Aims: Steroid injections are used for subacromial pain syndrome and can be administered via the anterolateral or posterior approach to the subacromial space. It is not currently known which approach is superior in terms of improving clinical symptoms and function. This is the protocol for a randomized controlled trial (RCT) to compare the clinical effectiveness of a steroid injection given via the anterolateral or the posterior approach to the subacromial space. Methods: The Subacromial Approach Injection Trial (SAInT) study is a single-centre, parallel, two-arm RCT. Participants will be allocated on a 1:1 basis to a subacromial steroid injection via either the anterolateral or the posterior approach to the subacromial space. Participants in both trial arms will then receive physiotherapy as standard of care for subacromial pain syndrome. The primary analysis will compare the change in Oxford Shoulder Score (OSS) at three months after injection. Secondary outcomes include the change in OSS at six and 12 months, as well as the Pain Numeric Rating Scale (0 = no pain, 10 = worst pain), Disabilities of Arm, Shoulder and Hand questionnaire (DASH), and 36-Item Short-Form Health Survey (SF-36) (RAND) at three months, six months, and one year after injection. Assessment of pain experienced during the injection will also be determined. A minimum of 86 patients will be recruited to obtain an 80% power to detect a minimally important difference of six points on the OSS change between the groups at three months after injection. Conclusion: The results of this trial will demonstrate if there is a difference in shoulder pain and function after a subacromial space steroid injection between the anterolateral versus posterior approach in patients with subacromial pain syndrome. Aims: Frozen shoulder is a common, painful condition that results in impairment of function. Corticosteroid injections are commonly used for frozen shoulder and can be given as glenohumeral joint (GHJ) injection or suprascapular nerve block (SSNB). Both injection types have been shown to significantly improve shoulder pain and range of motion. It is not currently known which is superior in terms of relieving patients’ symptoms. This is the protocol for a randomized clinical trial to investigate the clinical effectiveness of corticosteroid injection given as either a GHJ injection or SSNB. Methods: The Therapeutic Injections For Frozen Shoulder (TIFFS) study is a single centre, parallel, two-arm, randomized clinical trial. Participants will be allocated on a 1:1 basis to either a GHJ corticosteroid injection or SSNB. Participants in both trial arms will then receive physiotherapy as normal for frozen shoulder. The primary analysis will compare the Oxford Shoulder Score (OSS) at three months after injection. Secondary outcomes include OSS at six and 12 months, range of shoulder movement at three months, and Numeric Pain Rating Scale, abbreviated Disabilities of Arm, Shoulder and Hand score, and EuroQol five-level five-dimension health index at three months, six months, and one year after injection. A minimum of 40 patients will be recruited to obtain 80% power to detect a minimally important difference of ten points on the OSS between the groups at three months after injection. The study is registered under ClinicalTrials.gov with the identifier NCT04965376. Conclusion: The results of this trial will demonstrate if there is a difference in shoulder pain and function after GHJ injection or SSNB in patients with frozen shoulder. Intro duction: Awake fiberoptic bronchoscope-guided intubation is the preferred airway management strategy in a patient with a difficult airway and limited mouth opening. The aim is to compare the efficacy of nebulization and airway nerve block using lignocaine to provide airway anesthesia in awake patients using fiberoptic bronchoscope for nasal intubation to make the patient comfortable, cooperative, and agreeable. Methods: In this randomized, prospective, single-blinded study, sixty adult patients scheduled for oral cancer surgeries were divided into two equal groups. Nebulization with 4% lignocaine (10 ml) was performed in Group N, and superior bilateral laryngeal and trans-laryngeal recurrent laryngeal nerve blocks used 2% lignocaine (2 ml) were given in Group B. Nasotracheal intubation using fiberoptic bronchoscope were performed. Intravenous midazolam and fentanyl were used for sedation and analgesia. We recorded the ease of intubation, coughing episodes, vocal cord status, hemodynamic variables, patient recall, and lignocaine toxicity. Student's t-test and Chi-square tests were used for appropriate data. Statistical significance was considered for P < 0.05 (SPSS Version 20, IBM, USA). Results: The intubation time was shorter with better intubating conditions in Group B (120.3 ± 42.6 s) than Group N (200.4 ± 60.3 s) (P < 0.001). Cough severity and patient recall for untoward effects were lower in Group B. Intubation were done successfully in all patients. Conclusion: Airway nerve blocks provided better airway anesthesia characterized by easier fiberoptic intubation and patient comfort than lignocaine nebulization. Nebulization can be considered a suitable alternative when nerve blocks are not feasible due to distorted neck anatomy.

Abstract Background The purpose of the study was to find out correlation between severity of disease and vaccine in COVID-19 pandemic. Primary objective was to know occurrence of post-vaccination breakthrough infections in hospitalized patients and secondary objective was to know of COVID-19 vaccine to prevent severe infection, morbidity, and mortality of patients. This retrospective observational multicentric study included 325 confirmed COVID-19 patients on NRBM/BIPAP/ventilator admitted in high dependency unit (HDU)/intensive care unit (ICU) were divided into based on severity of symptoms and vaccination status. We included adult patients having positive RTPCR (reverse transcription polymerase chain reaction) COVID-19 test/high-resolution computed tomography scan (HRCT) thorax suggestive of COVID-19 pneumonia. Patients who developed adverse reaction post-vaccination, pregnant patient, and lactating mother were excluded from the study. Results Total 325 confirmed COVID-19-positive patients were studied. Statistical analysis with SPSS version 26 and data were analyzed by chi-square test and P value < 0.05 were considered as significant. Total duration of hospital stay was less in vaccinated patients compared to non-vaccinated patients. Oxygen requirement was also less in vaccinated patients. Vaccinated patients developed less severe infection than non-vaccinated patients. Conclusions COVID-19 vaccination is very effective against severity of disease. It reduces hospitalization, oxygen requirement, and mortality.

Abstract Background Mucormycosis is uncommon, progressive fungal infection with mortality rate on higher side. The anesthetic management of surgical debridement in mucormycosis is challenging. We evaluated the anesthetic management of mucormycosis patients associated with problems of airway management, predisposing factors, and adverse effect of antifungal therapy who underwent surgical resection of necrotized tissues. Results Fifty-six patients presented with uncontrolled diabetes mellitus. All patients had history of COVID-19 infection and received steroid during treatment. Nasal endoscopic debridement was done in 43 patients, total maxillectomy in 14 patients. Temporalis flap was needed in five patients, and orbital exenteration was required in three patients. Central venous catheter was inserted in 17 patients, and invasive arterial monitoring was done in ten patients. All patients administered lyophilized amphotericin B (deoxycholate) in combination with surgical debridement. Thirteen patients were shifted to ICU for further management and continuous hemodynamic monitoring. Fifteen patients were expired, and the mortality rate was 26.31%. Conclusions Challenges include difficult intubation and renal function impairment due to prolonged antifungal therapy. Postoperative ICU management is important in mucormycosis patients due to comorbidities and rapid progressive infection after surgery.

Background and Aims: Dexmedetomidine is a selective α2 agonist used as an adjuvant to attenuate sympathetic response to laryngoscopy and tracheal intubation in general anesthesia. Recent studies have shown that dexmedetomidine decreases brain flow and cerebrospinal fluid pressure without cerebral ischemia. Hence, it is beneficial in neurosurgical procedures. This study was undertaken to analyze the efficacy of dexmedetomidine in neurosurgery for good perioperative hemodynamic stability with blunt sympathetic response to laryngoscopy and intubation. Materials and Methods: Sixty, American Society of Anesthesiologists grades II-IV patients between 18 and 50 years of age undergoing craniotomy under general anesthesia were divided randomly into two groups (thirty patients in each group). Group D: Dexmedetomidine was given as a bolus dose of 1 μg/kg diluted to a total volume of 20 ml with normal saline (0.9%) for 10 min before the induction of anesthesia. Group C: The patients received a similar volume of normal saline. Results: We observed a decrease in blood pressure and heart rate (HR) after intubation by dexmedetomidine, and they were highly significant on comparison (P < 0.0001). During intraoperative period, the patients in Group D had lower HR and blood pressure than baseline value as compared to Group C (P < 0.0001). Background and Aims: Supraglottic airway devices used in laparoscopic surgeries must be efficient to counter the increased peak airway pressure (PAWP) and airway leakage that can occur in laparoscopic surgeries. Ventilation settings for both groups set to tidal volume 5–6 ml/kg of predicted body weight, positive end-expiratory pressure 5 cm H2O, I: E ratio 1:2, and respiratory rate 12–16/min to maintain targeted ETCO2 of 30–35 cm H2O.
