8. Pegylated interferon‐α2a in cutaneous T‐cell lymphoma – a multicenter retrospective data analysis with 70 patients.

Abstract

Summary: Background: Interferon‐alpha is an important therapeutic option for the treatment of the cutaneous T‐cell lymphomas (CTCL). Since the approved recombinant interferon‐α‐2a (IFN‐α2a) has no longer been produced since January 2020, pegylated interferon‐α2a (pegIFN‐α2a) can be used as an alternative treatment, even though it is not approved for the treatment of CTCL. The aim of this multicentre study was to generate comprehensive data on the efficacy and tolerability of pegIFN‐α2a in the treatment of CTCL. Patients and Methods: A multicenter retrospective study was conducted with 70 patients with CTCL from twelve German skin centers. Results: In total, 70 patients were included in the study, with 57.2% male and a mean age of 58.8 ± 14.9 years. Mycosis fungoides was present in 71.4% of cases and Sézary Syndrome in 28.6%. An overall response rate of 55.2% was observed with pegIFNα‐2a therapy. In 50% of cases, therapy was discontinued after 63.6 ± 33.5 weeks. The most common reason for discontinuation was adverse events, which occurred in 68.6% of cases and which were classified as severe in 29.2%. Blood count changes, fatigue and liver toxicity occurred most frequently. Conclusions: Our analysis provides comprehensive data on the efficacy and tolerability of pegIFNα‐2a therapy in patients with CTCL. In terms of response rates and side effect profile, pegIFNα‐2a appears to be comparable to IFN‐α2a therapy. [ABSTRACT FROM AUTHOR]

12. The fifth edition of the WHO‐Classification – what is new for cutaneous lymphomas?

Abstract

Summary: The recently published 5th edition of the "World Health Organization classification of hematolymphoid tumors: lymphoid neoplasms" provides a hierarchical reorganization. In general, new (definitive) entities as well as tumor‐like lesions were included. Primary cutaneous B‐cell lymphomas (CBCL) received a thorough review. A new class/family of cutaneous follicle center lymphomas was defined. Primary cutaneous marginal zone lymphoma is now presented as a separate entity independent from extranodal marginal zone lymphoma of mucosa‐associated lymphoid tissue. In primary cutaneous T‐cell lymphoma, former provisional entities were upgraded to definite entities. Sézary Syndrome was sorted into the class/family of mature T‐cell and NK‐cell leukemias. Additionally, a newly formed entity of primary cutaneous peripheral T‐cell lymphoma, NOS was created for CTCL entities that do not fit into the already described CTCL entities. The increasing importance of genomic and molecular data has already been recognized in classifying leukemias and systemic lymphomas. However, in PCL the genomic landscape has not yet been fully described and validated. Therefore, future research is necessary to describe the genomic and molecular mechanisms underlying the disease entities more clearly. 13. Spontaneous regression of primary cutaneous diffuse large B‐cell lymphoma, leg type: A case series and review of the literature.

Abstract

Primary cutaneous diffuse large B‐cell lymphoma, leg type (PCDLBCL, LT) is a subtype of cutaneous B‐cell lymphoma with unfavorable prognosis usually requiring aggressive polychemotherapy for disease control. Only single cases of spontaneous regression of PCDLBCL, LT are reported in the literature, peaking 3 months post‐biopsy following a clinical history of no longer than 1 year. Here, we report the first case of a spontaneously relapsing and remitting PCDLBCL, LT with complete regression after a clinical history of more than 9 years and thus an atypically indolent clinical course. The female patient presented with recurrent erythematous, non‐ulcerated, non‐raised plaques of the right lower leg for 6 years. Pathological workup and exclusion of a systemic disease confirmed the diagnosis of PCDLBCL, LT. Due to the history of repeated spontaneous remission, no therapy was initiated. Nine years after first occurrence the patient presented with complete clinical remission lasting for 64 months. We retrospectively identified four additional PCDLBCL, LT patients with spontaneous remission lasting up to 53 months. Our data provide evidence for a distinct PCDLBCL, LT patient subgroup that clinicians should be aware of and warrants a watch‐and‐wait treatment regime. [ABSTRACT FROM AUTHOR]

Abstract

Background Mycosis fungoides (MF) and Sézary syndrome (SS) are chronic malignant diseases that typically necessitate diverse strategies to achieve remission. Systemic interferon (IFN)-α (subtypes 2a and 2b) has been used to treat MF/SS since 1984; however, its production was recently stopped. The recombinant pegylated (PEG) form of IFN-α-2a remains the only alternative IFN treatment, although it has not been approved for use in MF/SS. Objectives To assess the effectiveness and safety of PEG-IFN-α-2a in monotherapy and in combination with other treatments using time to next treatment (TTNT) as a measure of clinical therapeutic benefit in a real-world setting. Methods We conducted an international, multicentre retrospective study of patients with MF and SS (of any stage) treated with PEG-IFN-α-2a from July 2012 to February 2022. Patients were included across 11 centres in 10 countries. The primary endpoints were to determine the TTNT of PEG-IFN-α-2a and adverse events (AEs) in MF/SS. Results In total, 105 patients were included [mean (SD) age 61 (13.1) years]; 42 (40.0%) had stage IA–IIA and 63 (60.0%) had stage IIB–IVB disease. PEG-IFN-α-2a was combined with other therapies in 67 (63.8%) patients, most commonly with extracorporeal photopheresis (36%) and bexarotene (22%). Patients with stage I–IIA disease achieved an overall response rate (ORR) of 57%; the ORR in those with stage IIB–IVB disease was 51%. Combination treatment resulted in a median TTNT of 10.4 months (range 0.6–50.7) vs. 7.0 months (range 0.7–52.4) for those who received monotherapy (P < 0.01). Overall, the mean (SD) TTNT was 9.2 (10.6) months and the ORR was 53.3% (n = 56). A complete response was seen in 13% of patients and a partial response in 40%. AEs were described in 68.6% (n = 72) of patients. Flu-like symptoms (n = 28; 26.7%), lymphopenia (n = 24; 22.9%) and elevated liver function (n = 10; 9.5%) were the most frequently reported. Grade 3–4 AEs were reported in 23 (21.9%) patients, mostly related to myelosuppression. Conclusions PEG-IFN-α-2a for MF/SS resulted in an ORR of 53.3% and a mean (SD) TTNT of 9.2 (10.6) months. Combination regimens were superior to monotherapy and doses of 180 µg PEG-IFN-α-2a weekly were related to a higher ORR. [ABSTRACT FROM AUTHOR]

Abstract

Simple Summary: Cutaneous T-cell lymphomas (CTCLs) constitute a group of rare lymphoproliferative malignancies primarily manifesting in the skin. This study aimed to assess a combined treatment approach utilizing mogamulizumab and extracorporeal photopheresis for a subset of patients in an advanced stage of this disease, which typically challenges therapy and carries an unfavorable prognosis. The present study aimed to understand the impact of this novel treatment combination on skin- and blood-related symptoms and its associated side effects. This retrospective study included 11 patients with Sézary syndrome (SS) or mycosis fungoides (MF). Encouragingly, three-fourths of the patients showed positive responses, with improvements in skin-related symptoms, a decrease in malignant cells in the blood, and a minimum progression-free survival of 7.2 in the skin and 7.6 months in the blood. Overall, the treatment demonstrated good tolerance. If confirmed through larger-scale studies, this combined therapy could potentially establish a new therapeutic option for patients with advanced-stage cutaneous T-cell lymphoma. Background: Primary cutaneous T-cell lymphomas (CTCLs) are rare lymphoproliferative malignancies characterized by significant morbidity and mortality in advanced disease stages. As curative approaches apart from allogeneic stem cell transplantation are lacking, establishing new treatment options, especially combination therapies, is crucial. Methods: This retrospective study included 11 patients with SS or MF receiving therapy with mogamulizumab in combination with ECP from four European expert centers. The response rates in the skin and blood as well as treatment use and adverse events (AE) were described. Results: 8/11 patients (73%) showed an overall response (OR) in the skin. The mean mSWAT decreased from 98.2 ± 40.8 to 34.6 ± 23.8. The overall response rate (ORR) in the blood was 64% with two complete responses. During combination therapy, the mean number of Sézary cells decreased from 3365.3 × 106/L before treatment to 1268.6 × 106/L. The mean minimum known period without progress was 7.2 months in the skin and 7.6 months in the blood. The most common AEs were mogamulizumab-associated rash (MAR) (45.5%), anemia (27.3%), lymphocytopenia (27.8%), and infusion related reaction (16.7%). No AE led to treatment discontinuation. Conclusions: Our study presents the combination of mogamulizumab and ECP as an effective therapy in the blood and skin in CTCL with good tolerability, similar to mogamulizumab monotherapy. Abstract

In den letzten Jahren gab es im Bereich der Therapien für primär kutane T‑Zell-Lymphome unter anderem mit der Zulassung neuer zielgerichteter Therapien zahlreiche neue Entwicklungen. Auch wenn bisher keine kurativen Therapieoptionen existieren, lassen sich mit den zur Verfügung stehenden Therapien und insbesondere deren Kombinationen oft gute Ansprechraten erzielen. Die Therapieentscheidung richtet sich stets nach der genauen Diagnose, dem Erkrankungsstadium und den bereits erfolgten Vortherapien. Dieser Beitrag gibt eine Übersicht über die aktuellen Therapieoptionen bei Mycosis fungoides und Sézary-Syndrom und orientiert sich hierbei an der kürzlich erschienenen, überarbeiteten S2k-Leitlinie zu kutanen Lymphomen (Update 2021). Zudem stellen wir vielversprechende, jedoch (noch) nicht zugelassene Therapien vor, die zumindest teilweise bereits jetzt im Rahmen eines Off-Label-Einsatzes zur Anwendung kommen können.
