8. Prediction of visceral adipose tissue magnitude using a new model based on simple clinical measurements.

Aims: Waist circumference (WC) is a reliable obesity surrogate but may not distinguish between visceral and subcutaneous adipose tissue. Our aim was to develop a novel sex-specific model to estimate the magnitude of visceral adipose tissue measured by computed tomography (CT-VAT). Methods: The model was initially formulated through the integration of anthropometric measurements, laboratory data, and CT-VAT within a study group (n=185), utilizing the Multivariate Adaptive Regression Splines (MARS) methodology. Subsequently, its correlation with CT-VAT was examined in an external validation group (n=50). The accuracy of the new model in estimating increased CT-VAT (>130 cm2) was compared with WC, body mass index (BMI), waist-hip ratio (WHR), visceral adiposity index (VAI), a body shape index (ABSI), lipid accumulation product (LAP), body roundness index (BRI), and metabolic score for visceral fat (METS-VF) in the study group. Additionally, the new model's accuracy in identifying metabolic syndrome was evaluated in our Metabolic Healthiness Discovery Cohort (n=430). Results: The new model comprised WC, gender, BMI, and hip circumference, providing the highest predictive accuracy in estimating increased CT-VAT in men (AUC of 0.96 ± 0.02), outperforming other indices. In women, the AUC was 0.94 ± 0.03, which was significantly higher than that of VAI, WHR, and ABSI but similar to WC, BMI, LAP, BRI, and METS-VF. It's demonstrated high ability for identifying metabolic syndrome with an AUC of 0.76 ± 0.03 (p<0.001). Conclusion: The new model is a valuable indicator of CT-VAT, especially in men, and it exhibits a strong predictive capability for identifying metabolic syndrome. [ABSTRACT FROM AUTHOR]

Metabolically healthy obesity (MHO) refers to obese individuals with a favorable metabolic profile, without severe metabolic abnormalities. This study aimed to investigate the potential of follistatin, a regulator of metabolic balance, as a biomarker to distinguish between metabolically healthy and unhealthy obesity. This cross-sectional study included 30 metabolically healthy and 32 metabolically unhealthy individuals with obesity. Blood samples were collected to measure the follistatin levels using an enzyme-linked immunosorbent assay (ELISA). While follistatin did not significantly differentiate between metabolically healthy (median 41.84 [IQR, 37.68 to 80.09]) and unhealthy (median 42.44 [IQR, 39.54 to 82.55]) individuals with obesity (p = 0.642), other biochemical markers, such as HDL cholesterol, triglycerides, C-peptide, and AST, showed significant differences between the two groups. Insulin was the most significant predictor of follistatin levels, with a coefficient of 0.903, followed by C-peptide, which exerted a negative influence at −0.624. Quantile regression analysis revealed nuanced associations between the follistatin levels and metabolic parameters in different quantiles. Although follistatin may not serve as a biomarker for identifying MHO and metabolically unhealthy obesity, understanding the underlying mechanisms that contribute to metabolic dysfunction could provide personalized strategies for managing obesity and preventing associated complications. Objective: The aim of the study was to evaluate the effectiveness and safety of insulin glargine 300 U/mL (Gla-300) in insulin-naive patients with type 2 diabetes mellitus (T2DM) inadequately controlled on oral antidiabetic drug (OADs) treatment in Turkey. Methods: One hundred eight patients from 20 centers enrolled in the study. Starting from baseline, Gla-300 was self-administered subcutaneously and once daily in the evening. The primary outcome was the mean change in glycated hemoglobin A1c (HbA1c) from baseline to week 24. Results: The mean (±SD) Hb1Ac level of 9.4% (±0.8) at baseline decreased to 7.5% (±0.9) at week 12 (P < .1) and to 7.3% (±0.9) at week 24 (P < .1). Although none of the patients were within the target Hb1Ac level of ≤7% at baseline, the percentage of patients who achieved the target Hb1Ac level was 30.4% at week 12 and increased to 42.9% at week 24. Gla-300 treatment achieved the Hb1Ac target in 21 (19.4%) patients without experiencing a hypoglycemic event and in 27 (25.0%) patients who experienced at least one hypoglycemic event. For each self-monitoring blood glucose time point, significant improvements were observed as compared to baseline (P < .001). Statistically significant improvement (P < .001) was seen in the treatment satisfaction questionnaire - status version scores between baseline and week 24. Conclusion: This study indicated that Gla-300 is effective to provide a successful glycemic control with low risk of hypoglycemia added to OADs in insulin-naive patients with T2DM, and it has the potential to improve the quality of life of patients. Introduction: Diabetic nephropathy is one of the major complications of diabetes mellitus. It is the most common cause of end-stage renal disease. Neutrophil gelatinase-associated lipocalin (NGAL) is a biomarker that increases before serum creatinine in early acute kidney injury. We examined in this study the hypothesis that urinary NGAL levels may be elevated as an indicator of early diabetic nephropathy in patients with type 2 diabetes mellitus. Methods: This study included a total of 78 patients (51 females and 27 males, mean age 57.74±8.97 years). The patient group consisted of 38 patients (24 females and 14 males, mean age 58.68 ± 8.36 years) and the control group consisted of 40 healthy subjects (27 females and 13 males, mean age 56.85 ± 9.53 years). The groups were compared in terms of anthropometric, demographic, biochemical data, and urinary NGAL levels. Urinary NGAL levels were measured using enzyme-linked immunosorbent assay. Results: No significant difference was found in urinary NGAL levels between the patient and control groups (7.53±8.49, 9.04±9.98, p>0.05, respectively). Albumin/creatinine ratios were significantly higher in the patient group than in the control group (0.14±0.09, 0.08±0.02, p<0.05, respectively). No significant correlation was found between urinary NGAL levels and disease duration, body mass index, glomerular filtration rate, waist circumference, albumin/creatinine ratio, and HbA1c (p>0.05 for all). Discussion and Conclusion: The results of our study show that urinary NGAL levels do not have sufficient predictive value to identify early diabetic nephropathy. [ABSTRACT FROM AUTHOR]

28. Effect of Defensive Medicine on Clinical Practices of Emergency Medicine Physicians.

Objective: To determine the perspectives of emergency medicine physicians on defensive medicine practices under the influence of malpractice thought, the frequency of these practices, and their relationship with crowded emergency departments, and to reveal the factors affecting physicians in this practice. Materials and Methods: A questionnaire consisting of questions including positive and negative defensive medicine practices was prepared and delivered to emergency medicine physicians who were in a patient-physician relationship by hand or e-mail. Results Physicians answered the following questions: protecting themselves from possible medical errors, keeping more detailed records (87.4%), requesting additional examinations when not necessary (56.8%), keeping the patient under observation for long hours (47.2%), asking for additional consultations (32.7%), avoiding treatments with high complications (17.3%), referring patients (17%), trying to avoid invasive procedures (11%) and prescribing extra medications (10.6%). Conclusion: Emergency medicine physicians preferred positive defensive medicine practices over negative defensive medicine practices. It has been observed that the pressure of the relatives of the patients and the threat of lawsuits were the main factors influencing defensive medicine practices. [ABSTRACT FROM AUTHOR]

- Abstract
Background: The risk of cardiovascular disease is correlated with the frequency and control of associated risk factors in diabetes mellitus and may vary according to country. We evaluated risk factors for cardiovascular disease, cardiovascular events, and the use of preventive medications in patients with diabetes mellitus using the Prospective Urban and Rural Epidemiological Türkiye cohort. Methods: Patients with diabetes mellitus versus without diabetes mellitus were compared for risk factors, cardioprotective drugs (angi otens in-co nvert ing enzyme inhibitors or angiotensin-II receptor antagonists, statins, and antiplatelets), and cardiovascular events. The primary outcome was major cardiovascular events (composite of cardiovascular death, myocardial infarction, stroke, or heart failure). Results: Among 4041 participants, 549 (13.6%) had diabetes mellitus. The mean age (54.8 ± 8.4 vs. 49.3 ± 9.0 years, P < .001) and proportion of women (65.4% vs. 59.9%, P = .014) were higher in diabetics compared with non-diabetics. Hypertension, history of coronary heart disease, and use of statin, antiplatelets, and angiotensin-converting enzyme inhibitors or angiotensin-II receptor antagonists were more common in diabetics; however, the use of these medications at baseline was lower than optimal even in patients with diabetes mellitus and concomitant coronary heart disease (statin 31.2%, antiplatelets 46.9%, and angiotensin-converting enzyme inhibitors or angiotensin-II receptor antagonists 54.7%). During 11.5 years of follow-up, major cardiovascular events occurred in 288 (7.1%) patients, and the risk was higher in diabetics [hazard ratio (95% confidence interval) 1.71 (1.30-2.24); P < .001]. The increase in the risk of future events was comparable for those with diabetes mellitus alone without cardiovascular disease [hazard ratio 1.62 (1.20-2.20)] versus those with cardiovascular disease alone without diabetes mellitus [hazard ratio 1.31 (0.83-2.07)] and was additive in those with both conditions [hazard ratio 2.79 (1.65-4.69)]. The risk of major coronary events (myocardial infarction, angina, percutaneous, or surgical coronary intervention) was also higher in diabetes mellitus [hazard ratio 1.64 (1.26-2.15); P < .001]. Conclusion: Patients with diabetes mellitus have a higher risk of major cardiovascular events, and the risk is comparable to that observed in those with cardiovascular disease but no diabetes mellitus. Amaç: Diyabetik nöropati sıklıkla hastanın şikayeti olursa veya önlenebilecek komplikasyonlar gelişmişse tanı almaktadır. Tip 2 diyabetli (T2DM) hastalarda diyabetik periferik nöropati (DPN) varlığının bildirildiğinden daha sık olduğu hipotezinden yola çıkarak bu hasta grubunda DPN sıklığını ve ilişkili olduğu durumları araştırmayı planladık.Yöntem: Diyabet polikliniklerimize başvuran 336 tip 2 diyabetli hasta çalışmaya alındı. Anamnezleri, demografik ve antropometrik bilgileri, kullandıkları ilaçlar ve laboratuvar verileri kaydedildi. Diyabetik nöropatiyi saptamak amacıyla MNSI (Michigan Neuropathy Screening Instrument) skorlaması ve Semmes-Weinstein monofilaman testi uygulandı.Bulgular: Bu 336 hastanın (yaş ortalaması 57,9±10 yıl, %63,7’si kadın) ortalama diyabet süresi 9,5±7,8 yıldı. HbA1c, açlık kan şekeri ve vücut kütle indeksi ortalaması sırasıyla %8,7±2, 182±89 mg/dl ve 32±5,8 kg/m2 bulundu. DPN sıklığı %38,7 (n=130) saptandı. Daha önce nöropati tedavisi başlanmış olan 32 hasta (%9.5) ilaç kullanımları nedeniyle diğer analizlere katılmadı. Diyabetik nöropatisi olan grupla (n=107) olmayan grup (n=197) arasında kadın cinsiyet (%71 vs %59,9, p=0,054), yaş (60,6 yıl vs 55,9 yıl, p, Objective: Diabetic neuropathy is often diagnosed only if the patient complains or after the development of complications. We hypothesized that diabetic peripheral neuropathy (DPN) is more common than reported and aimed to investigate the incidence and related conditions of DPN in type 2 diabetic (T2DM) patients.Methods: Three hundred and thirty six patients with type 2 diabetes who presented to our diabetes outpatient clinics were recruited. Patients’ history, demographic and anthropometric characteristics, medications and laboratory data were recorded. MNSI (Michigan Neuropathy Screening Instrument) and Semmes-Weinstein monofilament test were applied to detect diabetic neuropathy.Results: In the 336 patients (mean age, 57.9±10 years; 63.7% female), the mean duration of diabetes was 9.5±7.8 years. The goal of interlaboratory comparisons is to verify the competence of accredited or non-accredited laboratories, including verification of the reported measurement uncertainties, whenever possible. An interlaboratory comparison consists of measurement results of a laboratory and a reference laboratory which is competent to run the comparison and provides traceability at the required uncertainty level. In Turkey, the demand for an interlaboratory comparison in the humidity field increased drastically in parallel to an increasing number of accredited laboratories in this field. Therefore, a national interlaboratory comparison in the scope of relative humidity in the range between 11 %rh and 95 %rh at $$23\, {}^{\circ }\hbox {C}$$ was initiated by TUBITAK UME in 2011. Two hygrometers were calibrated at TUBITAK UME in the above-mentioned range and characterized in terms of long-term stability, temperature dependence, and hysteresis. One of the hygrometers is used throughout this comparison. The second one was kept aside as a backup device in case of a malfunction of the first device. Organization and evaluation of the interlaboratory comparison in the scope of relative humidity were performed according to the EN ISO/IEC 17043:2010 “Conformity assessment—general requirements for proficiency testing” among thirteen national laboratories. Evaluation of comparison results are performed by normalized error analysis. It was found that among the 95 realized points of comparison, 85 (89 %) had $$E_{n} \le 1$$ and only 10 (11 %) had $$E >1$$ .

This work describes a porous, silicon-based humidity sensor operating under the capacitive transduction principle. One of the aims of this work is to determine the main parameters such as sensitivity, linearity, hysteresis, and time response to step humidity changes of four samples via measurements of their capacitance. The parameters gauged by capacitance measurements were used to explain the dynamics of its operation. The most sensitive sample caused changes in relative humidity of $$74\hbox { nF}{\cdot }\%\hbox {rh}^{-1}$$ . Hysteresis of at least 3.6 % to 4.6 % was found to occur. Humidity sensor samples synthesized from nanoporous silicon were also analyzed by scanning electron microscopy, image processing, and Raman spectra red shifts. As a result of these measurement and analysis of this work, the best synthesis conditions and nanopore surface and sub-surface diameters for producing high performing humidity sensors were identified. Another aim of this work is to find the optimal pore size from the analysis of image processing and Raman spectra. The optimal porous sizes in relation to the analyzed sensor’s characteristics were found to be between 4 nm and 26 nm. The novelty of this work is to establish the relationship between the capacitance measurements with image processing of SEM images and Raman spectral measurements. The mechanical stability of the samples was also gauged over 3 months utilizing both capacitance and Raman spectral measurements.
