2. From repatriation to rematriation:dismantling the attitudes and potentials behind the repatriation of Sámi heritage

Abstract

In this dissertation, I study the many aspects related to the return or repatriation of Sámi heritage, especially in Finland. For a long time, repatriation was a subject that was not openly discussed in Finland. In a process that seemed sudden, the National Museum of Finland decided to repatriate the Sámi collection to the Sámi Museum Siida in Inari in 2017. My dissertation examines contexts for this turn. I ask what kind of processes have led to the birth of the collections and to their repatriation. Why were objects collected, of what kind, and how do these collections differ between Europe and the Nordic countries and between the Nordic countries? How does the repatriation situation differ from one Nordic country to another, and finally, what is the situation in Europe? What have been the opinions and views of Western museums and cultural heritage workers about repatriation? This contextual knowledge helps to understand the slow process in understanding the meaning of repatriation and the reasons that triggered the rapid change. I also ask why is repatriation important to Sámi people? Is repatriation a sufficient concept when the meaning of this phenomenon is considered from the Sámi perspective? I approach the subject in four different articles which deal with the perspectives of majority museums and researchers on the one hand and with the Sámi on the other. I examine the difficulties of repatriation through the eyes of Finnish archaeologists, the many ontologies of the Sámi collections of the National Museum of Finland, and the history of the collecting of Sámi culture through one collector, T. I. Itkonen. Two articles especially study the meaning of these collections for today’s Sámi society, including the research and revival project concerning a Sámi hat used by Sámi women in certain areas between 1750 and 1910. Kysyn myös sitä, miksi palauttaminen on tärkeää saamelaisille. Onko repatriaatio riittävä käsite, kun tämän ilmiön merkitystä tarkastellaan saamelaisesta näkökulmasta? Käsittelen aihetta neljässä eri artikkelissa, jotka käsittelevät yhtäältä enemmistön museoiden ja tutkijoiden ja toisaalta saamelaisten näkökulmia. Tarkastelen repatriaation vaikeuksia suomalaisten arkeologien silmin, Kansallismuseon saamelaiskokoelmien lukuisia ontologioita sekä saamelaiskulttuurin keräämisen historiaa yhden keräilijän eli T. I. Itkosen kautta. Kahdessa artikkelissa tutkin erityisesti kokoelmien merkitystä nykypäivän saamelaisyhteiskunnalle, erityisesti tutkimus- ja elvytysprojektia, jonka keskiössä oli saamelaisnaisten tietyillä alueilla vuosina 1750‐1910 käyttämä päähine, ládjogahpir. Näen repatriaation dekolonialisaatioprosessina saamelaisten ja valtaväestön museoiden välillä, jossa molempien osapuolten on kohdattava ”jaettu” epämiellyttävä historia. The second phase is indigenisation, the continuation of repatriation in Sámi society, which brings life back to the objects. I call it rematriation. 23. A hybrid model for diagnosing sever aortic stenosis in asymptomatic patients using phonocardiogram

Abstract

This study presents a screening algorithm for severe aortic stenosis (AS), based on a processing method for phonocardiographic (PCG) signal. The processing method employs a hybrid model, constituted of a hidden Markov model and support vector machine. The method benefits from a preprocessing phase for an enhanced learning. The performance of the method is statistically evaluated using PCG signals recorded from 50 individuals who were referred to the echocardiography lab at Linköping University hospital. All the individuals were diagnosed as having a degree of AS, from mild to severe, according to the echocardiographic measurements. The patient group consists of 26 individuals with severe AS, and the rest of the 24 patients comprise the control group. Performance of the method is statistically evaluated using repeated random sub sampling. Results showed a 95% confidence interval of (80.5%-82.8%) /(77.8%- 80.8%) for the accuracy/sensitivity, exhibiting an acceptable performance to be used as decision support system in the primary healthcare center., QC 20160229

28. Chest pain and ischaemic heart disease in primary care

Abstract

BACKGROUND: Chest pain is the main symptom of first presentation with ischaemic heart disease (IHD). Little is known about the incidence of IHD among patients consulting the general practitioner (GP) for chest pain. AIMS: To estimate the occurrence of IHD among patients consulting for chest pain, to study the results of the bicycle exercise test, and to estimate the incidence of IHD in the population. DESIGN OF STUDY: Prospective descriptive study. SETTING: Three primary health centres in south-eastern Sweden. METHOD: All patients without a current IHD diagnosis, aged 20 to 79 years, and consulting for a new episode of chest pain, were included consecutively. The outcome was classified as IHD, possible IHD or not IHD, according to the results of a postal questionnaire, an exercise test or hospital care. Data from the hospital registry on patients with a diagnosis of IHD were analysed retrospectively. RESULTS: Out of 38,075 GP consultations, 577 (1.5%) were for chest pain. IHD was diagnosed in 41 (8%) of the chest pain patients, in 441 (83%) the diagnosis was excluded, and in 50 (9%) the diagnosis was judged as being uncertain. Even though the diagnostic criteria were strict, the exercise tests led to a diagnostic conclusion in 77% of the cases, most frequently a normal test result. Combining data from primary and hospital care, the yearly incidence of IHD was 6.5 diagnosed per 1000 inhabitants (aged 20 to 79 years old). CONCLUSION: The incidence of a new episode of chest pain bringing the patients to the GP was low. Eight per cent of the patients received an IHD diagnosis, and in 9% further investigation or clinical assessment is needed.

31. Little effect on p63 but significant effect on miR-21 and miR-125b by NB-UVB phototherapy on psoriatic lesions

Abstract

Psoriasis is an inflammatory skin disease in which dysregulation of p63, a member of the p53 family and crucial for skin development and maintenance, has been shown. Though currently incurable, many therapies are available including narrowband ultraviolet B (NB-UVB) phototherapy. To further elucidate the role of p63 in psoriasis and increase our understanding of the mechanisms of phototherapy, we studied the effects of NB-UVB treatment on p63 expression. Expression of p53 was also studied due to its functional role in the response of skin to UV. In addition, we investigated expression of miR-203, miR-125b and miR-21, as these microRNAs are p63 and/or p53 regulators and their involvement in psoriasis pathogenesis has previously been suggested. Skin biopsies from 12 psoriasis patients were collected before, during and at the final session of phototherapy. Real time RT-PCR and immunohistochemistry showed that epidermal p63 mRNA and protein levels were not significantly affected following phototherapy, whereas a significant increase in p53 mRNA expression and protein accumulation was found. NB-UVB treatment also significantly affected expression of miR-21 and miR-125b, whereas individual clinical improvement seemed related to p53 status only. Our results indicate that even though NB-UVB phototherapy causes diverse molecular changes, induction of p53 is pivotal for successful treatment of psoriasis, and unresolved p63 abnormality in the treated epidermis of psoriasis patients further indicate a role for p63 in psoriasis.

32. Improved contact tracing for Chlamydia trachomatis with experienced tracers, tracing for one year back in time and interviewing by phone in remote areas

Abstract

Objectives: To evaluate the Swedish model for contact tracing and especiallythe "Västerbotten model" with centralised, extended contactinterview periods, sometimes by telephone. Methods: Using questionnaires, the contact tracing and interview procedurewas evaluated during 2002, followed by an evaluation of contactinterviewing by phone in 2005–6. Results: Patients with diagnosed Chlamydia trachomatis infection reportedon average 2.5 sexual contacts, 3.0 contacts when contact interviewingwas performed at the clinic, and 2.3 contacts when performedby phone. 65% of the sexual contacts with a known test resultwere infected. Conclusion: Centralised contact tracing, exploring the sexual history forat least 12 months back in time, shows good results. Combinedwith screening of certain risk groups it is probably one effectiveway of preventing C trachomatis infections. Preventing C trachomatisby primary prevention such as information and counselling is,however, still of great importance.

40. Long-term follow-up of thoratec ventricular assist device bridge-to-recovery patients successfully removed from support after recovery of ventricular function

Abstract

Background: In certain forms of severe heart failure there is sufficient improvement in cardiac function during ventricular assist device (VAD) support to allow removal of the device. However, it is critical to know whether there is sustained recovery of the heart and long-term patient survival if VAD bridging to recovery is to be considered over the option of transplantation. Methods: To determine long-term outcome of survivors of VAD bridge-to-recovery procedures, we retrospectively evaluated 22 patients with non-ischemic heart failure successfully weaned from the Thoratec left ventricular assist device (LVAD) or biventricular assist device (BVAD) after recovery of ventricular function at 14 medical centers. All patients were in imminent risk of dying and were selected for VAD support using standard bridge-to-transplant requirements. There were 12 females and 10 males with an average age of 32 (range, 12-49). The etiologies were 12 with myocarditis, 7 with cardiomyopathies (4 post-partum [PPCM], 1 viral [VCM], and 2 idiopathic [IDCM]), and 3 with a combination of myocarditis and cardiomyopathy. BVADs were used in 13 patients and isolated LVADs in 9 patients, for an average duration of 57 days (range, 11-190 days), before return of ventricular function and successful weaning from the device. Post-VAD survival was compared with 43 VAD bridge-to-transplant patients with the same etiologies who underwent cardiac transplantation instead of device weaning. Results: Nineteen of the 22 patients are currently alive. Three patients required heart transplantation, 1 within 1 day, 2 at 12 and 13 months post-weaning, and 2 died at 2.5 and 6 months. The remaining 17 patients are alive with their native hearts after an average of 3.2 years (range, 1.2-10 years). The actuarial survival of native hearts (transplant-free survival) post-VAD support is 86% at 1 year and 77% at 5 years, which was not significantly different (p = 0.94) from that of post-VAD transplanted patients, also

Abstract

OBJECTIVE: The reliability of conventional scalar ECG for diagnosis of perioperative myocardial infarction (PMI) in cardiac surgery has been questioned. For the diagnosis of myocardial infarction in general vectorcardiography (VCG) is superior to ECG. Therefore, the usefulness of conventional VCG and computerized analysis of spatial VCG changes for diagnosis of PMI were studied. DESIGN: VCG registrations were obtained from 218 patients undergoing coronary surgery. The spatial QRS vector loop area of each VCG registration was calculated and the loop area before surgery compared with the loop area after surgery. Conventional VCG criteria for myocardial infarction and set values for loop area reduction were related to sustained elevation of plasma troponin-T and clinical course. RESULTS: Both conventional VCG criteria and spatial changes translated better than Q-waves on scalar ECG into elevation of biochemical markers of myocardial injury and impaired clinical course. CONCLUSION: VCG appears superior to conventional ECG as regards detection of myocardial injury in coronary surgery. Computerized programs have facilitated the registration and the interpretation of VCG and this methodology deserves further evaluation in cardiac surgery.

Abstract

There are relatively few studies on female athletes examining cardiac size and function and how these measures relate to maximal oxygen uptake ( VO2max). When determining sports eligibility, it is important to know what physiological adaptations and characteristics may be expected in female athletes, taking body and cardiac size into account. The purposes of this study were (a) to compare right and left heart dimensions and function in female endurance athletes ( ATH) and in non-athletic female controls of similar age ( CON); and (b) to explore how these measures related to VO2max. Forty-six ATH and 48 CON underwent a maximal bicycle exercise test and an echocardiographic examination at rest, including standard and color tissue Doppler investigation. All heart dimensions indexed for body size were larger in ATH (all P < 0.01). The diastolic mitral E/A ratio was 27% higher in ATH ( P < 0.001) while systolic left and right atrio-ventricular longitudinal displacement was 7% ( P = 0.002) and 15% ( P < 0.001) larger in ATH, respectively. Half (50.3%) of the variability in VO2max could be explained by left ventricular end-diastolic volume. Our results could be useful in evaluating female endurance athletes with suspected cardiac disease and contribute to understanding differences between female athletes and non-athletes.
