Myopericarditis is an illness with normally favorable prognosis and mostly seen in young adults. It is known that myopericarditis can be recurrent. However, it cannot be clarified why it is recurrent. We are presenting two cases including a 16-year-old and a 17-year-old patients gone through myopericarditis for 3 times. We did a detailed research to introduce etiological reasons in two patients. We studied infectious reasons, autoimmune diseases, immunological and rheumatic diseases including mainly viral pathogens. During the myopericarditis attacks of patients, no pathological finding was identified other than being monocytosis in full blood counts, moderate sedimentation, CRP increase and raised troponin in laboratory data. We determined all viral pathogens known as agent as negative in both patients of ours. We think that patients particularly with more than one recurrent myopericarditis should be evaluated with a different perspective.

Atriyal Septal Defekt (ASD) tüm konjenital kalp hastalıklarının içinde %5-10 oranında izole bir anomali olarak görülür. ASD 1000 canlı doğumda yaklaşık 2 prevalansı olan yaygın bir konjenital bozukluktur. Günümüzde birçok kronik hastalık izleminde yaşam kalitesinin önemi üzerinde durulmaktadır. Çocuğun hastalığı söz konusu olduğunda, hastalığın tanı ve tedavisi, hastalığın sonlanması durumlarında anne ve babanın tutum ve davranışları ile, bu dönemde üstlendikleri görevler arasındaki farklar da önemli bir ayrıntıdır. Ancak, kalp hastalığı olan çocuklarda, hastanın kendi algılarını ya da çocuğun psikolojik ve sosyal işlevlerini algılayan ebeveyn algılarını, çocukların sağlıkla ilişkili yaşam kalitesini değerlendiren çalışmalar azdır. Konjenital kalp hastalığı tanısı alan çocuklar ve ailelerinde yaşam kalitesinde azalma olduğu belirtilmektedir. Vaktinde ameliyat edilen izole ASD'lerde, beklenti ise; normal bir yaşam süresi ve kalitesidir. Gazi Üniversitesi Tıp Fakültesi Hastanesinde pediatrik hastalarda hem transkateter hem de cerrahi olarak ASD onarımları yıllardır başarılı şekilde yapılmaktadır. Çalışmaya, Gazi Üniversitesi Tıp Fakültesi çocuk kardiyolojisi bölümünde takipli yaşları 8-18 arasında değişen ek kronik hastalığı olmayan 15 cerrahi ASD kapatma, 15 transkateter ASD kapatma uygulanan ve genel pediatri polikliniğine başvuran herhangi bir ek kronik ya da kalp hastalığı öyküsü olmayan 30 sağlıklı çocuk olmak üzere toplam 60 çocuk ve ebeveynlerinden biri dahildi. Çalışmamızda çocukluk çağı takipleri sırasında transkateter veyacerrahi ASD kapatma uygulanan ve yaşıtlarına benzer günlük aktivitelere devam edenhastalar ile kardiyak ve kronik hastalığı olmayan kontrol hastalarında yaşam kalitesi veebeveyn tutumlarını değerlendirdik aralarındaki ilişkiyi saptadık, kendi aralarında ve sağlıklı grupla kıyaslama yaptık. Ayrıca kapatma uygulanan hasta gruplarının kapatılma süreçleri ile ilgili tecrübelerini içerik analizini kullanarak inceledik. Çalışmanın sonuçlarındaysa transkateter ve cerrahi ASD kapatma uygulanan hasta gruplarının yaşam kalitesi ve ebeveynlerin çocuk yetiştirme tutumu tüm alt boyutlarda sağlıklı grup ve birbirleri ile benzer olup istatistiksel olarak anlamlı bir fark saptanmamıştır. Cerrahi kapatma uygulanan özellikle kız hastalarda mevcut olan skar dokusunun rahatsızlık oluşturduğu ve ebeveyn tutumlarının karşı cinse göre farklılık gösterdiği tespit edilmiştir. Başarılı bir şekilde cerrahi kapatma uygulanan günümüz koşullarında hastalara ve ebeveynlerine uygulanan cerrahi insizyon ile ilgili daha çok bilgi verilip onlara seçme hakkı sunulmasına ihtiyaç vardır. Transkateter veya cerrahi kapatma uygulanan hastaların kapatılma süreçleri ile ilgili anıları sorgulandığında sosyal destek almayan hastalarda korku ve kaygının ön planda olduğu olumsuz anı kodlamalarının mevcut olduğu tespit edilmiştir. ASD kapatma uygulanan çocukların yaşam kalitelerinin, ebeveyn tutumlarının, onların kapatılma süreci ile ilgili beklentilerinin ve anılarının değerlendirilmesi, beklentilerinin tanımlanması ve onların sağlık ihtiyaçlarına odaklanan müdahale stratejilerinin detaylandırılması gibi değerli bilgiler edinmemize, bu değerlendirmeler ile sosyal ve psikolojik destek için ailelerle işbirliği yapılması ve beklentiler doğrultusunda bireye uygun girişimlerin seçilmesi gibi amaçlara yol gösterici olacaktır. Bu nedenle daha fazla araştırma yapılması gerekmektedir. Atrial Septal Defect (ASD) is seen as an isolated anomaly in all congenital heart diseases by 5-10%. ASD is a common congenital disorder with a prevalence of about 2 per 1000 live births. Nowadays, the importance of quality of life is emphasizedin many chronic disease monitoring. In the case of the child's disease, the differences between the attitudes and behaviors of the parents and the tasks they undertake in this period are also an important detail in the process the diagnosis and treatment of the child's disease and the end of the disease. However, in children with heart disease, studies assessing the patient's own perceptions or parents' perceptions of the child's psychological and social functions and children's health-related quality of life are rare. It has been reported that there is a decrease in quality of life in children with congenital heart disease and their families. In isolated ASDs operated on time, the expectation; is a normal life process and quality. In the pediatric patients of Gazi University Faculty of Medicine, both transcatheter and surgical ASD repairs have been successfully performed for many years.The study is included a total of 60 children and anyone of their parents, 15 surgical ASD closure, 15 transcatheter ASD closure children aged 8-18 years with no additional chronic disease followed at Gazi University Faculty of Medicine, Department of Pediatric Cardiology and 30 healthy children without any additional history of chronic or heart disease admitted to the general pediatric outpatient clinic. In our study, we evaluated quality of life and parental attitudes of the patients who underwent transcatheter or surgical ASD closure during childhood and who continued their daily activities similarto their peers and control patients with no cardiac or chronic disease, we found the relationship between them and we made comparisons amongst themselves and the healthy group. We also examined the experience of closure processes of patient groups using content analysis. In the results of the study, the quality of life of the patients who underwent transcatheter and surgical ASD closure and their parental attitudes were similar in all subscales with a healthy group and no statistically significant difference was found. It has been determined that scar tissue present in girls with surgical closure is discomfortable and parental attitudes differ according to opposite sex. In today's conditions, where surgical closure is successfully performed, more information about the surgical incision applied to patients and their parents should be given and they should be given the right to choose. When the memories of patients undergoing transcatheter or surgical closure were questioned, it was determined that in patients without social support, negative moment encodings fear and anxiety in the foreground has been found to be present. It will be guiding forus to get valuable information like evaluation of quality of life, parental attitudes, of the expectations of their closure process and memories of children with ASD closure application, definition of their expectations and elaboration of intervention strategies focusing on their health needs; for aims like cooperation with families for social and psychological support and selection of appropriate initiatives in accordance with expectations with these assessments. Therefore, further research is required.

Short QT sydrome (SQT) is one of the rarely seen arythmia causes related to sudden cardiac death. An important part of the patients are diagnosed during the examination due to cardiac arrest. Therefore, diagnosing asymptomatic patients is lifesaving. In this case report, we presented a five-year old asymptomatic female patient how to put the SQT diagnosis. There was no complaint of the patient who had been directed to pediatric cardiology polyclinic due to murmur. In her electrocardiography (EKG), QTc duration was determined in lower limits. However, when the patient's family history was examined in a detailed way, suspicious deaths were recognized. In the patient's 24-hour holter EKG; SQT was diagnosed by establishing average QTc duration as 340 ms together with sharp, narrow T waves. Satolol treatment was initiated to the patient. To diagnose asymptomatic SQT patients, a decent family history must be obtained. Spot EKG is insufficient to rule out the diagnosis. All patients with suspicious death histories in their familieis must be directed to pediatric cardiologists.

WOS: 000329790100002 PubMed: 25536721 Background/aim: The purpose of this study is to find the frequency of contrast-induced nephropathy (CIN) and to show the risk factors in the development of CIN and the diagnostic utility of serum cystatin C (CysC) and serum and urine neutrophil gelatinase-associated lipocalin (NGAL) during childhood following cardiac angiography. Materials and methods: In this prospective study, we studied 46 children with congenital heart disease. The levels of serum creatinine, serum CysC, and serum NGAL were measured at 4, 24, and 48 h, while levels of urine NGAL and urine creatinine were measured at 4 to 8 and 48 h following cardiac angiography. Results: According to serum creatinine levels, with a cutoff value of 4.1 mL/kg for development of CIN, sensitivity, specificity, area under the receiver-operating characteristic curve, and positive likelihood ratio were calculated as 69%, 70%, 0.67, and 2.29, respectively. The levels of serum CysC and serum creatinine significantly increased at 4, 24, and 48 h after the application of the contrast agent. Conclusion: The results of this study show that according to the definition of CIN, the incidence of CIN is significantly increased in pediatric patients with congenital heart disease. Moreover, the results support that serum CysC levels may allow the detection of CIN after cardiac angiography, like serum creatinine in present study.

Amaç: Çalışmada antrasiklin kullanılarak tedavi edilmiş pediatrik kanser hastalarının uzun dönem takiplerinde görülebilen subklinik kardiyotoksisite ile fonksiyonel egzersiz kapasitesi arasındaki ilişki incelenmiştir.Gereç ve Yöntemler: Kemoterapi tedavisi 9-16 yıl önce tamamlanmış ve kalp yetmezliği bulguları olmayan 16 hasta ile 16 sağlıklı kontrol çalışmaya alındı. Ejeksiyon fraksiyonu, fraksiyonel kısalma ve miyokardiyal performans indeksi ölçümleri ekokardiyografik olarak alındı. Subklinik kardiyotoksisite açısından miyokardiyal performans indeks sınırı 0.37 olarak kabul edildi. Fonksiyonel egzersiz kapasitesi 6-dakika yürüme testi ile değerlendirildi.Bulgular: Hastaların miyokardiyal performans indeks değerleri belirgin olarak kontrol grubundan daha yüksek saptandı (p, Objective: We aimed to evaluate the functional exercise capacity and its relation to echocardiographic measurements in pediatric cancer survivors who were treated with anthracyclines.Material and Methods: Sixteen patients who completed chemotherapy 9-16 years ago and who have no symptoms of heart failure were enrolled in the study, together with 16 healthy subjects. Ejection fraction, fractional shortening, and myocardial performance index were calculated by echocardiographic evaluation. Patients with a myocardial performance index > 0.37 were accepted as having subclinical cardiotoxicity. Functional exercise capacity was evaluated by the sixminute walk test. results: Patients had significantly higher values of myocardial performance index than control subjects (p < 0.001). The six-minute walking distance was found to be lower in patients than that of the control subjects (p= 0.02). Furthermore, in patients with subclinical cardiotoxicity, the difference was more significant (p= 0.001). Six minute walking distance and myocardial performance index were well correlated with each other (r= -0.6 p= 0.03) and with cumulative anthracycline dosage (r= -0.6 p= 0.04 and r= 0.6 p= 0.02 respectively).conclusion: Long-term pediatric cancer survivors may have subclinicalcardiotoxicity despite having no clinical symptoms. Patients with subclinical cardiotoxicity should be taken into account for having impaired functional exercise capacity and correspondingly low quality of life. Six-minute walk test could be a useful indicator for subclinical cardiotoxicity since it is well correlated with myocardial performance index

Kalp kateterizasyonu ve anjiyografinin kardiyovasküler sistem hastalıklarının tanı ve tedavisindeki rolü son 20 yılda çok önemli değişikliklere uğramıştır. Gerek konjenital gerekse edinsel kalp hastalıklarında tanısal ve tedavi amaçlı kalp kateterizasyonu uygulanmaktadır. Çalışmamızda Gazi Üniversitesi Tıp Fakültesi Hastanesi Çocuk Kardiyolojisi Kliniği'nde Ocak 2004-Eylül 2014 tarihleri arasında tanısal ve girişimsel amaçla yapılan kalp kateterizasyonu ve anjiyografi işlemleri retrospektif olarak değerlendirilmiştir. Ocak 2004- Eylül 2014 tarihleri arasında 1007 olguya, 1002'si tanısal, 214'ü girişimsel olmak üzere toplam 1216 kalp kateterizasyonu ve anjiyografi işlemi uygulandığı belirlendi. Tanısal kalp kateterizasyonu işlemi uygulanan olgularda konulan tanıların %72 ile asiyanotik kalp hastalığı, %16,4 ile siyanotik kalp hastalığı, %2,8 ile kardiyomyopati olduğu belirlendi. Girişimsel kalp kateterizasyon işlemlerinin %29,4'ünü PDA'nın coil ile embolizasyonu, %20'sini endomiyokardiyal biyopsi, %17,7'sini ASD kapatılması, %12,1'ini pulmoner balon valvüloplasti, %5,1'ini koarktasyon balon anjiyoplasti, %3,7'sini atriyal balon septostomi, %3,2'sini aortik balon valvüloplasti, %3,2'sini pace yerleştirilmesi, %2,3'ünü PDA'nın ADO ile embolizasyonu, %0,9'unu koarktasyonun stent ile giderilmesi, %0,9'unu koroner AV fistül embolizasyonu, %0,4'ünü duktusa stent yerleştirilmesi, %0,4'ünü aortopulmoner kollateral anjiyoplasti işlemlerinin oluşturduğu belirlendi. İşlemlerin %12,4'ünda komplikasyon geliştiği saptanmış olup bu komplikasyonların %2,6'sinin major, %9,8'unun minör komplikasyonlar olduğu; en sık görülen minör komplikasyonun dolaşım bozukluğu (%33,7), en sık görülen major komplikasyonun ise disritmi (%17,7) olduğu belirlendi. Kalp kateterizasyonu her büyüklükteki bebek ve çocukta güvenle uygulanabilen bir yöntemdir. Ancak teknik ve ekipmanlardaki ilerlemeye rağmen kateterizasyon işlemlerinde komplikasyon görülmesi olasıdır. İşlem sırasında uygulanacak kontrast madde ve radyasyon miktarlarının olabilecek en az miktarda tutulmasına dikkat edilmelidir. Ülkemizde bu konuda yapılacak çalışmalara ihtiyaç vardır 10 Years' Retrospective Evaluations of Pediatric Patients Who Were Made Cardiac Catheterization In twenty years, the application of pediatric cardiac catheterization have changed dramatically. Pediatric cardiac catheterization laboratories are still being used for both diagnostic and therapeutic interventional procedures. In this presented study, 1216 pediatric cardiac catheterization procedures which were performed between January 1th 2004- September 1th, 2014 were evaluated. 1002 diagnostic, 214 therapeutic cardiac catheterization and angiocardiography procedure were applied to 1007 cases. It was determined that 70.3% part of the procedures were applied to the cases with a diagnoses of acyanotic and 14.8%part of the procedures were applied to the cases with a diagnoses of cyanotic heart disease respectively. Transcatheter percutaneous therapeutic procedures were consisted of closing PDA with coil or ADO in 31.7%, endomyocardial biopsy in 20%, closing ASD in 17.7%, pulmoner balloon valvuloplasty in 12.1%, coarctation balloon angioplasty in 5.1%, atrial balloon septostomy in 3.7%, aortic balloon valvuloplasty in 3.2%, pacemaker implantation in 3.2%, coarctation stent implantation in 0.9%, embolization of AV fistula in 0.9%, patent ductus arteriosus stenting in 0.4%and aortopulmonary collateral angioplasty in 0.4% of all procedures. Complications were detected in 12.4% of all procedures (2.6% major, 9.8% minor complication). The most detected minor complication was transient vascular disturbance which was found as 33.7% of all complications and the most detected major complication was arrhythmia which was found as 17.7% of all complications. The progresses in the technology, equipment and materials that can be used in children have become smaller in recent years. Despite all developments in cardiac catheterization procedure, it still has serious complication risks. A basic understanding of radiation doses, contrast agents, the risks of their administration, the choice of the available agents and premedication regimens for high-risks patients beneficial in preparing patients for their contrast enhanced imaging examinations. Physicians should be able to recognize and threat the various types of possible adverse reactions and should use as less contrast agents and radiation as they can.

To describe the epidemiology and geographical distribution of congenital heart disease. A retrospective population-based study was conducted using data collected in a large tertiary care hospital in Turkey. Materials and methods: The medical records of 1300 patients with congenital heart disease admitted to the Gazi University Department of Pediatric Cardiology for catheter angiography from 1997-2007 were reviewed. The patients were divided into 7 groups according to geographical distribution, and each group was further divided into 2 groups, for simple (group 1) and complex (group 2) congenital heart disease. Results: Among the 1300 cases, there was no difference between the regions regarding mean age, mean diagnostic age, and female/male (F/M) ratio. In 3 regions that have similar geographic and climatic features, a high incidence of complex cardiac malformations was observed (P < 0.05). Conclusion: This is the first epidemiological study of the distribution of congenital heart disease in Turkey. Although etiological data is not sufficient to explain the findings, this study may play an important role in further prospective studies about this issue.

WOS: 000298589000046 PubMed: 21810152 Background: The aim of this study was to assess the prevalence of persistent left superior vena cava (PLSVC) in patients with all types of congenital heart defects and to determine the congenital heart anomalies accompanying PLSVC. Methods: The present study is based on a retrospective review of 1205 children who consecutively underwent cardiac catheterization from 2000 to 2007. In order to determine the existence of PLSVC, all the subjects routinely underwent superior vena cava (SVC) injection during angiography at the catheter laboratory of the study center. Results: The prevalence of PLSVC was computed to be 6.1% for the present study population. Transthoracic echocardiography was able to detect PLSVC in 32 children (2.6%) whereas angiography diagnosed PLSVC in 74 children (6.1%). The mean age of the patients with PLSVC was 40.09 +/- 50.21 months. A communication between the right and left SVC was determined in 27% of the children who were diagnosed with PLSVC after angiography was performed (20 out of 74). A statistically significant association was present between PLSVC and other congenital cardiac anomalies, including ventricular septal defect (n = 42, 56.8%), atrial septal defect (n = 31, 41.9%), pulmonary stenosis (n = 19, 25.7%), atrioventricular septal defect (n = 10, 13.5%), patent ductus arteriosus (n = 6, 8.1%) and cor triatriatum (n = 3, 4.1%). Conclusion: Transthoracic echocardiography usually visualizes dilated coronary sinus in association with PLSVC. However, SVC injection should be performed in patients undergoing angiography so that morbidity and mortality related with persistent left superior vena cava can be avoided during cardiovascular surgery.

Endotel disfonksiyonu ve vasküler inflamasyon pulmoner hipertansiyon patogenezinde önemli rol almaktadır. Inter sellüler adezyon molekülü 1’in ekspresyonunun dolayısıyla serum sICAM-1 düzeyinin artması bir çok endotel disfonksiyonuyla giden hastalıkla ilişkili bulunmuştur. Bu çalışmada doğuştan kalp hastalığına ikincil gelişen PAH’da biyobelirteç olarak serum ICAM-1’in kullanılabilirliğine aynı zamanda kateter anjiyografi ve ekokardiyografi bulgularıyla ICAM-1’in ilişkisine bakılmıştır. Çalışmaya, PAH tanısı alan 30, soldan sağa şantı olup PAH gelişmemiş 20 DKH ile 20 sağlıklı kontrol grubundan oluşan toplam 70 olgu dahil edildi. Olguların detaylı öykü ve fizik incelemeleri, antropometrik ölçümleri, tele, EKG, EKO, kateter anjio ve tam kan bulguları daha önceden hazırlanmış olan formlara kaydedildi. Tüm olgulardan alınan kan örneklerinde ICAM-1 düzeyleri ELISA yöntemiyle çalışıldı. Bu çalısmaya non-iyonik düsük osmolariteli KM olan iyopromid (769 mg/ml) kullanılarak tanısal amaçlı kateter anjiyografi yapılan yasları 2 ay ile 16 yas arasında değisen (9 siyanotik 37 asiyanotik) 46 hasta dahil edilmistir. Hastalardan islem öncesi ve islem sonrası 4, 4-8, 24 ve 48. saatlerde serum ve idrar örnekleri alınmıstır. Bu çalısmada; KMN oranı, risk faktörleri ve serum Sistatin C , serum NGAL, idrar NGAL düzeylerinin erken tanıdaki rolü saptanmaya çalısılmıstır. KMN; kateter anjiyografi islemi öncesi alınan serum Cr düzeyinde ilk 48 saat içinde 0,5mg/dl ’den fazla veya bazal değere göre % 25 ve üzerinde artıs olarak tanımlandığında, KMN gelisme oranı 16/46 (% 34,7) olarak saptanmıstır. Hastalar KMN gelismeyenler (Grup 2) ve KMN gelisenler (Grup 3) olmak üzere 2 gruba ayrıldığında; KMN gelisen grupta KMV(cc/kg) düzeyinin daha yüksek olduğu görülmüstür (5,0±2,3cc/kg ‘e 3,7±2,0 cc/kg, p:0,051). KMV(cc/kg) için kesim değeri 4,09 cc/kg kabul edildiğinde sensitivite, spesifite, negatif ve pozitif tahmini değerler sırasıyla; % 68,7, % 70, % 50,6 ve % 50,1 olarak saptanmıstır.
