慢性肝疾患にみられる耐糖能異常の機序を追求する一環として合併する膵内外分泌機能障害の実態を明らかにし,耐糖能との関連性を検討した.慢性肝疾患101例にPancreozymin secretin試験とブドウ糖負荷試験を行い,大多数の例で糖負荷後の血中インスリン反応を,一部の例ではアルギニン静注負荷後の血中インスリン,グルカゴン動態も検討した.膵外分泌機能障害を呈した例は非硬変群の40%,肝硬変群の55%に達したが,その大多数は軽度障害であり,この段階では耐糖能や内分泌機能に対する影響は認められなかった.中等度以上の外分泌機能障害は非硬変の大酒家で多くみられ,耐糖能異常やインスリン低反応,グルカゴン低反応を伴う例が多かった.慢性肝疾患全体としては膵障害は耐糖能異常の発現に著しく関与しているとは言えないが,大酒歴を有する非硬変例で耐糖能低下がみられる時には膵障害の合併も考えて,その検索をすることが大切である.

Evidence is present that plasma contains non-specific factors which interfere with the 30K glucagon assays. A correction can be made for these interference factors because the factors can be quantitated following absorption of glucagon with charcoal-dextran. Using a correction factor the range of fasting plasma immunoreactive glucagon (IRG) in 12 totally pancreatectomized patients was below detectable limit. Fasting levels of IRG were determined on the plasma from 25 liver cirrhotics complicated by abnormal GTT, 13 pancreatic diabetics with chronic calcified pancreatitis (CCP), 25 adult-onset primary diabetics and 25 healthy subjects. When all samples were measured using no correction factor, the mean levels of IRG were 358 +/- 24 (mean +/- SE), 170 +/- 26, 178 +/- 16 and 178 +/- 7 pg/ml, respectively. Using a correction factor the mean level of IRG were 177 +/- 26, 16 +/- 4, 39 +/- 9 and 20 +/- 4 pg/ml, respectively. The mean values of the interference factor were not significantly different among all five groups. During an arginine infusion the interference factor remained unchanged despite an increase in IRG. It is available but not always necessary to apply a correction factor for 30K glucagon radioimmunoassay.

The general profile of pain in the evolution of pancreatitis was analysed in relation to exocrine and endo:rine pancreatic function in 127 patients with primary chronic pancreatitis followed up over 3 years. Pain decreased or disappeared in 67.8% and 55.9% of calcifying and non-calcifying pancreatitis, respectively. While pancreatic exocrine function remained abnormal in spite of an improvement of pain in 72% of 18 patients with calcifying pancreatitis, it improved with the amelioration of pain in 64.0% of 25 patients with non-calcifying pancreatitis during the follow-up period. Alcohol abstinence seems most important for pain relief in patients with non-calcifying pancreatitis but not calcifying pancreatitis. Changes in glucose tolerance test were not related with those in pain. In calcifying pancreatitis, 69.2% of patients with calcifying pancreatitis were diabetic or became so, while 66.7% of patients with non-calcifying pancreatitis remained non-diabetic during the observation period.

In an attempt to study pancreatic A cell function at an early stage of pancreatic diabetes secondary to chronic pancreatitis, both intravenous glucose (0.5 g/kg) and insulin (0.5 U/kg) tolerance tests were performed on seven dogs with pancreatolithiasis and seven normal control dogs. Experimental pancreatolithiasis was produced 12 months after incomplete ligation of the greater pancreatic duct. Impaired glucose tolerance and reduced plasma insulin response to an intravenous glucose load were observed in the calcified group. Pancreatic release of glucagon to insulin-induced hypoglycaemia was diminished, but suppressive response of the hormone to glucose-induced hyperglycaemia was well preserved. These data suggest that the A cell response to hypoglycaemia may be impaired earlier than that to hyperglycaemia in pancreatic diabetes secondary to pancreatolithiasis.
