Abstract

BACKGROUND. The significant risks posed to mothers and fetuses by COVID-19 in pregnancy have sparked a worldwide debate surrounding the pros and cons of antenatal SARS-CoV-2 inoculation, as we lack sufficient evidence regarding vaccine effectiveness in pregnant women and their offspring. We aimed to provide substantial evidence for the effect of the BNT162b2 mRNA vaccine versus native infection on maternal humoral, as well as transplacentally acquired fetal immune response, potentially providing newborn protection. METHODS. A multicenter study where parturients presenting for delivery were recruited at 8 medical centers across Israel and assigned to 3 study groups: vaccinated (n = 86); PCR-confirmed SARS-CoV-2 infected during pregnancy (n = 65), and unvaccinated noninfected controls (n = 62). Maternal and fetal blood samples were collected from parturients prior to delivery and from the umbilical cord following delivery, respectively. Sera IgG and IgM titers were measured using the Milliplex MAP SARS-CoV-2 Antigen Panel (for S1, S2, RBD, and N). RESULTS. The BNT162b2 mRNA vaccine elicits strong maternal humoral IgG response (anti-S and RBD) that crosses the placenta barrier and approaches maternal titers in the fetus within 15 days following the first dose. Maternal to neonatal antiCOVID-19 antibodies ratio did not differ when comparing sensitization (vaccine vs. infection). IgG transfer ratio at birth was significantly lower for third-trimester as compared with second trimester infection. Lastly, fetal IgM response was detected in 5 neonates, all in the infected group. CONCLUSION. Antenatal BNT162b2 mRNA vaccination induces a robust maternal humoral response that effectively transfers to the fetus, supporting the role of vaccination during pregnancy. FUNDING. Abstract

Despite notable scientific and medical advances, broader political, socioeconomic and behavioural factors continue to undercut the response to the COVID-19 pandemic1,2. Here we convened, as part of this Delphi study, a diverse, multidisciplinary panel of 386 academic, health, non-governmental organization, government and other experts in COVID-19 response from 112 countries and territories to recommend specific actions to end this persistent global threat to public health. The panel developed a set of 41 consensus statements and 57 recommendations to governments, health systems, industry and other key stakeholders across six domains: communication; health systems; vaccination; prevention; treatment and care; and inequities. In the wake of nearly three years of fragmented global and national responses, it is instructive to note that three of the highest-ranked recommendations call for the adoption of whole-of-society and whole-of-government approaches1, while maintaining proven prevention measures using a vaccines-plus approach2 that employs a range of public health and financial support measures to complement vaccination. Other recommendations with at least 99% combined agreement advise governments and other stakeholders to improve communication, rebuild public trust and engage communities3 in the management of pandemic responses. The findings of the study, which have been further endorsed by 184 organizations globally, include points of unanimous agreement, as well as six recommendations with >5% disagreement, that provide health and social policy actions to address inadequacies in the pandemic response and help to bring this public health threat to an end.

Abstract

PURPOSE: To compare marginal implant bone loss (MBL), survival, and radiographic evidence of success of dental implants among smokers and nonsmokers. MATERIALS AND METHODS: Consecutive records of 161 patients (aged 23 to 89 years, mean 57 years) treated with a total of 646 implants between the years 1995 and 1998 were examined. Patients were divided into 3 groups: nonsmokers, moderate smokers, and heavy smokers. Tobacco exposure was calculated by cigarettes per day and by pack-years. Follow-up ranged from 1 to 7 years (mean 3.8 years). Postoperative panoramic radiographs obtained before implant exposure and annually thereafter were analyzed for MBL changes. The influence of smoking and other variables on MBL was analyzed at all implant sites. RESULTS: Generally, smokers had more MBL than nonsmokers (0.153 +/- 0.092 mm and 0.047 +/- 0.048 mm, respectively; P < .001). When each jaw was examined separately, smoking had a greater effect on MBL in the maxilla than in the mandible (0.158 +/- 0.171 mm versus 0.146 +/- 0.158 mm, respectively; P < .001). Furthermore, in the maxilla, heavy smokers had the greatest amount of MBL (0.1897 +/- 0.1825 mm), followed by moderate smokers (0.123 +/- 0.156 mm) and nonsmokers (0.0460 +/- 0.070 mm) (P < .001). In the mandible, there was no distinction between heavy and moderate smokers, and both had greater MBL than nonsmokers (P < .001). Only 3 of the 646 implants failed; the cumulative survival rate was 99.5%. Overall radiographic success rate was 93.2%. Nonsmokers had a higher radiographic success rate (97.1%) than smokers (87.8%) (P < .001). CONCLUSIONS: This study demonstrated a relationship between MBL and smoking habits. Abstract

Fugitive dust, the source of which is primarily the earth's soil, is observed at high concentrations particularly in dry environment. The future of bacterial biosensing is using outdoor instruments with immediate response. However, dust contamination can interfere with bacterial measurement because they, and the measuring pores, are in the same size range. Utilizing our instrument with porous silicon ordered array to observe dust, we found a 0.1 optical density threshold for sieved dust to induce a detectable change in the magnitude of the measurement (optical thickness (OT)). Thus, sieving the dust allows us to conduct outdoor bacterial sensing measurements without dust interference.

Abstract

Purpose: To assess the growth hormone (GH) response to the Wingate anaerobic test (WAnT) among children with short stature and suspected GH deficiency. We hypothesized that the GH response to the WAnT would be similar to the GH response to a commonly used pharmacologic provocation test. Methods: Ten children (6 males and 4 females, age range 9.0–14.9 years) participated in the study. Each participant performed 2 tests: a standard all-out WAnT, cycling for 30 s against constant resistance, and a standardized pharmacologic test (clonidine or glucagon). Blood samples for GH were collected before and 10, 30, 45, and 60 min after the beginning of exercise. In addition, we collected pre- and post-exercise blood lactate levels. Results: There was a significant increase in GH levels after the WAnT, yet in 9 of 10 participants, this increase was below the threshold for GH sufficiency. Peak GH after the WAnT was significantly lower compared to the pharmacologic GH provocation tests (with 9 of 10 demonstrating GH-sufficient response). Conclusion: The traditional WAnT cannot be used as a GH provocation test. Further research is needed to develop anaerobic exercise protocols sufficient to promote GH secretion.

Abstract

The flora of periodontal pockets > 6 mm and the rate of alveolar bone loss of one diseased site in each of eighteen adult beagle dogs were compared during a 6 month pretreatment period and a subsequent 12 month tetracycline treatment period. The dogs were divided into the following three groups and treated as follows: untreated controls (5), 250 mg tetracycline daily (5), and 500 mg tetracycline daily (7). An attempt was made to correlate the flora in deep periodontal pockets and the subsequent rate of alveolar bone loss in untreated control beagles and beagles treated with tetracycline (250 mg per day and 500 mg per day). The bacterial flora and the rate of bone loss of one site in each dog was compared during a six-month pretreatment period and a subsequent twelve-month tetracycline treatment period. The cultivable flora was classified according to cellular morphology, Gram stain, and atmospheric tolerance. The periodontal pocket flora was predominantly a Gram negative anaerobic flora at all time periods throughout the study. The flora of the untreated control dogs spontaneously shifted to a more Gram negative flora at 18 months of the study. In the treated dogs, after six months of tetracycline treatment, there was a significant shift to a more Gram negative flora which returned to baseline proportions after twelve months of treatment. A decrease in the numbers of bacteria sensitive in vitro to tetracycline was found in the course of the treatment period. No significant relationship was found between the cultivable flora of a specific site and the rate of alveolar bone loss for that site. The data suggest that in advanced periodontal disease in the beagle dogs studied a Gram negative anaerobic flora was predominantly cultivated. 46. Adhesive force: the underlying cause of the disc anchorage to the fossa and/or eminence in the temporomandibular joint—A new concept

Abstract

The commonly held cause for using the closed lock technique is the prevention of disc sliding by using the non-reducible disc. The purpose of this study was to re-evaluate the pathogenesis of sudden and persistent severely limited mouth opening associated with a total lack of disc sliding in view of the fact that it is promptly released by lavage of the upper compartment of the temporomandibular joint (TMJ). The proposed pathogenesis is based upon earlier clinical and laboratory findings of the first author coupled with pertinent informtaion culled from the literature. It is suggested that sliding of the disc in the TMJ is enabled due to the presence of phospholipids protected by hyaluronic acid (HA) that constitute an efficient lubrication system. Joint overloading may be associated with uncontrolled production of reactive oxygen species (ROS) that causes degradation of the HA, followed by the exposure of the phospholipids to lysis by phospholipase A2 (PLA2). The denuded, smooth and elastic articular surfaces that possess high surface energy become strongly adherent when placed in direct physical contact with each other. The presence of an extremely thin film of fluid (sub-boundary lubrication) between the mating surfaces may cause even higher adhesion. These adhesive forces are probably responsible for the flexible disc anchorage to the fossa and/or eminence. They also explain the immediate release of the disc and rehabilitation of its sliding following arthrocentesis. Since it is uncommon for two opposing surfaces to be stripped bare and to become adherent, the likelihood of anchored disc phenomenon (ADP) occurring and recurring is very low.
