2. Using the socio-ecological model to appraise perspectives on health workforce retention and intention to leave in Malawi and Tanzania: a qualitative longitudinal study.

Introduction: The chronic health workforce shortage poses a significant setback to achieving universal health coverage. Health authorities continually develop and implement human resources for health policies and interventions to alleviate the crisis, including retention policies. However, the success of such policies and interventions is tangential to the alignment with health workers' expectations. The aim of this study was to explore perspectives on health workforce retention and intention to leave among health workers and policy-makers from rural and remote areas of Malawi and Tanzania. Methods: Semi-structured interviews were conducted with 120 participants - 111 rural and remote mid-level health workers, and nine policy-makers in Malawi and Tanzania - for a period of 3 years, 2014-2017. The semi-structured interviews were conducted face to face, and follow-up interviews were conducted through emails or social media. By using the socio-ecological model as a framework for analysis, the emerging themes were mapped out and linked. Results: Health workers related their perspectives on retention and intention to leave to the individual (intrapersonal), family (interpersonal/microsystem), and community (institutional/mesosystem) factors, whereas policy-makers focused their views mainly on the individual (intrapersonal) factors and retention policies at the national level (macrosystem). Conclusion: Policy-makers and health workers in rural and remote settings in Malawi and Tanzania recognise the factors influencing health workforce retention, and intention to leave at the individual level. However, while policy-makers focus mainly on national-level retention policies, health workers focus on retention aspects related to the family and the surrounding community - a clear misalignment. Therefore, health authorities need to align health policies to health workers' expectations to bridge this gap, improve access to the health workforce in rural and remote populations, and improve health outcomes. NPY is an important central orexigenic hormone, but little is known about its peripheral actions in human adipose tissue (AT) or its potential paracrine effects. Our objective was to examine NPY's role in AT, specifically addressing NPY protein expression, the effect of NPY on adipokine secretion, and the influence of insulin and rosiglitazone (RSG) on adipocytederived NPY in vitro. Ex vivo human AT was obtained from women undergoing elective surgery [age: 42.7 [+ or -] 1.5 yr (mean [+ or -] SE), BMI: 26.2 [+ or -] 0.7 kg/[m.sup.2]; n = 38]. Western blot analysis was used to determine NPY protein expression in AT depots. Abdominal subcutaneous (AbSc) adipocytes were isolated and treated with recombinant (rh) NPY, insulin, and RSG. NPY and adipokine levels were measured by ELISA. Our results were that NPY was localized in human AT and adipocytes and confirmed by immunohistochemistry. Depot-specific NPY expression was noted as highest in AbSc AT (1.87 [+ or -] 0.23 ODU) compared with omental (Om; 1.03 [+ or -] 0.15 ODU, P = 0.029) or thigh AT (Th; 1.0 [+ or -] 0.29 ODU, P = 0.035). Insulin increased NPY secretion (control: 0.22 [+ or -] 0.024 ng/ml; 1 nM insulin: 0.26 [+ or -] 0.05 ng/ml; 100 nM insulin: 0.29 [+ or -] 0.04 ng/ml; 1,000 nM insulin: 0.3 [+ or -] 0.04 ng/ml; P < 0.05, n = 13), but cotreatment of RSG (10 nM) with insulin (100 nM) had no effect on NPY secretion. Furthermore, adipocyte treatment with rh-NPY downregulated leptin secretion (control: 6.99 [+ or -] 0.89 ng/ml; 1 nmol/l rh-NPY: 4.4 [+ or -] 0.64 ng/ml; 10 nmol/l rh-NPY: 4.3 [+ or -] 0.61 ng/ml, 100 nmol/l rh-NPY: 4.2 [+ or -] 0.67 ng/ml; P < 0.05, n = 10) but had no effect on adiponectin or TNF-[alpha] secretion. We conclude that NPY is expressed and secreted by human adipocytes. NPY secretion is stimulated by insulin, but this increment was limited by cotreatment with RSG. NPY's antilipolytic action may promote an increase in adipocyte size in hyperinsulinemic conditions. - Abstract
South Asians have a higher risk of type 2 diabetes mellitus (T2DM) and cardiovascular disease (CVD) than white Caucasians, for a given BMI. Premature biological ageing, assessed by reduction in telomere length (TL), may be mediated by factors resulting from altered metabolic profiles associated with obesity. We hypothesise that ethnicity and metabolic status represent detrimental factors contributing to premature biological ageing. Therefore we assessed TL in two South Asian, age and BMI-matched cohorts [T2DM (n = 142) versus non-T2DM (n = 76)] to determine the effects of BMI, gender, lipid and CVD profile on biological ageing. Genomic DNA was obtained from the UKADS cohort; biochemical and anthropometric data was collected and TL was measured by quantitative real-time PCR. Our findings indicated a gender-specific effect with reduced TL in T2DM men compared with non-T2DM men (P = 0.006). Additionally, in T2DM men, TL was inversely correlated with triglycerides and total cholesterol (r = -0.419, P <0.01; r = -0.443, P <0.01). In summary, TL was reduced amongst South Asian T2DM men and correlated with triglycerides and total cholesterol. This study highlights enhanced biological ageing among South Asian, T2DM men, which appears to be tracked by changes in lipids and BMI, suggesting that raised lipids and BMI may directly contribute to premature ageing.

Aims - A common variant, rs9939609, in the FTO (fat mass and obesity) gene is associated with adiposity in Europeans, explaining its relationship with diabetes. However, data are inconsistent in South Asians. Our aim was to investigate the association of the FTO rs9939609 variant with obesity, obesity-related traits and Type 2 diabetes in South Asian individuals, and to use meta-analyses to attempt to clarify to what extent BMI influences the association of FTO variants with diabetes in South Asians. Methods - We analysed rs9939609 in two studies of Pakistani individuals: 1666 adults aged = 40 years from the Karachi population-based Control of Blood Pressure and Risk Attenuation (COBRA) study and 2745 individuals of Punjabi ancestry who were part of a Type 2 diabetes case–control study (UK Asian Diabetes Study/Diabetes Genetics in Pakistan; UKADS/DGP). The main outcomes were BMI, waist circumference and diabetes. Regression analyses were performed to determine associations between FTO alleles and outcomes. Summary estimates were combined in a meta-analysis of 8091 South Asian individuals (3919 patients with Type 2 diabetes and 4172 control subjects), including those from two previous studies. Results - In the 4411 Pakistani individuals from this study, the age-, sex- and diabetes-adjusted association of FTO variant rs9939609 with BMI was 0.45 (95% CI 0.24–0.67) kg/m2 per A-allele (P = 3.0 × 10-5) and with waist circumference was 0.88 (95% CI 0.36–1.41) cm per A-allele (P = 0.001). The A-allele (30% frequency) was also significantly associated with Type 2 diabetes [per A-allele odds ratio (95% CI) 1.18 (1.07–1.30); P = 0.0009]. A meta-analysis of four South Asian studies with 8091 subjects showed that the FTO A-allele predisposes to Type 2 diabetes [1.22 (95% CI 1.14–1.31); P = 1.07 × 10-8] even after adjusting for BMI [1.18 (95% CI 1.10–1.27); P = 1.02 × 10-5] or waist circumference [1.18 (95% CI 1.10–1.27); P = 3.97 × 10-5]. 15. Is it possible to predict improved diabetes outcomes following diabetes self-management education: a mixedmethods longitudinal design.

- Abstract
Objective: To predict the diabetes-related outcomes of people undertaking a type 2 Diabetes Self- Management Education (DSME) programme from their baseline data. Design: A mixed-methods longitudinal experimental study. 6 practice nurses and 2 clinical academics undertook blind assessments of all baseline and process data to predict clinical, behavioural and psychological outcomes at 6 months post-DSME programme. Setting: Primary care. Participants: -31 people with type 2 diabetes who had not previously undertaken DSME. Intervention: All participants undertook the Diabetes Manual 1:1 self-directed learning 12-week DSME programme supported by practice nurses trained as Diabetes Manual facilitators. Outcome variables: Glycated haemoglobin (HbA1c), diabetes knowledge, physical activity, waist circumference, self-efficacy, diabetes distress, anxiety, depression, demographics, change talk and treatment satisfaction. These variables were chosen because they are known to influence self-management behaviour or to have been influenced by a DSME programme in empirical evidence. Results: Baseline and 6-month follow-up data were available for 27 participants of which 13 (48%) were male, 22 (82%) white British, mean age 59 years and mean duration of type 2 diabetes 9.1 years. Significant reductions were found in HbA1c t(26)=2.35, p=0.03, and diabetes distress t(26)=2.30, p=0.03, and a significant increase in knowledge t(26)=-2.06, p=0.05 between baseline and 6 months. No significant changes were found in waist circumference, physical activity, anxiety, depression or self-efficacy. Accuracy of predictions varied little between clinical academics and practice nurses but greatly between outcome (0-100%). The median and mode accuracy of predicted outcome was 66.67%. Accuracy of prediction for the key outcome of HbA1c was 44.44%. Diabetes distress had the highest prediction accuracy (81.48%). Conclusions: Clinicians in this small study were unable to identify individuals likely to achieve improvement in outcomes from DSME. DSME should be promoted to all patients with diabetes according to guidelines. [ABSTRACT FROM AUTHOR]

Pay for performance initiatives have been proposed as a means of reducing health care inequalities in primary care. As a part of its strategy to improve standards in primary care the UK Government in 2004 introduced a new contract for primary care physicians linking pay to performance (Quality and Outcomes Framework-QOF). We examined the impact of this initiative on south Asian patients with type 2 diabetes in UK and compared them with the targets set by various national and international bodies. 1492 south Asian patients with type 2 diabetes recruited from 18 general practices in Birmingham and Coventry to the United Kingdom Asian Diabetes Study(UKADS) were studied. Data on blood pressure, total cholesterol and HbA1c at baseline and at 1 year after implementation of contract was collected. The percentage of patients achieving the QOF targets (BP < 145/85 mmHg, Total Cholesterol <5 mmol, HbA1c<7.5%) at baseline and at 1 year was compared against internationally accepted targets for diabetes (BP< 130/80, Total Cholesterol <4 mmol, HbA1c<7%). At baseline the percentages of patients achieving the QOF and international targets were -BP 45 v19, Total Cholesterol 63 v 26 and HbA1c 40 v 29 respectively. After 1 year of implementation of the contract the corresponding percentages were- BP 55 v 28, Total Cholesterol 75 v 36 and HbA1c 43 v 31 respectively. Linking pay to performance initiative improves general standards of care but falls significantly short of meeting the targets recommended by various national and international advisory bodies for patients with diabetes. Central obesity is strongly associated with sub-clinical inflammation, insulin resistance (IR) and type 2 diabetes mellitus (T2DM), but the intracellular mechanisms involved in the pathogenesis of central obesity remain unclear. Recent studies have implicated NF κB and c-Jun N terminal kinase (JNK) as central molecules linking insulin action and inflammation in mice. Our study aimed to investigate (1) the central inflammatory intracellular signaling pathways involving NF κB and JNK activation in abdominal subcutaneous (AbdSc) and omental (Ore) human adipose tissue, (2) the influence of adiposity and diabetic status on the activation of the NF κB and its key intermediates and JNK within these AT depots and (3) the effects of TNFα, a known mediator of inflammation in mice, on NF κB and JNK activity in human AbdSc adipocytes. Our data showed that ex vivo IKKβ, IKKγ, I κBα and I κB α -P protein expression was increased in Om AT compared with AbdSc AT (p<0.01). The ratio of active NF κB to total NF κB expression (NF κB ratio) was upregulated in Om AT compared with AbdSc in both lean and obese subjects (p<0.05). Whilst, phosphospecific JNK1 was increased 5.8 fold in the AbdSc depot, (p<0.01), compared with Om AT. In in vitro TNF α treated AbdSc adipocytes NF κB activity increased over time (2-48 hr, p<0.05), however JNK activity remained unchanged. In conclusion, these findings suggest distinct inflammatory signaling pathways with increased NF κB in human Om AT and JNK in AbdSc AT. In addition, NF κB was influenced by obesity and diabetic status, whilst TNF α treated AbdSc lead to activation of NF κB and not JNK. Consequently, NF κB appears to play a more central role in inflammatory mediated metabolic disease than JNK, highlighting NF κB as a potential key target for therapeutic intervention. South Asians have a higher risk of type 2 diabetes mellitus (T2DM) and cardiovascular disease (CVD) than white Caucasians, for a given BMI. Premature biological ageing, assessed by reduction in telomere length (TL), may be mediated by factors resulting from altered metabolic profiles associated with obesity. We hypothesise that ethnicity and metabolic status represent detrimental factors contributing to premature biological ageing. Therefore we assessed TL in two South Asian, age and BMI-matched cohorts [T2DM (n=142) versus non-T2DM (n=76)] to determine the effects of BMI, gender, lipid and CVD profile on biological ageing. Genomic DNA was obtained from the UKADS cohort; biochemical and anthropometric data was collected and TL was measured by quantitative real-time PCR. Our findings indicated a gender-specific effect with reduced TL in T2DM men compared with non-T2DM men (P=0.006). Additionally, in T2DM men, TL was inversely correlated with triglycerides and total cholesterol (r=−0.419, P<0.01; r=−0.443, P<0.01). In summary, TL was reduced amongst South Asian T2DM men and correlated with triglycerides and total cholesterol. This study highlights enhanced biological ageing among South Asian, T2DM men, which appears to be tracked by changes in lipids and BMI, suggesting that raised lipids and BMI may directly contribute to premature ageing.
