Abstract

Purpose: To assess radiation therapy (RT)-induced vasculitis in patients with non-small cell lung cancer (NSCLC) by examining changes in the uptake of 18F-fluoro-D-deoxyglucose ([18F]FDG) by positron emission tomography/computed tomography (PET/CT) images of the ascending aorta (AA), descending aorta (DA), and aortic arch (AoA) before and after proton and photon RT. Method: Thirty-five consecutive locally advanced NSCLC patients were definitively treated with proton (n = 27) or photon (n = 8) RT and concurrent chemotherapy. The patients were prospectively enrolled to undergo [18F]FDG-PET/CT imaging before and 3 months after RT. An adaptive contrast-oriented thresholding algorithm was applied to generate mean standardized uptake values (SUVmean) for regions of interest (ROIs) 3 mm outside and 3 mm inside the outer perimeter of the AA, DA, and AoA. These ROIs were employed to exclusively select the aortic wall and remove the influence of blood pool activity. SUVmeans before and after RT were compared using two-tailed paired t-tests. Results: RT treatments were associated with increased SUVmeans in the AA, DA, and AoA—1.9%, 0.3%, and 1.3% for proton and 15.8%, 9.5%, and 15.5% for photon, respectively. There was a statistically significant difference in the ∆SUVmean (post-RT SUVmean − pre-RT SUVmean) in patients treated with photon RT when compared to ∆SUVmean in patients treated with proton RT in the AA (p = 0.043) and AoA (p = 0.015). There was an average increase in SUVmean that was related to dose for photon patients (across structures), but that was not seen for proton patients, although the increase was not statistically significant. Conclusion: Our results suggest that patients treated with photon RT for NSCLC may exhibit significantly more RT-induced inflammation (measured as ∆SUVmean) in the AA and AoA when compared to patients who received proton RT. Knowledge gained from further analyses in larger cohorts could aid in treatment planning and help prevent the significant morbidity and mortality associated with RT-induced vascular complications. 16. Magnetic characterisation of thin film NiFe/FeMn multilayers

Abstract

Exchange biased thin films form the basis of many high speed electronic devices such as hard disc drives, field sensors and microwave communication devices. It is important to have a detailed understanding of the electromagnetic properties of such thin films in order to determine their suitability for specific applications. As the operating speeds of electronic devices increase the necessary characterisation range expands into' the microwave region i.e. the gigahertz frequency range. One of the most important properties in investigating thin films for potential applications is permeability. The aim of this thesis was to characterise the magnetic properties and permeabilities of exchange biased thin films. Multilayers of the structure [Ni80Fe20 (x nm)/ Fe50Mn50 (y nm)]z were deposited using a sequential sputtering system where x is the thickness of the ferromagnetic layer, y is the thickness of the antiferromagnetic layer and z is the number of times the bilayer is repeated. The hysteresis loops of the multilayers were measured using a vibrating sample magnetometer to investigate their multiple reversal stages. A two port coplanar waveguide permeameter was built to measure the permeability spectrum of the multilayers at various applied magnetic fields. The multilayer hysteresis loops exhibited a stepped structure corresponding to multiple reversals. Each reversal stage was assigned to a particular NiFe layer based on the magnetisation ratios, exchange bias and NiFe layer thickness. NiFe layers of the same thickness with the same relative position within a stack appeared to have the same value of exchange bias. The resonant permeabilities of the multilayers shifted towards higher frequencies as the applied field was increased, in agreement with ferromagnetic resonance theory. This indicated that the resonances originated from the multilayers and were not due to systematic errors or artefacts. Trends within the sample range were observed but further work is needed to confirm these observations.

23. Intervention Fidelity for a Complex Behaviour Change Intervention in Community Pharmacy Addressing Cardiovascular Disease Risk

Abstract

Background: Delivery of cardiovascular disease (CVD) prevention programs by community pharmacists appears effective and enhances health service access. However, their capacity to implement complex behavioural change processes during patient counselling remains largely unexplored. This study aims to determine intervention fidelity by pharmacists for behavioural components of a complex educational intervention for CVD prevention. After receiving training to improve lifestyle and medicines adherence, pharmacists recruited 70 patients aged 50-74 years without established CVD, and taking antihypertensive or lipid lowering therapy. Patients received five counselling sessions, each at monthly intervals. Researchers assessed biomedical and behavioural risk factors at baseline and six months. Pharmacists documented key outcomes from counselling after each session. Most patients (86%) reported suboptimal cardiovascular diets, 41% reported suboptimal medicines adherence, and 39% were physically inactive. Of those advised to complete the intervention, 85% attended all five sessions. Pharmacists achieved patient agreement with most recommended goals for behaviour change, and overwhelmingly translated goals into practical behavioural strategies. Barriers to changing behaviours were regularly documented, and pharmacists reported most behavioural strategies as having had some success. Meaningful improvements to health behaviours were observed post-intervention. Findings support further exploration of pharmacists' potential roles for delivering interventions with complex behaviour change requirements.

Abstract

Pulmonary fibrosis is characterized by extracellular deposition in the lung primarily collagen but also other ECM molecules. The primary cell type responsible for this is the myofibroblast, and this can be induced by various stressors and signals. Infections be they bacterial or viral can cause pulmonary fibrosis (PF). In 2019, severe acute respiratory syndrome coronavirus 2 (SAR-CoV-2) originated in Wuhan, China, has led to a worldwide pandemic and can lead to acute respiratory distress and lung fibrosis. The virus itself can be cleared, but patients may develop long-term PF, which can be debilitating and life-limiting. There is a significantly perturbed immune response that shapes the fibrotic response leading to fibrosis. Given the importance of PF irrespective of cause, understanding the similarities and differences in pathogenesis caused by SARS-CoV-2-induced PF may yield new therapeutic targets.
