A finite element model for the stress analysis of circular arches strengthened with composite materials is developed. The formulation uses the principle of virtual work, the Bernoulli-Euler curved beam theory for the arch and the composite reinforcement, and a high-order kinematic assumption that satisfies the compatibility and (with the constitutive laws) the tangential equilibrium conditions of the adhesive. The character of the masonry arch is introduced through the constitutive equations with a distinction between the masonry units and the mortar joints. Convergence and numerical studies that support using high-order shape functions and examine the capabilities of the model are presented.

A successful GnRH treatment for hypogonadotropic hypogonadism is described. A 40 year old infertile patient due to Kallmann's syndrome, associated with obesity and NIDDM, was treated with HCG (5,000 IU/week) for 8 years. In an attempt to induce spermatogenesis HMG (75 IU/daily) was added for 2 years. During the combined gonadotropin treatment semen analysis indicated an improvement from azoospermia to 2 x 10(6) sperm/ml. Since semen quality remained poor and obviously no pregnancy was achieved, the last regime was replaced by a pulsatile GnRH treatment (5 mcg/90 minutes). Following 6 months sperm count increased up to 15 x 10(6) associated with normal motility and morphology. Patient's wife conceived after the second intrauterine insemination and delivered a full term healthy baby.

The bending behavior of circular, fully or partially bonded (delaminated), sandwich plates with a “soft” core and composite laminated face sheets of general layup is investigated. The analytical model derived follows the principles of the High-Order Sandwich Plate Theory and it is based on equilibrium and compatibility requirements. The variational principle of virtual work is employed for the derivation of the field equations along with the boundary and continuity conditions. The delamination considered forms a penny shaped disbonded zone, in which the delaminated faces can slip horizontally with respect one to another, yet they may be in contact accommodating vertical normal compressive stresses. The core is assumed a 3D elastic medium and the composite face sheets are modeled using the classical lamination theory, which yields coordinate dependent constitutive relations. The governing equations take the form of partial differential equations with variable coefficients. The solution procedure adopts the Galerkin method with truncated Fourier series in the circumferential direction and the multiple-points shooting method in the radial direction. Numerical results concerning typical fully and partially bonded sandwich plates are presented and discussed. The results reveal the effect of the anisotropy of the face sheets on the response of the structure and the influence of the delaminated area on the behavior of the sandwich plate.

A closed-form high-order analytical approach for analysis of concrete beams strengthened with externally bonded fiber-reinforced polymer (FRP) stripes, on the tensile faces of the RC beam in positive and negative bending moment regions, is presented. The mode is based on equilibrium and compatibility requirements in and between all constituents of the strengthened beam, i.e. the concrete beam, the FRP stripe, the adhesive layers and the interfaces between the various parts. The governing equations of the mathematical model of the strengthened beam are derived and an analytically closed-form solution is determined. A numerical example of a typical continuous reinforced concrete beam strengthened with externally bonded FRP stripes is discussed with emphasis on the shear and transverse normal (peeling) stress concentrations at the adhesive-concrete and adhesive-FRP interfaces at the edges of the FRP stripes. Results concerning the edge stresses at three characteristic locations that have been determined using the closed-form high-order model are discussed. In the sequel a summary, conclusions, and recommendations for the design of the strengthened beam are presented.

Between January 1984 and August 1991, 511 cases of extrauterine pregnancies were diagnosed by laparoscopy in our department. In 374 cases salpingectomy was performed: 184 by explorative laparotomy, and 190 by operative laparoscopy. Patients were scheduled for salpingectomy if one or more of the following criteria were fulfilled: (i) a ruptured tube which was surgically unsuitable for conservation; (ii) no interest in future fertility; (iii) tubes with ectopic gestation previously operated on; (iv) a previous tubal pregnancy on the same side, which was treated expectantly. Salpingectomy was performed via operative laparoscopy with bipolar diathermy forceps and laparoscopic scissors. Pregnancy rates, i.e. intra-uterine and repeat extra-uterine, were evaluated. The reproductive performance following salpingectomy did not differ significantly, whether by laparotomy or laparoscopy: the intra-uterine pregnancy rate was 78 and 64%, respectively and the repeat ectopic pregnancy rate was 12 and 6%, respectively. Salpingectomy via laparoscopy can be performed safely with a low incidence of complications, with subsequent reproductive performance comparable to laparotomy.

To study the effect of pregnancy on kidney function in patients with familial Mediterranean fever (FMF) with amyloidosis.A retrospective analysis relating kidney function at term to kidney function at conception and to blood pressure and colchicine treatment before and during pregnancy in 17 patients with 29 pregnancies found among more than 3000 patient files in our FMF clinic.Following pregnancy, 7 patients (24% of pregnancies) experienced a decline in renal function. Urine proteinor = 2 g/24 h at conception was present in all pregnancies which sustained deterioration in contrast to 6 of 22 which did not (p0.001). Serum creatinineor = 1.5 mg/dl at conception was present in 3 patients, all of whom experienced deterioration of renal function during pregnancy (p0.01). Neither colchicine dose nor elevated blood pressure correlated with status of renal function at term.Our findings suggest a possible deleterious effect of pregnancy on amyloid nephropathy and that this effect may be associated with more advanced renal disease at conception.

A case of parovarian serous tumor of borderline malignancy inadvertently found during cesarean section in a 27-year-old multigravida is reported. Seventeen months after diagnosis there is no clinical evidence of disease. The histologic appearance of the tumor was identical with that of tumors of ovarian origin, however, whether its biologic behavior is similar as well is not known.

11. Reproductive outcome after microsurgical treatment of tubal pregnancy in women with a single fallopian tube

Whether restoration of tubal function following conservative surgery for ectopic pregnancy was achieved can only be estimated in patients with a single tube. Between 1978 and 1983, 22 women with a tubal pregnancy in their single tube underwent conservative microsurgical treatment. In 18 women dorsal salpingotomy was performed, in 3 the tube was gently milked (squeezing the products of conception through the fimbria), and in 1 the segment containing the products of conception was excised and the tube anastomosed. Twenty-one patients desired pregnancy. The mean follow-up period was 21.4 months. Of the patients desiring pregnancy, 76% conceived. The intrauterine pregnancy rate was 47.6%, and the repeat ectopic pregnancy rate was 42.8%. This ectopic pregnancy rate was significantly higher than those quoted in recent literature. Although the study group was a very-high-risk group, restoration of tubal function was achieved in a high proportion of cases. The intrauterine pregnancy rate was high and higher than the best results available with in vitro fertilization and embryo transfer, which is the only treatment available after bilateral salpingectomy. Therefore, conservative surgery is definitely indicated in such patients despite the high incidence of repeat ectopic pregnancy.

Prenatal real-time ultrasonographic diagnosis of exophthalmus is presented. Diagnosis was made at the 35th week of gestation in a fetus of a patient affected with Crouzon syndrome (craniofacial dysotosis). Recognition of exophthalmus as a part of Crouzon syndrome and the easy visualization of the eye balls and palpebrae in the third trimester made the diagnosis possible.

16. A learned artisan debates the system of the world: Le Clerc versus Mallemant de Messange.

- Abstract
Sébastien Le Clerc (1637-1714) was the most renowned engraver of Louis XIV's France. For the history of scientific publishing, however, Le Clerc represents a telling paradox. Even though he followed a traditional route based on classic artisanal training, he also published extensively on scientific topics such as cosmology and mathematics. While contemporary scholarship usually stresses the importance of artisanal writing as a direct expression of artisanal experience and know-how, Le Clerc's publications, and specifically the work on cosmology in his Système du monde (1706-1708), go far beyond this. By reconstructing the debate between Le Clerc and the professor Mallemant de Messange on the authorship of this 'system of the world', this article argues that Le Clerc's involvement in publishing ventures shaped his identity both as an artisan and as a scientific author. Whereas the Scientific Revolution supposedly heralded a change from the world of 'more or less' to the 'world of precision', this article shows how an artisan could be more 'precise' than the learned scholar whose claims he disputed, and points to the importance of the literary field as a useful lens for observing the careers of early modern scientific practitioners.

Israel is among the few countries in the world where sex reassignment surgery (SRS) is performed in public hospitals. Today, the only center performing SRS in Israel is the Sheba Medical Center. A multidisciplinary committee grants authorization permitting surgery after a follow-up period of two years. We found that this multidisciplinary approach leads to the best therapeutic results. We briefly present the pre-operative evaluation, operative approach, post-operative course, final results and complications of sex reassignment surgery. This article aims to inform physicians of this therapeutic option, and the indications for referral.

This study aimed to design formulations of hydrophobic herbicides, alachlor and metolachlor, by adsorbing them on the clay mineral montmorillonite preadsorbed by the small organic cation phenyltrimethylammonium (PTMA). An adsorption model that considers electrostatics and specific binding and the possibility of cation adsorption above the cation exchange capacity (CEC) could explain and yield predictions for PTMA adsorption in the presence of NaCl concentrations from 0 to 500 mM. Adsorption of alachlor and metolachlor from aqueous solution on a clay mineral preadsorbed by PTMA was determined by GC and modeled by Langmuir equation. Herbicide interactions with the organoclay were studied by Fourier transform infrared spectroscopy. Leaching of herbicides was determined by a bioassay using a column technique and Setaria viridis as a test plant. The adsorbed amounts of alachlor and metolachlor on montmorillonite preadsorbed by PTMA at a loading of 0. 5 mol/kg (Mont-PTMA0.5) were higher than at a loading up to the CEC, that is, 0.8 mol/kg, and were higher than those obtained by using several other organic cations. Herbicide formulations based on Mont-PTMA0.5 yielded the largest shifts of the infrared peaks of the herbicides. These formulations based on Mont-PTMA0.5 gave slower release and showed improved weed control in comparison with formulations based on other organoclays. These formulations maintained herbicidal activity in the topsoil and yielded the most significant reduction in herbicide leaching.

The effect of maternal use of colchicine on fetuses is unknown. The children of 116 women with Familial Mediterranean Fever (225 completed pregnancies) were studied. There was no unusual frequency of fetal abnormality among women taking colchicine before or during pregnancy. Colchicine treatment does not apparently harm mother or child.

- Abstract
The effect of pregnancy on kidney function was studied in 29 pregnancies of 17 patients with familial Mediterranean fever (FMF) and amyloidosis. Pregnancy associated deterioration of renal function occurred in seven patients who had advanced renal disease at conception, marked by serum creatinine > or = 1.5 mg/dl or urine protein > or = 2 g/24 h. This finding suggests that the severity of renal disease at conception may predict the fate of kidney function during pregnancy and puerperium.

23. Reproductive outcome after microsurgical treatment of tubal pregnancy in women with a single fallopian tube.

- Abstract
Whether restoration of tubal function following conservative surgery for ectopic pregnancy was achieved can only be estimated in patients with a single tube. Between 1978 and 1983, 22 women with a tubal pregnancy in their single tube underwent conservative microsurgical treatment. In 18 women dorsal salpingotomy was performed, in 3 the tube was gently milked (squeezing the products of conception through the fimbria), and in 1 the segment containing the products of conception was excised and the tube anastomosed. Twenty-one patients desired pregnancy. The mean follow-up period was 21.4 months. Of the patients desiring pregnancy, 76% conceived. The intrauterine pregnancy rate was 47.6%, and the repeat ectopic pregnancy rate was 42.8%. This ectopic pregnancy rate was significantly higher than those quoted in recent literature. Although the study group was a very-high-risk group, restoration of tubal function was achieved in a high proportion of cases. The intrauterine pregnancy rate was high and higher than the best results available with in vitro fertilization and embryo transfer, which is the only treatment available after bilateral salpingectomy. Therefore, conservative surgery is definitely indicated in such patients despite the high incidence of repeat ectopic pregnancy.

- Abstract
Prenatal real-time ultrasonographic diagnosis of exophthalmus is presented. Diagnosis was made at the 35th week of gestation in a fetus of a patient affected with Crouzon syndrome (craniofacial dysotosis). Recognition of exophthalmus as a part of Crouzon syndrome and the easy visualization of the eye balls and palpebrae in the third trimester made the diagnosis possible.
