1. The Effectiveness of Individualized Morphosyntactic Target Identification and Explicit Intervention Using the SHAPE CODING System for Children with Developmental Language Disorder and the Impact of Within-Session Dosage

Abstract

Purpose: We investigated the effectiveness of a highly individualized morphosyntactic intervention using the SHAPE CODING™ system delivered at different dosages. Method: Eight children with developmental language disorder aged 8;0-10;10 (years;months) received 10 hr of explicit individualized intervention for morphosyntax delivered in 30-min individual sessions once per week for 20 weeks. Following at least four baseline probe tests, two grammatical targets per session received explicit instruction until they reached criterion (90%), when the next target was introduced. To control for session length and teaching episode density, either both targets received 20 teaching episodes per session or one target received 10 teaching episodes and the other 30. Maintenance testing of completed targets was also carried out. Results: Scores on probe tests post-intervention were significantly higher than during the baseline phase (d = 1.6) with no change during the baseline or maintenance phases. However, progress during the intervention phase was highly significant. One participant showed significantly faster progress with intervention, while one (with the lowest attention score) made little progress. When considering progress relative to cumulative intervention sessions, progress was faster with 30 teaching episodes per session and slower with 10. However, when cumulative teaching episodes were used as the predictor, all three within-session dosages showed very similar rates of progress, with the odds of a correct response increasing by 3.9% for each teaching episode. The targets that were achieved required an average of 40-60 teaching episodes. Conclusions: With the exception of one participant, the individualized intervention was highly effective and efficient. Thus, the individualized target identification process and intervention method merit further research in a larger group of children. The cumulative number of teaching episodes per target provided across sessions appeared to be key. Thus, clinicians should aim for high teaching episode rates, particularly if the number of sessions is constrained. Otherwise, intervention scheduling can be flexible.

Abstract

Maternal obesity is associated with increased risk of diabetes, cardiovascular disease and hypertension in adult offspring. Midlife hypercholesterolemia and hypertension are risk factors for Alzheimer's disease, suggesting that the ageing brain may be impacted by early life environment. We found that exposure to a high fat diet during gestation and lactation induced changes in multiple components of the neurovascular unit, including a downregulation in apolipoprotein E and fibronectin, an upregulation in markers of astrocytes and perivascular macrophages and altered blood vessel morphology in the brains of adult mice. Feeding of high fat diet after weaning increased lipid droplets in the brain and influenced the fatty acid composition of phosphatidylcholine and phosphatidylethanolamine species, but did not affect the neurovascular unit. Sustained high fat diet over the entire lifespan resulted in additional decreases in levels of pericytes and collagen IV, changes in phospholipid composition and impaired perivascular clearance of Beta-amyloid (A-Beta) from the brain. In humans, vascular A-Beta load was significantly increased in the brains of aged individuals with a history of hypercholesterolemia. These results support a critical role for early dietary influence on the brain vasculature across the lifespan, with consequences for the development of age-related cerebrovascular and neurodegenerative diseases., Comment: 45 pages, 7 figures

Abstract

Purpose – Guided by several of the 9 essentials of what it means to be a PDS (NAPDS, 2021), authors share a plea to rethink the teacher burnout-attrition-staffing crisis with a call toward a moral imperative of recentering an ethic of care. Many schools are operating under anti-care practices which directly undermine teacher wellness in part due to secondary traumatic stress, rising workload demands and intensive student needs. Design/methodology/approach – Reflecting a compilation of teacher voices, including participants from three research studies and the collective decades-worth experience of educator scholars, this paper presents a synthesis of educator burnout and the role of educator wellness within trauma-informed social emotional learning initiatives. Findings – The practical model of educator resilience offers a potential solution to burnout and attrition by prioritizing care for teachers individually and collectively prior to addressing care for students. Originality/value – The model articulates educator resilience as the motivational force of life within a school community focused on an ethic of care that drives the collective and individuals within the collective to be their best. This aligns with foundation principles of PDS schools and Goodlad and colleagues’ decades-old call to foreground the moral dimensions of teaching in school reform (1990).

7. Understanding Capacity for Implementing New Interventions: A Qualitative Study of Speech and Language Therapy Services for Children with Speech Sound Disorder

Abstract

Background: Many speech sound disorder (SSD) interventions with a long-term evidence base are 'new' to clinical practice, and the role of services in supporting or constraining capacity for practice change is underexplored. Innovations from implementation science may offer solutions to this research-practice gap but have not previously been applied to SSD. Aim: To explain variation in speech and language therapy service capacity to implement new SSD interventions. Methods & Procedures: We conducted an intensive, case-based qualitative study with 42 speech and language therapists (SLTs) in three NHS services (n = 39) and private practice (n = 3) in Scotland. We explored therapists' diverse experiences of SSD practice change through individual interviews (n = 28) or self-generated paired (n = 2) or focus groups (n = 3). A theoretical framework (Normalization Process Theory) helped us understand how the service context contributed to the way therapists engaged with different practice changes. Outcomes & Results: We identified six types ('cases') of practice change, two of which involved the new SSD interventions. We focus on these two cases ('Transforming' and 'Venturing') and use Normalization Process Theory's Cognitive participation construct to explain implementation (or not) of new SSD interventions in routine practice. Therapists were becoming aware of the new interventions through knowledge brokers, professional networks and an intervention database. In the Transforming case, new SSD interventions for selected children were becoming part of local routine practice. Transforming was the result of a favourable service structure, a sustained and supported 'push' that made implementation of the new interventions a service priority, and considerable collective time to think about doing it. 'Venturing' happened where the new SSD interventions were not a service priority. It involved individual or informal groups of therapists trying out or using one or more of the new interventions with selected children within the constraints of their service context. Conclusions & implications: New, evidence-based SSD interventions may be challenging to implement in routine practice because they have in common a need for therapists who understand applied linguistics and can be flexible with service delivery. Appreciating what it really takes to do routine intervention differently is vital for managers and services who have to make decisions about priorities for implementation, along with realistic plans for resourcing and supporting it.

8. Beyond Public/Private: Instituting Transformations in the Plastic University

Abstract

As we re-imagine the role and value of the university, we need to pose new questions about knowledge and institutionality at a moment of intersecting crises. This essay presents a case study of a university in Western Canada, one shaped by the impacts of intensive extraction from human and more-than-human beings and now facing the challenge of reinventing itself in response to acute political, environmental and economic pressures. Approaching fossil fuel industries within a wider context of settler-occupying states' investments in 'natural' resources, I explore tensions between the university's racial, colonial, patriarchal and capitalist formation, its current investments, and its capacity for institutional transformation. Andre Keet's application of Catherine Malabou's philosophy to develop Decentered Critical University Studies (DCUS) is linked to some theoretical frameworks developed by Indigenous Feminist and Science and Technology Studies (STS) scholars to support an analytical distinction between extractive forms of knowledge that I call 'plastic' and the 'plasticity' inherent in relational ways of knowing human and more-than-human beings. An examination of the role of nonprofit research institutes at the University informs my conclusion that academic integrity is less a function of whether a university is public or private than of the relationships it institutes between human and more-than-human beings in place.

Abstract

Abstract Background Therapeutic options for early-stage hepatocellular carcinoma (HCC) in individual patients can be limited by tumor and location, liver dysfunction and comorbidities. Many patients with early-stage HCC do not receive curative-intent therapies. Stereotactic ablative body radiotherapy (SABR) has emerged as an effective, non-invasive HCC treatment option, however, randomized evidence for SABR in the first line setting is lacking. Methods Trans-Tasman Radiation Oncology Group (TROG) 21.07 SOCRATES-HCC is a phase II, prospective, randomised trial comparing SABR to other current standard of care therapies for patients with a solitary HCC ≤ 8 cm, ineligible for surgical resection or transplantation. The study is divided into 2 cohorts. Cohort 1 will compromise 118 patients with tumors ≤ 3 cm eligible for thermal ablation randomly assigned (1:1 ratio) to thermal ablation or SABR. Cohort 2 will comprise 100 patients with tumors > 3 cm up to 8 cm in size, or tumors ≤ 3 cm ineligible for thermal ablation, randomly assigned (1:1 ratio) to SABR or best other standard of care therapy including transarterial therapies. The primary objective is to determine whether SABR results in superior freedom from local progression (FFLP) at 2 years compared to thermal ablation in cohort 1 and compared to best standard of care therapy in cohort 2. Secondary endpoints include progression free survival, overall survival, adverse events, patient reported outcomes and health economic analyses. Discussion The SOCRATES-HCC study will provide the first randomized, multicentre evaluation of the efficacy, safety and cost effectiveness of SABR versus other standard of care therapies in the first line treatment of unresectable, early-stage HCC. It is a broad, multicentre collaboration between hepatology, interventional radiology and radiation oncology groups around Australia, coordinated by TROG Cancer Research. Abstract

Below the surface layers of the ocean, there are ecosystems full of undiscovered life. Scientists love to ask questions like, “Who is there?” and “What are they doing?” An important question scientists are beginning to ask is, “How will these living things react to warmer waters, loss of oxygen, or pollution?” To answer these questions, scientists build equipment to observe life in the deep sea. We built an ocean lander named BEEBE, with a camera, sensors, and waterproof casing. BEEBE helped us study deep-sea ecosystems near the coast of California and learn about the animals that live there. We can use what we learned to recognize vulnerable communities and the threats some ocean animals face. An ocean lander like BEEBE can be a great tool to learn more about coastal, deep-sea ecosystems around the world!

20. Addressing Issues of Equity Using the Cross-Pollination of Universal Design for Learning and Culturally Responsive Teaching

Abstract

This article provides a rationale for the crosspollination of two asset-based pedagogies: Universal Design for Learning (UDL) and culturally responsive teaching (CRT) and specific examples that illustrate the important role that strong school- university partnerships play to address potential barriers to UDL and CRT implementation. Concrete examples of implementation at one university along with specific teacher and student actions further illustrate implementation of this crosspollination. Since teaching and learning are culturally situated and vary across and within cultural and linguistic groups (Gay, 2010), it is important for pre- and in-service teachers to understand this cross-pollination when using the UDL Guidelines as an implementation tool in their classrooms to proactively identify and address potential barriers to student learning while sustaining their students' cultural and linguistic identities. This article articulates the authors' vision and journey towards operationalizing this cross-pollination of UDL and CRT (Thorius and Waitoller, 2017).

21. Predicting Learning Interactions in Social Learning Networks: A Deep Learning Enabled Approach

Abstract

We consider the problem of predicting link formation in Social Learning Networks (SLN), a type of social network that forms when people learn from one another through structured interactions. While link prediction has been studied for general types of social networks, the evolution of SLNs over their lifetimes coupled with their dependence on which topics are being discussed presents new challenges for this type of network. To address these challenges, we develop a series of autonomous link prediction methodologies that utilize spatial and time-evolving network architectures to pass network state between space and time periods, and that models over three types of SLN features updated in each period: neighborhood-based (e.g., resource allocation), path-based (e.g., shortest path), and post-based (e.g., topic similarity). Through evaluation on six real-world datasets from Massive Open Online Course (MOOC) discussion forums and from Purdue University, we find that our method obtains substantial improvements over Bayesian models, linear classifiers, and graph neural networks, with AUCs typically above 0.91 and reaching 0.99 depending on the dataset. Our feature importance analysis shows that while neighborhood and path-based features contribute the most to the results, post-based features add additional information that may not always be relevant for link prediction., Comment: This work was published in the IEEE/ACM Transactions on Networking

22. A qualitative study of how structural vulnerability shaped COVID-19 testing behaviors in Portland, Maine

Abstract

BackgroundPeople with structural vulnerabilities (including immigrants, people who use drugs, and those who are unhoused or uninsured) are more likely to experience COVID-19 testing disparities relative to other groups. We documented barriers and facilitators to COVID-19 testing and explored how structural vulnerabilities created and/or exacerbated COVID-19 testing barriers.MethodsBetween 2021 and 2022, we conducted semi-structured interviews with 34 members of structurally vulnerable populations and 27 key informants who provide health and social services to them. Our abductive analysis was iterative, utilizing both inductive and deductive coding processes. Recognizing that adequate and appropriate testing for COVID-19 is a complex health behavior that involves both decision-making and issues related to access, we developed a hybrid model of COVID-19 testing behavior to organize reported barriers. We then used that model for more in-depth analysis of structural vulnerabilities in the context of testing.ResultsOur model of testing behaviors provides a framework for understanding the many barriers and facilitators relevant to COVID-19 testing. After identifying locally-reported barriers, we found that specific conditions—economic precarity, legal precarity, the confusing U.S. healthcare landscape, English-exclusive environments, and stigmatizing medical encounters—make adequate and appropriate testing less likely by making COVID-19 testing feel riskier (entailing legal, financial, and psycho-social risks) and making healthcare, and thus vicariously testing, more difficult to access.ConclusionThe COVID-19 pandemic exposed disparities in health care delivery. To avoid under-testing and its associated health consequences during the next pandemic, public health efforts should address structural conditions to ameliorate risks and bolster testing infrastructure to improve access.

Abstract

Background Speech impairments are common with Parkinson’s disease (reported prevalence 68%), increasing conversational demands, reliance on family and social withdrawal. Objective(s) The PD COMM trial compared the clinical and cost-effectiveness of two speech and language therapy approaches: Lee Silverman Voice Treatment LOUD and National Health Service speech and language therapy for the treatment of speech or voice problems in people with Parkinson’s disease to no speech and language therapy (control) and against each other. Design PD COMM is a phase III, multicentre, three-arm, unblinded, randomised controlled trial. Participants were randomised in a 1 : 1 : 1 ratio to control, National Health Service speech and language therapy or Lee Silverman Voice Treatment LOUD via a central computer-generated programme, using a minimisation procedure with a random element, to ensure allocation concealment. Mixed-methods process and health economic evaluations were conducted. Setting United Kingdom outpatient and home settings. Participants People with idiopathic Parkinson’s disease, with self-reported or carer-reported speech or voice problems. We excluded people with dementia, laryngeal pathology and those within 24 months of previous speech and language therapy. Interventions The Lee Silverman Voice Treatment LOUD intervention included maximum effort drills and high-effort speech production tasks delivered over four 50-minute therapist-led personalised sessions per week, for 4 weeks with prescribed daily home practice. National Health Service speech and language therapy content and dosage reflected local non-Lee Silverman Voice Treatment speech and language therapy practices, usually 1 hour, once weekly, for 6 weeks. Trained, experienced speech and language therapists or assistants provided interventions. The control was no speech and language therapy until the trial was completed. Main outcome measures Primary outcome: Voice Handicap Index total score at 3 months. Secondary outcomes: Voice Handicap Index subscales, Parkinson’s Disease Questionnaire-39; Questionnaire on Acquired Speech Disorders; EuroQol-5D-5L; ICEpop Capabilities Measure for Older Adults; Parkinson’s Disease Questionnaire – Carers; resource utilisation; and adverse events. Assessments were completed pre-randomisation and at 3, 6 and 12 months post randomisation. Results Three hundred and eighty-eight participants were randomised to Lee Silverman Voice Treatment LOUD (n = 130), National Health Service speech and language therapy (n = 129) and control (n = 129). The impact of voice problems at 3 months after randomisation was lower for Lee Silverman Voice Treatment LOUD participants than control [−8.0 (99% confidence interval: −13.3, −2.6); p = 0.001]. There was no evidence of improvement for those with access to National Health Service speech and language therapy when compared to control [1.7 (99% confidence interval: −3.8, 7.1); p = 0.4]. Participants randomised to Lee Silverman Voice Treatment LOUD reported a lower impact of their voice problems than participants randomised to National Health Service speech and language therapy [99% confidence interval: −9.6 (−14.9, −4.4); p 70 years); disease severity measured using the Hoehn and Yahr staging (1.0–2.5, 3.0–5.0) and severity of speech measured using the Voice Handicap Index (VHI) total score (≤ 33, mild 34–44, moderate 45–61, severe > 61). To avoid any possibility of the treatment allocation becoming predictable, a random element was included within the randomisation process. Once the participant was randomised into the trial, they were given a unique trial identifier and the treatment allocation was confirmed by e-mail to the site. The primary outcome measure for the trial was patient-reported VHI total score at 3 months. The VHI measures the psychosocial consequences of voice disorders and provides an overall perception of effectiveness of voice-related communication. The VHI comprises of 30 questions (0–120 negatively scored) divided into emotional, functional and physical subscales (0–40 subscale score). Secondary outcomes were: the VHI subscales; the Parkinson’s Disease Questionnaire-39, a validated, health-related quality-of-life measure specific to PD; the Questionnaire on Acquired Speech Disorders, self-reported participation restriction related to speech and communication; the participant-rated EuroQol5D (5-level version) which provides a simple descriptive profile and a single index value for health status; the ICEpop Capabilities Measure for Older Adults (ICECAP-O), a measure of capability in older people for use in economic evaluations; a disease-specific participant resource usage questionnaire; carer quality of life, measured using the PD Questionnaire – carer; and adverse events. For participants in either SLT arm, any vocal strain or abuse believed to be associated with treatment was identified by the therapists at the participants’ SLT session and was reported in the adverse event log. All participant-reported resource usage forms completed for the trial were checked to ensure that no vocal strain or abuse had occurred in outpatient appointments with ENT specialists. At the 12-month clinical visit, the medical professional also checked whether any adverse events had occurred since entering the trial. There were two nested studies within this trial: a process evaluation and an economic evaluation. A process evaluation of the content of the interventions examined participants’ and therapists’ reported content of a subset of the two SLT interventions. Individual participant therapy data recorded by participants and therapists during the PD COMM trial were extracted from treatment record forms and therapy notes. Extraction category headings were piloted and reflected the Template for Intervention Description and Replication intervention reporting guidelines, with ongoing refinement as extraction progressed and components of the interventions were identified which were inadequately accounted for in the initial framework. The process evaluation of the implementation of the interventions used normalisation process theory which is highly attuned to the challenges of complex interventions as it encourages looking at systems as a whole. We considered that its conceptual framework would assist the interpretation and synthesis of data and analysis to explain what implementation processes took place and the interactions and gaps between the PD COMM interventions, the changing context, speech and language therapists and their practice. A within-trial economic evaluation in the form of cost–utility analysis was conducted from the perspective of the NHS. The primary results were expressed in terms of cost per quality-adjusted life-year (QALY) gained at 12 months for the three trial comparisons. Additional secondary analyses were performed from the NHS, personal social services perspective, broader societal perspective and using the capability approach, which uses broader measures of capability well-being. Results The mean age of participants in PD COMM was approximately 70 years old, and 74% were male (286/388). The mean duration of PD was between 5 and 6 years, with a broad range from newly diagnosed to over 30 years, and the majority (61%) were in Hoehn and Yahr stage of 2 or less (

Abstract

Releasing charge into natural droplet systems such as fog and clouds offers a route to influence their properties. To facilitate charge release across a wide range of altitudes and meteorological circumstances—such as developing clouds—a charge emitter has been developed for integration with the conventional cloud-seeding flares carried by crewed cloud-seeding aircraft. This allows charge emitters to be used alongside, or instead of, conventional particle releasing flares. The charge emitter flare system is self-contained and self-powered, and includes internal monitoring and recording of its operating parameters. Using this “flare emitter” approach, successful charge emission has been demonstrated in level flight, at 3 km altitude, likely to have exceeded natural ion concentrations by several orders of magnitude. This quantitative verification of successful charge emission can underpin further physically based experiments on the effectiveness of charge release in cloud seeding.

Abstract

Bio‐logging has revealed much about high‐latitude seabird migratory strategies, but migratory behaviour in tropical species may differ, with implications for understanding nutrient deposition. Here we use combined light‐level and saltwater immersion loggers to study the year‐round movement behaviour of adult red‐footed boobies Sula sula rubripes from the Chagos Archipelago, tropical Indian Ocean, to assess migratory movements and estimate nutrient deposition rates based on the number of days they spent ashore. Light levels suggest that red‐footed boobies are resident in the Chagos Archipelago year‐round, although there are large latitudinal errors this close to the equator. Immersion data also indicate residency with tracked birds returning to land every one or two days. Spending an average of 79.86 ± 2.80 days and 280.84 ± 2.64 nights per year on land allows us to estimate that the 21 670 pairs of red‐footed boobies deposit 37.34 ± 0.56 tonnes year−1 of guano‐derived nitrogen throughout the archipelago. Our findings have implications for tropical seabird conservation and phylogenetics, as well as for assessing the impact of seabird nutrients on coral reef ecosystems.

32. Leveraging Social Media and Scholarly Discussion for Educator Empowerment

Abstract

This paper shares insights from an international community of educators who have been using social media as a virtual space for a scholarly reading group: #edureading. The collection of educator narratives presented in this paper show how social networks on Twitter and Flipgrid were used as inclusive environments for teacher-led professional development. This paper is both a report of research involving five practitioners inquiring into their collective experience, and an exercise in building the scholarly capacity of the #edureading group. The accessibility of the social media platforms, as well as the collaborative, inquiry-based approach to scholarly reading, emerge as key themes in the educator narratives. The findings of this research emphasise that professional learning occurring in virtual spaces is open to social mediation using the norms of social networks, rather than the norms of workplaces, jurisdictions or education sectors, and that this can lead to a greater sense of empowerment for educators.

Abstract

The role of droplet charge in stratiform clouds is one of the least well understood areas in cloud microphysics and is thought to affect cloud radiative and precipitation processes. Layer clouds cover a large proportion of the Earth's surface and are important in regulating the planetary radiation budget. Using a new remote sensing method developed at our University Observatory, we demonstrate that charge in the base of stratiform clouds is typically of negative polarity, as expected from theory considering the vertical current flow into and out of the cloud. More detailed vertical charge structure of layer clouds can be found using balloon-carried instruments. Our previous research using in situ balloon observations has demonstrated that, on average, the bulk charge polarity and location agrees with theoretical predictions of positive charge at the upper edge and negative charge at the lower edge. Abstract

IntroductionAdvances in motion capture technology include markerless systems to facilitate valid data collection. Recently, the technological reliability of this technology has been reported for human movement assessments. To further understand sources of potential error, biological reliability must also be determined. The aim of this study was to determine the day-to-day reliability for a three-dimensional markerless motion capture (MMC) system to quantify 4 movement analysis composite scores, and 81 kinematic variables.MethodsTwenty-two healthy men (n = 11; X¯±SD; age = 23.0 ± 2.6 years, height = 180.4.8 cm, weight = 80.4 ± 7.3 kg) and women (n = 11; age = 20.8 ± 1.1 years, height = 172.2 ± 7.4 cm, weight = 68.0 ± 7.3 kg) participated in this study. All subjects performed 4 standardized test batteries consisting of 14 different movements on four separate days. A three-dimensional MMC system (DARI Motion, Lenexa, KS) using 8 cameras surrounding the testing area was used to quantify movement characteristics. 1 × 4 RMANOVAs were used to determine significant differences across days for the composite movement analysis scores, and RM-MANOVAs were used to determine test day differences for the kinematic data (p 50% total variability).DiscussionCombined, these findings effectively add to the body of literature suggesting sufficient reliability for MMC solutions in capturing kinematic features of human movement.

Abstract

Background The long-term outcomes of COVID-19 hospitalisation in individuals with pre-existing airway diseases are unknown. Methods Adult participants hospitalised for confirmed or clinically suspected COVID-19 and discharged between 5 March 2020 and 31 March 2021 were recruited to the Post-hospitalisation COVID-19 (PHOSP-COVID) study. Participants attended research visits at 5 months and 1 year post discharge. Clinical characteristics, perceived recovery, burden of symptoms and health-related quality of life (HRQoL) of individuals with pre-existing airway disease (i.e., asthma, COPD or bronchiectasis) were compared to the non-airways group. Results A total of 615 out of 2697 (22.8%) participants had a history of pre-existing airway diseases (72.0% diagnosed with asthma, 22.9% COPD and 5.1% bronchiectasis). Abstract

Esta revisión bibliográfica busca brindar información amplia y clara de los posibles riesgos a los que pueden estar expuestas las mujeres a lo largo de su ciclo menstrual teniendo contacto con diferentes compuestos con actividad de disrupción endocrina, que están en las toallas higiénicas. Empresas comercializadoras de productos de higiene íntima, al pasar los años y la evolución de las tecnologías, innovan en características, como el uso de fragancias, tamaño y presentación al público; pero han dejado atrás detalles como la calidad de las materias primas y su minuciosa revisión, que dentro de sus múltiples tareas tiene como fin identificar la presencia de disruptores endocrinos, situación que a largo plazo podría contribuir al deterioro de la salud de la población femenina, resaltando que las compañías no están obligadas a presentar al público los componentes de estos productos íntimos. Teniendo en cuenta que dicho producto es de los más usados en el mundo por la población femenina, se vuelve de suma urgencia la comunicación de los posibles efectos negativos sobre la salud que los disruptores endocrinos pueden causar. Entre los problemas de la salud está la aparición temprana de la menopausia, posibles problemas de fertilidad y cáncer.

44. Medication Required: P*RN – Promoting Healthy Attitudes and Improved Access to Pornography in Wathwood Hospital

Abstract

Aims The key aim of this project was to update and modernise the hospital procedure on how patients access pornographic material whilst detained at Wathwood Hospital. Within the update, we aimed to promote inclusivity and acceptance of all patients’ sexual identities as well as utilising the opportunity to emphasise healthy consenting sexual relationships. Patients in Forensic Mental Health settings are often inpatients for a significant amount of time, with multiple restrictions imposed on their private and family life. Current policy allows patients to purchase pornography for private use in their own room. It must be material of the same nature available in a main street outlet. In practice, material is usually purchased in DVD format from the Amazon website and subsequently screened for suitability by Security staff and finally approval by the Responsible Clinician. There have been numerous incident reports involving the trading of pornographic material. Methods Qualitative semi-structured group interviews (up to 5 people at a time) were conducted with patients in the medium-secure forensic services of Wathwood Hospital. They were recruited from the fortnightly Patient Forum. Anonymised questionnaires involving Likert scales and free text response spaces were also distributed at the Patient Forum. Data gathered investigated the percentage of patients who were aware of the current procedure, if they felt it worked well and what they thought the impact of accessing pornographic material might be. Staff were invited to complete a similar anonymised questionnaire, again considering their opinions on the positive or negative impacts of pornography for patients. In addition, we gathered data on whether there was a difference on the degree of comfort/discomfort about pornography, depending on whether the material involved opposite sex or same sex couples. In total, there were 40 survey participants. Results Some key areas for concern were found, for example, only 17% of staff and 16% of patients thought the current policy works well despite 69% of staff and 84% of patients feeling it is a patient's right to access pornography. Free text and focus group feedback established many benefits to it. It was clear that there were some areas of difficulty in the hospital policy, which would benefit from being refreshed. Conclusion Staff and patients overall feel that access to pornography is important for many of the patients. We identified areas for improvement in how this is accessed and a need to continually be considering the need to consider meeting the holistic needs for the patients.

Abstract

The critical role of AMPA receptor (AMPAR) trafficking in long-term potentiation (LTP) of excitatory synaptic transmission is now well established, but the underlying molecular mechanism is still uncertain. Recent research suggests that PSD-95 captures AMPARs via an interaction with the AMPAR auxiliary subunits-transmembrane AMPAR regulatory proteins (TARPs). To determine if such interaction is a core minimal component of the AMPAR trafficking and LTP mechanism, we engineered artificial binding partners, which individually were biochemically and functionally dead but which, when expressed together, rescue binding and both basal synaptic transmission and LTP. These findings establish the TARP/PSD-95 complex as an essential interaction underlying AMPAR trafficking and LTP.
