Plutonium zwierleini is a large plutoniumid centipede of great evolutionary interest, occurring with isolated populations along the western Mediterranean area, from Spain to Italy. Due to its rarity and the extreme paucity of available records, P. zwierleini is among the least known Mediterranean chilopods, and scarce information is currently available on its ecology and natural history. Based on an extensive sampling effort carried out in Sicily between 2022 and 2023, we here provide additional occurrence localities for the species across Sicily, and new insights into its ecology. Overall, 29 novel Sicilian records of P. zwierleini, scattered across 21 localities, were collected thus increasing its known Sicilian distribution area by 117%, and the number of localities by 110%. The species was found in a wide range of habitats such as open areas, woods, buildings, and caves, characterizing Plutonium zwierleini as a habitat generalist, whose fine ecological preferenda need to be further explored. Moreover, to explore the diet and behaviour of the species, some specimens were kept in captivity. The captive individuals fed mostly on dead or poorly mobile soft-bodied prey and inert food, without ever displaying predatory behaviour; this suggests that, contrarily to what is currently assumed, P. zwierleini might be a scavenger rather than a predator. The potential distribution of Plutonium zwierleini in Sicily was inferred based on georeferenced occurrence records and climatic variables. The implemented MaxEnt model forecasts the possible occurrence of P. zwierleini on the whole island, with the single exception of its south-easternmost part, possibly due to the local pattern of precipitation seasonality. We hope that the present work might pave the way for further surveys aimed at a better understanding of the ecology of Plutonium zwierleini and the collection of new data in the other regions inhabited by this secretive species. - Abstract
Angiogenesis inhibitor Bevacizumab (BVZ) may lead to the development of adverse effects, including hypertension and cardiac ischemia. Whether assessment of changes in myocardial strain by two-dimensional speckle-tracking echocardiography (2D-STE) can be of value in detecting BVZ-mediated cardiotoxicity at an earlier stage is not known. We investigated whether 2D-STE can non-invasively detect early evidence of cardiotoxicity in metastatic colorectal cancer (mCRC) patients treated with BVZ. Between January and June 2019, 25 consecutive patients (71.8 ± 7.5 year/old, 17 males) with mCRC were prospectively enrolled. Patients underwent physical examination, blood tests, and conventional 2D-transthoracic echocardiography implemented with 2D-STE analysis, at baseline and at 3 and 6 months following treatment with BVZ (15 mg/kg every 15 days) + 5-fluorouracil/folinic acid plus oxaliplatin (FOLFOX i.v.). At 6-month follow-up, we assessed occurrence of global longitudinal strain (GLS) impairment (> 15% decrease in GLS compared with baseline) as primary end-point and a new-onset systemic hypertension (secondary end-point). On average, GLS showed a progressive significant impairment after BVZ, from − 17.4 ± 3.2% at baseline to − 16 ± 2.9% (p = 0.003) at 6-month follow-up; > 15% decrease in GLS (primary end-point) was detected in 9 patients (36%). All other strain parameters remained unchanged. New-onset systemic hypertension (secondary end-point) was diagnosed in five patients (20%). No significant changes were observed in serial high-sensitivity cardiac troponin I measurements. No patient developed significant changes in LV size or LV ejection fraction; no case of clinically symptomatic HF was observed during BVZ-treatment. Measurement of GLS by 2D-STE analysis can effectively detect BVZ-mediated cardiotoxicity at an early stage.

Subterranean ecosystems are among the most widespread environments on Earth, yet we still have poor knowledge of their biodiversity. To raise awareness of subterranean ecosystems, the essential services they provide, and their unique conservation challenges, 2021 and 2022 were designated International Years of Caves and Karst. As these ecosystems have traditionally been overlooked in global conservation agendas and multilateral agreements, a quantitative assessment of solution-based approaches to safeguard subterranean biota and associated habitats is timely. This assessment allows researchers and practitioners to understand the progress made and research needs in subterranean ecology and management. We conducted a systematic review of peer-reviewed and grey literature focused on subterranean ecosystems globally (terrestrial, freshwater, and saltwater systems), to quantify the available evidence-base for the effectiveness of conservation interventions. We selected 708 publications from the years 1964 to 2021 that discussed, recommended, or implemented 1,954 conservation interventions in subterranean ecosystems. We noted a steep increase in the number of studies from the 2000s while, surprisingly, the proportion of studies quantifying the impact of conservation interventions has steadily and significantly decreased in recent years. The effectiveness of 31% of conservation interventions has been tested statistically. We further highlight that 64% of the reported research occurred in the Palearctic and Nearctic biogeographic regions. Assessments of the effectiveness of conservation interventions were heavily biased towards indirect measures (monitoring and risk assessment), a limited sample of organisms (mostly arthropods and bats), and more accessible systems (terrestrial caves). - Abstract
Pectus excavatum (PE) may cause symptoms and alter cardiopulmonary function. Left ventricular (LV) and right ventricular (RV) function have been reported to be impaired in PE subjects. However, this issue has not been systematically investigated with respect to the degree of chest wall abnormality. We aimed to evaluate the influence of severity of chest shape abnormality on myocardial strain parameters in PE subjects. We studied 30 healthy subjects (55.8 ± 14.0 year/old, 18 males) with PE, assessed by the ratio of chest transverse diameter over the distance between sternum and spine (modified Haller index, MHI, >2.5), and 30 controls (MHI ≤2.5) matched by age, sex, and cardiovascular risk factors. Participants underwent 2-dimensional (2D) transthoracic echocardiography implemented with 2D-speckle tracking echocardiography. Right-heart and left-heart chamber dimensions, and stroke volume, were significantly reduced in PE subjects (all P< 0.0001). While LV ejection fraction, E/A, and E/e’, did not significantly differ between the 2 groups, all LV and RV strain and strain rate parameters were severely reduced in subjects with PE (P < 0.0001). Importantly, in PE subjects, but not in controls, LV global longitudinal strain, LV global circumferential strain, LV global radial strain, and RV free wall systolic strain, were all linearly correlated to MHI (all P < 0.0001). In healthy subjects with PE, abnormal chest anatomy progressively impairs myocardial strain. However, this impairment is not due to subclinical myocardial dysfunction; it might reflect intraventricular dyssynchrony due to compressive phenomena, or technical limitations of strain methodology, due to chest wall abnormality.

OBJECTIVE: An increased cardiovascular mortality and morbidity has been widely reported in patients with atrial fibrillation (AF). In this study, a subanalysis of the AntiThrombotic Agents Atrial Fibrillation (ATA‐AF) is performed with the aim to evaluate estimated glomerular filtration rate (eGFR) as an independent prognostic marker of cardiovascular mortality and morbidity in patients with AF. METHODS AND RESULTS: The ATA‐AF study enrolled 7148 patients with AF, in 360 Italian centers. The eGFR was calculated from data reported in patient notes or hospital database. This post‐hoc analysis included 1097 AF patients with eGFR data available and 1‐year clinical follow‐up. According to many studies, extracorporeal dialysis with convective methods is associated with better clinical outcomes and a survival benefit compared to diffusive techniques. However, there is no full agreement on the actual superiority of this kind of renal replacement therapy on hard end-points such as mortality. We performed a retrospective epidemiological cohort study to provide "real-world" evidence on the impact of convective and non-convective dialysis techniques on all-cause and cardiac mortality and biochemical outcomes among dialysis patients in Sicily, the southernmost region of Italy. Data of all incident adult patients (N = 6529) who have started chronic extracorporeal dialysis over the period 2009-2015 were retrieved from the Sicilian Registry of Nephrology, Dialysis and Transplantation. There were 1558 patients receiving convective techniques (23.86%). Overall mortality rate was 45.21% with a significant difference between convective (31.39%) and non-convective (49.55%) groups (P 0.0001). After adjustment for potential confounders in multiple Cox regression models of increasing complexity, the mortality risk remained significantly lower for patients treated with convective methods (HR, 0.581; 95%CI, 0.525 to 0.643; P 0.0001). Moreover, the convective group had a better blood chemistry profile, improved dialysis efficacy, and reduced mortality rate from cardiac diseases compared to the non-convective group. As a sensitivity analysis, patients were categorized according to propensity score quartiles and the hazard ratio for both all-cause and cardiac mortality was significantly lower for the convective group in each quartile. In conclusion, despite the observational and retrospective design, the results of the present study further support the use of convective therapies for the treatment of end-stage renal disease.

In the general understanding, and also in scientific practice, technology and society are viewed as two distinct entities. Related to this view is the assumption that technology and human experience are quite different and unconnected and also the idea that modernity has uprooted, de-contextualized and disembodied technical rationality. Taking a contrary approach, this study represents a theoretical exploration aimed at showing that in the domain of technological development, there are significant margins for maneuver in which to recuperate and valorize human and social action. As a work of theoretical sociology or social epistemology, this thesis approaches its subject from the theoretical background of the philosophy and sociology of technique. The historical and conventional assumptions of this theoretical background, it is argued, have been and continue to be characterized by a hegemonically defined essentialist paradigm. This paradigm has been fiercely counteracted by two opposed approaches, critical theory and pragmatism. The present work combines these approaches, usually considered mutually incompatible, for the development of a new theoretical gaze or perspective. The aim has been to engage in a theoretical research oriented to a new philosophy of praxis in order to instigate a critical and constructivist approach to technology. The main result expected of this work is the provision of a problematized and multifaceted semantic map leading to a multidimensional conceptual re-integration of skilled experience in human technical action. Technologie en samenleving worden vaak als twee afzonderlijke entiteiten gezien. Gerelateerd aan deze opvatting is de vooronderstelling dat technologie ontkoppeld is van de menselijke ervaring en dat de moderniteit ook gekenmerkt wordt door een technische rationaliteit die op zichzelf staat. Ondanks de dominante positie van deze perceptie van technologie in wetenschappelijke en algemene publieke kringen, wordt in deze studie een tegenovergestelde theoretisch verkenning uitgevoerd. Dit onderzoek is erop gericht om aan te tonen dat de mens juist wel een aanzienlijke manoeuvreerruimte heeft op het gebied van technologische ontwikkeling en dat de menselijke ervaring en het sociale handelen wel in het centrum van de belangstelling kan komen te staan en opnieuw gewaardeerd kunnen worden. Als een theoretisch sociologisch of sociaal epistemologisch werk worden technologie en samenleving in dit proefschrift benaderd vanuit een techniek filosofische en techniek sociologische achtergrond. De historische en conventionele vooronderstellingen van dit theoretische referentiekader worden gekenmerkt – zo wordt betoogd – door een essentialistisch paradigma van technologieontwikkeling dat een hegemoniale positie heeft verworven en bepalend is voor ons kijken naar techniek in de huidige tijd. Toch zijn er twee filosofische stromingen geweest – de kritische theorie en het pragmatisme - die zich sterk hebben afgezet tegen de hegemoniale positie van het essentialistische technologische paradigma. Hoewel deze twee benaderingen vaak als onderling tegenstrijdig en incompatibel zijn beschouwd, wordt in dit proefschrift geprobeerd deze twee benaderingen samen te brengen voor de ontwikkeling van een nieuw theoretisch perspectief; een nieuwe lens van waaruit naar technologieontwikkeling kan worden gekeken. Het proefschrift beoogt een theoretisch onderzoek uit te voeren dat erop gericht is een bijdrage te leveren aan de ontwikkeling van een nieuwe filosofie van de praxis om van daaruit een kritisch en constructivistische technologie benadering te gaan ontwikkelen. Het belangrijkste resultaat dat van deze studie verwacht kan worden is de schets van een veelzijdig semantische kaart die de hegemoniale positie van het essentialistische paradigma problematiseert en een aanzet geeft tot een multi-dimensionele en nieuwe integratie van de menselijke ervaring in het technisch handelen.

- Abstract
Chronic aortic regurgitation (AR) is associated with a unique pattern of left ventricular (LV) volume and pressure overload, leading to LV remodelling. LV torsional motion, a key component of LV performance, can be altered in this setting. We aimed to assess the impact of LV remodelling on LV torsional dynamics parameters using speckle-tracking echocardiography (STE) in asymptomatic AR patients. We prospectively enrolled 60 patients with chronic AR and LVEF > 50 % and 55 healthy controls. LV rotation, twisting and untwisting were assessed using STE. Patients with AR had higher LV diameters, volumes and mass, a more spherical LV shape than controls, but similar LVEF. In AR patients we found reduced peak LV apical rotation and decreased (2.1 ± 0.8 vs 2.9 ± 0.9°/cm, p < 0.001) and delayed (time to peak LV twist: 0.94 ± 0.12 vs 0.99 ± 0.09, p = 0.004) peak LV torsion. Also, peak LV untwisting velocity was decreased (−123.5 ± 41.5 vs −152.3 ± 55.0°/s, p = 0.002) due to lower peak LV apical diastolic rotation rate. LV shape influenced LV torsional dynamics, a more spherical LV displaying reduced peak LV apical rotation and diastolic rotation rate and decreased LV twist. A more hypertrophied LV had a lower peak LV torsion, peak LV apical diastolic rotation rate and peak LV untwisting velocity. LV apical rotation and torsion are decreased and LV twist is delayed in patients with chronic AR and normal LVEF, detecting early subclinical LV dysfunction before LVEF declines. Also, LV untwisting is reduced in these patients. LV remodelling impairs LV torsional dynamics parameters in this setting.
