Abstract

Electromagnetic (EM) multipath interference is difficult to address with passive approaches due to two physical restrictions - the shared frequency of the initial and interfering signals and their variable incident angles. Thus, to address multipath interference, the spatial impedance must be adjusted in response to the incident angles of multiple signals with the same frequency, which is impossible with classic linear time-invariant (LTI) systems. We present a design concept for metasurface-based spatial filters to overcome LTI behavior and suppress multipath interference signals using a time-varying interlocking mechanism without any active biasing systems. The proposed devices are coupled to the first incoming wave to adjust the spatial impedance and suppress delayed waves in the time domain, which is validated numerically and experimentally. This study opens a new avenue for passive yet time-varying selective EM metasystems, enabling the adjustment of spatially complicated EM waves and fields even at the same frequency., Comment: 11 pages, 14 figures

8. Three-dimensional magnetic resonance imaging-based statistical shape analysis and machine learning-based prediction of patellofemoral instability

Abstract

Abstract This study performed three-dimensional (3D) magnetic resonance imaging (MRI)-based statistical shape analysis (SSA) by comparing patellofemoral instability (PFI) and normal femur models, and developed a machine learning (ML)-based prediction model. Twenty (19 patients) and 31 MRI scans (30 patients) of femurs with PFI and normal femurs, respectively, were used. Bone and cartilage segmentation of the distal femurs was performed and subsequently converted into 3D reconstructed models. The pointwise distance map showed anterior elevation of the trochlea, particularly at the central floor of the proximal trochlea, in the PFI models compared with the normal models. Principal component analysis examined shape variations in the PFI group, and several principal components exhibited shape variations in the trochlear floor and intercondylar width. Multivariate analysis showed that these shape components were significantly correlated with the PFI/non-PFI distinction after adjusting for age and sex. Our ML-based prediction model for PFI achieved a strong predictive performance with an accuracy of 0.909 ± 0.015, and an area under the curve of 0.939 ± 0.009 when using a support vector machine with a linear kernel. This study demonstrated that 3D MRI-based SSA can realistically visualize statistical results on surface models and may facilitate the understanding of complex shape features.

Abstract

Abstract: Objectives: This study aimed to analyze Japanese youth athletes’ mental health and resilience during the COVID-19 pandemic, as well as the potential correlations between their demographic factors and mental health outcomes. Methods: Participants were recruited from a single high school with a student population encompassing national-level competitors (NCs), local-level competitors (LCs), and general students (i.e., others; OTs), aiming to reduce the impact of differences in environmental conditions affecting pandemic-related stress. In March 2021, the students completed an anonymous online survey comprising questions on demographics and COVID-19, as well as the following scales: Kessler 6 (K6), Patient Health Questionnaire (PHQ-9), Generalized Anxiety Disorder Scale (GAD-7), and the Resilience Scale. Differences and correlations among these groups’ scores were analyzed while controlling for covariates. Results: Of 1,022 students (51.3% girls), 22.3% were NCs, 38.6% were LCs, and 39.0% were OTs. NCs reported significantly lower scores on the K6, PHQ-9, and GAD-7, as well as significantly higher resilience scores, compared with LCs and OTs. Resilience scores had a significantly negative correlation with the K6, PHQ-9, and GAD-7 scores. Female gender, participation in cultural clubs, and indoor sports were factors associated with scores above the cut-off on each scale. COVID-19 testing, history of illness, and family history were not associated with any of the psychological evaluation measures. Conclusion: Based on responses to the self-reported questionnaires, NCs appear to have better mental health than their peers, while resilience has a potential association with improved mental health during the COVID-19 pandemic.

Abstract

In this study, we numerically demonstrate how the response of recently reported circuit-based metasurfaces is characterized by their circuit parameters. These metasurfaces, which include a set of four diodes as a full wave rectifier, are capable of sensing different waves even at the same frequency in response to the incident waveform, or more specifically the pulse width. This study reveals the relationship between the electromagnetic response of such waveform-selective metasurfaces and the SPICE parameters of the diodes used. First, we show that reducing a parasitic capacitive component of the diodes is important for realization of waveform-selective metasurfaces in a higher frequency regime. Second, we report that the operating power level is closely related to the saturation current and the breakdown voltage of the diodes. Moreover, the operating power range is found to be broadened by introducing an additional resistor into the inside of the diode bridge. Our study is expected to provide design guidelines for circuit-based waveform-selective metasurfaces to select/fabricate optimal diodes and enhance the waveform-selective performance at the target frequency and power level., Comment: 9 pages, 9 figures

Abstract

Cardiovascular disease is one of the most important complications in girls and women with Turner syndrome (TS). Although the latest international guideline provides useful suggestions for the management of cardiovascular diseases in TS, some unknown cardiac conditions warrant physicians’ attention and awareness. Here, we have reported two adult cases wherein significant cardiovascular diseases were detected during the transition period. The first case patient was diagnosed with aortic crank deformity and left subclavian artery aneurysm at 14 years based on the report of cardiac catheterization, computed tomography angiography, and cardiac magnetic resonance imaging, which had remained undetected by annual evaluations using transthoracic echocardiography (TTE). This case emphasizes the importance of cardiac reevaluation during the transition period. The second case patient was diagnosed with moderate mitral valve regurgitation (MR) due to mitral valve prolapse at 18 years through TTE, although the first evaluation at 7 years by TTE detected slight MR without any clinical concerns. The condition however progressed to severe MR at 28 years, requiring mitral valvuloplasty. MR is the most common valve disease worldwide, which makes it challenging to comprehend whether the condition is a complication. However, the condition requiring surgery at this age is extremely rare, which implies the possibility of early progression. Because almost all literature on cardiovascular complications in TS is cross-sectional, further information about longitudinal cardiovascular conditions is vital for optimal care for girls and women with TS. The two cases reported in this article provide significant information for improving lifelong cardiovascular health issues in TS.

Abstract

Background: Individuals with food addiction (FA) compose a distinct subset of people with obesity who are less responsive to weight loss interventions. An emerging field of study explores the role of telomere length in disease processes. Some evidence suggests that obesity is associated with telomere shortening; however, we are not aware of studies examining telomere length in obesity subtypes. Therefore, we explored whether FA and levels of obesity were associated with telomere shortening. Methods: We enrolled 120 adults (aged 19–70) with overweight/obesity (BMI ≥ 25); half were positive for severe food addiction (FA+), and half were negative for food addiction (FA−) (Yale Food Addiction Scale 2.0). Participants completed a demographic form and provided a saliva sample (Oragene saliva DNA collection kit). Telomere length was analyzed using the monoplex quantitative polymerase chain reaction (qPCR). Data were analyzed using descriptive statistics, t-tests, and ANOVAs (α = 0.05). Results: Participants with overweight (mean = 1.40 t/s, SD = 0.40) had longer telomeres (p = 0.013) than those with morbid obesity (mean = 1.15 t/s, SD = 1.29). Telomere length did not differ (p = 0.306) between persons who were FA− (mean = 1.26 t/s, SD = 0.26) and those who were FA+ (mean = 1.32 t/s, SD = 0.34). The youngest participants (mean = 1.39 t/s, SD = 0.33) had longer telomeres (p = 0.006) than the oldest participants (mean = 1.18 t/s, SD = 0.19). Conclusion: Those who were morbidly obese had the shortest telomere lengths. Interestingly, however, there were not significant telomere length differences in the food addicted vs. nonfood-addicted subtypes.

Abstract Heart disease-related deaths have increased in recent decades, with most patients dying of sudden cardiac arrest. In such instances, the effect of regular electrocardiogram (ECG) measurements is minimal. Therefore, long-term ECG monitoring has become increasingly important. In this paper, we report a non-adhesive high accuracy ECG monitoring system that can be used in various scenarios without interfering with daily activities. The ECG ultra-thin film electrode is made by water-resistant material based on poly(3,4-ethylenedioxythiophene) poly(4-styrenesulfonate) (PEDOT: PSS) electrode doped with ethylene glycol (EG) and xylitol, to improve the noise signal caused by sweat. The optimal ratio of the three ingredients of PEDOT: PSS/xylitol/EG was determined experimentally to accommodate the ECG monitoring. By using the proposed selectively closed multi-channel single-lead logic circuit, the noise of ECG signal received from the proposed film electrode can be successfully reduced during broad-area electrode measurements, thus to improve ECG measurement accuracy.

17. Three-dimensional sectional measurement approach for serial volume changes in shoulder muscles after arthroscopic rotator cuff repair

Abstract

Purpose: This study assessed the serial volume changes in multiple shoulder muscles simultaneously following arthroscopic rotator cuff repair (ARCR) by a three-dimensional (3D) modeling-based sectional measurement. These volume changes were correlated with background preoperative factors. Methods: Four consecutive magnetic resonance imaging scans (preoperatively and postoperatively at 3, 6, and 12 months) of 33 shoulders from 31 patients who underwent arthroscopic rotator cuff repair were examined. We focused on the sectional volume differences of the supraspinatus, infraspinatus, teres minor, and subscapularis between preoperatively and 3 months postoperatively (Dif.pre.3mo) and between 3 and 12 months postoperatively (Dif.3.12mo). The correlation between volume differences and clinical/demographic parameters was determined by a multivariate analysis. Results: No statistically significant differences were observed for most serial changes in the shoulder muscle volumes. The tear-site muscles (supraspinatus and infraspinatus) showed similar tendencies for volume changes, whereas the non-tear-site muscles (teres minor and subscapularis) differed. A negative correlation was observed between Dif.pre.3mo and Dif.3.12mo for the supraspinatus, infraspinatus, and teres minor. These perioperative volume differences might correlate with tear size and symptom duration in the supraspinatus, as well as with a history of steroid injections and work and sports activity levels in the infraspinatus and teres minor. Conclusion: The serial volume changes in multiple shoulder muscles after ARCR measured using our 3D sectional approach exhibited different tendencies and clinical implications depending on the primary and non-primary site of tears. Our method may serve as a potential indicator to facilitate muscle recovery and prevent the progression of postoperative muscle atrophy.

18. Association between mental health social worker staffing in psychiatric emergency wards and readmission outcomes: A nationwide survey in Japan

Abstract

Abstract Aim The mental healthcare system in Japan is transitioning from institution‐based to community‐based treatment. To prevent prolonged hospitalization and community integration of psychiatric patients, mental health social workers (MHSWs) are pivotal in coordinating post‐discharge arrangements for psychiatric inpatients. This study aimed to propose a care model to improve clinical outcomes in psychiatric emergency wards in Japan. Methods We conducted a mail‐in questionnaire survey targeting medical facilities with psychiatric emergency wards. We collected data of the psychiatric care system, including facility profiles, staffing conditions and caseloads, and the provided psychiatric services and treatment options. Using multiple regression analyses, we explored associations between these data and clinical outcomes, focusing on the average number of days for hospitalization and the integration of patients into a community. Results Data were collected from 82 facilities (response rate, 45.8%). The average number of days for hospitalization and community integration were 64.7 and 327.9 days, respectively. The caseloads for MHSWs were significantly associated with longer hospitalization (β = 0.31, p = 0.009) and shorter duration of community living (β = −0.28, p = 0.027). Conclusion The clinical performance in psychiatric emergency wards surpassed the Japanese government's targets regarding these outcomes. We found that heavy caseloads on MHSWs were associated with worse clinical outcomes for patients in psychiatric emergency wards. These findings suggest that reducing MHSW caseloads (≤20 cases) may be a potential interventional strategy to prevent prolonged hospitalization and promote successful community integration of patients.

Abstract

BackgroundMost genetic analyses that have attempted to identify a locus or loci that can distinguish patients with treatment-resistant schizophrenia (TRS) from those who respond to treatment (non-TRS) have failed. However, evidence from multiple studies suggests that patients with schizophrenia who respond well to antipsychotic medication have a higher dopamine (DA) state in brain synaptic clefts whereas patients with TRS do not show enhanced DA synthesis/release pathways.Patients and methodsTo examine the contribution (if any) of genetics to TRS, we conducted a genetic association analysis of DA-related genes in schizophrenia patients (TRS, n = 435; non-TRS, n = 539) and healthy controls (HC: n = 489).ResultsThe distributions of the genotypes of rs3756450 and the 40-bp variable number tandem repeat on SLC6A3 differed between the TRS and non-TRS groups. Regarding rs3756450, the TRS group showed a significantly higher ratio of the A allele, whereas the non-TRS group predominantly had the G allele. The analysis of the combination of COMT and SLC6A3 yielded a significantly higher ratio of the putative low-DA type (i.e., high COMT activity + high SLC6A3 activity) in the TRS group compared to the two other groups. Patients with the low-DA type accounted for the minority of the non-TRS group and exhibited milder psychopathology.ConclusionThe overall results suggest that (i) SLC6A3 could be involved in responsiveness to antipsychotic medication and (ii) genetic variants modulating brain DA levels may be related to the classification of TRS and non-TRS.

Abstract

Abstract Aim Hepatocellular carcinoma (HCC) with bile duct invasion (BDI) (BDIHCC) has a poor prognosis. Moreover, due to the paucity of reports, there is no consensus regarding optimal management of this clinical condition yet. The aim of this study was to clarify the efficacy and safety of proton beam therapy (PBT) for BDIHCC. Methods Between 2009 and 2018, 15 patients with BDIHCC underwent PBT at our institution. The overall survival (OS), local control (LC), and progression-free survival (PFS) curves were constructed using the Kaplan-Meier method. Toxicities were assessed using the Common Terminology Criteria of Adverse Events version 4.0. Results The median follow-up time was 23.4 months (range, 7.9–54.3). The median age was 71 years (range, 58–90 years). Many patients were Child A (n = 8, 53.3%) and most had solitary tumors (n = 11, 73.3%). Additionally, most patients had central type BDI (n = 11, 73%). The median tumor size was 4.0 cm (range, 1.5–8.0 cm). The 1-, 2-, and 3-year OS rates were 80.0%, 58.7% and 40.2%, respectively, and the corresponding LC and PFS rates were 93.3%, 93.3%, and 74.7% and 72.7%, 9.7%, and 0.0%, respectively. Acute grade 1/2 dermatitis (n = 7, 46.7%), and grades 2 (n = 1, 6.7%) and 3 (n = 1, 6.7%) cholangitis were observed. Late toxicities such as grade 3 gastric hemorrhage and pleural effusion were observed. No toxicities of grade 4 or higher were observed. Conclusions PBT was feasible with tolerable toxicities for the treatment of BDIHCC.

Abstract

Extracellular vesicles and exomere nanoparticles are under intense investigation as sources of clinically relevant cargo. Here we report the discovery of a distinct extracellular nanoparticle, termed supermere. Supermeres are morphologically distinct from exomeres and display a markedly greater uptake in vivo compared with small extracellular vesicles and exomeres. The protein and RNA composition of supermeres differs from small extracellular vesicles and exomeres. Supermeres are highly enriched with cargo involved in multiple cancers (glycolytic enzymes, TGFBI, miR-1246, MET, GPC1 and AGO2), Alzheimer's disease (APP) and cardiovascular disease (ACE2, ACE and PCSK9). The majority of extracellular RNA is associated with supermeres rather than small extracellular vesicles and exomeres. Cancer-derived supermeres increase lactate secretion, transfer cetuximab resistance and decrease hepatic lipids and glycogen in vivo. This study identifies a distinct functional nanoparticle replete with potential circulating biomarkers and therapeutic targets for a host of human diseases.

Abstract

Abstract In this study, we numerically demonstrate how the response of recently reported circuit-based metasurfaces is characterized by their circuit parameters. These metasurfaces, which include a set of four diodes as a full wave rectifier, are capable of sensing different waves even at the same frequency in response to the incident waveform, or more specifically the pulse width. This study reveals the relationship between the electromagnetic response of such waveform-selective metasurfaces and the SPICE parameters of the diodes used. In particular, we draw conclusions about how the SPICE parameters are related to (1) the high-frequency operation, (2) input power requirement and (3) dynamic range of waveform-selective metasurfaces with supporting simulation results. First, we show that reducing a parasitic capacitive component of the diodes is important for realization of the waveform-selective metasurfaces in a higher frequency regime. Second, we report that the operating power level is closely related to the saturation current and the breakdown voltage of the diodes. Moreover, the operating power range is found to be broadened by introducing an additional resistor into the inside of the diode bridge. Our study is expected to provide design guidelines for circuit-based waveform-selective metasurfaces to select/fabricate optimal diodes and enhance the waveform-selective performance at the target frequency and power level. Our results are usefully exploited to ensure the selectivity based on the pulse duration of the incident wave in a range of potential applications including electromagnetic interference, wireless power transfer, antenna design, wireless communications, and sensing.

Abstract

We present the case of a patient with spontaneous isolated common femoral artery (CFA) dissection (SICFAD). A 62-year-old woman was referred to our department with intermittent claudication a few days after experiencing tightness and weakness in the right leg while exercising at the gym. Ankle-brachial index was 0.68 on the right side. Ultrasonography and computed tomography angiography showed localized dissection in the right CFA. Endarterectomy with patch angioplasty was performed. The postoperative course was uneventful and the patient was discharged on postoperative day 5. We speculate that chronic repetition of both mechanical stretching of the intima in the posterior wall of the CFA in hip flexion during exercise and high wall shear stress at the same site may have represented major contributors to SICFAD in this case. Spontaneous dissection limited to the CFA has not been reported previously.

Abstract

Abstract Aim The spread of the novel coronavirus infection (coronavirus disease 2019 [COVID‐19]) has caused behavioral changes and mental illness in patients and their attendants during its early phase. The present study aimed to examine the association between precautionary behaviors against COVID‐19 and psychosocial factors in outpatients with pre‐existing disease and their attendants. Methods We conducted a cross‐sectional paper‐based questionnaire survey in Chiba University Hospital on 1019 patients and 513 attendants, and a web‐based questionnaire survey in Japan on 3981 individuals from the general population. We evaluated the participants' anxiety about COVID‐19, depression, health anxiety, and precautionary behaviors. Results Regarding knowledge and anxiety about COVID‐19, the protective factors for the high precautionary behaviors group were knowledge of COVID‐19 (odds ratio [OR] = 1.178, 95% confidence interval [CI]: 1.099–1.263), anxiety about the spread of COVID‐19 (OR = 1.348, 95% CI: 1.243–1.461), and anxiety about infecting someone with COVID‐19 (OR = 1.135, 95% CI: 1.039–0.239). Regarding psychosocial factors, the protective factors for the high precautionary behaviors group were patients (OR = 1.759, 95% CI: 1.056–2.929), their attendants (OR = 3.892, 95% CI: 1.416–10.700), health anxiety (OR = 2.005, 95% CI: 1.451–2.772), and nondepression states (OR = 1.368, 95% CI: 1.004–1.864). Conclusion Our findings suggest that patients and their attendants may perform high precautionary behaviors. Health anxiety and nondepression states may be associated with high precautionary behaviors.

Abstract

Abstract Background The coronavirus disease 2019 (COVID-19) pandemic is widespread; however, accurate predictors of refractory cases have not yet been established. Circulating extracellular vesicles, involved in many pathological processes, are ideal resources for biomarker exploration. Methods To identify potential serum biomarkers and examine the proteins associated with the pathogenesis of refractory COVID-19, we conducted high-coverage proteomics on serum extracellular vesicles collected from 12 patients with COVID-19 at different disease severity levels and 4 healthy controls. Furthermore, single-cell RNA sequencing of peripheral blood mononuclear cells collected from 10 patients with COVID-19 and 5 healthy controls was performed. Results Among the 3046 extracellular vesicle proteins that were identified, expression of MACROH2A1 was significantly elevated in refractory cases compared to non-refractory cases; moreover, its expression was increased according to disease severity. In single-cell RNA sequencing of peripheral blood mononuclear cells, the expression of MACROH2A1 was localized to monocytes and elevated in critical cases. Consistently, single-nucleus RNA sequencing of lung tissues revealed that MACROH2A1 was highly expressed in monocytes and macrophages and was significantly elevated in fatal COVID-19. Moreover, molecular network analysis showed that pathways such as “estrogen signaling pathway,” “p160 steroid receptor coactivator (SRC) signaling pathway,” and “transcriptional regulation by STAT” were enriched in the transcriptome of monocytes in the peripheral blood mononuclear cells and lungs, and they were also commonly enriched in extracellular vesicle proteomics. Conclusions Our findings highlight that MACROH2A1 in extracellular vesicles is a potential biomarker of refractory COVID-19 and may reflect the pathogenesis of COVID-19 in monocytes.

Abstract

Abstract Background Aneurysm of a coronary artery branch with a fistula is extremely rare. Here, we present a case of giant aneurysm of the left circumflex artery branch with a fistula to the coronary sinus treated successfully with aneurysmectomy. Case presentation A 58-year-old woman was referred to our hospital due to an abnormal pericardial mass found by multidetector computed tomography. Imaging examination revealed a dilated left circumflex artery branch with a 30-mm aneurysm. Coronary angiography confirmed a left circumflex artery branch aneurysm with a fistula to the coronary sinus. As percutaneous occlusion of the aneurysm by catheterization was considered unsuccessful, the aneurysm was resected, and the fistula was occluded surgically with excellent outcome. Pathological examination suggested that congenital factors may have contributed to the development of the aneurysm. Computed tomography showed no recurrence of the aneurysm at 1-year postoperative follow-up. Conclusions We presented a case of giant aneurysm of the left circumflex artery branch with a fistula to the coronary sinus. This is the first report of the combination of a giant coronary artery branch aneurysm with a fistula to the coronary sinus. Surgical aneurysmectomy should be considered in such cases to avoid fatal aneurysmal complications.

Abstract

Genetic mechanisms influencing COVID-19 susceptibility are not well understood. Here, the authors analyzed whole blood RNA-seq data of 465 Japanese individuals with COVID-19, highlighting thousands of fine-mapped variants affecting expression and splicing of genes, as well as the presence of COVID-19 severity-interaction eQTLs.

Abstract

This study explored the pathophysiology of bipolar disorder (BD) and schizophrenia (SZ) by examining the associations between the two disorders and single nucleotide polymorphisms (SNPs) involved in the dopamine signaling system. This was a case-controlled, exploratory, and multicenter study. A total of 1048 patients with BD (495 male; mean age, 49.6 ​± ​15.0 years), 2106 patients with SZ (1159 male, 49.6 ​± ​15.0 years), and 2240 healthy controls (HCs) (917 male, 42.3 ​± ​14.2 years) were included, and all the volunteers were Japanese. SNPs at tyrosine hydroxylase rs10770141 ​C-824T, catechol-O-methyltransferase rs4680 ​G/A(Val158Met), dopamine receptor D2 gene (DRD2) rs1799732 -141C Ins/Del, and DRD2/ANKK1 (Taq1A) rs1800497 ​C/T were examined. Binomial logistic regression analyses were performed to analyze the four SNPs, age, and sex. C allele and heterozygous CT in Taq1A were associated with an increased risk of BD. A comparison of the BD and HC groups revealed a significant association between heterozygous CT in Taq1A and BD in female participants. Heterozygous CT in Taq1A showed a significant association with BD as compared to SZ. DRD2 Taq1A polymorphism (CT heterozygotes) is associated with a high risk of BD in the Japanese population, particularly in females. DRD2 genetic predisposition in the dopamine signaling system and sex-specific factors may be associated with the pathophysiology of BD.
