13. The Right Place, The Right Time

Abstract

Superintendents are in the right place at the right time to take advantage of a golden opportunity to reverse a trend that threatens the nation's schools and communities. They are now uniquely positioned to stop the retreat of good-hearted, well-intentioned citizens from public life. In this article, the authors present perpetual traps that include high ideals blended with traditional approaches to working with the public, and these are: (1) selling preordained decisions; (2) manufacturing representation; (3) dumping information; (4) cutting engagement short; (5) doing it alone; and (6) buying into the politicians and pundits. They take a brief look at three school districts that are building school-community partnerships in order to illustrate how creating an organizational structure for reconnecting schools and communities is working. In their work with the leaders of these school districts, they have discovered three constants associated with their success in building effective partnerships with their communities: (1) The partnership is community-based; (2) The partnership is all-inclusive; and (3) The partnership is supported by a courageous school leader.

Abstract

Realizing how difficult it is to pass tax issues today, many school boards with all good intentions place them on the ballot before the money is really needed. They do this so if the tax issues are not approved, they have another chance to get them passed before serious educational cuts have to be made. In states where school districts must rely on the passage of a local tax referendum to fund at least part of their operations, this is a common practice. However, in trying to be responsible by asking for the money before it is really needed, often what school officials actually end up doing is to trigger a chain reaction of negative side effects, or unintended consequences, that surface later down the road. To complicate matters, the growing parade of tax issue attempts begins to wear down both school district employees and the citizens who are working to pass them. After a while, school officials grow weary of losing and decide to appease the no-voters by freezing staff salaries, cutting athletics, eliminating busing and making other cuts and reductions. However, since no-voters have an insatiable appetite for making cuts, these reductions only serve as additional proof to them that the school district has more money than it needs.

Background: The use of smartphones and multimedia messaging service (MMS) continues to increase in day to day orthopaedic clinical practice. However, there is limited evidence to support the safe utilisation of MMS.Objectives: The aim of this study was to correlate the performance of MMS imaging to picture archiving and communication systems (PACS) imaging within the setting of diagnosis and management of ankle fractures.Methods: The ankle fracture radiograph series of 82 consecutive patients were evaluated by five orthopaedic consultant specialists. A questionnaire regarding diagnosis and preferred management was completed separately for each patient using smartphone and PACS images. Statistical analysis was performed using Intraclass Correlation Coefficient (ICC).Results: Ankle fracture diagnosis showed strong to excellent correlation both inter- and intraobserver MMS vs PACS when using the Weber (0.815, 0.988), Anatomical (0.858, 0.988), and AO classification systems (0.855, 0.985). MMS was less reliable than PACS in determining many management options.Conclusion: The reliability of ankle fracture classification using MMS image viewing was not significantly different to interpretation on PACS workstations. Smartphone use in ankle fracture classification is supported by this study. Smartphone use was less accurate than PACS in devising management plans and future use should be limited to making only initial plans that must be corroberated with PACS and intraoperative findings prior to definitive fixation. Abstract

Since the first novel gene discovery for a Mendelian condition was made via exome sequencing (ES), the rapid increase in the number of genes known to underlie Mendelian conditions coupled with the adoption of exome (and more recently, genome) sequencing by diagnostic testing labs has changed the landscape of genomic testing for rare disease. Specifically, many individuals suspected to have a Mendelian condition are now routinely offered clinical ES. This commonly results in a precise genetic diagnosis but frequently overlooks the identification of novel candidate genes. Such candidates are also less likely to be identified in the absence of large-scale gene discovery research programs. Accordingly, clinical laboratories have both the opportunity, and some might argue a responsibility, to contribute to novel gene discovery which should in turn increase the diagnostic yield for many conditions. However, clinical diagnostic laboratories must necessarily balance priorities for throughput, turnaround time, cost efficiency, clinician preferences, and regulatory constraints, and often do not have the infrastructure or resources to effectively participate in either clinical translational or basic genome science research efforts. For these and other reasons, many laboratories have historically refrained from broadly sharing potentially pathogenic variants in novel genes via networks like Matchmaker Exchange, much less reporting such results to ordering providers. Efforts to report such results are further complicated by a lack of guidelines for clinical reporting and interpretation of variants in novel candidate genes. Nevertheless, there are myriad benefits for many stakeholders, including patients/families, clinicians, researchers, if clinical laboratories systematically and routinely identify, share, and report novel candidate genes. To facilitate this change in practice, we developed criteria for triaging, sharing, and reporting novel candidate genes that are most likely to be promptly validated as underlying a Mendelian condition and translated to use in clinical settings.

Abstract

Cows with an extended interval from calving to first ovulation (post partum period) have increased intervals from calving to conception and are more likely to be culled compared with cows with a short post partum period. Treatment options for cows with an extended post partum period include hormonal and management strategies. Hormonal treatments result in the majority of treated animals displaying oestrus with a subsequent luteal phase of normal duration and improved pregnancy rates compared with untreated controls. Hormonal interventions also tend to have more predictable outcomes compared with management changes, such as manipulating body condition or dietary intakes after calving, and usually have some oestrus synchronization effect, thus facilitating the use of artificial insemination. However, responses to any treatment are variable and are related to those factors that influence duration of the PPI, such as body condition and parity. In a study on a commercial farm of 700 dairy cows experiencing poor reproductive performance, cows were assigned to two different treatment protocols based on their ovarian function. This was carried out over a 12 month period. Cows with a normal corpus luteum and cystic corpus luteum were assigned to treatment with PGF2α and anoestrus cows and follicular cyst cows were assigned to treatment with GnRH. Success was based on return to oestrus within 6 days with PGF2α treatment and return to oestrus within 25 days with GnRH treatment. 35% of the cows were treated with PGF2α, of which 56% showed return to oestrus within 6 days after the first veterinarian visit. After 2 more subsequent visits, 10% of the initial treatment pool with PGF2α remained open. 39% of the cows were treated with GnRH. Within 25 days, 51% of the cows displayed oestrus. Following two more subsequent visits, again 10% of the non responsive cows remained open after continued GnRH treatment. The long interval from parturition to first service (123 days), the low rate of oestrus response and poor pregnancy rate (20%) prior to treatment are consonant with inadequate oestrus detection.

Abstract

Although initial success rates for coronary angioplasty have improved, the rate of restenosis within 6 months of the procedure has persisted at 30 to 40%. The relation of restenosis to initial success, recurrence of symptoms and risk factors suggests that high grade or total lesions, long lesions, lesions in the proximal left anterior descending artery or in saphenous grafts, and the absence of intimai dissection after angioplasty are associated with an increased risk of restenosis. Unstable angina, male sex and diabetes are clinical factors associated with a greater risk of restenosis. Pathologic specimens suggest that plaque splitting and disruption are found acutely after angioplasty, but that restenosis occurs as an ex- cessive reparative, proliferative response of smooth muscle cells leading to recurrent luminal narrowing.

Abstract

Accessory pathway electrograms are rarely recorded in patients with Wolff-Parkinson-White syndrome. In one patient, during electrophysiologic study, simultaneous local ventricular (V) accessory pathway (AP) and atrial (A) deflections were recorded during bipolar catheter endocardial mapping over the pathway. Analysis of changes in electrographic intervals during performance of the ventricular extrastimulus technique allowed characterization of the retrograde conduction properties of the pathway. As coupling intervals were decreased, an initial increase was seen in the AP2A2interval with subsequent ventriculoatrial block between the accessory pathway and atrium. When coupling intervals were further decreased, the V2AP2interval lengthened with ultimate block between the ventricle and accessory pathway. These findings support the concept of impedance mismatch as the cause of conduction block in accessory pathways with the distal junction of the accessory pathway being the most vulnerable.

Abstract

The late restenosis rate after emergent percutaneous transluminal coronary angioplasty for acute myocardial infarction was assessed by performing outpatient fallow-up cardiac catheterization in 79 (87%) of 91 consecutive patients who had been discharged from the hospital with a successful coronary angioplasty. The majority of patients (90%) received high dose intravenous thrambolytic therapy with streptokinase in addition to angioplasty. Similar follow-up data were obtained in 206 (90%) of 228 consecutive patients who had successful elective angioplasty during the same period. The interval from angioplasty to follow-up was 28 ± 9 weeks for the myocardial infarction group and 30 ± 11 weeks for the elective group.

Abstract

Background: Pelvic skeletal asymmetry can result in rotational differences and morphologic bony prominence variance between the left and right hemipelvis. When selecting bony reference points for modern computed tomography-based robotic total hip arthroplasty planning, it is unclear which bony landmarks are the most reliable and accurate, especially in the presence of significant pelvic asymmetry., Methods: A retrospective study was conducted utilizing a database of computed tomography scans. Multiple bony landmarks in the pelvis and femur were selected for comparison, with the aim of measuring pelvic asymmetry. Specifically, the study measured the average difference in lateral offset between the left and right hemipelvis caused by pelvic asymmetry. Landmarks were also compared to determine the impact of pelvic asymmetry on hip length, femur length, and limb length discrepancies. Furthermore, a scenario was simulated in the software whereby a total hip replacement was inserted, potentially changing the hip length. The impact of pelvic reference point selection on the measurement of this simulated change in hip length was examined., Results: This study population showed widespread pelvic asymmetry. The anatomical landmarks of the opposite side cannot be relied upon for predicting the anatomy of the affected side. The center of rotation axis is more reliable than the inferior obturator foramen axis for hip length discrepancy due to pelvic asymmetry ( P < .05)., Conclusions: Current computer-assisted surgery THR software reports measurements of global offset and hip length that do not consider pelvic asymmetry. Surgeons are not given confidence ranges to represent the potential impact of asymmetry on the global offset and hip length values. Abstract

Purpose: Proximal femur fractures in geriatric patients are associated with increased morbidity and mortality. This study investigates Brain Natriuretic Peptide immunoassay levels taken at the time of hospital admission in predicting cardiac complications and mortality in geriatric patients with a proximal femur fracture., Methods: A single-site prospective cohort study at a large tertiary care, level 1 trauma centre was conducted on all consecutive geriatric patients aged greater than 60 years who sustained a proximal femur fracture. Investigators collected Brain Natriuretic Peptide levels from venous blood samples on admission to the Emergency Department. The main outcome measurements were inpatient cardiovascular complications, and all-cause mortality at 30-day, 90-day, one-year, and nine-years., Results: Over a one-year period, 112 patients were enrolled. The average age was 82.7 years, and the average follow up was 6 years and 6 months (range, 2 days to 9 years). No patients were lost to follow up. There were 44 new or exacerbations of pre-existing cardiac complications requiring management recorded in 30 (26.8%) patients. Mortality at 30 days was 9.8%, 90 days was 16.1%, and one year was 24.1%, with deceased patients having a statistically significant elevated Brain Natriuretic Peptide immunoassay on hospital admission. The Kaplan-Meier graph demonstrated a trend towards increasing Brain Natriuretic Peptide and adverse survivorship risk. The Charlson Comorbidity Index was statistically significant in predicting overall survival probability., Conclusion: Brain Natriuretic Peptide immunoassay on hospital admission may be utilised to identify patients at risk of cardiac complications and mortality to guide further investigations, operative planning, the consent process, and post-operative monitoring., Competing Interests: The authors have no relevant financial or non-financial interests to disclose., (© 2022 Professor P K Surendran Memorial Education Foundation. The decision on which technique to use to perform a total knee arthroplasty has become much more complicated over the last decade. The shortfalls of mechanical alignment and kinematic alignment has led to the development of a new alignment philosophy, functional alignment. Functional alignment uses preoperative radiographic measurements, computer-aided surgery, and intraoperative assessment of balance, to leave the patient with the most "normal" knee kinematics achievable with minimal soft-tissue release. 49. Takotsubo Cardiomyopathy Associated with Primary Hyperthyroidism Secondary to Toxic Multinodular Goiter.

Abstract

Takotsubo cardiomyopathy is increasingly recognized as a distinct clinical entity. An association with thyroid disease has been identified in several case reports, mostly in the context of Grave disease. We report the case of a 67-year-old woman presenting with takotsubo cardiomyopathy and thyrotoxicosis secondary to toxic multinodular goiter. Previously unreported, this suggests that this association is related to the direct effects of thyroid hormone, rather than an autoimmune mechanism. Thyroid disease should be considered in patients presenting with takotsubo cardiomyopathy.
