We report an outbreak of Shiga toxin-producing Escherichia coli (STEC) associated paediatric haemolytic uraemic syndrome linked to the consumption of raw cow’s milk soft cheeses. From 25 March to 27 May 2019, 16 outbreak cases infected with STEC O26 (median age: 22 months) were identified. Interviews and trace-back investigations using loyalty cards identified the consumption of raw milk cheeses from a single producer. Trace-forward investigations revealed that these cheeses were internationally distributed.

We describe a Salmonella Poona outbreak involving 31 infant cases in France. Following outbreak detection on 18 January 2019, consumption of rice-based infant formula manufactured at a facility in Spain was identified as the probable cause, leading to a recall on 24 January. Whole genome sequencing analysis linked present outbreak isolates to a 2010–11 S. Poona outbreak in Spain associated with formula manufactured in the same facility, indicating a persistent source of contamination.

Introduction Le 3 juin 2019, l’Agence regionale de sante (ARS) Centre-Val de Loire a recu une declaration d’hepatite A (VHA) chez un cuisinier d’un restaurant de la region. Au 17 juin, 4 nouveaux cas de VHA en lien avec le restaurant etaient declares a l’ARS dont 2 formes severes. Le 21 juin, le Centre national de reference (CNR) isolait chez 2 cas un nouveau genotype « 1B », isole uniquement aux Pays-Bas en avril 2019 (4 cas) et en France en mai 2019 (1 cas). Sante Publique France (SpFrance) et l’ARS ont pris en charge l’investigation ce signal sanitaire. Les objectifs etaient de confirmer la nature epidemique et proposer des mesures de gestion adaptees. Materiels et methodes A compter du 18 juin, l’ARS interrogeait systematiquement tout cas declare de VHA sur les restaurants frequentes lors des 6 semaines precedant les symptomes et demandait l’envoi au CNR des souches des cas declarant un lien avec le restaurant. Le CNR signalait a SpFrance tout cas de VHA survenu en France dont la souche correspondante etait retrouvee afin de permettre la recherche d’une eventuelle exposition au restaurant. SpFrance appuyait les investigations et assurait le suivi epidemiologique de ce cas groupe. Resultats Entre le 3 juin et le 31 juillet, 16 cas de VHA en lien avec le restaurant, tous de genotype 1B, ont ete identifies. Douze cas ont ete declares a l’ARS et 4 cas ont ete identifies via le CNR. Cinq cas etaient employes du restaurant, 9 etaient des clients et 2 etaient des contaminations intrafamiliales. Les cas etaient âges de 17 a 61 ans et le sex-ratio H/F etait de 2,2 (11/5). Dix cas ont developpe un ictere, 13 ont ete hospitalises et 5 ont developpe des formes severes. Le 14 juin, la Direction departementale de protection des populations (DDPP) realisait une visite du restaurant mettant en evidence plusieurs non-conformites d’hygiene. Le 21 juin, la vaccination etait proposee a l’ensemble du personnel du restaurant et 11 des 14 professionnels presents ont ete vaccines. Le 9 juillet, une contre-visite de la DDPP a permis de constater la mise en oeuvre des actions correctives demandees. Aucun cas en lien avec l’episode n’a ete rapporte apres le 31 juillet. Conclusion Les informations disponibles orientaient vers une transmission secondaire a partir du cas index, et non une contamination alimentaire. La proportion elevee de cas severes a conduit a s’interroger sur une fermeture eventuelle de l’etablissement mais le risque a ete maitrise suite a la mise en place des mesures d’hygiene. Cet episode de cas groupe souligne l’importance de rappeler la recommandation de vaccination contre l’hepatite A pour les professionnels impliques dans la preparation alimentaire et la necessite de vacciner l’ensemble des personnes exposees dans un delai court des l’apparition d’un cas.

International audience; Ticks are one of the principal arthropod vectors of human and animal infectious diseases. Whereas the prevalence of tick-borne encephalitis virus in ticks in Europe is well studied, there is less information available on the prevalence of the other tick-borne viruses (TBVs) existing worldwide. The aim of this study was to improve the epidemiological survey tools of TBVs by the development of an efficient high-throughput test to screen a wide range of viruses in ticks. In this study, we developed a new high-throughput virus-detection assay based on parallel real-time PCRs on a microfluidic system, and used it to perform a large scale epidemiological survey screening for the presence of 21 TBVs in 18 135 nymphs of Ixodes ricinus collected from five European countries. This extensive investigation has (i) evaluated the prevalence of four viruses present in the collected ticks, (ii) allowed the identification of viruses in regions where they were previously undetected. In conclusion, we have demonstrated the capabilities of this new screening method that allows the detection of numerous TBVs in a large number of ticks. This tool represents a powerful and rapid system for TBVs surveillance in Europe and could be easily customized to assess viral emergence.
