3. Beyond translations, perspectives for researchers to consider to enhance comprehension during consent processes for health research in sub-saharan Africa: a scoping review

Abstract

Abstract Background Literature on issues relating to comprehension during the process of obtaining informed consent (IC) has largely focused on the challenges potential participants can face in understanding the IC documents, and the strategies used to enhance comprehension of those documents. In this review, we set out to describe the factors that have an impact on comprehension and the strategies used to enhance the IC process in sub-Saharan African countries. Methods From November 2021 to January 2022, we conducted a literature search using a PRISMA tool. We searched electronic databases (PubMed, EMBASE, EBSCOHOST) to identify relevant peer reviewed studies. We then reviewed the references of these articles to find additional literature that might have been missed through the initial search. We were particularly interested in full text articles in English that focused on the IC process in SSA published between 2006 and 2020. We included systematic reviews, and studies from Western and Asian countries that included data about SSA. We excluded articles that focused on medical interventions and studies that did not require IC. Results Out of the 50 studies included most were multi-country (n = 13) followed by single country studies in South Africa (n = 12); Kenya, Tanzania, Uganda (n = 5) each; Gambia, Ghana and Nigeria (n = 2)each ; and one each for Botswana, Malawi, Mali, Mozambique. We identified three areas of focus: (1) socio-cultural factors affecting IC; (2) gaps in the ethical and legal frameworks guiding the IC process; and (3) strategies used to improve participants’ understanding of IC. Conclusion Our review showed wide recognition that the process of achieving IC in SSA is inherently challenging, and there are limitations in the strategies aimed at improving comprehension in IC. We suggest that there is a need for greater flexibility and negotiation with communities to ensure that the approach to IC is suited to the diverse socio-cultural contexts. We propose moving beyond the literal translations and technical language to understanding IC comprehension from the participants’ perspectives and the researchers’ views, while examining contextual factors that impact the IC process.

Abstract

Background: Understanding disruptive behaviours from the perspective of radiographers is important, as this professional group uses hazardous radiation in the execution of their duties, making patient safety of utmost concern.Objective: Determine the disruptive behaviours involving radiographers at central hospitals in Harare Metropolitan Province, Zimbabwe.Methods: A descriptive cross-sectional quantitative study was carried out at central hospitals in Harare Metropolitan Province, Zimbabwe, where 100 radiographers were randomly selected.Results: Overall, 83% of radiographers had been exposed to an incident of DB in the preceding 12 months. Reported types of disruptive behaviour included: Verbal abuse (81%), sexual abuse (21%) and physical abuse (4%). Of the 21 radiographers that suffered sexual abuse, the majority 71 % (n=15) were female while 29% (n=6) were males. Prevalence odds ratio revealed that female radiographers were 1.8 times more likely than their male counterparts to be victims of the workplace sexual abuse (95% C.I.: 0 – 3.04). A significant 69% were abused by patients and their families/escorts, p=.001.Conclusion: More than 8 out of 10 radiographers in this study were exposed to disruptive behaviours, mostly from the patients and patient’s family or escorts. A framework to increases awareness and address these behaviours is recommended.Keywords: Disruptive behaviour; radiographers; Zimbabwe.

22. South African Zulu Widows in a Time of Poverty and Social Change

Abstract

Interviews were carried out with 16 South African Zulu widows. Much of what the widows had to say seemed like what one might hear from widows in economically developed countries, but there were also striking differences. All the widows lived in poverty, and for some their grief seemed much more about the poverty than about the husband's death. Most widows observed a year of traditional "ukuzila" mourning practices, which made them a threat to others and which is difficult to carry out in the social upheaval of modern South Africa. Widowhood was in some cases a struggle with witchcraft--as cause of the death or as an accusation directed at the widow.

26. The ethical-legal requirements for adolescent self-consent to research in sub-Saharan Africa: A scoping review

Abstract

Support for the enrolment of adolescents in research has been constrained by uncertainties in parental involvement, and the lack of clarity in the ethical and legal frameworks. We conducted a scoping review to examine articles that explored the opinion of scholars on the question of adolescent consent and conditions for parental waivers in research in sub-Saharan Africa (SSA). Guided by the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) tool, we searched electronic databases (PubMed, EMBASSE, EBSCOHOST) and also reviewed the references of articles identified for additional relevant literature. We included full text English articles focusing on adolescent consent and parental waivers in SSA that were published between 2004 and 2020. We excluded studies focusing on healthcare, theses, and reviews. We reviewed a total of 21 publications from South Africa (n = 12), Kenya (n = 4) and Botswana, Malawi, Nigeria, Uganda and Zimbabwe (n = 1 each). We identified four broad thematic issues: the current position regarding parental waivers and self-consent; parental involvement in the consent process; the role of community approval or consent when adolescent self-consent approaches were used; and complexities and ambiguities in legal requirements and ethical guidelines on adolescent consent. Our findings show inconsistencies and ambiguities in the existing legal and ethical frameworks within and across different countries, and underscore the need for consistent and clearer guidance on parental waivers and adolescent self-consent. Harmonization of the legal and ethical frameworks taking into account varying contexts is critically important to ensure research on adolescents in SSA meets adolescents' specific unmet needs.

29. Participant understanding of informed consent in a multidisease community-based health screening and biobank platform in rural South Africa

Abstract

BACKGROUND: In low- and middle-income settings, obtaining informed consent for biobanking may be complicated by socio-economic vulnerability and context-specific power dynamics. We explored participants experiences and perceptions of the research objectives in a community-based multidisease screening and biospecimen collection platform in rural KwaZulu-Natal, South Africa. METHODS: We undertook semi-structured in-depth interviews to assess participant understanding of the informed consent, research objectives and motivation for participation. RESULTS: Thirty-nine people participated (individuals who participated in screening/biospecimen collection and those who did not and members of the research team). Some participants said they understood the information shared with them. Some said they participated due to the perceived benefits of the reimbursement and convenience of free healthcare. Most who did not participate said it was due to logistical rather than ethical concerns. None of the participants recalled aspects of biobanking and genetics from the consent process. CONCLUSIONS: Although most people understood the study objectives, we observed challenges to identifying language appropriate to explain biobanking and genetic testing to our target population. Engagement with communities to adopt contextually relevant terminologies that participants can understand is crucial. Researchers need to be mindful of the impact of communities' socio-economic status and how compensation can be potentially coercive.

Abstract

South Africa faces the formidable challenge of redressing the imbalances created by repressive apartheid policies. These policies resulted in the erosion of civil society typified by unequal and inequitable access to resources and opportunities. This condition resulted in the attrition of the asset base of families and exposed the majority of African families to physical, political and economic disasters. Yet, despite these challenges, South African families have always maintained a certain extraordinary power, which guards their systems, thereby enabling them to sustain themselves, often under the most difficult conditions. 38. Challenges of developing a district child welfare plan in South Africa: lessons from a community-engaged HIV/AIDS research project.

Abstract

The Amajuba Child Health and Wellbeing Research Project measured the impact of orphaning due to HIV/AIDS on South African households between 2004 and 2007. Community engagement was a central component of the project and extended through 2010. We describe researcher engagement with the community to recruit participants, build local buy-in, stimulate interest in study findings, and promote integration of government social welfare services for families and children affected by HIV/AIDS. This narrative documents the experience of researchers, drawing also on project reports, public documents, and published articles, with the objective of documenting lessons learned in this collaboration between researchers from two universities and a community in South Africa during a period that spanned seven years. This experience is then analyzed within the context of an applied research, community-engagement framework. [ABSTRACT FROM AUTHOR]

Abstract

Water is the source of life for human beings, animals and plants also need water for them to grow. The scourge of water pollution in our country due to various reasons is of great concerns and if left unattended will have dire consequences. Uncontrolled water pollution results in health hazards to human beings, animals and other living things. In terms of our Constitution the state as a custodian of all natural resources has a duty to ensure that water as a scarce resource is preserved for the present and future generations. Legislative measures are needed in order to ensure that same prevails. Legislation imposes measure such as a permit and its condition that must be respected by the holder of such permit. Failure to observe the condition of the permit is punishable by law for both corporates and human beings. Most environmental crimes are caused not by a deliberate intention or negligence but by poor or ineffective management systems. Public education is important in ensuring that water pollution does not take place. Authorities must also be strict in ensuring compliance with permits and prevention of water pollution as prevention is better that cure. This dissertation examines water pollution legislation in South Africa, India and the United Kingdom and offers a comparative analysis and recommendations to South Africa. The three countries are developed and also allocated in three different continents. The examination and analysis of how they fight water pollution gives a chance to South Africa on how it may improve its legislation and maintain its water quality.

Abstract

South Africa is a frontline country for malaria elimination in the southern African region. It has three malaria‐endemic provinces, each with its own transmission pattern. The elimination of malaria depends, in part, on controlling and/or eliminating vectors responsible for transmission. Sustained entomological surveillance is an important factor to consider when shifting from a control to elimination framework. The Ehlanzeni district in Mpumalanga province is a key entomological sentinel surveillance area. It is one of the malaria‐endemic districts in South Africa with higher rates of malaria incidences. As such, entomological data about the Anopheles gambiae Giles (Diptera: Culicidae) complex have been collected in this province over a substantial period. These data are stored in a pre‐existing institutional database. An analysis of the trends that can be observed from this database has not been performed before. This retrospective (longitudinal) analysis provides a summary of the An. gambiae complex vector composition in this region from 2009 to 2021. Routine surveillance data were correlated with climatic data (obtained from the NASA LaRC POWER project database) for the same period to assess the role of climatic factors in vector dynamics. This review also identifies a number of limitations in the data collection process across the sampling period and provides recommendations on how to strengthen the database going forward. The most abundant member of the An. gambiae complex since 2009 in the province was An. merus Dönitz followed by An. arabiensis Patton. Collection methods used showed that human landing catches were successful for collecting An. arabiensis, while pit traps were the most effective in collecting An. merus and An. quadriannulatus Theobald. The latter two species were mainly collected in spring, whereas An. arabiensis abundance was larger during autumn collections. Vector abundance was not significantly correlated with annual climatic data. The information gained from this database provides insights into the vector dynamics of the Ehlanzeni district of the Mpumalanga province. [ABSTRACT FROM AUTHOR]

Abstract

Background: In low- and middle-income settings, obtaining informed consent for biobanking may be complicated by socio-economic vulnerability and context-specific power dynamics. We explored participants experiences and perceptions of the research objectives in a community-based multidisease screening and biospecimen collection platform in rural KwaZulu-Natal, South Africa., Methods: We undertook semi-structured in-depth interviews to assess participant understanding of the informed consent, research objectives and motivation for participation., Results: Thirty-nine people participated (individuals who participated in screening/biospecimen collection and those who did not and members of the research team). Some participants said they understood the information shared with them. Some said they participated due to the perceived benefits of the reimbursement and convenience of free healthcare. Most who did not participate said it was due to logistical rather than ethical concerns. None of the participants recalled aspects of biobanking and genetics from the consent process., Conclusions: Although most people understood the study objectives, we observed challenges to identifying language appropriate to explain biobanking and genetic testing to our target population. Engagement with communities to adopt contextually relevant terminologies that participants can understand is crucial. Researchers need to be mindful of the impact of communities' socio-economic status and how compensation can be potentially coercive., (© The Author(s) 2020.
