM and blastocyst ploidy (rs = 0.329, P > 0.05). But the decrease in quality of TE significantly correlated with the aneuploidy in blastocysts (rS = 0.458, P Conclusion Existing data regarding the relationship between embryo morphology and its ploidy are conflicting.Results of the present study point to a significant relationship of the trophectoderm morphology with the chromosomal abnormalities in preimplantation embryos. As no correlation between ICM morphology and blastocyst karyotype has not been found out the development of new approaches is relevant to choose the best embryo for the next PGS examination.

25. FORMATION OF EXCESSIVE AMOUNT OF ENDOGENOUS CARBON MONOXIDE AND INCREASE OF CARBOXYLATED HEMOGLOBIN CONTENT IN PATIENTS WITH POLYTRAUMA.

Abstract

There are serious conditions for increasing the production of endogenous carbon monoxide (CO) in patients with polytrauma. Any damage to the cell membranes under conditions of traumatic injury, shock, acute respiratory failure, destruction of the blood cells against the background of hemorrhages, blood transfusions, and blood reinfusions can contribute to the enhancement of products. The purpose of the study was the investigation of endogenous CO production in the body of patients with polytrauma by determining the percentage of carboxylated hemoglobin (HbCO%) in the blood using photoplethysmometric technology, as well as determining the degree of danger of the specified products to the body of patients. We have investigated the percentage of HbCO in the blood of 137 patients with polytrauma. 66 patients, upon admission to the clinic, showed prevailing symptoms of traumatic hypovolemic. 36 patients manifested the syndrome of acute respiratory failure on the background of a chest injury with a lung contusion. Another 35 patients had no manifestations of acute insufficiency of the systemic circulation and acute respiratory failure. The results of their survey were used for comparison. The HbCO% content was determined by a photoplethysmometric method using a Masimo Rainbow Rad-57 apparatus (USA) along with indicators of heart rate, capillary blood oxygen saturation (SpO2%) and volume peripheral capillary perfusion (perfusion index). In patients admitted to the hospital in a state of shock, all of these indicators were monitored for 24 hours from the start of urgent surgery. To determine the relationship between the severity of hemolysis and the intensity of production of endogenous CO in patients with polytrauma, 40 patients were examined for the concentration of free hemoglobin in the blood plasma using the benzidine method. The blood carboxylated hemoglobin content was found to be the greater according to the severity of shock, by definition, in accordance with the magnitude of the shock index. The maximum HbCO% blood content in patients who were in a state of shock was ascertained during reperfusion, an hour after the start of an urgent surgical intervention. In patients with thoracic injury, the content of carboxylated hemoglobin was the greater, the more severe were the manifestations of acute respiratory failure syndrome, which was detected according to the severity of arterial hypoxemia. The severity of hemolysis turned out to be an independent risk factor for increased production of endogenous CO in patients with polytrauma. The excessive production of endogenous CO in patients with polytrauma was stated to stand for an additional threat to the viability of the body due to the formation of carboxylated hemoglobin and increased hypoxia. A probable mechanism is the activation of free radical oxidation processes with the damage of cell membranes.
