Background: 2-year follow-up data from our randomised controlled trial showed that peroral endoscopic myotomy is associated with a significantly higher efficacy than pneumatic dilation as initial treatment of therapy-naive patients with achalasia. Here we report therapeutic success rates in patients treated with peroral endoscopic myotomy compared with pneumatic dilation at the 5-year follow-up. Methods: We did a multicentre, randomised controlled trial in six hospitals in the Netherlands, Germany, Italy, Hong Kong, and the USA. Adults aged 18–80 years with newly diagnosed symptomatic achalasia (based on an Eckardt score >3) were eligible for inclusion. Patients were randomly assigned (1:1) to peroral endoscopic myotomy or pneumatic dilation using web-based randomisation with a random block size of 8 and stratification according to site. Randomisation concealment for treatment type was double blind until official study enrolment. Treatment was unmasked because of the different technical approach of each procedure. Patients in the pneumatic dilation group were dilated with a single series of 30–35 mm balloons. The need for subsequent dilations in the pneumatic dilation group, and the need for dilation after initial treatment in the peroral endoscopic myotomy group, was considered treatment failure. The primary outcome was therapeutic success (Eckardt score ≤3 in the absence of severe treatment-related complications and no need for retreatment). Analysis of the primary outcome was by modified intention to treat, including all patients randomly assigned to a group, excluding those patients who did not receive treatment or were lost to follow-up. This paper reports about the feasibility study carried out by the Dutch Kadaster to automatically generalise the largest scale topographical data set maintained by the Kadaster (i.e. TOP10NL) from the 1:1k topographical object oriented data set, which is currently being collected and structured by organisations that need to maintain public space such as municipalities, the railway company and provinces. The two data sets do not only differ in scale but also with respect to objectives, source data, application domain, providers, acquisition method and rules, and definition of topology. This paper reports about on going developments in the map production process of the Dutch Kadaster (who also hold the mapping agency). In March 2013, the Dutch Kadaster will produce the 1:50k map series fully automatically from 1:10k data for the first time. From that moment on, this new product will replace the existing 1:50k map. It is the first time that a complete topographic map is generalised with no human interaction and that the resulting map is good enough to replace the existing map. The implementation is a 100% automated generalisation workflow that produces a countrywide map series at scale 1:50k from 1:10k data within 50 hours. Therefore a 1:50k map delivery is foreseen with every 1:10k data update, i.e. five times a year. The research that achieved the fully automated generalisation workflow was explained in several publications (Stoter et al, 2009; 2011; 2012). The automated workflow for the generalisation of 1:50k maps from TOP10NL data is currently being extended to produce the 1:100k map series. This workshop paper focuses on the generalisation problem for which one third of the sub models were developed: generalisation of the road network. The Dutch data models at 1:10k and 1:50k have two properties that characterise this process. At first 1:10k data contains road polygons for every road wider than 2 meters that represent the real widths of the roads, while 1:50k maps contain road centerlines that cover complete roads and that have an attribute indicating the width of the road. The second property that characterises the generalisation of roads is that data at all scale form a planar partition, i.e. no gaps or overlap are allowed. Op dit moment worden de topografische producten van het Kadaster op schaal 1:10.000, 1:50.000, 1:100.000 en 1:250.000 - die te samen de Basisregistratie Topografie (BRT) vormen - apart van elkaar geproduceerd en beheerd. Kartografen werken de producten op schaal 1:50.000 en kleiner bij op basis van het 1:10.000 product (TOP10NL) volgens generalisatierichtlijnen. Dit vergt relatief veel tijd, te meer omdat de kleinere schalen stapsgewijs van elkaar worden afgeleid (iedere schaal van de schaal eronder). Om de productielijn efficiënter maar ook consistenter te maken, is een onderzoeksteam van het Kadaster sinds 2011 bezig met een studie naar de implementatie van automatische generalisatie. In Geo-Info 2012-2 (blz. 6-10) beschreven wij hoe het Kadaster onderzoek doet naar de toepassing van automatische generalisatie voor de nieuwe productielijn van topografische kaarten (in eerste instantie op schaal 1:50.000 en 1:100.000). De resultaten zijn veelbelovend, zoals te zien is op deze pagina’s. Hiernaast staat een 1:50.000 kaart afgebeeld (op schaal) die is gemaakt door middel van volledige automatische generalisatie van TOP10NL data.
