Abstract Background Sexual assault is an important social issue that influences women’s lives. Despite a significant increase in 2018, there is no reliable information available in the Kingdom of Bahrain about female sexual assaults. The aim of this study is to investigate the pattern of reported sexual assaults among living female victims in the Kingdom of Bahrain. All medicolegal reports of sexual assaults in living female victims from 2016 to 2020 were included. Reported cases were studied for sociodemographic data of victims, assailant data, assault circumstances, and medicolegal findings and procedures. Results The prevalence of female sexual assault allegations was 7%. During the studied period, there were 272 cases of sexual assault overall. Most cases were in the age group 18–30 years (41.2%). In 58.1% of assaults, the assailant was the boyfriend or partner of the victim. The medicolegal examination took place within 2 days after assault in only 32%. Forensic examination of sexual assault victims revealed extragenital injuries in only 30.9% of cases. Sexual assaults involving mixed penetration occurred in 66.9% of victims, followed by vaginal penetration (30.1%), then anal penetration (2.9%). The final medicolegal opinion confirmed positive assault in 38.6% of allegations and negative assault in 32.4% of allegations, while allegations that have no firm medicolegal conclusion were 29%. Conclusions Despite the efforts made in Bahrain, there is an underestimated problem of sexual violence against females which needs a robust multilevel strategy including the roles of education, law, and non-governmental institutions.

Abstract Background Many viral infections can cause hearing loss due to affection of cochlear hair cells or neurogenic pathway. Although, the damage secondary to viral infections is mainly cochlear affection; auditory brainstem can be affected as well. It was predicted that SARS-COV-2 infection can similarly affect the auditory system. This study aimed to detect affection in auditory system and if present investigate the possible site of lesion (up to the level of the brain stem) in relation to COVID-19 infection. Methods This is a case control study, where the study group constituted of thirty adults, diagnosed with COVID-19 at least 2 weeks prior to testing and up to 6 months, without previous auditory complaints pre-COVID-19 or other risk factors that could affect the auditory pathway. Fifteen adult participants that were age and gender matched to the study group with no previous history of covid-19 infection constituted the control group. Audiological evaluations done to all participants were pure-tone and speech audiometry, tympanometry, transient-evoked otoacoustic emission with and without contralateral suppression and auditory brainstem response measurements. Results The study group showed significantly worse pure tone thresholds at high frequencies 4 and 8 kHz (p < 0.01), significantly worse transient-evoked otoacoustic emission signal to noise ratio at 2800 Hz and 4000 Hz (p < 0.05) and significantly lower total suppression index (p 0.05). Conclusion COVID-19 had subtle effect on cochlear basal turn, and it is shown that the auditory efferent system may also be affected, while the auditory nerve and afferent brainstem pathways seems to be spared. Moreover, the absence of the symptoms of auditory dysfunction postcovid-19 does not guarantee normal auditory functions.

Breast cancer is among the most prevalent tumors worldwide. In this study, in-situ forming implants (ISFIs) containing rosuvastatin calcium were prepared using three types of poly (D, L-lactic-co-glycolic acid) (PLGA), namely, PLGA 50/50 with ester terminal and PLGA 75/25 with ester or acid terminal. Additionally, polydimethylsiloxane (PDMS) was added in concentrations of 0, 10, 20, and 30% w/v to accelerate matrix formation. The prepared ISFIs were characterized for their rheological behaviors, rate of matrix formation, and in-vitro drug release. All the prepared formulations revealed a Newtonian flow with a matrix formation rate between 0.017 and 0.059 mm/min. Generally, increasing the concentration of PDMS increased the matrix formation rate. The prepared implants’ release efficiency values ranged between 46.39 and 89.75%. The ISFI containing PLGA 50/50 with 30% PDMS was selected for further testing, as it has the highest matrix formation rate and a promising release efficiency value. Copper-selenium nanoparticles were prepared with two different particle sizes (560 and 383 nm for CS1 and CS2, respectively) and loaded into the selected formulation to enhance its anticancer activity. The unloaded and loaded implants with rosuvastatin and copper-selenium nanoparticles were evaluated for their antibacterial activity, against Gram-positive and negative microorganisms, and anticancer efficacy, against MCF-7 and MDA-MB-231 cell lines. The results confirmed the potency of rosuvastatin calcium against cancer cells and the synergistic effect when loaded with smaller particle sizes of copper-selenium nanoparticles. This formulation holds a considerable potential for efficient breast cancer therapy.

Energy performance-based contracts (EPCs) offer a promising solution for enhancing the energy performance of buildings, which is an overarching step towards achieving Net Zero Carbon Buildings, addressing climate change and improving occupants’ comfort. Despite their potential, their execution is constrained by difficulties that hinder their diffusion in the architecture, engineering, construction, and operation industry. Notably, the Measurement and Verification process is considered a significant impediment due to data sharing, storage, and security challenges. Nevertheless, there have been minimal efforts to analyze research conducted in this field systematically. A systematic analysis of 113 identified journal articles was conducted to fill this gap. A paucity of research tackling the utilization of digital technologies to enhance the implementation of EPCs was found. Consequently, this article proposes a framework integrating Digital Twin and Blockchain technologies to provide an enhanced EPC execution environment. Digital Twin technology leverages the system by monitoring and evaluating energy performance in real-time, predicting future performance, and facilitating informed decisions. Blockchain technology ensures the integrity, transparency, and accountability of information. Moreover, a private Blockchain infrastructure was originally introduced in the framework to eliminate high transaction costs related to on-chain storage and potential concerns regarding the confidentiality of information in open distributed ledgers.

The complement system is a fundamental part of the innate immune system that plays a key role in the battle of the human body against invading pathogens. Through its three pathways, represented by the classical, alternative, and lectin pathways, the complement system forms a tightly regulated network of soluble proteins, membrane-expressed receptors, and regulators with versatile protective and killing mechanisms. However, ingenious pathogens have developed strategies over the years to protect themselves from this complex part of the immune system. This review briefly discusses the sequence of the complement activation pathways. Then, we present a comprehensive updated overview of how the major four pathogenic groups, namely, bacteria, viruses, fungi, and parasites, control, modulate, and block the complement attacks at different steps of the complement cascade. We shed more light on the ability of those pathogens to deploy more than one mechanism to tackle the complement system in their path to establish infection within the human host.

Managed aquifer recharge (MAR) is considered an innovative method for storing water in the subsurface. In this work, multi-criteria decision analysis (MCDA) was used to delineate potential groundwater recharge areas for MAR implementation in Western Delta using reclaimed wastewater. By employing geographical information systems (GIS) and pairwise comparison matrix (PCM), a modified approach was utilized for the development of the suitability map by capturing the interlinkages between a specified MAR technique (spreading methods) and MAR suitability mapping processes. The developed approach was created with a range of constraining and factorial considerations. Based on the findings, MAR potential recharge zones included four main suitability classes. The presence of high-suitability areas was mainly delineated in the northeast part, particularly around the left side of the Nile River valley. Areas of low suitability were located around the west-north side where the hydrological criterion seems to hinder the implementation due to the low productivity of the hydrogeologic layer. The developed methodology reflected the importance of specific determining factors (i.e., slope and depth to the water table) that govern the successful implementation of infiltration basins and maximize the benefits from soil aquifer treatments effects when taken into account with other hydrogeological and socio-economic variables. Introduction: Aluminium (Al) is accumulated in the brain causing neurotoxicity and neurodegenerative disease like Alzheimer's disease (AD), multiple sclerosis, autism and epilepsy. Hence, attenuation of Al-induced neurotoxicity has become a “hot topic“ in looking for an intervention that slow down the progression of neurodegenerative diseases.Objective: Our study aims to introduce a new strategy for hampering aluminum chloride (AlCl3)-induced neurotoxicity using a combination of sesamol with the probiotic bacteria; Lactobacillus rhamnosus (L. rhamnosus) and also to test their possible ameliorative effects on AlCl3-induced hepatotoxicity.Methods: Sprague-Dawley male rats were randomly divided into five groups (n = 10/group) which are control, AlCl3, AlCl3 + Sesamol, AlCl3 + L. rhamnosus and AlCl3 + Sesamol + L. rhamnosus. We surveilled the behavioral, biochemical, and histopathological alterations centrally in the brain and peripherally in liver.Results: This work revealed that the combined therapy of sesamol and L. rhamnosus produced marked reduction in brain amyloid-β, p-tau, GSK-3β, inflammatory and apoptotic biomarkers, along with marked elevation in brain free β-catenin and Wnt3a, compared to AlCl3-intoxicated rats. Also, the combined therapy exerted pronounced reduction in hepatic expressions of JAK-2/STAT-3, inflammatory (TNF-α, IL-6, NF-κB), fibrotic (MMP-2, TIMP-1, α-SMA) and apoptotic markers, (caspase-3), together with marked elevation in hepatic PPAR-γ expression, compared to AlCl3 -intoxicated rats. Behavioral and histopathological assessments substantiated the efficiency of this combined regimen in halting the effect of neurotoxicity.Discussion: Probiotics can be used as an add-on therapy with sesamol ameliorate AlCl3 -mediated neurotoxicity and hepatotoxicity.

Organophosphorus pesticides (OP) are used extensively in many arenas including agriculture and industry leading to humans and agroecosystems disorders. Malathion is one of the OP that are used in agriculture to control pest and protect crops. Also, they harm non-target organisms and affect cruelly water sources, air, and soil quality. The present study aimed to isolate and identify a potent bacterial isolate capable of degrading malathion. Bacterial strain that isolated from Al Fayoum governorate, Egypt exhibited high efficiency for malathion biodegradation. Biodegradation process using minimal salt medium (MSM) supplemented with different malathion concentrations indicated that the bacterium was able to degrade and use malathion as a sole carbon source up to 700 mg/l at 37°C.The potent strain that exhibited biodegradation potential was identified as Bacillus sp. FYM31 and deposited into GenBank with the accession number OK325597. HPLC proved the effectiveness of malathion removal by Bacillus sp. FYM31 after 12 days of incubation to the level of 70.1% malathion (700 mg/l) degradation. Organophosphorus hydrolase (opd) gene was detected in the potent Bacillus sp. FYM31 strain. Due to the widespread usage of malathion in Egypt's agricultural areas, Bacillus sp. FYM31 can help bio-remediate the polluted areas.

The goal of this study is to compare between the blind technique and US guided technique in insertion of the LMA and confirming its position. A 102 patients who underwent elective surgery were randomly selected and grouped into 2 groups, 51 in each group. Group B: using the blind technique in positioning of the LMA and confirming its position by the fiberoptic score and group U: using the US in positioning the LMA and confirming its position by the fiberoptic score The laryngeal mask insertion time for 1st group was significantly shorter than that for the 2nd group. The fiberoptic glottic view scores for 2nd group was significantly better than that for the ones for 1st group. The side effects and complications had no statistical difference between the two groups.

Both thymoquinone (TQ) and thymol (T) have been proved to possess a positive impact on human health. In this research, we aimed to investigate the effect of these compounds separately and together on the Attention-deficit/hyperactivity disorder (ADHD)-like behavior induced by monosodium glutamate (MSG) in rats. Forty male, Spargue Dawley rat pups (postnatal day 21), were randomly allocated into five groups: Normal saline (NS), MSG, MSG+TQ, MSG+T, and MSG+TQ+T. MSG (0.4 mg/kg/day), TQ (10 mg/kg/day) and T (30 mg/kg/day) were orally administered for 8 weeks. The behavioral tests proved that rats treated with TQ and/or T showed improved locomotor, attention and cognitive functions compared to the MSG group with more pronounced effect displayed with their combination. All treated groups showed improvement in MSG-induced aberrations in brain levels of GSH, IL-1β, TNF-α, GFAP, glutamate, calcium, dopamine, norepinephrine, Wnt3a, β-Catenin and BDNF. TQ and/or T treatment also enhanced the mRNA expression of Nrf2, HO-1 and Bcl2 while reducing the protein expression of TLR4, NFκB, NLRP3, caspase 1, Bax, AIF and GSK3β as compared to the MSG group. However, the combined therapy showed more significant effects in all measured parameters. All of these findings were further confirmed by the histopathological examinations. Current results concluded that the combined therapy of TQ and T had higher protective effects than their individual supplementations against MSG-induced ADHD-like behavior in rats.

Abstract Background In children with spastic diplegia, efficient use of the upper extremity especially the hands play an important role in participation in daily living. Hands can perform heavy activities and also enable to perform extremely gentle, skillful, and precise activities. Inadequate hand strength and fine motor skills may lead to functional limitations. This study was planned to investigate the pinch grip strength and fine manual control in children with spastic diplegic cerebral palsy and to determine if there is a relation between them. Thirty children with diplegic cerebral palsy and 30 normal developed children from both sexes between 5 and 10 years old were included in this study as diplegic and control groups. For all children, pinch grip strength measured by Baseline Mechanical Pinch Gauge and Bruininks-Oseretsky Test of Motor Proficiency, Second Edition used to evaluate fine manual control. Results Children with diplegic cerebral palsy have significant impairment in the pinch grip strength (tip, tripod, and key) and the fine manual control (fine motor precision and integration) compared to their healthy peers of the same age. There was large positive significant correlation (r > 0.5, p < 0.05) between tip and tripod pinch strength, and fine motor precision and medium positive significant correlation (r = 0.47, p < 0.05) between key pinch and fine motor precision in children with spastic diplegia. Also, there was large positive significant correlation between tip pinch and fine motor integration (r = 0.54, p < 0.05). The correlations are small and medium positive between tripod and key pinch strength, and fine motor integration respectively but they are not statistically significant. Conclusions Pinch grip strength and fine manual skills are affected in children with spastic diplegia. Also, there is a significant correlation between the pinch strength and activities require precise control of the hand.

Tourniquet is a well-established model of hind limb ischemia–reperfusion (HLI/R) in rats. Nevertheless, measures should be taken to alleviate the expected injury from ischemia/ reperfusion (I/R). In the present study, 30 adult male Sprague-Dawley rats were randomly divided into 5 groups (n = 6): control, HLI/R, HLI/R given candesartan (1 mg/kg, P.O); HLI/R given Coenzyme Q10 (CoQ10) (10 mg/kg, P.O); HLI/R given candesartan (0.5 mg/kg) and CoQ10 (5 mg/kg). The drugs were administered for 7 days starting one hour after reperfusion. Candesartan and CoQ10 as well as their combination suppressed gastrocnemius content of angiotensin II while they raised angiotensin-converting enzyme 2 (ACE2) activity, angiotensin (1–7) expression, and Mas receptor mRNA level. Consequently, candesartan and/or CoQ10 reversed the oxidative stress and inflammatory changes that occurred following HLI/R as demonstrated by the rise of SOD activity and the decline of MDA, TNF-α, and IL-6 skeletal muscle content. Additionally, candesartan and/or CoQ10 diminished gastrocnemius active caspase-3 level and phospho-p38 MAPK protein expression. Our study proved that CoQ10 enhanced the beneficial effect of candesartan in a model of tourniquet-induced HLI/R by affecting classical and non-classical renin-angiotensin system (RAS) pathway. To our knowledge, this is the first study showing the impact of CoQ10 on skeletal muscle RAS in rats.

This paper introduces a new concept for water security, the Irrigation Water Security Quality-based Index. Through this concept, the irrigation water security is presented based on quantity and quality. The effect of water quality parameters on agriculture productivity, soil deterioration, and public health was evaluated. The probability of harm was used in the risk analysis and assessment. The developed index was applied to evaluate the Egyptian irrigation system, 10% reduction in the Nile’s supply, and four adaptation scenarios as an illustrative example. Drainage water reuse is adopted in Egypt to provide a sufficient supply and improve the water security. However, this improved water security doesn’t consider the water’s quality. The current Egyptian condition and 10% Nile reduction showed water insecurity. Expanding drainage and sewage water treatment improved the developed index to high security levels. This highlights the importance of drainage water treatment in attaining a sustainable high water security level.

MRSA colonizing skin, mucous membranes, and the anterior nares without clinical symptoms is termed “colonizing MRSA”. Upon manifestation of clinical symptoms, it is termed “infectious MRSA”. Here, we characterize and differentiate colonizing and infectious MRSA, and analyze the phenotypic-genotypic and antibiotic susceptibility correlations.Methodology: Clinical MRSA isolates were recovered from intensive care units (ICUs) of two major Egyptian hospitals and their biofilm formation ability was tested. Antibiograms against 16 antibiotics were determined, in addition to the minimum inhibitory concentrations (MICs) of vancomycin and linezolid. The entire collection was typed by enterobacterial repetitive intergenic consensus (ERIC)-PCR, as well as multi-locus sequence typing (MLST). Representative resistance and virulence genes were detected by PCR amplification.Results: Forty-nine isolates were confirmed as MRSA, of which 30 isolates were infectious and 19 were colonizing. Versatile resistance patterns were observed in both groups of isolates. We report a higher tendency for biofilm-formation and borderline minimum inhibitory concentrations among infectious isolates. A Positive antibiotic correlation was observed between susceptibility to protein synthesis inhibitors and cell wall inhibitors. Positive correlations were observed between isolation site and rifampicin resistance: nasal samples were enriched in rifampicin-resistant isolates, while urine and blood samples were enriched in susceptible ones. Furthermore, biofilm formation ability was slightly associated with amikacin resistance, and an association between teicoplanin resistance and the presence of the Panton-Valentine leukocidin gene was the only significant phenotype-genotype correlation observed. Finally, ERIC typing and MLST had congruent results.Conclusion: Linezolid and vancomycin are still the most convenient choice for MRSA treatment. ERIC PCR and MLST show promising typing combination that could be easily used periodically for tracking the genotypic changes of MRSA, especially within the healthcare facilities. The aim of this study was to investigate the influence of replacing egg yolk with trehalose in Tris-citric-yolk extender on post-thawing of Barki and Rahmani ram spermatozoa parameters namely, progressive motility, viability, abnormalities, membrane integrity, and acrosome integrity. Fifteen ejaculates from 3 rams presenting each breed were collected by using the artificial vagina. Accepted semen samples were pooled and kept under 37°C then were immediately distributed on extenders with replaced egg yolk levels of 5, 4, 3, 2, 1and 0 % by respective levels 0, 25, 50, 75, 100, and 125 mM of trehalose. After an equilibration period, cooled semen was aspirated into 0.5mL straws and were frozen and stored in the liquid nitrogen. After 24 h, straws were thawed in a 37°C water bath for the 60s. Results revealed no significant differences between the two studied breeds of ram for all post-thawing spermatozoa parameters. The extender containing 50 mM trehalose and 3% egg yolk gave the highest post-thawing sperm motility and membrane integrity (41.43% and 44.47 %), while the lowest values (34.29 % and 37.40 %) were obtained from the extender containing 125 mM trehalose and 0% EY and was highly significant in total sperm abnormalities. Post-thawing sperm viability and acrosome integrity were not significantly affected by substituting different levels of trehalose for egg yolk. In conclusion, reducing egg yolk dawn to 1 % versus using high concentrations of trehalose (up to 100 mM) did not affect sperm characteristics, but complete elimination of egg yolk from the extender reduced post-thawing characteristics.

Loss of excess water through free subsurface drainage networks is a major cause of inefficiency in Egypt's irrigation system, where the drainage water flows continuously and quickly with nutrients from the soil profile. This research aims at evaluating the impact of controlled drainage and irrigation scheduling on applied irrigation water, soil salinity and crop yield. Actual field data have been collected for growing seasons 2015 and 2016 in El-Baradi area in the Nile Delta. DRIANMOD-S model was used to simulate irrigation and drainage management practices for wheat and maize crops over 9 years. The simulation showed that, controlled drainage system increases the average relative yield of maize crop 6% compared to conventional drainage system, with 14% reduction in irrigation water.Using evenly irrigation gifts with decreasing 14% of the applied irrigation water increased the relative yield of wheat crop by 2.0 and 9.4% in conventional and controlled drainage systems, respectively. While the average relative yield for maize crop increased by 8.6% for conventional drainage system and decreased by 10.2% in controlled drainage system, the loss in yield for both systems was attributed mainly to salinity stress.
