Abstract

We measured the current induced loss of vortex correlation in YBCO crystals using the pseudo DC flux transformer geometry. We find that the current $I_{cut}$ at which the top and bottom voltage differ is a linear function of temperature independent of pinning. Although the vortex correlation length at $I_{cut}$ coincides with the sample thickness, the experimental data show that $I_{cut}$ is sample thickness independent. Abstract

Transfusion is a specific cause of acute kidney injury (AKI) after cardiac surgery. Whether there is an association between the composition of blood products and the onset of AKI is unknown. The present study suggests that the transfusion of packed red blood cells containing a high amount of myeloid-related protein 14 (MRP_14) could increase the incidence of AKI after cardiac surgery. In a mouse model, MRP_14 increased the influx of neutrophils in the kidney after ischemia-reperfusion and their ability to damage tubular cells. Higher concentrations of MRP_14 were found in packed red blood cells from female donors or prepared by whole blood filtration.

Abstract

BACKGROUND\ud \ud Dupilumab, a monoclonal antibody against interleukin (IL)-4 receptor alpha that inhibits IL-4/IL-13 signalling is indicated in dermatology for the treatment of moderate-to-severe atopic dermatitis (AD) in adult and adolescent patients 12 years and older and severe AD in children 6-11 years, who are candidates for systemic therapy. Dupilumab received Early Access to Medicines Scheme (EAMS) approval for adults in March 2017.\ud \ud OBJECTIVES\ud \ud The purpose of this study was to assess the efficacy outcomes of treatment with dupilumab in EAMS.\ud \ud METHODS\ud \ud A retrospective analysis of adult patients enrolled in the dupilumab EAMS in the UK. Scores were assessed at baseline and follow up, including the Eczema Area and Severity Index (EASI), Investigator’s Global Assessment Score (IGA) and Dermatology Life Quality Index (DLQI).\ud \ud RESULTS\ud \ud Data were available for 57 adult patients treated with dupilumab for at least 12 weeks; 73.6% of patients had\ud received prior treatment with 3 or 4 immunosuppressants. Baseline scores for the EASI and DLQI were 27.93 (standard \ud deviation, SD 13.09) and 18.26 (SD 6.18) respectively. Abstract

This concluding chapter engages with the individual contributions to this volume and shows how they represent an important response to the otherwise simplistic perspectives on Africa’s ‘new middle class’ or ‘emerging middle class’, perspectives that have appeared in both the global media and in publications produced by global institutions. Spronk reflects, firstly, on the terms ‘African’ and ‘middle class’. Problematic assumptions lie behind the use of both terms in the present debate, assumptions that compound the dangers of treating the African middle class as an ontological given. She then analyses three matters emerging from the book: the debates about the category of the ‘middle class’, the question of which material characteristics we should focus on when studying the middle classes, and the immaterial or symbolic qualities of studying the middle class. Lastly, she discusses the potential of theorizing the middle class from Africa, so as to incorporate the global North as just one of many sites in a world of plurality that can be, and must be, read from Africa as much as it is read from anywhere else.

29. Treatment outcomes of patients with Atopic Dermatitis (AD) treated with dupilumab through the Early Access to Medicines Scheme (EAMS) in the UK.

Abstract

BACKGROUND Dupilumab, a monoclonal antibody against interleukin (IL)-4 receptor alpha that inhibits IL-4/IL-13 signalling is indicated in dermatology for the treatment of moderate-to-severe atopic dermatitis (AD) in adult and adolescent patients 12 years and older and severe AD in children 6-11 years, who are candidates for systemic therapy. Dupilumab received Early Access to Medicines Scheme (EAMS) approval for adults in March 2017. OBJECTIVES The purpose of this study was to assess the efficacy outcomes of treatment with dupilumab in EAMS. METHODS A retrospective analysis of adult patients enrolled in the dupilumab EAMS in the UK. Scores were assessed at baseline and follow up, including the Eczema Area and Severity Index (EASI), Investigator's Global Assessment Score (IGA) and Dermatology Life Quality Index (DLQI). RESULTS Data were available for 57 adult patients treated with dupilumab for at least 12 weeks; 73.6% of patients had received prior treatment with 3 or 4 immunosuppressants. Baseline scores for the EASI and DLQI were 27.93 (standard deviation, SD 13.09) and 18.26 (SD 6.18) respectively. AD severity scores showed statistically significant improvement at week 16+4 weeks (p <0.001 for all). The mean change in EASI was 14.13 points with 66.7% and 36.7% achieving a 50% (EASI-50) and 75% (EASI-75) improvement in EASI, respectively at 16+/- 4 weeks. IGA scores improved by at least two categories for 75% patients. DLQI scores decreased by a mean of 9.0 points, with 80% patients demonstrating a MCID 4-point improvement. For 85% patients, clinicians rated the treatment response as being either 'better' (19%) or 'much better' (65%). CONCLUSIONS Dupilumab is associated with a significant and clinically relevant improvements in AD as measured by patient- and physician-reported outcome measures. Importantly, the clinical efficacy, despite the refractory disease of this EAMS cohort, is comparable to that previously reported in clinical trials. [ABSTRACT FROM AUTHOR]

Abstract

© 2017 Australian College of Mental Health Nurses Inc. Australia is a country rich in cultural diversity, with Indigenous Australians having specific cultural values and a variety of spoken languages. In addition, the increasing number of people from migrant and refugee backgrounds requires that health professionals be able to communicate effectively with people from a wide range of cultural backgrounds. This is particularly relevant when undertaking a mental health assessment, because members of diverse communities often face the dual vulnerability of marginalization and stigmatization. This paper reports on the development and evaluation of a virtual teaching and learning resource that prepares health students to be culturally competent in mental health assessment. Four online interprofessional learning journeys were developed. Evaluation of the learning resources was conducted across three participating Australian universities. Quantitative evaluation involved pre- and post-testing using an empathy scale, the Mental Health Nursing Clinical Confidence Scale, and the Cultural Competence Questionnaire informed by the theory of planned behaviour. Qualitative data from focus group interviews explored participants’ experiences of using the guided learning journey. Participants reported changes from pretest to post-test in their empathy and attitudes towards culturally and linguistically diverse consumers with significant positive changes in cultural competence, empathy, and attitudes. There was strong satisfaction with the learning materials, indicating that participants valued this ‘real world’ learning experience. Results require cautious interpretation, given recruitment difficulties in the evaluation phase. However, these learning journeys appear to have potential to be an effective way to challenge attitudes and perceptions, as well as increase cultural competence towards culturally and linguistically diverse consumers.

Abstract

Summary: This study summarizes the use, since its inception, of the Cutaneous Lupus Disease Area and Severity Index (CLASI) as an outcome measure in clinical studies. We systematically searched the MEDLINE, PubMed, EMBASE and Cochrane databases for papers including the term 'cutaneous lupus disease area and severity index' and its abbreviations up to August 2017, identifying 205 abstracts. Following shortlisting, two independent physicians critically reviewed 71 papers for data extraction. We found that a limited number of high‐quality studies used the CLASI scoring as an outcome measure. We concluded that further validation is necessary to identify the effectiveness of the CLASI in the assessment of cutaneous lupus erythematosus subtypes. The use of standardized core patient‐ and physician‐reported outcome measures may reduce heterogeneity and allow comparisons between patients enrolled in clinical trials. This would improve the relevance within clinical practice, where the use of CLASI is currently limited. [ABSTRACT FROM AUTHOR]

Abstract

We have fabricated and tested GMR magnetic sensors that operate in the CPP mode. This work is a continuation of the ultra-high density magnetic sensor research introduced at INTERMAG 96. We have made two significant modifications to the process sequence. First, contact to the sensor is made through a metal conduit deposited in situ with the multilayers. This deposition replaces electroplating. This configuration ensures a good electrical interface between the top of multilayer stack and the top contact, and a continuous, conductive current path to the sensor. The consequences of this modification are an increase in yield of operational devices to [greater than or equal to]90% per wafer and a significant reduction of the device resistance to [less than or equal to]560 m[ohms] and of the uniformity of the device resistance to [less than or equal to]3%. Second, the as-deposited multilayer structure has been changed from [[Cu 30 [Angstrom]/Co 20 [Angstrom]].sub.18] (third peak) to [[Cu 20.5 [Angstrom]/Co 12 [Angstrom]].sub.30] (second peak) to increase the GMR response. The best second peak CPP GMR response from a single device is 39%. The sensitivity of that device is 0.13 %/Oe.

34. A high temperature stable spin valve sensor head for high density applications

Abstract

In this paper, we will report a read-only spin valve that has a trackwidth of 1.4 [[micro]meter], a read gap of 0.23 [[micro]meter], and with a stripe height of 0.75 [[micro]meter]. It had a corrosion resistant exchange pinning layer, [Co.sub.90][Fe.sub.10]/FeMnRh, that provided an exchange bias of 225 Oe. It produces a very stable signal and a normalized amplitude of 1 mv/[[micro]meter]/(memu/[cm.sup.2]). Furthermore its asymmetry is lower that 6%. The [Delta]R/R of the spin valve film was 5.2% with the following structure:Ta(5nm)/NiFe(2nm)/[Co.sub.90][Fe.sub.10](5nm)/Cu(2.8nm) /[Co.sub.90][Fe.sub.10](1 .2nm)/NiFe(2nm)/FeMnRh(10nm)/Ta(5nm). The spin valve heads were soaked for 96 hours in a corrosion test chamber that was operated at a temperature of 85 C and a humidity of 85%. The sensor element, the exchange bias film, the hard magnetic longitudinal bias, and the conducting leads were free from corrosion after the corrosion test. Furthermore the spin valve read-only heads were electro-migration tested in an oven operated at 120 [degrees] C, and the heads survived with the operating current up to 9 ma.

Abstract

Lupus profundus is a rare variant of cutaneous lupus erythematosus (CLE), which can result in difficulty establishing a diagnosis and initiating appropriate management. We recommend that antimalarials, singularly or in combination, should be considered as first line treatment in all subtypes of cutaneous lupus, including lupus profundus, as advocated by the 2021 British Society of Rheumatology-endorsed British Association of Dermatology guidance for cutaneous lupus ([8]). 40. Radiation dosing in the investigation and follow-up of urolithiasis: Comparison between historical and contemporary practices

Abstract

PURPOSE: Computed tomography (CT) is the gold standard imaging modality for the diagnosis and follow-up of urolithiasis. Before the use of CT, intravenous urography (IVU) was the imaging modality of choice. CT remains contentious because of the cancer risk related to radiation exposure above a threshold level. We aimed to compare the radiation exposure dose to the average patient with urolithiasis in the era of CT with that of IVU. MATERIALS AND METHODS: Our hospital medical records database was searched for patients who presented to the Emergency Department over a 1-month period in 1990 with a diagnosis of renal colic. Patients with the same presentation, from the same month, in 2013 were also identified. A total of 14 patients from each year fulfilled the inclusion criteria. The estimated effective radiation exposure dose for each patient was calculated by using data from population-based studies. RESULTS: The median effective radiation dose per patient in the 1990 group, for initial diagnosis and subsequent follow-up, was 4.05 mSv (interquartile range [IQR], 3.7-4.4 mSv). The corresponding median dose in the 2013 group was 4.2 mSv (IQR, 4.2-4.9 mSv), and there was no evidence of a statistical difference between the groups (p=0.8). CONCLUSIONS: Despite the contentiousness related to the use of serial CT scanning, our study demonstrated that for radiological investigation and follow-up of urolithiasis, the estimated effective radiation exposure dose to each patient is only marginally higher than in the era of IVU, with improvements in length of hospital stay and time to definitive diagnosis.
