1. Testing of pandemic ventilators under early and agile development

Abstract: Aiming to address clinical requirements subsequent to SARS-CoV-2-related pulmonary disease, multiple research groups and industry groups carried out intensive studies to develop pandemic ventilators (PDVs). In vitro testing to critically evaluate the specific performance of the developed apparatuses is an essential requirement. This study presents a test protocol which promotes a test-oriented, iterative, and agile assessment and consecutive development of such PDVs. It allows for fast identification of specific characteristics of each PDV in the individual test features. The test protocol includes an evaluation of the accuracy of control systems and instruments at changing parameters, the oxygen dynamics, and the response to trigger signals. The test environment is a mechanical lung, which allows reproducing various lung mechanics and to simulate active breathing cycles. A total of three PDVs that are under development were iteratively tested, with a Hamilton T1 as a reference. Continuous testing of the PDVs under development enables quick identification of critical application aspects that deserve further improved. Based on the present test protocol, the ventilators demonstrate a promising performance justifying continued development.

Abstract: Abstract Background Diagnosis of acute appendicitis (AA) and decisions about its treatment remain among the most common dilemmas of pediatric surgical teams. Monitoring of immune response may be of importance for this purpose. Our aim was to measure and analyze serum and peritoneal fluid cytokines, in children who had undergone surgery for suspected AA. Methods Prospective investigation of serum and peritoneal fluid cytokine values was performed in 127 consecutive patients. According to the pathohistological findings, patients were divided into three groups: normal/early, uncomplicated and complicated AA. Determination of cytokine concentrations for 20 different cytokines was done using a commercial flow cytometry kit: Human Inflammation 20 plex BMS 819. Results Statistically significant differences in serum cytokine values between pathohistological groups were found for IP-10, MIP-1α and IL-10. Preoperative cut-off values of IP-10, MIP-1α and IL-10 between groups were obtained using ROC curve analysis. Positive correlations between serum and peritoneal concentrations were recorded for most of the analyzed cytokines. Conclusion IP-10, MIP-1α and IL-10 showed potential in assessment of AA in children. Confirmatory studies with a larger number of patients are required to prove reliability of these biomarkers.

3. Socioeconomic status and in-hospital cardiac arrest: A systematic review

Abstract: Aim: To perform a review of the literature on the association between socioeconomic status and risk of and outcomes after in-hospital cardiac arrest. Data sources: PubMed and Embase were searched on January 24, 2020 for studies evaluating the association between socioeconomic status and risk of and/or outcomes after in-hospital cardiac arrest. Two reviewers independently screened the titles/abstracts and selected full texts for relevance. Data were extracted from included studies. Risk of bias was assessed using the Quality In Prognosis Studies (QUIPS) tool. Results: The literature search yielded 4960 unique records. We included nine studies evaluating the association between socioeconomic status and risk of and/or outcomes after in-hospital cardiac arrest. All studies were observational cohort studies, of which seven were from the USA. Seven studies were in an adult population, while two studies were in a pediatric population. Results were overall inconsistent although some studies found a higher in-hospital cardiac arrest incidence in patients from low-income communities. There was no clear association between other socioeconomic factors (i.e. education, occupation, marital status, and insurance) and risk of or outcomes after in-hospital cardiac arrest. Due to the scarcity and heterogeneity of available studies, meta-analyses were not performed. Conclusion: There are limited data regarding the association between socioeconomic status and risk of and outcomes after in-hospital cardiac arrest and further research is warranted. Understanding the association between socioeconomic status and in-hospital cardiac arrest may reveal strategies to mitigate potential inequalities.

Abstract: Academic degrees following author names are often included in medical research papers. However, it remains unclear how many journals choose to include academic degrees and whether this is more common in certain types of journals. We examined the 100 highest impact medical journals and found that only 24 medical journals reported academic degrees. Moreover, this was substantially more common in journals based in North America compared with Europe. Further research is required to explore the implications of listing academic degrees on the readers’ attitude towards research quality.

Abstract: This paper presents a control philosophy for multiterminal DC grids, which are embedded in the main AC grid. DC transmission lines maintain higher power flow at longer distances compared with AC lines. The voltage losses are also much lower. DC power transmission is good option for Russian north. Arctic seashore regions of Russia don't have well developed electrical infrastructure therefore power line lengths are significant there. Considering above it is possible to use DC grids for supply mining enterprises in Arctic regions (offshore drilling platforms for example). Three different control layers are presented in an hierarchical way: local, primary and secondary. This whole control strategy is veriﬁed in a scaled three-nodes DC grid. In one of these nodes, a modular multilevel converter (MMC) is implemented (ﬁve sub-modules per arm). A novel model-based optimization method to control AC and circulating currents is discussed. In the remaining nodes, three-level voltage source converters (VSC) are installed. For their local controllers, a new variant for classical PI controllers are used, which allow to adapt the values of the PI parameters with respect to the measured variables. Concerning the primary control, droop control technique has been chosen. Regarding secondary level, a new power ﬂow technique is suggested. Unbalance conditions are also veriﬁed in order to show the robustness of the whole control strategy.

Abstract: Background. Neutrophil-to-lymphocyte ratio (NLR), monocyte-to-lymphocyte ratio (MLR), platelet-to-lymphocyte ratio (PLR), and mean platelet volume-to-platelet count (MPV/PC) ratio are readily available parameters that might have discriminative power regarding outcome. The aim of our study was to assess prognostic value of these biomarkers regarding outcome in critically ill patients with secondary sepsis and/or trauma. Methods. A total of 392 critically ill and injured patients, admitted to surgical ICU, were enrolled in a prospective observational study. Leukocyte and platelet counts were recorded upon fulfilling Sepsis-3 criteria and for traumatized Injury Severity Score > 25 points. Patients were divided into four subgroups: peritonitis, pancreatitis, trauma with sepsis, and trauma without sepsis. Results. NLR and MPV/PC levels were significantly higher in nonsurvivors (AUC/ROC of 0.681 and 0.592, resp., in the peritonitis subgroup; 0.717 and 0.753, resp., in the pancreatitis subgroup); MLR and PLR did not differ significantly. There was no significant difference of investigated biomarkers between survivors and nonsurvivors in trauma patients with and without sepsis except for PLR in the trauma without sepsis subgroup (significantly higher in nonsurvivors, AUC/ROC of 0.719). Independent predictor of lethal outcome was NLR in the whole cohort and in the peritonitis subgroup as well as MPV in the pancreatitis subgroup. Also, there were statistically significant differences in MPV/PC, MLR, and PLR values regarding nature of bacteremia. In general, the lowest levels had been found in patients with Gram-positive blood cultures. Conclusions. NLR and MPV were very good independent predictors of lethal outcome. For the first time, we demonstrate that nature of bacteremia influences MPV/PC, MLR, and PLR. In heterogeneous cohort subgroup, analysis is essential.

Abstract: Background: Hypertrophic pyloric stenosis in otherwise healthy neonates frequently requires urgent surgical procedure but anaesthesia care may result in respiratory complications, such as hypoxaemia, pulmonary aspiration of gastric contents, and postoperative apnoea. The primary aim was to study whether or not the incidence of difficult airway management and of hypoxaemia in neonates undergoing pyloric stenosis repair was higher than that in neonates undergoing other surgeries. Methods: Data on neonates and infants undergoing anaesthesia and surgery for pyloric stenosis were extracted from the NEonate and Children audiT of Anesthesia pRactice In Europe (NECTARINE) database, for secondary analysis. Results: We identified 310 infants who had anaesthesia for surgery for pyloric stenosis. Difficult airway management (more than two attempts at laryngoscopy) was higher in children with pyloric stenosis when compared with the entire NECTARINE cohort (7.9% [95% confidence interval {CI}, 5.22–11.53] vs 4.4% [95% CI, 1.99–6.58]; relative risk [RR]=1.81 [95% CI, 1.21–2.69]; P=0.004), whereas transient hypoxaemia with oxygen saturation

Abstract: AIM: To investigate how socioeconomic status was associated with the risk of in-hospital cardiac arrest in Denmark.METHODS: We conducted a matched case-control study based on data from nationwide registries in Denmark. A total of 3,449 cases with in-hospital cardiac arrest in 2017 and 2018 were matched at the index time based on age and sex with up to 10 controls from the total Danish population and a hospitalized patient population, respectively. Household income, household assets, and education were used as measures of socioeconomic status. Conditional logistic regression was used to assess the association between socioeconomic status and the risk of in-hospital cardiac arrest.RESULTS: Across all analyses of cases and controls, high household income, high household assets, and higher education were associated with decreased odds of in-hospital cardiac arrest. In the analyses of cases and background controls, high household income was associated with 0.45 (95% CI: 0.40, 0.52) times the odds of in-hospital cardiac arrest compared to low household income, which was similar for household assets. Compared to basic education, higher education was associated with 0.50 (95% CI: 0.43, 0.58) times the odds of in-hospital cardiac arrest. The results attenuated marginally after adjustment for comorbidities. Similar albeit attenuated findings were observed in the analyses of cases and hospitalized controls.CONCLUSIONS: In this matched case-control study, high socioeconomic status was associated with lower odds of in-hospital cardiac arrest compared to low socioeconomic status. The findings were consistent across household income, household assets, and education and persisted after adjustment for comorbidities. Strategies are needed to address the socioeconomic inequalities observed in the risk of in-hospital cardiac arrest.

14. Intra-cardiac arrest transport and survival from out-of-hospital cardiac arrest: A nationwide observational study

Abstract: Aim: To assess whether intra-cardiac arrest transport as compared to continued on-scene resuscitation was associated with improved clinical outcomes among out-of-hospital cardiac arrest patients in Denmark. Methods: This was an observational study using data from population-based registries in Denmark. Adults (aged ≥ 18 to ≤ 65 years) with an out-of-hospital cardiac arrest attended by Emergency Medical Services (EMS) between 2016 and 2018 were included. The primary outcome was survival to 30 days. Time-dependent propensity score matching was used to match patients transported to the hospital within 20 minutes of EMS arrival to patients with assumed on-scene resuscitation (with or without subsequent intra-cardiac arrest transport) at risk of being transported within the same minute. Results: The full cohort included 2,873 cardiac arrests. The median age was 56 (quartiles: 48 to 62) years, 1987 (69%) were male, and 104 (4%) were transported within 20 minutes. A total of 87 transported patients were matched to 87 patients at risk of being transported based on the propensity score. Although not reaching statistical significance, in comparison with on-scene resuscitation, intra-cardiac arrest transport was associated with increased survival to 30 days (risk ratio, 1.55; 95%CI, 0.99–2.44; P = 0.06). Similar associations were observed for return of spontaneous circulation and survival to one year. Conclusions: Among patients aged 18–65 years, intra-cardiac arrest transport was associated with a non-significant increase in survival within 20 minutes of EMS on-scene arrival. However, the results did not eliminate the potential for bias and the results should be interpreted carefully.

Abstract: U radu je prikazana vremenska analiza emisije zagađujućih materija u vazduh na primeru dve najveće termoelektrane „Nikola Tesla A“ i „Nikola Tesla B“. Ogranak „Termoelektrane Nikola Tesla“ je najveći proizvođač električne energije u Jugoistočnoj Evropi. Ima 14 blokova čija je ukupna instalisana snaga 3.430 MW. U sastavu Ogranak „Termoelektrane Nikola Tesla“ su TE Nikola Tesla A (sa 6 blokova) 1650 MW projektovane snage i TE Nikola Tesla B (sa 2 bloka) 1.340 MW električne snage. U okviru „Termoelektrane Nikola Tesla A“ se nalaze blokovi A1 sa 660 MW, A2 sa 660 MW, A3 sa 932 MW, A4 sa 943 MW, A5 sa 934 MW i blok A6 sa 934 MW toplotne snage, dok u okviru „Termoelektrane Nikola Tesla B“ se nalaze dva bloka B1 i B2 sa po 1809 MW i 1826 MW toplotne snage. Trend emisije je prikazan za tri zagađujuće materije, sumpor-dioksid, azotove okside i praškaste materije za 2022., 2021. i 2020. godinu. Dimni gasovi koji sadrže zagađujuće materije se ispuštaju u vazduh preko dimnjaka visine 150 metara (blokovi А1, А2 и А3) и 220 metara (blokovi А4, А5 и А6) na „Termoelektrani Nikola Tesla A“ i 280 metara (blokovi B1 i B2) na „Termoelektrani Nikola Tesla B“. U skladu sa zakonskim zahtevima redovno vrše se kontinualna merenja, dok se periodična merenja vrše za proveru ispravnosti kontinualnih merenja.

Abstract: Background: We investigated hospital-level variation in outcomes after in-hospital cardiac arrest (IHCA) in Denmark, and assessed whether variation in outcomes could be explained by differences in patient characteristics. Methods: Adult patients (≥18 years old) with IHCA in 2017 and 2018 were included from the Danish IHCA Registry (DANARREST). Data on patient characteristics and outcomes were obtained from population-based registries. Predicted probabilities, likelihood ratio tests, intraclass correlation coefficients (ICCs), and median odds ratios (ORs) were calculated for return of spontaneous circulation (ROSC), survival to 30 days, and survival to 1 year. Results: A total of 3340 patients with IHCA from 24 hospitals were included. We found that hospital-level variation in outcomes after IHCA existed across all measures of variation. The unadjusted median OR for ROSC, survival to 30 days, and survival to 1 year were 1.28 (95% confidence interval [CI]: 1.24, 1.45), 1.38 (95% CI: 1.33, 1.60), and 1.44 (95% CI: 1.39, 1.70), respectively. The unadjusted ICC suggest that 2.0% (95%: 1.6%, 4.4%), 3.3% (95%: 2.7%, 6.8%), and 4.3% (95%: 3.5%, 8.6%) of the total individual variation in ROSC, survival to 30 days, and survival to 1 year was attributable to hospital-level variation. These results decreased but persisted in the analyses adjusted for select patient characteristics. Conclusions: In this study, we found that outcomes after IHCA varied across hospitals in Denmark. However, only about 2%–4% of the total individual variation in outcomes after IHCA was attributable to differences between hospitals, suggesting that most of the individual variation in outcomes was attributable to patient-level variation.

Abstract: Background: Neonates and infants are susceptible to hypoxaemia in the perioperative period. The aim of this study was to analyse interventions related to anaesthesia tracheal intubations in this European cohort and identify their clinical consequences. Methods: We performed a secondary analysis of tracheal intubations of the European multicentre observational trial (NEonate and Children audiT of Anaesthesia pRactice IN Europe [NECTARINE]) in neonates and small infants with difficult tracheal intubation. The primary endpoint was the incidence of difficult intubation and the related complications. The secondary endpoints were the risk factors for severe hypoxaemia attributed to difficult airway management, and 30 and 90 day outcomes. Results: Tracheal intubation was planned in 4683 procedures. Difficult tracheal intubation, defined as two failed attempts of direct laryngoscopy, occurred in 266 children (271 procedures) with an incidence (95% confidence interval [CI]) of 5.8% (95% CI, 5.1e6.5). Abstract: BACKGROUND Little is known about current clinical practice concerning peri-operative red blood cell transfusion in neonates and small infants. Guidelines suggest transfusions based on haemoglobin thresholds ranging from 8.5 to 12��g��dl-1, distinguishing between children from birth to day 7 (week 1), from day 8 to day 14 (week 2) or from day 15 (��week 3) onwards. OBJECTIVE To observe peri-operative red blood cell transfusion practice according to guidelines in relation to patient outcome. DESIGN A multicentre observational study. SETTING The NEonate-Children sTudy of Anaesthesia pRactice IN Europe (NECTARINE) trial recruited patients up to 60 weeks' postmenstrual age undergoing anaesthesia for surgical or diagnostic procedures from 165 centres in 31 European countries between March 2016 and January 2017. PATIENTS The data included 5609 patients undergoing 6542 procedures. Inclusion criteria was a peri-operative red blood cell transfusion. MAIN OUTCOME MEASURES The primary endpoint was the haemoglobin level triggering a transfusion for neonates in week 1, week 2 and week 3. Secondary endpoints were transfusion volumes, 'delta haemoglobin' (preprocedure - transfusion-triggering) and 30-day and 90-day morbidity and mortality. RESULTS Peri-operative red blood cell transfusions were recorded during 447 procedures (6.9%). The median haemoglobin levels triggering a transfusion were 9.6 [IQR 8.7 to 10.9] g��dl-1 for neonates in week 1, 9.6 [7.7 to 10.4] g��dl-1 in week 2 and 8.0 [7.3 to 9.0] g��dl-1 in week 3. The median transfusion volume was 17.1 [11.1 to 26.4] ml��kg-1 with a median delta haemoglobin of 1.8 [0.0 to 3.6] g��dl-1. Thirty-day morbidity was 47.8% with an overall mortality of 11.3%. CONCLUSIONS Results indicate lower transfusion-triggering haemoglobin thresholds in clinical practice than suggested by current guidelines. The high morbidity and mortality of this NECTARINE sub-cohort calls for investigative action and evidence-based guidelines addressing peri-operative red blood cell transfusions strategies. 26. Socioeconomic status and outcomes after in-hospital cardiac arrest

Abstract: AIM: To investigate the association between socioeconomic status and outcomes after in-hospital cardiac arrest in Denmark.METHODS: We conducted an observational cohort study based on nationwide registries and prospectively collected data on in-hospital cardiac arrest from 2017 and 2018 in Denmark. Unadjusted and adjusted analyses using regression models were performed to assess the association between socioeconomic status and outcomes after in-hospital cardiac arrest. Outcomes included return of spontaneous circulation (ROSC), survival to 30 days, survival to one year, and the duration of resuscitation among patients without ROSC.RESULTS: A total of 3,223 patients with in-hospital cardiac arrest were included in the study. In the adjusted analyses, high household assets were associated with 1.20 (95%CI: 0.96, 1.51) times the odds of ROSC, 1.49 (95%CI: 1.14, 1.96) times the odds of survival to 30 days, 1.40 (95%CI: 1.04, 1.90) times the odds of survival to one year, and 2.8 (95%CI: 0.9, 4.7) minutes longer duration of resuscitation among patients without ROSC compared to low household assets. Similar albeit attenuated associations were observed for education. While high household income was associated with better outcomes in the unadjusted analyses, these associations largely disappeared in the adjusted analyses.CONCLUSIONS: In this study of patients with in-hospital cardiac arrest, we found that high household assets were associated with a higher odds of survival and a longer duration of resuscitation among patients without ROSC compared to low household assets. However, the effect size may potentially be small. The results varied based on socioeconomic status measure, outcome of interest, and across adjusted analyses.

Abstract: Background/Aim. The inflammatory response is of utmost importance in bone healing, but the precise role of cells and cytokines remains unclear. In our study, we examined the association between interleukin-1? (IL-1?), tumor necrosis factors alpha (TNF-?), monocyte chemoattractant protein-1 (MCP-1) and interleukin-8 (IL-8) concentrations, fracture configuration, and callus formation. Methods. Serum cytokine concentrations were determined in 78 non-obese children with long bone fractures (group 1), 10 children with finger fractures (group 2), and 10 healthy controls (group 3). Blood samples were taken immediately after fracture upon hospital admission for groups 1 and 2. Differences in cytokine concentrations were analyzed among groups and catego-rized according to fracture configuration and callus formation. Results. IL-1? and TNF-? levels were lower in patients that went on to produce incomplete callus compared with patients that formed complete callus. Surprisingly, the average IL-1? concentration was highest in the healthy control group. The only significant correlation between IL-1? and TNF-? was in the group with moderate callus formation. MCP-1 level was slightly increased in all patient groups compared to controls, with no mutual difference. An average IL-8 level showed a clear decrease tendency from the group with incompletely formed callus toward the group with completely formed callus compared to controls, without significant difference. Children with epiphysiolysis had the lowest concentrations of cytokines compared with all other fracture types including transverse, oblique, and spiral. There were significantly lower concentrations of IL-1? and MCP-1 in patients with less fragment displacement compared with patients with greater fragment displacement. Conclusion. The systemic inflammatory response is important in physiological bone healing. High early production of IL-1?, TNF-?, and MCP-1 is associated with greater callus formation and better healing outcome, while increased IL-8 level is associated with poor callus formation and worse healing outcome. Our results indicate that epiphysiolysis and larger fragment displacement are associated with delayed fracture healing.

Abstract: As the production of metal-based nanomaterials increases, it is inevitable that nano-scale products and byproducts will enter the aquatic environment. In terms of global production, the most abundant nano-oxides are TiO2, CeO2 and Fe3O4 nanoparticles. Chironomus riparius is commonly used for ecotoxicological assessment and defining its histopathological biomarkers that showcase the toxic effect of tested nanoparticles should lead to a better understanding of the consequences of nanomaterial accumulation in aquatic ecosystems. In this study, a histological description of the digestive and excretory systems as well as the fat body structure of C. riparius larvae is provided. In addition, potential histological biomarkers of nano-oxide toxicity were determined based on the obtained histopathological alterations in organs. Vacuolization was observed in epithelial cells of midgut region I that were treated with nano-Fe3O4 as well as in Malpighian tubules treated with nano-Fe3O4 and nano-CeO2. Larvae exposed to nano-TiO2 showed alterations in the fat body and midgut region II tissue architecture. Additionally, shortening of the intestinal brush border was determined in groups exposed to nano-Fe3O4. These results reveal the high sensitivity of these organs, which can be used as biomarkers in histopathological assessment and therefore lead to further improvement of existing methodology in ecotoxicological studies.

Abstract: The increasing production and utilization of metal-oxide nanoparticles (NPs) such as titanium dioxide (TiO2), cerium dioxide (CeO2), and magnetite (Fe3O4) have raised concerns regarding their environmental impact. This study aimed to assess the sub-organismal biomarkers of Chironomus riparius larvae exposed to these NPs, focusing on target organs, potential DNA damage, oxidative stress parameters, and hemoglobin levels. Chronic exposure experiments were conducted using OECD guidelines, with histological analyses revealing shortened microvilli in the midgut of larvae treated with CeO2 and Fe3O4 NPs. DNA damage, as measured by the comet assay, was significantly higher in larvae treated with CeO2 and Fe3O4 NPs. Oxidative stress parameters, including thiobarbituric acid reactive substances (TBARS) and advanced oxidation protein products (AOPP), were elevated in groups treated with CeO2 and Fe3O4 NPs, with significantly higher activity of catalase (CAT) was also noticed in the latter group. Group treated with TiO2 NPs had increased activity of superoxide dismutase (SOD). Hemoglobin levels were significantly affected, with increased concentrations in TiO2 and Fe3O4 treated groups, and decreased levels in the CeO2 treated group. The results of the study support the multi-biomarker approach as highly beneficial in understanding the mechanisms of NPs toxicity in aquatic ecotoxicology by detecting histopathological biomarkers along with sub-organismal biomarkers such as DNA damage, oxidative stress parameters, and hemoglobin concentration.

Abstract: INTRODUCTION: While specific factors have been associated with outcomes after in-hospital cardiac arrest, the association between sex and outcomes remains debated. Moreover, age-specific sex differences in outcomes have not been fully characterized in this population.METHODS: Adult patients (≥18 years) with an index in-hospital cardiac arrest were included from the Danish In-Hospital Cardiac Arrest Registry (DANARREST) from January 1st, 2017 to December 31st, 2018. Population-based registries were used to obtain data on patient characteristics, cardiac arrest characteristics, and outcomes. Unadjusted and adjusted estimates for return of spontaneous circulation (ROSC), survival to 30 days, survival to one year, duration of resuscitation, and post-cardiac arrest time-to-death were computed.RESULTS: A total of 3266 patients were included, of which 2041 (62%) patients were male with a median age of 73 years (quartiles: 64, 80). Among 1225 (38%) female patients, the median age was 76 years (quartiles: 67, 83). Younger age was associated with higher odds of ROSC and survival. Sex was not associated with ROSC and survival in the unadjusted analyses. In the adjusted analyses, women had 1.32 (95%CI: 1.12, 1.54) times the odds of survival to 30 days and 1.26 (95%CI: 1.02, 1.57) times the odds of survival to one year compared to men. The overall association between sex and survival did not vary substantially across age categories, although female sex was associated with a higher survival within certain age categories. Among patients who did not achieve ROSC, female sex was associated with a shorter duration of resuscitation, which was more pronounced in younger age categories.CONCLUSIONS: In this study of patients with in-hospital cardiac arrest, female sex was associated with a shorter duration of resuscitation among patients without ROSC but a higher survival to 30 days and one year. While the overall association between sex and outcomes did not vary substantially across age categories, female sex was associated with a higher survival within certain age categories.

The aim of this study was to examine the association of the myeloid-derived suppressor cells (MDSCs) with the type of infecting microorganisms (Gram positive, Gram negative, polymicrobial) and underlying cause of secondary sepsis (peritonitis, pancreatitis, trauma). Methods. Totally, 40 critically ill patients with secondary sepsis were enrolled in the prospective study. Two patients without documented positive blood culture were excluded. We detected and enumerated both main subsets of MDSCs: granulocytic (G)-MDSCs and monocytic (M)-MDSCs on the Days 1 and 5. Blood was simultaneously drawn for a blood culture. The patients with different underlying causes of sepsis (peritonitis, pancreatitis, trauma) were perceived as separated groups and the frequencies and absolute numbers of their G-MDSCs and M-MDSCs were compared. Results. Both main MDSC subpopulations were accumulated significantly in Grampositive sepsis. Univariate logistic regression analyses of investigated variables regarding Gram-positive sepsis on the Day 5 revealed that G-MDSCs absolute number along with both MMDSCs frequency and absolute number had statistically significant power for predicting Gram-positive sepsis. Stepwise multivariate logistic regression analyses of the variables on the Day 5 determined that M-MDSCs absolute number was independent predictor of Gram-positive sepsis [odds ratio (OR) 1.012; p < 0.05]. Clinical accuracy of neutrophil (Ne)/GMDSCs (Ne/G-MDSCs) and monocyte (Mo)/M-MDSCs (Mo/M-MDSCs) ratios in predicting nature of bacteremia and outcome were investigated. Discriminative power of both Ne/G-MDSCs and Mo/M-MDSCs ratios in predicting Grampositive blood culture was statistically significant both on the Day 1 and Day 5 [areas under curve (AUCs): 0.684 and 0.692, and 0.707 and 0.793, respectively). Ne/G-MDSCs both on the Day 1 and Day 5 were statistically significant predictors of lethal outcome (AUCs: 0.694 and 0.678, respectively). There were no statistically significant differences in G-MDSCs and M-MDSCs among different three groups of patients regarding peritonitis, pancreatitis and trauma as causes of sepsis neither on the Day 1 nor on the Day 5. Conclusion. Gram-positive infectious agents were powerful inducers of MDSCs generation in sepsis. Also, underlying causes of secondary sepsis might not seem to influence the MDSCs accumulation.

The aim of our study was to determine whether frequency and/or absolute numbers of the MDSCs were associated with outcome in critically ill patients with secondary sepsis and/or septic shock. Methods. Total of 40 critically ill patients with secondary sepsis were enrolled in a prospective study. We detected and enumerated both main subsets of MDSCs: granulocytic (G)-MDSCs and monocytic (M)- MDSCs on the Day 1 (the day of hospital admission) and the Day 5 after the. The primary end-point was hospital mortality. Results. Increased frequencies and absolute numbers of subpopulations corresponding to MDSCs were associated with poor outcome. As far as relative kinetics was concerned, in both survivors and non-survivors, sepsis duration from 1th to 5th day was accompanied by an increase in MDSCs values of both investigated subpopulations. In contrast to findings of stepwise multivariate logistic regression analysis of the variables on the Day 1, on the Day 5 it was determined that the Sequential Organ Failure Assessment (SOFA) score (OR 2.350; p < 0.05) and G-MDSCs frequencies (OR 3.575; p < 0.05) were independent predictors of lethal outcome. Conclusion. These findings suggest harmful role of MDSCs in secondary sepsis.

Abstract: Introduction. Tracheoesophageal fistula (TEF) as a complication of balloon dilatation (BD) of corrosive esophageal stricture is a very rare and serious condition. Life threatening aspiration pneumonia requests urgent lungs' protection, but overall treatment strategy is not clearly defined. Case report. Twenty-month-old female child accidentally ingested a household bleach. Caustic injury of esophagus was healing with development of strictures of cervical and proximal thoracic esophagus. TEF was developed during the third BD. Healing of TEF and pulmonary infection was achieved by exclusion of the esophagus (pharyngostoma and feeding gastrostomy together) with prolonged tracheobronchial intubation and toilette. Retrosternal colon interposition was performed a year later, with excellent functional results over four-year follow-up. Conclusion. Esophageal exclusion in the first stage, and pharyngoesophageal reconstruction in the second stage, is a useful therapeutic option in the treatment of TEF caused by balloon dilatation of corrosive esophageal stricture in children.

Abstract: Background Diagnosis of acute appendicitis (AA) and decisions about its treatment remain among the most common dilemmas of pediatric surgical teams. Monitoring of immune response may be of importance for this purpose. Our aim was to measure and analyze serum and peritoneal fluid cytokines, in children who had undergone surgery for suspected AA. Methods Prospective investigation of serum and peritoneal fluid cytokine values was performed in 127 consecutive patients. According to the pathohistological findings, patients were divided into three groups: normal/early, uncomplicated and complicated AA. Determination of cytokine concentrations for 20 different cytokines was done using a commercial flow cytometry kit: Human Inflammation 20 plex BMS 819. Results Statistically significant differences in serum cytokine values between pathohistological groups were found for IP-10, MIP-1α and IL-10. Preoperative cut-off values of IP-10, MIP-1α and IL-10 between groups were obtained using ROC curve analysis. Positive correlations between serum and peritoneal concentrations were recorded for most of the analyzed cytokines. Conclusion IP-10, MIP-1α and IL-10 showed potential in assessment of AA in children. Confirmatory studies with a larger number of patients are required to prove reliability of these biomarkers.

Abstract: Background: Neonates and infants requiring anaesthesia are at risk of physiological instability and complications, but triggers for peri-anaesthetic interventions and associations with subsequent outcome are unknown. Methods: This prospective, observational study recruited patients up to 60 weeks' postmenstrual age undergoing anaesthesia for surgical or diagnostic procedures from 165 centres in 31 European countries between March 2016 and January 2017. The primary aim was to identify thresholds of pre-determined physiological variables that triggered a medical intervention. The secondary aims were to evaluate morbidities, mortality at 30 and 90 days, or both, and associations with critical events. Results: Infants (n=5609) born at mean (standard deviation [SD]) 36.2 (4.4) weeks postmenstrual age (35.7% preterm) underwent 6542 procedures within 63 (48) days of birth. Abstract: AIM: This observational cohort study aimed to identify factors associated with pulseless electrical activity (PEA) and asystole in in-hospital cardiac arrest (IHCA) patients and to determine whether differences in outcome based on the initial rhythm were explained by patient- and cardiac arrest characteristics.METHODS: Adults with IHCA from 2017 to 2018 were included from the Danish IHCA Registry (DANARREST). Additional data came from population-based registries. Unadjusted (RRs) and adjusted risk ratios (aRRs) were estimated for predictors of initial rhythm, return of spontaneous circulation (ROSC), and survival.RESULTS: We included 1495 PEA and 1285 asystole patients. The patients did not differ substantially in patient characteristics. Female sex, age >90 years, pulmonary disease, and obesity were associated with initial asystole. Ischemic heart disease and witnessed and monitored cardiac arrest were associated with initial PEA. In unadjusted and adjusted analyses, PEA was associated with increased ROSC (aRR = 1.21, 95% confidence interval [CI] 1.10; 1.33). PEA was also associated with increased 30-day and 1-year survival in the unadjusted analysis, while there was no clear association between the initial rhythm and 30-day (aRR = 0.88, 95% CI 0.71; 1.11) and 1-year (aRR = 0.85, 95% CI 0.69; 1.04) survival when patient- and cardiac arrest characteristics were adjusted for.CONCLUSION: In patients with IHCA presenting with PEA or asystole, there were no major differences in patient demographics and comorbidities. The patients differed substantially in cardiac arrest characteristics. Initial PEA was associated with higher risk of ROSC, but there was no difference in 30-day and 1-year survival.

Abstract: The paper analyses the complex position of Palestine in the modern international community. Introductory remarks create the necessary framework within which the analysis is carried out, and they consists of both the complex historical context and references to theories ofrecognition in international public law. The central analysis rests on two facts. The first observes Palestine in the context of multilateral diplomacy, analysing its relations with the United Nations, the European Union, and UNESCO. The second focuses on complex judicial practice on the international level, primarily through the relationship with the International Criminal Court and the implementation of the Rome Statute, as well as with the International Court of Justice, exclusively through the upcoming procedure of issuing an advisory opinion requested by the General Assembly, which will inevitably have to pay attention to the question of the international legal status of Palestine

Abstract: The paper studies the manifestation of cyberspace in the context of the principle of sovereignty as a fundamental principle of public international law. It is a young phenomenon that is still difficult to define in a clear way in the international community. The author analyzes whether, to what extent, and which formal sources of public international law apply to cyberspace. The analysis focuses on three central sources of public international law – international treaties, international customs and general legal rules. Cyberspace is then analyzed from two angles. The first, external aspect of the analysis seeks an answer to the question of whether, first of all, states as central subjects of international public law, but also the international community and to what extent, have sovereignty over cyberspace. Second, internal segment of the analysis is aimed at determining whether cyberspace has its own sovereignty. The paper briefly reviews the perspectives of the leading world powers, above all the United States of America, the People’s Republic of China and the Russian Federation. The author concludes that it is necessary to continue working on the regulation of cyberspace, primarily through the institutions of the United Nations. Only regulation at this level can provide the necessary legal regulations that will adequately regulate a specific area such as cyberspace.

Abstract: Background/Aim. Recent studies indicate that adipokines have an important role in bone physiology and pathology. Recent data indicate that adipokine leptin functions as a regulator of bone growth at multiple levels, systemically and locally. So far, it has been shown that leptin influences bone volume and bone mineral density in a population with metabolic and/or hormonal abnormality. Data concerning leptin values in non-obese children with fractures are scarce. Methods. This study included 93 non-obese children with long bone fractures (LBF), 14 children with short bone fractures (SBF), and 19 healthy children. Leptin concentration was determined in two blood samples (day 0 and day 21) and analyzed according to gender, fracture type, anatomical localization of the fracture, fracture topography, callus formation, and the healing outcome. Results. Children with LBF demonstrated significantly increased leptin levels compared to the control group (both day 0/day 21). In the control group, girls had significantly more leptin than boys. Leptin value was significantly influenced by anatomical localization since boys and girls with humerus fracture and girls with femur fracture had the highest average leptin concentration in the initial sample. Boys with incomplete callus formation had the highest leptin concentration (both day 0/day 21), significantly elevated compared to boys' samples in the control group, boys' samples with an intermediary and well-formed callus, and also increased compared to the initial samples of girls with incomplete callus. Better callus formation in girls was associated with an increment of leptin concentrations in the second over the initial sample. Girls with partially and satisfactorily formed callus had significantly increased leptin concentration in the second sample (day 21) compared to the boys' group. Conclusion. Leptin concentration was significantly increased (both samples) in children with LBF compared to children with SBF and corresponding controls. Leptin concentration was highly influenced by gender. High blood leptin concentrations in boys or low leptin concentrations in girls immediately upon fracture could be used to identify groups of children with incomplete callus formation. Uvod/Cilj.Novijestudije pokazuju da adipokini imaju važnu ulogu u fiziologiji i patologiji kostiju. Takođe, najnoviji podaci pokazuju da adipokin leptin funkcioniše kao regulator rasta kostiju sistemski i lokalno. Pokazano je da leptin utiče na volumen kostiju i mineralnu gustinu kostiju u populaciji sa metaboličkom i/ili hormonskom abnormalnošću. Podaci o vrednostima leptina kod negojazne dece sa frakturama su oskudni. Metode. U ovu studijubil a su uključena93 negojazna deteta sa prelomima dugih kostiju (LBF), 14 dece sa prelomima malih kostiju (SBF) i 19 zdrave dece. Koncentracija leptina određena je u 2 uzorka krvi (0. danai 21 . dana) i analizirana prema polu, tipu frakture, lokalizaciji anatomske frakture, topografiji frakture, formiranju kalusa i ishodu zarastanja.Rezultati.Deca sa LBF imala su značajnopovećane nivoeleptina u poređenju sa kontrolnom grupom u oba uzorka krvi ( 0. dana/21 . dana). U kontrolnoj grupi devojčice su imale značajno više nivoe leptina od dečaka. Na vrednost leptina značajno je uticala anatomska lokalizacija, jer su dečaci i devojčice sa prelomom humerusa i devojčice sa prelomom femura imali najveću prosečnu koncentraciju leptina u početnom uzorku. Dečaci sa nepotpuno for miranim kalusom imali su najveću koncentraciju leptina (u oba uzorka, 0. dana/21. dana),značajno višuu odnosu na kontrolne uzorke dečaka, uzorke dečaka s intermedijarnim i dobro formiranim kalusom, a takođe višu u odnosu na koncentracije leptina u početnim uzorcima djevojčica s nepotpunim kalusom. Bolje formiranje kalusa kod devojčica je bilo povezano sa poveć anjem koncentracije leptina u drugom (21. dan) u odnosu na početni uzorak(0. dan). Devojčice sa delimično i zadovoljavajuće formiranim kalusom imale su značajno višu koncentraciju leptina u drugom uzorku (21. dan ) u odnosu na grupu dečaka. Zaključak. Koncentracija leptina je značajno povećana (u oba uzorkakrvi ) kod dece sa LBF u poređenju sa decom sa SBF i odgovarajućim kontrolama. Koncentracija leptina je zavisna od pola. Visok nivo leptina u krvi kod dečaka ili niska koncentracija leptina kod devojčica odmah nakon preloma može se koristiti za identifikaciju grupa dece sa nepotpunim formiranjem kalusa.

Abstract: In this case study, the authors elaborate on the narrative structure oftransnational popular media events. Drawing from Dayan and Katz’s concept ofmedia eventsand Julia Sonnevend’s exceptional work oniconic global media events, they argue that fundamental changes in the way occurrences are being reported on and news is structured must be considered. Allowing for recent technological advancements, the role of the consumer and the compression of time in media use, the authors develop a methodological and theoretical framework fitting a more mundane and everyday life–based approach. They derive their results from the analysis of the “Podgorica Media Event,” a news cycle emerging from a racist incident during an international soccer game between England and Montenegro. Based on the body of 250 international news pieces, they identify aprimary mother narrationand adistinctive narrationas the typical ways of storytelling on a transnational level. While differing greatly in content, aspects of transnational popular media events serve to protect and reify the cultural background they are grounded in on a national level. Thus, we assume that sport, or, more specifically, soccer, may become political in media communication not by the impact of state government but by the consumers themselves choosing and developing a popular media event in the first place.

Abstract: BACKGROUND: Cardiac arrests are often categorized into two separate groups depending on the location of the arrest: in-hospital cardiac arrest (IHCA) and out-of-hospital cardiac arrest (OHCA). Despite this distinction, few studies have compared the two groups directly. The aim of this study was to compare patient characteristics, cardiac arrest characteristics, and outcomes for IHCA and OHCA patients.METHODS: Data on IHCA and OHCA in Denmark were obtained from two nationwide, prospective registries. All adult (≥ 18 years old) patients with index IHCA or OHCA from January 1, 2017 to December 31, 2018 were included. Supplementary information on outcomes, hospitalizations, and chronic diseases came from additional national registries. The primary outcome was 30-day survival and secondary outcomes were return of spontaneous circulation (ROSC) and 1-year survival.RESULTS: The study included 3,501 patients with IHCA and 8,846 patients with OHCA. The two groups were similar in demographics, most comorbidities, and initial cardiac arrest rhythm. In the unadjusted analysis, IHCA was associated with increased survival to 30 days compared to OHCA (risk ratio [RR] = 1.41; 95% CI, 1.30; 1.54) and 1 year (RR = 1.46; 95% CI, 1.33; 1.61). Adjusting for age, sex, and comorbidities did not change the RR substantially. When cardiac arrest characteristics were added to the model, the RR decreased from 1.51 (95% CI, 1.39; 1.65) to 1.06 (95% CI, 0.93; 1.20) for survival to 30 days and from 1.66 (95% CI, 1.50; 1.84) to 1.09 (95% CI, 0.94; 1.26) for survival to 1 year. In all subgroup analyses based on cardiac arrest characteristics (e.g. witnessed status), the association between location and outcome was substantially mitigated compared to the primary analyses.CONCLUSIONS: In this large, national study, we found that IHCA and OHCA patients were remarkably similar in demographics and most comorbidities. IHCA patients had better outcomes compared to OHCA patients, although these differences disappeared when comparing patients with similar cardiac arrest characteristics.

Abstract: Aim To perform a review of the literature on the association between socioeconomic status and risk of and outcomes after in-hospital cardiac arrest. Data sources PubMed and Embase were searched on January 24, 2020 for studies evaluating the association between socioeconomic status and risk of and/or outcomes after in-hospital cardiac arrest. Two reviewers independently screened the titles/abstracts and selected full texts for relevance. Data were extracted from included studies. Risk of bias was assessed using the Quality In Prognosis Studies (QUIPS) tool. Results The literature search yielded 4960 unique records. We included nine studies evaluating the association between socioeconomic status and risk of and/or outcomes after in-hospital cardiac arrest. All studies were observational cohort studies, of which seven were from the USA. Seven studies were in an adult population, while two studies were in a pediatric population. Results were overall inconsistent although some studies found a higher in-hospital cardiac arrest incidence in patients from low-income communities. There was no clear association between other socioeconomic factors (i.e. education, occupation, marital status, and insurance) and risk of or outcomes after in-hospital cardiac arrest. Due to the scarcity and heterogeneity of available studies, meta-analyses were not performed. Conclusion There are limited data regarding the association between socioeconomic status and risk of and outcomes after in-hospital cardiac arrest and further research is warranted. Understanding the association between socioeconomic status and in-hospital cardiac arrest may reveal strategies to mitigate potential inequalities.

Abstract: Ubiquinol is a fundamental component of cellular metabolism. Low ubiquinol levels have been associated with mortality. This was a substudy of a randomized trial in patients undergoing coronary artery bypass grafting. We drew blood before and after surgery. Ubiquinol or placebo was added to peripheral blood mononuclear cells for oxygen consumption (OCR) measurements. In vivo ubiquinol levels were lower postsurgery compared to presurgery (0.16 μmol/L [quartiles: 0.02-0.39], P = .01), although the difference disappeared when adjusting for hemoglobin levels ( P = .30). There was no difference in presurgical basal (1.0 mL/min/mg [95% confidence interval [CI]: −0.9 to 2.2], P = .08) and maximal (0.5 mL/min/mg [95% CI: −4.3 to 7.3], P = .56) OCR in cells receiving ubiquinol or placebo. There was a difference in postsurgical basal (1.1 mL/min/mg [95% CI: 0.9-1.6], P < .001) and maximal (4.2 mL/min/mg [95% CI: 0.3-7.0], P = .01) OCR between the groups. We found no association between ubiquinol and OCR levels (all P > .05).

Abstract: Background: Intraoperative application of positive end-end-expiratory pressure (PEEP) is recommended in mechanically ventilated children since it improves lung mechanics. However, inadequate level of applied pressure can increase risk of barotrauma. Objectives: To assess the impact of titrated PEEP on lung mechanics in mechanically ventilated children under general anaesthesia. Methods: This was an open label, single Tertiary Centre randomized controlled clinical trial. The study population included 70 preschool children, ASA I and II, scheduled for orthopaedic, reconstructive, abdominal or urological surgery. Children with upper respiratory tract infection, allergic to chosen anaesthetics, cardiorespiratory comorbidities were excluded. Patients were randomly assigned either to receive intraoperative PEEP titration form 5 - 11 cmH2O 20 minutes before the end of anaesthesia (intervention group) or to be ventilated until the end of anaesthesia conventionally with a positive end-expiratory pressure of 3 cmH2O (control group). Main outcomes: changes in dynamic lung compliance (Cdyn), peak airway pressure (PIP), mean airway pressure (Paw) between groups at the end of surgery; changes in PIP and Paw during PEEP titration and desaturation in interventional group during PEEP titration. Results: Seventy preschool children were recruited and analysed. Intraoperative positive end-expiratory pressure titration improved Cdyn in the intervention group comparing to control (ΔCdy -3.2 vs. 0.63; P < 0.001). PIP and Paw were higher in interventional group (ΔPIP -0.57vs. 0.11; P < 0.001, and ΔPaw -0.63 vs. 0.0; P < 0.001) and desaturation was not observed in interventional group. Conclusions: Stepwise titration of PEEP up to 11 cmH2O improves subsequent lung mechanics without causing of barotrauma.

Abstract: Introduction. Congenital diaphragmatic hernia (CDH) with concomitant esophageal atresia (EA) and tracheo-esophageal fistula (TEF) is a very rare condition, with a high mortality rate. Prematurity and congenital heart anomalies additionally increase the mortality rate. This situation is a great challenge for clinicians, requiring multidisciplinary work and adequate treatment strategy. Case report. We presented a premature twin newborn at the gestational age of 33/34 weeks with body mass of 1690 g. The existence of the left CDH was established on prenatal ultrasound exam in the 24th gestational week, and the diagnosis of EA with TEF was made on admittance to our hospital. The cardiac ultrasound exam revealed the total anomalous pulmonary venous connection (TAPVC). The first operation was performed on the day of admittance and consisted of left subcostal laparotomy, diaphragmatic repair, elastic occlusion of the gastroesophageal junction and gastrostomy. The ligation of TEF and esophagoplasty were done 13 days later in the second operation. The lethal outcome during the esophagoplasty was caused by the crisis of pulmonary hypertension and associated congenital heart anomaly (TAPVC). The presence of CDH and EA/TEF in association with TAPVC in a twin newborn has not been reported before in the literature. Conclusion. The treatment of newborns with CDH and EA/TEF requires multidisciplinary well-coordinated team work of pediatric surgeons, anaesthesiologists, neonatologists and pulmologists. The standard protocol for the management does not exist, but well-planned staged operations could enable greater survival rate.

Abstract: Background/Aim. Diagnosis of acute appendicitis (AA) remains the most common dilemma of pediatric surgical team. Our aim was to determine whether the neutrophil myeloperoxidase index (MPXI), in combination with other laboratory and clinical parameters, can be useful in diagnosis and follow-up of AA in children. Methods. A prospective investigation of MPXI values in 117 consecutive patients, planned for the surgical intervention due to AA, was performed. The patients were stratified into three groups according to the intraoperative finding: the normal/early, uncomplicated and complicated AA. Laboratory analyses were done preoperativly, on the 1st and on the 3rd postoperative days. Results. The statistically significant difference of MPXI values between the uncomplicated and complicated appendicitis before surgery and the positive correlations between the MPXI and C-reactive protein, as well as interleukin- 6, before surgery were found. Postoperatively, in the group of uncomplicated, as well as complicated AA, a significant decrease of MPXI was recorded. Conclusion. The MPXI may be used as an informative biomarker in the follow- up of AA in children. A wide reference range for the MPXI and individual differences in the values of MPXI in the healthy children, generate difficulties for its use for the initial diagnosis of acute appendicitis. Usefulness of MPXI determination decreases with a delayed diagnosis.

Abstract: Background. Neutrophil-to-lymphocyte ratio (NLR), monocyte-to-lymphocyte ratio (MLR), platelet-to-lymphocyte ratio (PLR), and mean platelet volume-to-platelet count (MPV/PC) ratio are readily available parameters that might have discriminative power regarding outcome. The aim of our study was to assess prognostic value of these biomarkers regarding outcome in critically ill patients with secondary sepsis and/or trauma. Methods. A total of 392 critically ill and injured patients, admitted to surgical ICU, were enrolled in a prospective observational study. Leukocyte and platelet counts were recorded upon fulfilling Sepsis-3 criteria and for traumatized Injury Severity Score > 25 points. Patients were divided into four subgroups: peritonitis, pancreatitis, trauma with sepsis, and trauma without sepsis. Results. NLR and MPV/PC levels were significantly higher in nonsurvivors (AUC/ROC of 0.681 and 0.592, resp., in the peritonitis subgroup; 0.717 and 0.753, resp., in the pancreatitis subgroup); MLR and PLR did not differ significantly. There was no significant difference of investigated biomarkers between survivors and nonsurvivors in trauma patients with and without sepsis except for PLR in the trauma without sepsis subgroup (significantly higher in nonsurvivors, AUC/ROC of 0.719). Independent predictor of lethal outcome was NLR in the whole cohort and in the peritonitis subgroup as well as MPV in the pancreatitis subgroup. Also, there were statistically significant differences in MPV/PC, MLR, and PLR values regarding nature of bacteremia. In general, the lowest levels had been found in patients with Gram-positive blood cultures. Conclusions. NLR and MPV were very good independent predictors of lethal outcome. For the first time, we demonstrate that nature of bacteremia influences MPV/PC, MLR, and PLR. In heterogeneous cohort subgroup, analysis is essential.

Abstract: PURPOSE: Alcohol-use disorders (AUDs) have been associated with increased sepsis-related mortality. As patients with AUDs are often thiamine deficient, we investigated practice patterns relating to thiamine administration in patients with AUDs presenting with septic shock and explored the association between receipt of thiamine and mortality.MATERIALS: We performed a retrospective cohort study of patients presenting with septic shock between 2008 and 2014 at a single tertiary care center. We identified patients with an AUD diagnosis, orders for microbial cultures and use of antibiotics, vasopressor dependency, and lactate levels≥4mmol/L. We excluded those who received thiamine later than 48h of sepsis onset.RESULTS: We included 53 patients. Thirty-four (64%) patients received thiamine. Five patients (15%) received their first thiamine dose in the emergency department. The median time to thiamine administration was 9 (quartiles: 4, 18) hours. The first thiamine dose was most often given parenterally (68%) and for 100mg (88%). In those receiving thiamine, 15/34 (44%) died, compared to 15/19 (79%) of those not receiving thiamine, p=0.02.CONCLUSIONS: A considerable proportion of patients with AUDs admitted for septic shock do not receive thiamine. Thiamine administration in this patient population was associated with decreased mortality.

Abstract: Mokra polja su inženjerski sistemi koji si koriste u procesu prečišćavanja otpadnih voda i unapređenju kvaliteta vode. Mokra polja su projektovani sistemi koji su izgrađeni da iskoriste prednosti procesa koji se dešavaju u prirodnim močvarama, uključujući vegetaciju, zemljište i prateću mikrobnu aktivnost za prečišćavanje otpadnih voda. Ovi sistemi se mogu koristi kao deo decentralizovanih sistema za tretman otpadnih voda i mogu se koristiti za prečišćavanje različitih vrsta otpadnih voda. U radu su prikazani različiti tipovi mokrih polja, njihova primena, kriterijumu za dizajn i načini prečišćavanja otpadne vode. Ovi sistemi se koriste kao sekundarni proces tretmana što znači da se otpadna voda prečišćava u primarnom koraku prečišćavanja pre ulaska u filter mokrih polja. Postoje različite vrste mokrih polja, kao što su mokra polja sa površinskim kretanjem otpadne vode, mokra polja sa podzemnim kretanjem otpadne vode, gde voda može horizontalno i vertikalno da se kreće, kao i hibridni sistem koji je kombinacija prethodno dva navedena sistema. Korišćenje mokrih polja za prečišćavanje otpadnih voda postaje sve popularniji sistem u različitim delovima sveta, kao u razvijenim zemljama tako i u zemljama u razvoju.
