Background:Spinal epidural abscess (SEA) is a rare but severe pyogenic infection of the epidural space. Case presentation:A 60-year-old female with a history High Blood Pressure, Chronic Renal Failure and the non-surgical descending aortic aneurysm was hospitalized 10 days before its symptoms in the intensive care unit for treatment of acute infective endocarditis caused byStaphococcus aureus.She presented 24 hours before a lower limb motor weakness, bladder and bowel dysfunction(urinary and anal incontinence).MRI spine with gadolinium demonstrating an epidural contrast-enhancing mass extending from mid Th10 to Th12 left-side with infiltration of the pedicles and extension to the soft tissues and is most consistent with epidural abscess. Within the next few hours the patient underwent decompressive laminectomy (level Th10-Th12) and surgical evacuation of the abscess. The post-operative cultures from the abscess were positive for Staphylococcus- aureus. Intravenous antibiotics were continued, and active neurorehabilitation was initiated. Immediate improvement of the paralysis and sphincter function was observed as a result of the surgical decompression. Conclusion: The incidence of spinal epidural abscess is increasing with the aging of the population and the frequency of immunocompromising diseases. The most common causative agent isStaphylococcus. Aureus.The signs and symptoms of epidural abscess are non-specific. Empiric antibiotic therapy and emergency surgical decompression improve the prognosis.

Tuberculosis is a major public health problem in the world. Spinal tuberculosis (Pott disease) is a frequently encountered extrapulmonary form of the disease. Cervical spinal tuberculosis is relatively rare. We report the case of a 66-year-old patient admitted for cervical Pott's disease managed surgically and the positive outcome. A patient with a history of pulmonary tuberculosis present 3 months ago persistent neck pain with tingling and heaviness in both upper limbs. The neurological examination was normal without any sensory or motor deficit. Spinal cord magnetic resonance imaging (MRI) showed a lesion centered on the vertebral body of C4 with spinal cord compression and epiduritis without signs of spinal cord injury. The patient underwent a corpectomy of C3 and C4 with an iliac graft and anterior cervical plate. The anatomopathological examination revealed a Pott disease. He was therefore put on antituberculous chemotherapy for 12 months. Three months later the neck pain and tingling disappeared in the upper limbs. Cervical Pott's disease is relatively rare. Surgical management is indicated in the case of spinal instability or spinal cord compression.

Background: Thoracic complications of ventriculoperitoneal (VP) cerebrospinal fluid shunting are rare and the diagnosis is difficult without neurological impairment. Case Description: We report a case of a 36-year-old woman who had a VP shunt in the right side when she was 13 years for a posterior fossa ependymoma and hydrocephalus. 23 years after surgery, she developed acute yellowfish cough and sputum, and the computed tomography scan found an intrathoracic cyst. She had a thoracotomy for the cyst and during surgery, we found the peritoneal catheter of the VP shunt, with a collected abscess in the left side. The patient was treated for the abscess and the VP shunt was removed. We also review the literature cases of thoracic complications after VP shunts. Conclusion: Thoracic abscess due to VP shunt migration is extremely rare and could happen after a long time delay VP shunt surgery.

Background: Cystic meningioma is a rare variety of meningioma. It represents 4% to 7% of all meningiomas. The authors report a case of intracranial cystic meningioma with a review of literature. Case Report: A 59-year-old female presented with a history of headache followed by left side hemiplegia. CT scan and MRI of the brain showed a right parietal tumor with double solid and cystic components thought to be pilocytic astrocytoma or metastasis preoperatively. At surgery, the extraaxial solid and cystic lesion had a well defined capsule that could be easily separated from the perilesional cortical surface. The tumor was totally removed and the cyst was punctioned. The histological study showed a cystic meningioma WHO Grade I. A follow-up at 10 months after surgery, the patient was going well and recovered from the hemiplegiaa. Conclusion: Cystic meningioma is an uncommon tumor that should be considered in the differential diagnosis of brain tumors with a cystic component. The preoperative diagnosis and management could be in some cases challenging.

Médecins et chercheurs se précipitent pour élargir les connaissances sur ce nouveau virus, sur sa propagation et sa virulence. Selon l’OMS, il ne s’agit pas d’une pneumonie infectieuse comme on le croyait au début mais une pandémie contagieuse, il s’est avéré que le virus peut entraîner des consignes graves aussi bien sur le plan respiratoire, moteur et psychologique chez les malades atteints du COVID 19. Dans ce contexte pandémique de CORONA Virus, le kinésithérapeute comme tout professionnel de santé se doit d’évaluer au cas par cas le rapport Bénéfice/Risque du patient, mais aussi pour lui-même en collaboration étroite avec tous les acteurs du staff médical et paramédical pour mieux prendre en charge ces patients. A 59-year-old female presented with severe headache and trouble of consciousness. The CT showed a subarachnoid hemorrhage FISHER IV with a hematoma of the right cerebellum. The angiography discovered a persistent left trigeminal artery associated with an aneurysm of the right posterior meningeal artery. The attempt of embolization failed and the patient suffered cerebral vasospasm and died. The persistence of a trigeminal artery is found in 0.1–0.6% of the population. It is the most frequent embryological carotid-vertebral anastomosis. It appears at the fifth week of development and disappears when the embryo reaches 5–7 mm. This embryological persistence is associated with numerous vascular abnormalities. The literature review showed the presence of those aneurysms in the trigeminal artery itself or the communicating artery. The association that we described is the first to our knowledge. Therefore, this association is particular and interesting to expose.

Background: Chordomas are rare benign, but locally aggressive tumors that are regularly encountered in the clivus and sacrum. Although they are rarely found in the thoracic spine, they can contribute not only to local bone destruction and spinal instability, lead to cord compression, and major neurological deficits. Case Description: A 56 year-old-male originally underwent a T12 laminectomy with debulking for a thoracic chordoma. Two years later, the lesion recurred contributing to a significant paraparesis. The new MR showed a T2 hyper intense lesion with huge epidural extension that warranted a 360° surgical decompression and fusion. The histopathology from both surgeries confirmed the diagnosis of a chordoma. Conclusion: Combined 360° decompression and fusion was warranted to resect a T12 chordoma that recurred 2 years following an original laminectomy with debulking procedure.

Les traumatismes de l'etage anterieur de la base du crâne representent 15 a 20% des traumatismes crâniens en general. Ils menacent les structures neuro-encephaliques sus jacentes et sont tres souvent responsables de breches osteo-meningees exposant au risque infectieux. Notre travail a concerne 136 dossiers exploitables de traumatisme de l'etage anterieur de la base du crâne colliges sur une periode de 10 ans entre janvier 2003 et decembre 2012. Le diagnostic a ete suspecte devant les signes cliniques evocateurs (ecchymose peri-orbitaire, rhinorrhee…) et a ete confirme dans la plupart des cas par la TDM. Le traitement ideal est la fermeture chirurgicale de la breche en association aux moyens medicaux (vaccination, anti-epileptiques, mesures de reanimation…) Le moment ideal de la reparation est au-dela de la 72eme heure apres la diminution de l'oedeme cerebral en cas d'absence d'une lesion intracrânienne necessitant une intervention en urgence. Notre equipe ne pratiquant pas la voie endoscopique, l'abord frontal est souvent indique. Le pronostic depend des lesions cerebrales associees et surtout de la presence d'une breche dont le diagnostic et la reparation doivent etre les plus rapides et les plus precis possibles. Ainsi toute rhinorrhee post-traumatique necessite une exploration systematique, le timing ideal: c'est la disparition de l'oedeme cerebral pour faciliter l'exploration, ceci est en general possible a partir de la 72eme heure sauf dans les cas associes a une autre lesion intra crânienne necessitant une exploration en urgence.

Le siège intraventriculaire est une localisation rare des cavernomes de l'encéphale. Le quatrième ventricule est le moins concerné de toutes les localisations. Nous rapportons le cas d'une patiente âgée de 52 ans qui présente depuis 12 mois un syndrome d'hypertension intracrânienne et trouble de l’équilibre. Elle s'est présentée aux urgences avec un GCS à 14, nuque subraide, un syndrome cérébelleux statokinétique avec une acuité visuelle basse et un oedeme papillaire bilatéral. Une TDM cérébrale réalisée aux urgences a objectivé un hématome du 4ème ventricule avec hydrocéphalie active triventriculaire. Une dérivation ventriculaire interne a été réalisée en urgence avec une bonne évolution clinique post opératoire. Le bilan a été complété par une IRM cérébrale objectivant un processus du quatrième ventricule évoquant un cavernome. Un abord direct a été réalisé permettant une exérèse totale du cavernome siégeant au sein du quatrième ventricule. L'anatomo-pathologie a confirmée le diagnostic. A notre connaissance, il s'agit du 13ème cas rapporté dans la littérature. Les cavernomes intraventriculaires représentent 2,5-10% de tous les cavernomes dont 9% est au niveau 4ème ventricule. Les patients sont souvent admis aux urgences suite au saignement de cette malformation angiomateuse. Le diagnostic est rendu accessible par les différentes séquences de l'IRM. Le traitement est souvent chirurgical vu le risque de resaignement. Le pronostic dépend de l’état initial du patient et de l'infiltration du plancher du quatrième ventricule.

Intracranial epidermoid cysts are one of the rare tumors of all intracranial tumors. They represent 0,2 to 1% of intracranial tumors and 7 % of tumors in the cerebellopontine angle. The pineal region is exceptionally subject to such kind of tumor. Cushing was the first to report the pineal localization of the epidermoid cyst in 1928. Up to now, 85 cases of pineal epidermoid cyst were cited in the literature. We report a clinical case concerning a 45 years old man who presented an intracranial hypertension during 18 months. The clinical examination found a hemiparesis with a facial hypoesthesis. The MRI showed a process of the pineal region. The patient underwent a surgery with a large resection. The histological examination confirms the epidermoid cyst. Many approaches were described in the literature. Les auteurs proposent des options therapeutiques dans la prise en charge des abces encephaliques, en fonction des parametres cliniques et radiologiques des patients, en degageant des facteurs pronostiques a long terme. Il s'agit d'une etude retrospective incluant tous les abces encephaliques pris en charge, sur une periode 13 ans, au service de Neurochirurgie de l'Hopital Ibn Sina de Rabat. L'analyse a porte sur : le Glasgow coma scale (GCS), la presence de deficit neurologique, l'imagerie cerebrale avec contraste, et la recherche d'une porte d'entree; le traitement a compris chirurgie et antibiotherapie, et le pronostic a ete evalue a long terme. 82 cas d'abces ont ete analyses, un score de GCS inferieur a 12 a ete trouve dans 24%. La porte d'entree a ete d'origine ORL dans 45%. La trepanoponction seule a ete decidee dans 34% des cas en situation d'urgence avec troubles de conscience. Elle a ete suivie d'une exerese de la coque dans 16% des cas lors de persistance ou recidive de la collection. La ponction stereotaxiques a ete realisee dans 10% des cas lors de localisations eloquentes et l'exerese d'emblee dans 38% des cas avec abces collecte non menacant. La mortalite operatoire a ete de 1,22%, la morbidite de 19,30%. Les facteurs de bon pronostic a long terme ont ete l'âge jeune, un GCS superieur a 12 a l'admission, et la trepanoponction suivie de l'exerese da la coque. L'amelioration du pronostic passe par le diagnostic precoce, un geste neurochirurgical precis et antibiotherapie adaptee, et le traitement des portes d'entree. Les tuberculomes intracrâniens représentent l'une des localisations les plus graves de la tuberculose, leur incidence varie en fonction du contexte représentant 0,2% des processus intracrâniens dans les pays occidentaux et 5 à 10% des masses intracrâniennes dans les pays en voie de développement. Nous rapportons une étude rétrospective de 125 cas. L'hypertension intracrânienne (45%) et le déficit neurologique (36%) sont les signes cliniques les plus fréquents. La lésion était localisée dans 60% des cas en sus-tentoriel et dans 40% des cas en sous-tentoriel. L'approche thérapeutique a consisté en un abord direct du tuberculome dans 67 cas (53%), une biopsie stéréotaxique dans 32 cas (25%), le traitement médical en première intention sans confirmation histologique dans 26 cas (20%). Avant 1993 notre service ne disposait pas de cadre de stéréotaxie, notre attitude thérapeutique consistait soit en un abord direct de la lésion dans 70% des cas, soit un traitement antituberculeux en première intention sans confirmation histologique (30%). Cette attitude était corrélée à une mortalité et morbidité non négligeables respectivement 3% et 10%. Après 1993; le taux d'abords direct a chuté a 38%, avec 47% de biopsies stéréotaxiques et seulement 13% des patients traités par antibacillaires sans preuve histologique. Ceci s'est accompagné d'une réduction significative de mortalité a 1,4% (p = 0,0003) et de morbidité a 2% (p = 0,0027).

Background: Coexistence of multiple primary intracranial tumors of different cell types has rarely been documented; the association of a meningioma and a glioma has been reported as the most common combination. Hereby, we report an unusual case of a temporal epidermoid cyst coexisting with an atypical meningioma. Case Presentation: A 37-year-old male presented with progressive symptoms of raised intracranial progression with progressive loss of vision without any neurological deficit. On admission, magnetic resonance imaging (MRI) revealed a right frontal lesion appearing hypointense T1, hyperintense T2 slightly enhanced after gadolinium and a second right temporal, isointense T1, hyperintense T2 non-enhancing lesion. A right frontotemporal craniotomy was performed that revealed two distinct lesions: The whitish temporal lesion with the pearl appearance reminding of an epidermoid cyst, the second lesion was extraaxial fibrous lesion arising from the falx. Pathology confirmed an atypical meningioma WHO Grade II and an epidermoid cyst. Conclusion: The simultaneous occurrence of primary intracranial tumors of different cell types is rare. Epidermoid cysts are slow growing lesions believed to arise from inclusion of ectodermal elements during neural tube closure, while meningiomas arise from arachnoidal cells; their association has rarely been reported previously.
