Rheumatoid arthritis (RA) is a systemic inflammatory autoimmune disease of unknown etiology. Current knowledge supports that chronic inflammation represents a crucial risk factor without prior development of atherosclerosis. Thus, the epidemiological findings of patients with RA have demonstrated an expressive increase in cardiovascular morbidity and mortality. In addition, these patients present a higher prevalence of coronary risk factors and, when associated with poor prognostic factors, increase the risk of ischemic heart disease.

Fundação de Amparo à Pesquisa do Estado de São Paulo (FAPESP) Coordenação de Aperfeiçoamento de Pessoal de Nível Superior (CAPES) OBJETIVO: Avaliar a efetividade da educação nutricional em escolas na redução ou prevenção de sobrepeso e obesidade em crianças e adolescentes. FONTES DOS DADOS: Busca sistemática em 14 bases de dados e cinco revisões sistemáticas por ensaios controlados randomizados realizados em escolas com o objetivo de reduzir ou prevenir o sobrepeso em crianças e adolescentes. O índice de massa corporal e o consumo de frutas e verduras foram utilizados como desfechos primário e secundário, respectivamente. Não houve restrição de data de publicação ou idioma, exceto para idiomas com logogramas. Excluímos estudos em populações com distúrbios alimentares, dislipidemia, diabetes e deficiência física ou mental, e que utilizassem medicamentos ou suplementos alimentares. A avaliação por título e resumo e a avaliação de qualidade foram realizadas de forma independente por dois pesquisadores. Utilizamos as orientações do Centre for Reviews and Dissemination para a elaboração de artigos de revisão em saúde e o programa EPPI-Reviewer 3. SÍNTESE DOS DADOS: Das 4.809 referências inicialmente encontradas, 24 artigos preencheram os critérios de inclusão. Os dados extraídos apresentam evidências de efeitos antropométricos positivos e de aumento no consumo de frutas e verduras. As intervenções com as seguintes características demonstraram ser efetivas: duração > 1 ano, introdução como atividade regular da escola, envolvimento dos pais, introdução da educação nutricional no currículo regular e fornecimento de frutas e verduras pelos serviços de alimentação da escola. CONCLUSÃO: As intervenções para reduzir o sobrepeso e a obesidade e aumentar o consumo de frutas e verduras foram efetivas nos estudos bem conduzidos. OBJECTIVE: To evaluate the effectiveness of school-based nutrition education in reducing or preventing overweight and obesity in children and adolescents. SOURCES: Systematic search in 14 databases and five systematic reviews for randomized controlled trials conducted in schools to reduce or prevent overweight in children and adolescents. Body mass index and fruit and vegetable intake were used as primary and secondary measures of outcome, respectively. There was no restriction by date of publication or language, except for languages with structured logograms. We excluded studies on specific populations presenting eating disorders, dyslipidemia, diabetes, and physical or mental disabilities, as well as studies that used drugs or food supplements as components of the intervention. The assessment by title and abstract and the quality assessment were performed independently by two researchers. We used the Centre for Reviews and Dissemination's guidance for undertaking reviews in health care and the software EPPI-Reviewer 3. SUMMARY OF THE FINDINGS: From the initially retrieved 4,809 references, 24 articles met the inclusion criteria. The extracted data show that there is evidence of positive effects on anthropometry and of increase in fruit and vegetable consumption. Characteristics of the interventions that demonstrated effectiveness are: duration > 1 year, introduction into the regular activities of the school, parental involvement, introduction of nutrition education into the regular curriculum, and provision of fruits and vegetables by school food services. CONCLUSION: Interventions in schools to reduce overweight and obesity, as well as to increase fruits and vegetable consumption, have demonstrated effectiveness in the best-conducted studies. OBJECTIVE: To investigate the presence of psychiatric disorders in patients with chest pain not responsive to treatment. METHOD: We evaluated 18 patients judged by their physicians to have a chest pain not responsive to usual treatment, which included anti-pain medicines and investigation and treatment of possible etiological causes such as coronary artery disease, and gastroesophageal reflux disease. A psychiatrist interviewed the patients using the Present State Examination and made the diagnosis based on the DSM-III-R criteria. Current major depression was diagnosed in 6 (30%) patients, somatization in 1 (6%) and panic disorder in 1 (6%) patient. Seven patients were receiving tricyclics antidepressant with doses >; 75 mg/day. DISCUSSION: Patients were receiving doses of tricyclics antidepressants efficacious for pain but not for major depression. It is possible that the low dose of antidepressants used to treat pain may partially ameliorate depressive symptoms, making the appropriate diagnosis and treatment of major depression even more difficult, consequently contributing to the persistence of pain and other complains. Considering the wide alternatives to effectively treat depression, a focus on detection and treatment of major depression in patients with chest pain is warranted by clinicians and researchers. OBJETIVO: Investigar a presença de transtornos psiquiátricos em pacientes com dor torácica de origem não cardíaca que não respondem aos tratamentos regulares. MÉTODO: Dezoito pacientes com dor torácica sem origem cardíaca e considerados por seus clínicos como não respondentes aos tratamentos regulares instituídos foram avaliados por um psiquiatra treinado. As entrevistas foram realizadas com base no Present State Examination e os diagnósticos psiquiá-tricos, de acordo com os critérios do Manual de Diagnóstico e Estatística da Associação Psiquiátrica Americana, 3ª Edição Revisada (DSM-III-R). RESULTADOS: Depressão maior no momento da avaliação foi diagnosticada em 6 (30%) pacientes, somatização em 1 (6%) e transtorno do pânico em 1 (6%) paciente. Sete pacientes estavam recebendo antidepressivos tricíclicos com doses < 75 mg/dia. CONCLUSÕES: A baixa dose de ADTs usadas para o tratamento da dor nesses pacientes pode ter melhorado parcialmente os sintomas depressivos, tornando mais difíceis o diagnóstico e o tratamento apropriado(s) da depressão e, assim, contribuindo para a persistência da dor e outras queixas. As futuras pesquisas deverão focalizar a eficácia do tratamento da depressão nesses pacientes e o impacto deste no alívio da dor torácica não cardíaca.

Health education practices are central to preventing heart disease. The great challenge remains how to promote the adoption of healthy behaviors that can reduce risk factors for heart disease. In São Paulo, Brazil, the Healthy Lifestyle Multiplier Students program is based on studies that train older students ("multipliers") as peer educators for younger students. These students influence each others' cultural development and psychosocial growth, encourage one another to diversify their social relationships, and transform teaching-learning experiences. This intervention focused on physical activity, healthy diet, and the health effects of cigarette and alcohol consumption.

Evidence based practice is the use of the best scientific evidence to support the clinical decision making. The identification of the best evidence requires the construction of an appropriate research question and review of the literature. This article describes the use of the PICO strategy for the construction of the research question and bibliographical search. La práctica basada en evidencias permite la elección de la mejor evidencia científica para subsidiar la toma de decisión clínica. Para lo cual, se requiere de una adecuada construcción de la pregunta de investigación y de la revisión de la literatura. Este artículo describe el uso de la estrategia PICO para la construcción de la pregunta de investigación y la búsqueda bibliográfica. Prática baseada em evidências é a utilização da melhor evidência científica para subsidiar a tomada de decisão clínica. Identificar a melhor evidência requer adequada construção da pergunta de pesquisa e de revisão da literatura e este artigo descreve o uso da estratégia PICO para a construção da pergunta de pesquisa e busca bibliográfica.

No presente artigo discutiremos como avaliar criticamente as informações obtidas. Para tanto, serão revistos conceitos que a epidemiologia clínica tem colocado à disposição da prática clínica baseada em evidências. A pesquisa clínica busca desenvolver meios diagnósticos e terapêuticos medindo associações, ou relações de causa/efeito, entre um fator em estudo e um desfecho clínico. Onde o fator em estudo é o sintoma, sinal propedêutico, teste laboratorial, exame de imagem, ou tratamento. E o desfecho clínico é o reconhecimento da doença, cura, morte ou limitação. É um erro básico de interpretação tomar os marcadores intermediários como desfechos clínicos. Os desenhos de estudo na área clínica apresentam quatro enfoques principais: diagnóstico, prognóstico, terapêutico, dano/etiológico. Experimentos animais, estudos anatômicos, fisiológicos, genéticos, farmacológicos, análises econômicas são necessários à formação básica do médico, no entanto, não colaboram diretamente para a tomada de decisão clínica. Os desenhos de estudo são classificados de acordo com a presença de grupo controle, seguimento dos pacientes ao longo do tempo e presença de intervenção dos pesquisadores no sentido de modificar a evolução dos pacientes. A hierarquia da força de evidência científica está fundamentada nestas características e na susceptibilidade aos vícios decorrentes do tipo desenho de estudo. Revisões sistemáticas são consideradas de maior força do que os estudos primários que lhe deram origem, e quando envolvem ensaios clínicos randomizados são colocadas no topo da hierarquia. Desde 1998 é crescente a proporção de "diretrizes clínicas baseadas em evidências" quando comparada às revisões sistemáticas, ou às publicações sobre diretrizes em geral, embora as primeiras sejam ainda em número reduzido. A avaliação crítica do artigo deve responder uma questão clínica, e ser consistente quanto à adequação do desenho de estudo e ao controle dos vícios. Concluímos que se deve oferecer atualização metodológica aos profissionais interessados, e colocar a informação já avaliada à disposição de todos, elaborando e divulgando diretrizes baseadas em evidências. Evidence based health care begins with a clinical question and the search on data bases to retrieve the relevant information, that was the issue of two preceding articles of this series. At present it will be discussed how to critically appraise the medical literature using the clinical epidemiological methodology. Clinical research aims to develop diagnostic and therapeutic procedures measuring association and causality between the exposure and outcome. In this case the exposures are signs, symptoms, laboratorial or image exam, and therapy intervention. It is a mistake to take surrogate end-points instead of clinical outcomes. The main types of clinical study design are diagnostic, prognostic, therapeutic and harm/etiology. Experimental, physiologic and animal studies are useful for the medical undergraduate education, but do not contribute with clinical decisions. The study designs are classified according with the presence of a control group, patient's follow-up, and therapy interventions. The evidence hierarchy was done by the previous characteristics and the presence of systematic bias. Systematic reviews are stronger than the primary observational studies and are on the top when they revised randomized clinical trial. Since 1998 the proportion of evidence based practice guidelines was increasing compared with systematic reviews or other types of practice guidelines, although the former still are in a few numbers. The article critical appraisal must answer the clinical question, and need to have consistent study design and bias under control. In conclusion we ought to offer methodological actualization to interested physicians and put the information already critically assessed on evidence-based practice guidelines.

Background: Ice-pack is widely used for alleviating postpartum perineal pain sustained after birth related perineal trauma. However, it lacks robust evidence on timing and frequency of applications, to ensure the effective and safe use of this therapy. Aims: To evaluate if a 10 min ice-pack application relieved postpartum perineal pain and if the analgesic effect was maintained for up to 2 h. Methods: A randomised controlled trial conducted from December 2012 to February 2013 with 69 primiparous women ≥18 years old, 6–24 h postpartum, with perineal pain ≥ 3, who had not received anti-inflammatory medication or analgesics after childbirth, who were randomised to a single ice-pack application on the perineum for 10 min or standard care. The primary and secondary outcomes were a reduction ≥ 30% in perineal pain intensity, immediately after the application and the maintenance of the analgesic effect for up to 2 h, respectively. Findings: Immediately post-intervention, the proportion of women whose perineal pain decreased ≥ 30%was significantly higher in the experimental group. Within 2 h, there was no significant difference in the pain levels in both groups. Within 2 h, for 61.9% and 89.3% of women in the experimental and control group, respectively, the perineal pain levels remained unchanged. For the remaining participants, perineal pain was increasing after an average time of 1 h 45 min and 1 h 56 min for the experimental and control groups, respectively. Conclusion: By applying an ice-pack for 10 min to the perineum, effective pain relief is achieved, that is maintained for between 1 h 45 min and 2 h. Refereed/Peer-reviewed
