Purpose To analyze refractive and topographic changes secondary to Descemet membrane endothelial keratoplasty (DMEK) in pseudophakic eyes with Fuchs’ endothelial dystrophy (FED). Methods Eighty-seven pseudophakic eyes of 74 patients who underwent subsequent DMEK surgery for corneal endothelial decompensation and associated visual impairment were included. Median post-operative follow-up time was 12 months (range: 3–26 months). Main outcome measures were pre- and post-operative manifest refraction, anterior and posterior corneal astigmatism, simulated keratometry (CASimK) and Q value obtained by Scheimpflug imaging. Secondary outcome measures included corrected distance visual acuity (CDVA), central corneal densitometry, central corneal thickness, corneal volume (CV), anterior chamber volume (ACV) and anterior chamber depth (ACD). Results After DMEK surgery, mean pre-operative spherical equivalent (± SD) changed from + 0.04 ± 1.73 D to + 0.37 ± 1.30 D post-operatively (p = 0.06). CDVA, proportion of emmetropic eyes, ACV and ACD increased significantly during follow-up. There was also a significant decrease in posterior corneal astigmatism, central corneal densitometry, central corneal thickness and corneal volume over time (p = 0.001). Only anterior corneal astigmatism and simulated keratometry (CASimK) remained fairly stable after DMEK. Conclusion Despite tendencies toward a hyperopic shift, changes in SE were not significant and refraction remained overall stable in pseudophakic patients undergoing DMEK for FED. Analysis of corneal parameters by Scheimpflug imaging mainly revealed changes in posterior corneal astigmatism pointing out the relevance of posterior corneal profile changes during edema resolution after DMEK.

Following this premise, the paper focused on the justification of ethical micro financing in Nigeria with consideration of experiences worldwide that Islamic micro financing is one of the fastest-growing segments in both the domestic and international financial system. This study also intends to inspire the espousal of alternative ethical finance to tie together its benefits in Nigeria. A structured questionnaire was distributed to the Bauchi metropolis; thereby, a convenience sampling technique was used. The data collected were analyzed using frequency tables, percentages, and simple tabulation. This study believed that poverty is not created by poor people. It is created by the institutions and policies which surround them. Loans offer people the chance to take initiatives in business and agriculture to make earnings that enable them to pay off debt. This paper largely recommended that the acceptance of Islamic and ethical microfinancing experiences are the foremost component of the struggle against poverty and can be used to ease the discouraging impact of the high unemployment rate among the youth in Nigeria and other Muslim countries.

Zusammenfassung Hintergrund Die Etablierung der Mikrochirurgie hat in den letzten Jahrzehnten eine enorme Verbesserung der operativen Versorgung von interdisziplinären Fällen ermöglicht. Gleichzeitig steigt hierdurch aber auch der Ressourcenaufwand der mitbehandelnden plastischen Chirurgie. Ziel der vorliegenden Arbeit war es daher, den Zusammenhang zwischen Erlössteigerung und Ressourcenaufwand der rekonstruktiven Mikrochirurgie an einem Universitätsklinikum der Maximalversorgung in der interdisziplinären Zusammenarbeit darzustellen. Methoden An einem Standort einer plastisch-chirurgischen Abteilung einer Universitätsklinik wurden in den Jahren 2018 und 2019 alle Fälle der interdisziplinären Zusammenarbeit fallbegleitend verfolgt. Aus dem Gesamtkollektiv wurden die interdisziplinären chirurgischen Fälle identifiziert und prospektiv mit Hinblick auf die stationären Behandlungstage, operativen Ressourcenaufwand und die ökonomische Entwicklung (DRG-Erlöse vor und nach mikrochirurgischer Rekonstruktion) ausgewertet. Ergebnisse In den Jahren 2018 und 2019 wurden insgesamt 68 freie mikrochirurgische Lappenplastiken in 64 interdisziplinären Fällen durchgeführt. 62 der Fälle erfüllten die Kriterien einer interdisziplinären chirurgischen Behandlung. Bei Betrachtung des Beitrages der Plastischen Chirurgie an der ökonomischen Entwicklung und dem damit verbundenen Resourcenaufwand zeigt sich eine signifikante Zunahme in allen erhobenen Parametern (p Schlussfolgerung Die rekonstruktive Mikrochirurgie führt bei interdisziplinären chirurgischen Fällen, durch Veränderung der DRG, zu einem signifikanten Anstieg des Erlöses. Allerdings lässt sich auch ein deutlicher Anstieg des Ressourcenverbrauches beobachten. Zudem sind diese zusätzlichen Kosten nicht immer adäquat in der DRG abgebildet. Dies betrifft vor allem DRGs mit hoher initialer Bewertungsrelation, die durch Eingriffe der rekonstruktiven Mikrochirurgie zum Teil nicht aufgewertet wurden. Zur Sicherstellung einer zeitgemäßen, individuellen, patientenorientierten und leitliniengerechten Patientenversorgung besteht daher dringender Bedarf an einer Anpassung des (G-)DRG-Systems an den Mehraufwand. Zudem muss bei interdisziplinären chirurgischen Fällen eine klare interne Leistungsverrechnung, gemäß dem chirurgischen Ressourcenaufwand, erfolgen.

Any misconception under Islam is considered as a misinterpretation of Islam despite the nature and disparity in understanding Islamic view scholarly and religious perspectives. The ignorance and non-religious individuals find it difficult to differentiate between renowned scholars and students ward manifested. It is recorded that scholars differ in terms of their views as experts, some speak on areas that are not their field of studies which generate faulty logic reasoning, deliberate distortion, and misinterpretations from the so-called scholars in Nigeria. Boko Haram emerged based on negative misconceptions and perceived some extreme views that are in conjunction and misleading towards Islam. To root out the insurgency from any society, there must be effective majors to be addressed, such as the root causes for the emergence, reasons for misconception, good governance and Justice within society and regulating the system of da’awa and dua’at (views preaching for and qualification of a preacher). Therefore, the paper will deliberate such issues through the writings of experts and suggest lasting solutions to such issues.

Für uns als Plastische Chirurgen und Handchirurgen ist es von zentralerBedeutung, dass wir uns als verantwortungsbewusste Menschen und Ärzte dafür einsetzen, die Übertragung von Viren zu verringern und die Ressourcen zur Behandlung von Patienten, die an der Krankheit schwer erkrankt sind, frei geben. Die vorliegende Arbeit gibt einer Übersicht zu aktuellen Entwicklungen und Erkenntnissen für das Fachgebiet Plastische Chirurgie und für die Handchirurgie.Gleichzeitig müssen wir über unsere Fachgesellschaften aktiv Kriterien und Regularien zur Versorgung von an COVID-19 erkrankten Patienten mitgestalten, den Erhalt unserer Notfallversorgung sichern und sinnvolle Exit-Strategien erarbeiten. Naturgemäß kann dabei gerade bei der rasanten Entwicklung der Erkenntnisse in der aktuellen Pandemie nur eine Momentaufnahme gegeben werden.

The paper focuses on evaluating and comparing the justification of Islamic and conventional modes of finance in Nigeria. This study also intends to inspire the adoption of alternative ethical finance to harness its benefits in Nigeria. The acclaimed one hundred billion Sukuk bond launched by the Federal Government of Nigeria is just one of the critical plus Nigeria as a nation can benefit from Islamic finance. A structured questionnaire was distributed to the respondents in the Bauchi metropolis; thereby, the convenience sampling technique was used. Results were analyzed with Chisquare employed to test the hypothesis at a confidence level of 95% (0.05), that is, confidence and significance level, respectively. This study believed that Nigerians are quick to concern about their state of the economy. Therefore it’s suggested that there is an urgent need for everyone including the students of business and finance, the product developers for the emerging industry and banking personnel to have proper knowledge of the principles of Islamic finance vis-a-vis conventional finance about the essential requirements of different modes of finance and how they can be applied to various operations and services of banks and financial institutions in the dual economy. This paper further recommended that, when Islamic finance is properly instituted and practice as alternative ethical finance in the country, and more people apply for it, Nigeria economy would stand to be rejuvenated, boosted and made to be robust, embracing the Islamic financial system will present the best way the country’s economy can witness rapid improvement.

Advances in reconstructive microsurgery have led to significant improvements in the surgical care of complex interdisciplinary cases. At the same time, however, this also increases the resource expenditure of the plastic surgeon involved. This study aimed to analyse the relationship between increase in revenue and resource expenditure in reconstructive microsurgery at a university hospital of maximum care with regard to the treatment of interdisciplinary patients. In 2018 and 2019, all cases of interdisciplinary cooperation were followed up at one location of a plastic surgery department of a university clinic. The interdisciplinary surgical cases were identified from the collective and evaluated prospectively with regard to inpatient treatment days, surgical resource expenditure and economic development (DRG before and after microsurgical reconstruction). In 2018 and 2019, a total of 68 free microsurgical flaps were performed in 64 interdisciplinary cases. 62 of these cases met the criteria for interdisciplinary surgical treatment. Considering the contribution of plastic surgery to the economic development and the associated resource expenditure, there is a significant increase in all parameters (p 0.0001). Thus, the Case Mix Index (CMI) rose by 20.2 %, inpatient treatment days by 79.1 %, the number of surgical interventions by 62.4 %, cumulative incision-suture time by 131.4 % and total surgeon hours by 75.4 %. Reconstructive microsurgical procedures lead to a significant increase in revenue in interdisciplinary surgical cases. However, a significant increase in resource consumption is observed as well. Moreover, these additional costs are not always adequately reflected in the revenue of the DRG. This especially applies to DRGs with a high initial cost weight. To ensure modern, individual, patient-oriented and guideline-compliant patient care, there is, therefore, an urgent need to adapt the (G-)DRG system to the additional resource consumption. In addition, in the case of interdisciplinary surgical cases, a clear internal cost allocation must be carried out in accordance with the surgical resource expenditure. The first case of a SARS-Cov-2 virus infection was confirmed on January 27th in Munich. For both, plastic and hand surgeons it is crucial to act responsible, minimize the transmission of the virus and aid in reasonable and adequate allocation of resources for the treatment of affected patients during this pandemia. This article aims to provide an overview over the latest developments and insights that affect plastic and hand surgeons. At the same time plastic and hand surgeons are required to participate actively in the discussion of new regulatory measures that on one hand aim to ensure a proper medical care of COVID-19 patients and on the other hand need to guarantee coverage of all other patients. Furthermore exit - strategies after the pandemia need to be discussed by our societies.
