1. Evaluating the long-term outcomes of periodontal surgery vs. non-surgical treatment in aggressive periodontitis

Abstract

Background: Aggressive periodontitis is a severe form of periodontal disease characterized by rapid tissue destruction and tooth loss. The optimal treatment approach for managing this condition remains a topic of debate. Materials and Methods: A retrospective cohort study was conducted, involving patients diagnosed with aggressive periodontitis who received either surgical or non-surgical treatment between 2010 and 2020. Clinical and radiographic data were collected at baseline and regular intervals over a 5-year follow-up period. Surgical interventions included flap surgery, guided tissue regeneration, and bone grafting, while non-surgical treatments comprised scaling and root planning with or without adjunctive antibiotics. The primary outcomes assessed included changes in probing depth, clinical attachment level, tooth loss, and patient-reported quality of life measures. Results: A total of 120 patients were included in the study, with 60 patients in each treatment group. The surgical group demonstrated significantly greater reductions in probing depth and gains in clinical attachment level compared to the non-surgical group (P < 0.05). Tooth loss was significantly lower in the surgical group over the 5 years (P < 0.01). Patient-reported outcomes also favored the surgical group, with improved oral health-related quality of life. However, the surgical group had a higher incidence of postoperative complications. Conclusion: This study suggests that periodontal surgery yields superior long-term outcomes in the management of aggressive periodontitis compared to non-surgical treatment.

Abstract

This work investigates the potential utilization of Cu(i) as a reducing agent for the transformation of the platinum salt K2PtCl4, resulting in the production of stable nanoparticles. The synthesized nanoparticles exhibit a bimetallic composition, incorporating copper within their final structure. This approach offers a convenient and accessible methodology for the production of bimetallic nanostructures. The catalytic properties of these novel nanomaterials have been explored in various applications, including their use as artificial metalloenzymes and in the degradation of dyes. The findings underscore the significant potential of Cu(i)-mediated reduction in the development of functional nanomaterials with diverse catalytic applications. © 2023 RSC

Abstract

we developed a massive online open course (MOOC) through co-creation between medical students and academic staff on heart semeiotics and evaluated its teaching efficiency and user satisfaction."The Heart, Its Signs and Symptoms" was a MOOC aimed to teach medical students to collect clinical history and perform physical examination of patients presenting with chest pain, dyspnea, palpitations, or syncope. It consisted of 4 interactive cases with preliminary, interim and final evaluation tests. A group of medical students, with the supervision of a multidisciplinary team of University professors residents, health management and informatics researchers developed the MOOC.Users (n = 701) registered to the online platform over 1 month. Students from 34 of the 40 Italian medical faculties participated. 96% of those who started the online course completed at least one of the 4 modules. Among users completing both the preliminary test and the final exam (n = 239, 34% of those registered to the online platform), the percentage of subjects passing the evaluation test raised s from 74% to 95% after the course. Younger age, attendance of Universities in Southern Italy, and a lower number of correct answers during the pre-test predicted a greater increase in the number of correct answers; Almost all participants who provided a feedback (n = 238) felt satisfied by the course.Partnership of medical students and academic staff was a key asset for the success of the initiative. The high rate of involvement and positive feedback from learners suggest a still unmet need for online learning resources.

18. Geographic variation in inpatient costs for Acute Myocardial Infarction care: Insights from Italy

Abstract

Geographic variations in healthcare expenditures have been widely reported within and between countries. Nevertheless, empirical evidence on the role of organizational factors and care systems in explaining these variations is still needed. This paper aims at assessing the regional differences in hospital spending for patients hospitalized for Acute Myocardial Infarction (AMI) in Tuscany and Lombardy regions (Italy), which rank high in terms of care quality and that have been, at least until 2016, characterized by quite different governance systems. Generalized linear models are performed to estimate index, 30-day and one-year hospitalization spending adjusted for baseline covariates. A two-part model is used to estimate 31–365 day expenditure. Adjusted hospital spending for AMI patients were significantly higher in Lombardy compared with Tuscany. In Lombardy, patients experienced higher re-hospitalizations in the 31–365 days and longer length of stays than in Tuscany. On the other hand, no significant regional differences in adjusted mortality rates at both acute and longer phases were found. Comparing two regional healthcare systems which mainly differ in both the reimbursement systems and the level of integration between hospital and community services provides insights into factors potentially contributing to regional variations in spending and, therefore, in areas for efficiency improvement.

25. Patient satisfaction, patients leaving hospital against medical advice and mortality in Italian university hospitals: a cross-sectional analysis

Abstract

Background: Healthcare systems are increasingly focusing on outcomes that are the endpoints of care: patient health status and patient satisfaction. The availability of patient satisfaction (PS) data has encouraged research on its relationship with other outcomes, such as mortality. In Italy, an inter-regional performance evaluation system (IRPES) provides 13 regional healthcare systems with a multidimensional assessment of appropriateness, efficiency, financial sustainability, effectiveness, and equity. For university hospitals, IRPES includes the percentage of patients leaving hospital against medical advice (PLHAMA) and mortality rates at the ward level. This paper investigates the relationship between PS and PLHAMA across and within regional healthcare systems in Italy. Secondly, PLHAMA is used as a PS proxy to investigate its relationship with mortality at the ward level in the IRPES university hospitals. Methods: PLHAMA and mortality rates were gathered from administrative data, and PS scores from patient surveys. We explored the association between PS and PLHAMA through a correlation analysis, using data for the 13 IRPES regions. We tested this relationship also at the clinical directorate level in 28 hospitals in Tuscany (5482 interviewed patients in 100 clinical directorates). Secondly, we explored the association between PLHAMA and mortality at the ward level through correlation and regression analyses, using data of 405 wards of eight clinical specialties within 24 IRPES university hospitals. Results: Lower PLHAMA rates were associated with a higher PS in both regional and clinical directorate levels. A positive association between PLHAMA and mortality was shown at the ward level for IRPES university hospitals, with different results for medical and surgical clinical specialties. Abstract

Background: The COVID-19 pandemic is a complex global public health crisis presenting clinical, organisational and system-wide challenges. Different research perspectives on health are needed in order to manage and monitor this crisis. Performance intelligence is an approach that emphasises the need for different research perspectives in supporting health systems' decision-makers to determine policies based on well-informed choices. In this paper, we present the viewpoint of the Innovative Training Network for Healthcare Performance Intelligence Professionals (HealthPros) on how performance intelligence can be used during and after the COVID-19 pandemic.Discussion: A lack of standardised information, paired with limited discussion and alignment between countries contribute to uncertainty in decision-making in all countries. Consequently, a plethora of different non-data-driven and uncoordinated approaches to address the outbreak are noted worldwide. Comparative health system research is needed to help countries shape their response models in social care, public health, primary care, hospital care and long-term care through the different phases of the pandemic. There is a need in each phase to compare context-specific bundles of measures where the impact on health outcomes can be modelled using targeted data and advanced statistical methods. Performance intelligence can be pursued to compare data, construct indicators and identify optimal strategies. Embracing a system perspective will allow countries to take coordinated strategic decisions while mitigating the risk of system collapse.A framework for the development and implementation of performance intelligence has been outlined by the HealthPros Network and is of pertinence. Health systems need better and more timely data to govern through a pandemic-induced transition period where tensions between care needs, demand and capacity are exceptionally high worldwide. Health systems are challenged to ensure essential levels of healthcare towards all patients, including those who need routine assistance.Conclusion: Performance intelligence plays an essential role as part of a broader public health strategy in guiding the decisions of health system actors on the implementation of contextualised measures to tackle COVID-19 or any future epidemic as well as their effect on the health system at large. This should be based on commonly agreed-upon standardised data and fit-for-purpose indicators, making optimal use of existing health information infrastructures. The purpose of this paper is to present a project in order to verify -without the need of knowing the distance CERN-Gran Sasso- the discovery made by the OPERA Collaboration concerning the speed of the CERN neutrinos. The project consists of two parts. A simple one and a less simple one. Both have the great advantage of being totally independent of the knowledge of the distance, ≃ 732 km, between the two Labs, CERN and LNGS, where the neutrinos are produced and detected, respectively. The "simple" version of this project is based on the high-energy horizontal cosmic muons, which traverse LVD and OPERA detectors, thus allowing to cross-calibrate the timing systems of both experiments in a way which is totally independent of the TOF measurements of CNGS. This component of the project is being studied in collaboration with the OPERA group, as the time stabilities of both experiments are needed. In fact it is since a long time that the two groups are engaged with this problem. In this paper we will present and discuss the "less simple" part which allows to establish, at the highest possible level of accuracy, if (v > c) effects really exist.

Abstract

Spatial and temporal variation in interactions between algae, limpets (Patella spp.) and mussels (Mytilus galloprovincialis) were examined in low-shore habitats south of Livorno, on the west coast of Italy (western Mediterranean), from August 1993 to February 1995. Experiments were set up to: (1) contrast the response of the length of fronds and coverage of the brown alga Cystoseira compressa to the removal of algal turfs and mussels in habitats dominated by these organisms; (2) test whether any effect of mussels on C. compressa was consistent in space (at scales ranging from 1 to 1000 m) and as a function of the presence of limpets; (3) test for the temporal consistency in any effect of limpets on the development of macroalgae in habitat dominated by mussels and (4 ) test for spatlal consistency in the temporal patterns of algal recovery in this hab~tat. Removal of the algal turfs had no effect on the length of fronds of C. compressa; no effect was observed on its coverage but this result was probably confounded by artifacts. In contrast, mussels had both positive and negatlve effects on C. compressa. The mean length of fronds of this alga was significantly reduced by the removal of mussels. this effect being particularly evident after a period of severe storms occurring soon after the initiation of the experiment. A possible mechanism to explain this could involve the stabilisation of the basal fronds of the plants, because mussels were often packed around them. Conversely, the coverage of C. compressa increased significantly after the removal of the mussels, this effect probably resulting from the lnhlbition of recruitment of the alga. The effects of mussels were consistent in space and almost independent of the presence of limpets. In contrast, the herbivores influenced the temporal changes in abundance of the coarsely-branched algae and articulated corallines, but not those of the filamentous and foliose algae or of C. compressa. Finally, no space-time interactions were found in the analyses of variance, indicating that the course of succession was consistent among areas distributed along a stretch of shore about 1.5 km in length. The results of this study suggest that the assemblage of species dominated by mussels on the rocky coasts south of Livorno was not a highly interactive system. Most of the biological processes investigated were consistent in space and/or in time.

Abstract

Governance systems which are based on assumptions of purposive-rational action have received significant criticism. For example recent quality and performance frameworks of the English NHS have tended towards incentives and sanctions, and have been critiqued in terms of both the logic on which they are run as well as a lack of evidence for their success. Yet the limitation of much of these critical appraisals is the failure to propose concrete, empirically-grounded alternatives. Thus as a means of adding to the literature, this discussion paper seeks to perform three functions. Firstly it reviews the theoretical and empirical literature around governance in the English NHS as a basis of understanding the limitations of this ‘standards and sanctions’ dominated system. Secondly, it discusses findings from research into the governance system applied in Tuscany, Italy as evidence of the effectiveness of using the reputation of professionals and departments as a basis of facilitating quality development. Implications - for the English NHS and governance more widely - are then considered. A theoretically grounded alternative to purposive-rational approaches based on a more normative oriented understanding of human action and the ‘civilising processes’ of moral obligation is accordingly outlined.
