The manifestation of celiac artery compression syndrome in childhood is rarely described. At the same time, celiac artery compression syndrome remains one of the most discussed vascular diseases due to an unclear pathophysiological mechanism, difficulties in differential diagnosis, and the lack of a generally accepted algorithm for treating patients. In connection with the above, it seems relevant to analyze the features of managing children with celiac artery compression syndrome. As a result of the analysis of the scientific literature, we have come to the following conclusions: 1. Typical clinical characteristics of celiac artery compression syndrome include abdominal pain, epigastric murmur and weight loss associated with compression of the celiac artery and possibly celiac ganglia by the median arcuate ligament. 2. CT angiography and conventional catheter angiography are the gold standard for diagnosis. However, imaging findings should not be interpreted in isolation from clinical manifestations. 3. Surgical decompression of the celiac artery by resection of the median arcuate ligament is the treatment of choice for this syndrome. Both open surgery and laparoscopic techniques are safely performed with minimal morbidity and mortality. Endovascular treatment of compression celiac stenosis may have limited success.

The relevance of the article is due to the fact that in civil proceedings, electronic evidence can be considered the most popular method of obtaining evidence to date. However, in law enforcement practice, there are many gaps in the formation of the institution of electronic evidence. The advantage of electronic evidence in civil proceedings is quite obvious, which makes it necessary to take a different look at this type of evidence. The courts, certainly, strive to respect the interests of the parties when considering cases on the basis of competition and equality of the parties, but the study of judicial practice shows that the presence of electronic evidence stands out against the other evidence and acquires a special status that requires research, detailed legislative development and procedural regulation. At the same time, it should be noted that there has been insufficient research on this topic in the scientific and procedural doctrine; in this connection, the opinions of researchers and their assessment are given. The purpose of the article is to determine the legal nature of electronic evidence in civil proceedings, especially in the context of the development of digitalization and electronic information technologies. The procedural legal, formal legal, logical, dialectical and other methods of scientific research make up the methodological basis. Based on the results of the study, it was concluded that the legal nature of electronic evidence is closely related to modern technical means, and the position on the need to consolidate procedural measures, the use of information and communication tools to provide civil proceedings in the modern judicial system is also justified.

The paper presents the results of differential scanning calorimetry, powder thermogravimetry, scanning electron microscopy, and X-ray phase analysis of ceramics made of powder obtained by chemical dispersion of B95 alloy. The process of obtaining a new ceramic material is briefly described. The mechanism of formation of the microstructure observed is described using the results obtained.

The relevance. The prospects for increasing gas condensate – valuable raw material for oil refining and petrochemicals, are largely associated with the further development of the achimov gas condensate deposits in the north of Western Siberia. Large depths, abnormally high reservoir pressure and high temperature of low-productivity formations, along with the presence of carbon dioxide in the gas-condensate formation fluid, put forward, first of all, increased demands on the quality of well completion and hydraulic fracturing operations, the reliability of structures and materials of underground well equipment. These factors are responsible for the high cost of project implementation and operational risks of gas and condensate production, and therefore justification of safe operating conditions for achimov is an urgent scientific and technical task. The main aim of the research is the limiting thermobaric parameters determination of the technological operating modes of the achimov gas condensate wells, taking into account the production of CO2 in products. Objects: reservoir gas-condensate fluid and bottomhole equipment of three achimov gas-condensate wells with carbon steel tailpipes, unstable to CO2 corrosion. Methods: modeling the phase behavior of the produced reservoir fluid in bottomhole conditions by the GasCondNeft software. Results. Phase behavior thermodynamic calculations of the reservoir fluid, taking into account its moisture content, showed that a reservoir pressure decrease at the location of the three considered wells during the development of the achimov deposits for 20 years under project operation modes leads to the formation of a two-phase fluid (gas-unstable condensate) in bottomhole thermobaric conditions. Moreover, the aqueous liquid phase due to the high temperature of the flow at the well bottomhole is not formed during the entire calculation period. Hydrodynamic parameters calculations of the gas-liquid upward flow showed that high flow rates of well products provide conditions for the complete and continuous removal of unstable condensate by the gas flow from the bottom to the surface of all considered wells for the entire 20-year period, thereby preventing the physical and chemical conditions of electrolyte formation on tailpipes surface and carbon dioxide corrosion.

Types and characters of Andrei Platonov, his skill in creating artistic images, forms and ways of presenting the author's world – a topical historical and literary problem, which is considered in the context of modern research of Russian literature in the 1920s-1930s. This work is devoted to “innermost men” – Platonov author's discovery, certain artistic types, individualised, original and archetypal at the same time. They appeared as the result of Platonov's thought-making and language-making, superimposed on the colour of the epoch, historical, literary and cultural traditions. These and other difficult-to-grasp connections, intersections, and interactions were characterised in the course of research, which is based on the methodology of mythopoetic reconstruction of textual structures and discursive practices. Folklore and mythopoetic archetypes were established, and Platonov's creative motives were systematised. The most significant results of the work are the following. The existing ideas about the type of Platonov's artistic thinking are supplemented; the historical, literary and cultural context of the author's position formation is revealed; the functional side of mythopoetic motifs and archetypes in the creation of images is shown; new assessments of the content and form of a number of well-known works are given. Skill of Platonov is meaningful in the context of contemporary Russian prose of the 20th century, of neonatologists and word creation; augmented scientific understanding of complex phenomena in Russian literature of the first half of the 20th century.

The purpose of the work is to develop recommendations in the field of infrastructure management in the context of directions for the development of medical and health tourism. Infrastructure as a system is considered from the standpoint of ensuring the conditions for the implementation of an effective main (core) activity of medical organizations. The conclusions and recommendations presented in the article are based on the research results obtained using the methods of system analysis, formalization, classification, and expert assessment. The implementation of the recommendations outlined in the article will contribute to the development of medical tourism in the domestic and foreign markets of medical services.

The most widely used design scheme of synchronous machines with radially magnetized permanent magnets and a nonmagnetic magnet holder is considered. To calculate the armature reaction inductive reactances in these machines, it is proposed to use the conventional approach and the expressions obtained on its basis for the armature reaction inductive reactances of synchronous machines with electromagnetic excitation. The features of permanent magnet machines can be taken into account by special calculated coefficients included in these expressions. On the basis of the magnetic field analytical solution of the armature reaction, the special calculated coefficient for the design scheme with a cylindrical yoke of the inductor was determined taking into account the increased nonmagnetic gap between the armature magnetic core and inductor yoke core. By applying the Schwartz method with the use of a scalar magnetic potential, a new magnetic field analytical solution of the armature reaction with a polyhedral yoke of the inductor is obtained. On the basis of this solution, the field form factors of the longitudinal and transverse armature reaction were determined, which take into account the real geometry of this design scheme. Graphical dependences of the special calculated coefficients on the active zone geometric parameters are given.

The purpose of the article is to consider an important scientific historical and literary issue: the place of A. M. Remizov, an outstanding representative of Russian neorealism, in the structure of the so-called Yaroslavl text. For the first time Remizov's auteur myth is inscribed into the structure of a local text, to which type the Yaroslavl text belongs. This literary phenomenon is presented in the context of studying the author's individuality along the lines common to Russian prose: the enrichment of content, the search for new narrative and artistic forms. The following objectives of the work were also dealt with in the context of contemporary studies of the first half of the XX century Russian literature: to clarify the definitions of the Yaroslavl text and the Remizov myth; to determine the biographical, narrative, historical and literary contours of the latter; to identify and establish the place of archetypes and motives in this myth; and, finally, to incorporate myth into the conceptual space of the Yaroslavl text. The most significant results of the work are the following: the historical-literary and personal-biographical components of the Remizov myth are described; the role of folklore, literary motives and archetypes in the system of motives and images of Remizov's prose is established; the latter are shown in the context of his creative evolution. Remizov's auteur myth is integrated into the historical and literary space of the Yaroslavl text. Some observations are made on the philosophy, aesthetics, and poetics of Remizov's prose and the expressiveness of his writings. Diaries and documents are taken into account as a full-fledged link in the writer's legacy; the attitude to them as secondary, auxiliary material has been overcome. The approaches used in the work led to an original view of the writer’s skills and allowed to break the existing opinions about Remizov's type of artistic thinking. The article evaluates the artistic merits of some of the well-known works in a new way, clarifies the writer's position, shows its connection with mythology and folklore, with the functions of the components of external and internal forms. Remizov's skill is analyzed in the context of neo-mythologism and word-making relevant to the Russian prose of the XX century. Thus, scientific understanding of the complex phenomena in Russian literature of the first third of the XX century have been enriched.

The article addresses a comprehensive study of the quality indicators of the voltage produced by an aircraft synchronous generator. The study is carried out by applying a finite element analysis performed using the ANSYS Maxwell software package taking as an example the three-phase 115/200 V AC aircraft power supply system with a constant frequency of 400 Hz. To check the possibility of applying a finite element analysis, an analytical calculation of an aircraft synchronous generator was performed; its model was constructed in the ANSYS Maxwell software package environment, and the generator operation was simulated in its idle running mode and under the conditions of its balanced and unbalanced load. A finite element analysis of the model with an external electrical circuit connected to it is carried out. The construction and connection of electrical circuits makes it possible to implement the generator operation in different modes for obtaining the output phase voltage waveforms. Based on the simulation results, the electric power quality indicators (the amplitude coefficient, voltage modulation, voltage imbalance, phase voltage shift, distortion coefficient and harmonic components) have been estimated in the steady-state mode for their compliance with the normative values specified by the State Standard GOST R 54073-2017. By using the proposed method, it is possible to estimate the generated voltage quality indicators at the stage of developing an electromechanical converter, which will help reduce the time and cost of electrical machine design process.

The model coil with the racetrack geometry based on second generation High Temperature Superconducting tapes has been developed for an electrical machine where a winding pack is a stack of 2G HTS tapes. It is important to evaluate transport current AC losses and possible methods to reduce them in a winding. In a device made of 2G HTS tapes, the main the AC losses are the hysteresis ones. Only numerical simulation permits to predict them in full. The FEM model for calculation of the hysteresis losses developed before for 2G HTS power cables was modified for a stack of the 2G HTS tapes in a coil. In this paper the methods to increase a computational speed are presented. Possible ways to reduce AC losses are discussed also. The details of the model and comparisons of calculations with measurements of the AC loss are presented.

There is considered a possibility to obtain a new ceramic material of alloy V95 by means of chemical dispersion of chips and sintering aluminum hydroxide powder obtained. The rheological properties of the powder obtained are analyzed, with the aid of laser analyzer there is defined an average particles size and their specific surface, the X-ray phase analysis is carried out.

The paper describes manufacturing and testing of HTS double racetrack coils intended for use in stator windings of small-size motor, proposed for electric unmanned air vehicle (UAV). The level of AC losses of the armature characterizes required flow of liquid hydrogen (LH) coolant. Thus, it affects overall configuration of the UAV by LH-tank mass and volume. On the other side, motor will be driven by AC motor controller (inverter), so the armature currents will not be pure sinusoidal, and formed by pulse width modulation (PWM). In this work, the manufactured coil is tested on meander-form AC current with frequencies up to 200 Hz with and without solid ferromagnetic core. In addition, losses on PWM currents were also determined on a specific H-bridge to ensure functionality of HTS coils in such conditions.

Analysis of the global practice shows versatile innovative tools of the regional cluster institutional development: in the field of functional institutions it's introduction of the cluster ambassador function into its functional framework, development of training programs for cluster managers and facilitators, elaboration of cluster cooperation patterns with the consumer community; in the field of framework institutions it's introduction of the regional open-government pattern, establishment of specialized body system, encouragement of cluster members' integration into business and social networks; in the field of regulatory strategic institutions it's introduction of the practice of collecting relevant data on clusters beyond industrial and statistical data, development of a special strategy for regional cluster development, promotion of intersectoral and interregional cluster initiatives; in the field of mental institutions it's a shifting focus of strategic developments from competitive advantages to collaborative ones of the region, including marketing, branding and cluster identity establishment.

Brucellosis is a widespread zoonotic disease considered as an emerging and re-emerging disease with a resulting threat of public health and animal health. Official reports document an animal incidence in Kazakhstan of about 0.6% per year, and the country still registers high number of human cases annually . The main objective of this paper was to evaluate the distribution and economic impact of brucellosis in Kazakhstan. We analysed human disease incidence data obtained from the Government Sanitary & Epidemiological Service with the aim to estimate the burden of disease in terms of disability-adjusted life years (DALYs). We also estimated the economic impact in terms of monetary losses. Additionally, we mapped the geographical distribution of the disease throughout Kazakhstan. In total, 1,334 human cases of brucellosis were registered in 2015 in Kazakhstan that resulted in 713 DALYs. Around US Dollar 21 million was spent on compensation for animals that had to be slaughtered due to brucellosis, and an additional US Dollar 24 million was spent on testing animals. Animal brucellosis and human brucellosis occur throughout the whole country, some trends of which are reviewed in this paper. We estimated the burden of the disease and explored possible explanation for high human incidence rates. This paper is the first to estimate the human burden of disease and the economic costs in Kazakhstan. Both of these are substantial.

Possibility of heated petroleum application with various additives addition as solvents for asphaltene-resin-paraffin deposits was analyzed. An object of this investigation was chosen petroleum selected from tank farm of the Kazakhstan oil refinery and samples of bottom sediments (ARPD) formed during storage in tanks. Deposits type in line with ARPD classification was identified. Additives impact on petroleum capacity in relation to ARPD was investigated. The most effective additives to inhibiting and removing bottom settlings formed in petroleum storage tanks were determined.

Solid waste management is one of the main problems in large cities. The determination of the quantitative and qualitative composition of municipal solid waste is necessary for proper planning in the processing of such waste. This article presents the results of studies of the morphological composition, physico-chemical parameters and energetical potential of municipal solid waste (MSW) from the landfill of the city of Shymkent. Waste samples were taken from 10 different points of the landfill. The volume–mass parameters of the samples and their average density were determined. The combined waste samples mainly consisted of food waste, paper and cardboard, polymers and plastic, glass, metal, textiles, wood, leather, bones and rubber. Most of the waste, more than 30%, contains plastic products. The moisture of the combustible fraction of the samples is low and varies from 0.3% to 2.3%. The average ash content of the combustible fraction of the samples was determined and its average value is 18.4%. The yield of the volatile substances was determined and the average value per dry state is 78.7%. To determine the energetical potential of municipal solid waste, the gross calorific value was determined and the net calorific value was calculated, the average value of which is 24.15 MJ/kg. This shows the possibility of using solid waste as an energy source.

Pancreatic neuroendocrine tumors (PNETs) occur more frequently in men and are associated with higher mortality in males; however, the molecular basis for these sexual dimorphisms is unclear. Here, we demonstrate that PNETs are associated with the emergence of unique sex-specific transcriptomic differences that are not observed in non-neoplastic pancreatic islet tissues. We also show that while widespread sex-specific differences are present in the DNA methylation landscapes of control pancreatic islets, they are erased in PNETs. This includes a loss of imprinting with regards to many genes. These results implicate an emergence of sex-associated genetic and epigenetic dysregulations in PNETs.

The paper presents the computed data on water flow and heat transfer through a staggered tube bank typical for subsea cooling. The numerical study of unsteady buoyancy-induced flow through cooling units that could be used also as ocean thermal energy extractors have been performed for Gr = 2×105. Three computational models of the cooler are considered: the tube bank without any bounding walls, the tube bank with vertical bounding walls and the tube bank with the side walls and the bottom wall. The direct numerical simulation is applied to model the draft water flow and heat transfer at the Prandtl number of 6.09. Tube bank external heat transfer enhancement in the presence of the bounding walls is reported and discussed.

This paper deals with the assessment of draught rate (DR) in a ventilated university classroom (field lab). Based on one hour-long measurements with a sampling rate of 5 Hz of both air velocity and air temperature under recirculation mode of operation of four ceiling fan coils, DR time variation at 27 space points is obtained. Two cases that have a strong impact on the flow pattern in the investigated space are considered: an empty room and a furnished room. Moving intervals with a length of 1 min, 3 min, and 15 min are used to calculate DR.

This paper deals with assessing draught rate (DR) in a ventilated university classroom (field lab). Based on eight hours long continuous measurements of indoor air velocity, and air temperature, as well as the globe temperature under recirculation mode of operation of four ceiling fan coils, information about the time variation of air velocity, air temperature, mean radiant temperature and DR at four space points is obtained. Two cases are considered: an empty room and a furnished room.

The paper presents the computed data on air distribution in a test classroom with four ceiling diffusers supplying several interacting jets. Results of Unsteady Reynolds-Averaged Navies-Stokes (URANS) simulation under isothermal conditions are considered. The main objective of the study is the evaluation of the effect of supply diffuser operation mode on the flow structure and thermal comfort parameters. The paper presents the validation of the computed data by means of comparison with the full-scale measurement results obtained with omnidirectional thermo-anemometric sensors under summer conditions with no occupants. Assessment of draught rate in the occupied zone is presented and discussed.

The article is devoted to the study of losses in devices based on high-temperature superconductors of the 2nd generation. The complexity of the devices under study increases from a single rack coil to a winding assembled from several coils, and finally to an electric machine operating in generator mode. This is the way to experimentally study the behavior of 2nd generation high temperature superconductor (2G HTS) carrying a transport current in various conditions: self-field, external DC, and AC magnetic field. Attention is also paid to the losses in the winding during its operation from the inverter, which simulates the operating conditions in the motor mode of a propulsion system.

In less than nine months, the Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) killed over a million people, including >25,000 in New York City (NYC) alone. The COVID-19 pandemic caused by SARS-CoV-2 highlights clinical needs to detect infection, track strain evolution, and identify biomarkers of disease course. To address these challenges, we designed a fast (30-minute) colorimetric test (LAMP) for SARS-CoV-2 infection from naso/oropharyngeal swabs and a large-scale shotgun metatranscriptomics platform (total-RNA-seq) for host, viral, and microbial profiling. We applied these methods to clinical specimens gathered from 669 patients in New York City during the first two months of the outbreak, yielding a broad molecular portrait of the emerging COVID-19 disease. We find significant enrichment of a NYC-distinctive clade of the virus (20C), as well as host responses in interferon, ACE, hematological, and olfaction pathways. In addition, we use 50,821 patient records to find that renin–angiotensin–aldosterone system inhibitors have a protective effect for severe COVID-19 outcomes, unlike similar drugs. Finally, spatial transcriptomic data from COVID-19 patient autopsy tissues reveal distinct ACE2 expression loci, with macrophage and neutrophil infiltration in the lungs. These findings can inform public health and may help develop and drive SARS-CoV-2 diagnostic, prevention, and treatment strategies.

In the process of manufacturing products from high-temperature superconductors (HTS), quality control must be carried out. Traditionally, for HTS coils, electrical tests are carried out to determine critical current. In the case of an unacceptable result, it is necessary to determine the cause. Therefore, it is necessary to develop nondestructive testing methods. This article proposes a technology for manufacturing quality evaluation. It is based on determining the actual location of the tape and the gaps between the turns and rows of the coil and analyzing these values. For this purpose, samples were scanned using computed tomography (CT) with a Nordson Dage XD7600NT X-ray inspection system with a μCT module. The obtained data were analyzed using VolumeGraphics VGStudio 2.2 software. Furthermore, the proposed technology can be used as part of a predictive analysis of the state of HTS coils in the windings of electrical machines.

Pancreatic neuroendocrine tumors (PNETs) are rare neoplasms that arise from cells in the islets of Langerhans, with surgical resection presently recommended for tumors > 2cm. While many PNETs have the propensity to be indolent, some small tumors display aggressive features with early metastatic potential. We used machine learning to develop a predictive model of metastatic potential dependent upon the transcriptomic signature of primary PNET tissue. To build this model, RNA sequencing data was obtained from the primary tissue of 96 surgically-resected PNETs from various institutions. Two cohorts were generated with equally balanced metastatic PNET composition (15 (32.6%) vs. 13 (26.5%), p=0.52). A differential gene expression analysis identified 20 concordantly differentially expressed genes associated with metastatic status between the two cohorts. Unsupervised surrogate variable analysis estimated and adjusted for significant sources of variation not related to metastatic potential and mitigated unwanted noise and batch effects. A gene set enrichment analysis identified an additional 29 genes that most frequently contributed to the enriched biologic pathways extrapolated from the sequencing data. Log transformed, batch corrected TPM values for these 49 genes were combined with an additional 10 clinically-relevant genes, including ARX and PDX1, that are known to contribute to PNET signatures or oncogenesis. The datasets were subsequently randomized in a 1:1 ratio and informative features with respect to metastatic status were identified utilizing a Boruta algorithm, with a priori exclusion of highly-correlative genes and those that displayed near zero variance. Nine genes, including AURKA, ARX, CDCA8, CPB2, MYT1L, NDC80, PAPPA2, SFMBT1 and ZPLD1, were identified as sufficient to classify the localized or metastatic outcome. Distributed random forests (DRF), generalized linear models (GLM), gradient boosting machines (GBM) and extreme gradient boosting (XGBoost) models were trained utilizing these 9 genes. Training ROC ranged from 0.92 for DRF to 1 for XGboost. When applied to 47 independent validation samples, the testing sensitivity ranged from 75% for DRF to 94% for GBM; specificity ranged from 84% for DRF to 94% to XGboost and GLM; positive predictive value ranged from 72% for DRF to 86% for GLM; negative predictive value ranged from 88% for GLM to 97% to GBM. The degree of predictive agreement between models ranged from 64% to 91%. Taken together, we have developed a highly sensitive predictive model of the metastatic PNET phenotype that is based on expression of nine genes. Disparities exist in access to high-volume surgeons, who have better outcomes after thyroidectomy. The association of the Affordable Care Act's Medicaid expansion with access to high-volume thyroid cancer surgery centers remains unclear.The National Cancer Database was queried for all adult thyroid cancer patients diagnosed from 2010 to 2016. Hospital quartiles (Q1-4) defined by operative volume were generated. Clinicodemographics and adjusted odds ratios for treatment per quartile were analyzed by insurance status. An adjusted difference-in-differences analysis examined the association between implementation of the Affordable Care Act and changes in payer mix by hospital quartile.In total, 241,448 patients were included. Medicaid patients were most commonly treated at Q3-Q4 hospitals (Q3 odds ratios 1.05, P = .020, Q4 1.11, P.001), whereas uninsured patients were most often treated at Q2-Q4 hospitals (Q2 odds ratios 2.82, Q3 2.34, Q4 2.07, P.001). After expansion, Medicaid patients had lower odds of surgery at Q3-Q4 compared with Q1 hospitals (odds ratios Q3 0.82, P.001 Q4 0.85, P = .002) in expansion states, but higher odds of treatment at Q3-Q4 hospitals in nonexpansion states (odds ratios Q3 2.23, Q4 1.86, P.001). Affordable Care Act implementation was associated with increased proportions of Medicaid patients within each quartile in expansion compared with nonexpansion states (Q1 adjusted difference-in-differences 5.36%, Q2 5.29%, Q3 3.68%, Q4 3.26%, P.001), and a decrease in uninsured patients treated at Q4 hospitals (adjusted difference-in-differences -1.06%, P = .001).Medicaid expansion was associated with an increased proportion of Medicaid patients undergoing thyroidectomy for thyroid cancer in all quartiles, with increased Medicaid access to high-volume centers in expansion compared with nonexpansion states.

International audience; Electrical machines are important parts of different power systems. The application of high temperature superconductors (HTS) in electrical machines is very promising due to high transport currents. This paper reviews various topologies of superconducting motors and generators using HTS published in the literature in recent time. It begins with a brief presentation of the HTS material used in electrical machines. The description of AC losses and cryogenic systems is done afterwards. Then we offer a striking description of the various realizations of HTS electrical machines such as half HTS synchronous machines, fully HTS synchronous machine, machines with HTS bulks and stacks. Some of these machines are totally innovative compared to conventional ones and their operating principle is strictly related to the presence of HTS materials.

The article presents the results of experimental studies of a racetrack 2G HTS winding in the presence of an external constant magnetic field. To create high-performance devices based on 2G HTS tapes, it is very important to know what transport current can be provided by HTS coils. To clarify the level of transport current of HTS coils and make accurate predictions about it, a specialized bench was created for testing straight samples and racetrack coils in the presence of a variable magnetic field. Discrete sensors are used for measuring the field components perpendicular and parallel to the tape. Measurements of losses in the coil at different values of the external magnetic field with an alternating sinusoidal and high-frequency modulated current at several frequencies and amplitudes are performed and discussed.
