Background/Objectives: Treatment of patients with myocardial ischemic diseases crucially involves cardiac reperfusion (CR). However, oxidative stress and tissue lesions caused by CR may also lead to lethal complications, such as arrythmias and vasoplegic syndrome (VS). Although methylene blue (MB) has long been used to treat VS due to cardiac ischemia and reperfusion (CIR) and/or surgery because of its vascular effects, MB's effects on the heart are unclear. Therefore, we investigated the potential cardioprotective or arrhythmogenic effects of MB in an animal model of CIR. To this end, 12–16-week-old male Wistar rats were divided into four experimental groups: (a) rats subjected to SHAM surgery with no ischemia; (b) rats subjected to CIR and treated with a vehicle (SS + CIR); and (c) rats subjected to CIR and treated with 2 mg/kg i.v. MB before ischemia (MB + ISQ) or (d) after ischemia but before reperfusion (ISQ + MB). An ECG analysis was used to evaluate the incidence of ventricular arrhythmias (VAs), atrioventricular blocks (AVBs), and lethality (LET) resulting from CIR. After CIR, rat hearts were removed for histopathological analysis and lipid hydroperoxide (LH) measurements. Results: The incidence of VA, AVB, and LET was significantly increased in the MB + ISQ group (VA = 100%; AVB = 100%; LET = 100%) but significantly reduced in the ISQ + MB group (VA = 42.8%; AVB = 28.5%; LET = 21.4%) compared with the SS + CIR group (VA = 85.7%; AVB = 71.4%; LET = 64.2%). LH concentration was significantly reduced in both MB-treated groups, but myocardial injuries were increased only in the MB + ISQ group when compared with the SS + CIR group. Conclusions: These results indicate that MB produces a biphasic effect on CIR, with cardiotoxic effects when administered before cardiac ischemia and cardioprotective effects when administered after ischemia but before cardiac reperfusion. Acute myocardial infarction (AMI) is the main cause of morbidity and mortality worldwide and is characterized by severe and fatal arrhythmias induced by cardiac ischemia/reperfusion (CIR). However, the molecular mechanisms involved in these arrhythmias are still little understood. To investigate the cardioprotective role of the cardiac Ca
2+ /cAMP/adenosine signaling pathway in AMI, L-type Ca2+ channels (LTCC) were blocked with either nifedipine (NIF) or verapamil (VER), with or without A1 -adenosine (ADO), receptors (A1 R), antagonist (DPCPX), or cAMP efflux blocker probenecid (PROB), and the incidence of ventricular arrhythmias (VA), atrioventricular block (AVB), and lethality (LET) induced by CIR in rats was evaluated. VA, AVB and LET incidences were evaluated by ECG analysis and compared between control (CIR group) and intravenously treated 5 min before CIR with NIF 1, 10, and 30 mg/kg and VER 1 mg/kg in the presence or absence of PROB 100 mg/kg or DPCPX 100 µg/kg. The serum levels of cardiac injury biomarkers total creatine kinase (CK) and CK-MB were quantified. Both NIF and VER treatment were able to attenuate cardiac arrhythmias caused by CIR; however, these antiarrhythmic effects were abolished by pretreatment with PROB and DPCPX. The total serum CK and CK-MB were similar in all groups. These results indicate that the pharmacological modulation of Ca2+ /cAMP/ADO in cardiac cells by means of attenuation of Ca2+ influx via LTCC and the activation of A1 R by endogenous ADO could be a promising therapeutic strategy to reduce the incidence of severe and fatal arrhythmias caused by AMI in humans. - Abstract
Objective: This study aimed to evaluate the in vivo protective effect of the angico gum biopolymer in reducing the inflammatory response and preserving the integrity of the laryngeal and esophageal mucosa. Study Design: Animal study. Methods: A murine surgical model of gastroesophageal reflux disease was accomplished and subsequently treated with angico gum or omeprazole. On days 3 and 7 post surgery, samples of the larynx and esophagus, respectively, were collected to measure the level of inflammation (wet weight and myeloperoxidase activity) and mucosal integrity (transepithelial electrical resistance and mucosal permeability to fluorescein). Results: Angico gum and omeprazole decreased laryngeal inflammation (wet weight and myeloperoxidase activity) and dramatically improved the integrity of the laryngeal mucosa. It also reduced inflammation (decreased wet weight and myeloperoxidase activity) of the esophagus and preserved the barrier function (inferred by assessing the integrity of the mucosa). Conclusion: This study demonstrates the protective effect of angico gum in an experimental gastroesophageal reflux disease model. Angico gum attenuates inflammation and impairment of the mucosal barrier function not only in the larynx but also in the esophagus. Gastroesophageal reflux disease (GERD) can be defined as symptoms or mucosal damage produced by the abnormal reflux of gastric contents into the esophagus. Alternative ways of treatment must be considered since the standard therapy fails in all phenotypes of GERD, expressly in the most prevalent form (Non-Erosive Reflux Disease, NERD). We aimed to assess the esophageal mucosal protection effect of the biopolymer cashew gum (CG), a natural product obtained from bark of Brazilian northeastern tree (Anacardium occidentale L.), in mice (experimental model of NERD, non-erosive reflux disease) and in human esophageal biopsies. Experimental design was divided in “Pré-Clinical Studies: Murine NERD” and in “Human Oesophageal Biopsies Studies”. Firstly, mice were achieved by a surgical induction of NERD in which a pyloric sub-stenosis and ligation of forestomach were performed. After 7 days, animals were euthanized and samples of distal oesophagus taken to assess suggestive hallmarks of inflammation such as oedema, MPO activity and neutrophil infiltrate [histologic scores and keratinocyte-derived cytokine (KC)]. In functional studies, distal oesophageal samples of mice were mounted in Üssing chambers to evaluate Transepithelial Electrical Resistance (TER) and permeability to fluorescein. In human esophageal biopsies, it was evaluated TER and permeabiliy, but with pH adjustment to 2. As control in biopsies studies, alginate was used, besides sucralfate also was evaluated. In addition, an acute toxicity test was conducted in accordance with OECD guideline 423. Finally, we performed an epifluorescence microscopy to localize on the oesopageal epithelium the biopolymer previously labeled with FITC. Objective This study aimed to investigate the protective effect of epiisopiloturine hydrochloride (EPI), an imidazole alkaloid, on NAP-induced gastrointestinal damage in rats. Methods Initially, rats were pretreated with 0.5% carboxymethylcellulose (vehicle) or EPI (3, 10 and 30 mg/kg, p.o. or i.p., groups 3–5, respectively) twice daily, for 2 days. After 1 h, NAP (80 mg/kg, p.o.) was given. The control group received only vehicle (group 1) or vehicle + naproxen (group 2). Rats were euthanized on 2nd day, 4 h after NAP treatment. Stomachs lesions were measured. Samples were collected for histological evaluation and glutathione (GSH), malonyldialdehyde (MDA), myeloperoxidase (MPO), and cytokines levels. Moreover, gastric mucosal blood flow (GMBF) was evaluated. Results EPI pretreatment prevented NAP-induced macro and microscopic gastric damage with a maximal effect at 10 mg/kg. Histological analysis revealed that EPI decreased scores of damage caused by NAP. EPI reduced MPO (3.4 ± 0.3 U/mg of gastric tissue) and inhibited changes in MDA (70.4 ± 8.3 mg/g of gastric tissue) and GSH (246.2 ± 26.4 mg/g of gastric tissue). NAP increased TNF-α levels, and this effect was reduced by EPI pretreatment. Furthermore, EPI increased GMBF by 15% compared with the control group. Conclusion Our data show that EPI protects against NAP-induced gastric and intestinal damage by reducing pro-inflammatory cytokines, reducing oxidative stress, and increasing GMBF.

Background Sideroxylon obtusifolium (Roem. & Schult.) T.D. Penn., Sapotaceae family, is a medicinal species native to the Brazilian Northeastern region. The plant is popularly used as an anti-inflammatory and hypoglycemic. Purpose To evaluate the anti-inflammatory properties of the N-methyl-(2S,4R)-trans-4-hydroxy-l-proline (NMP) from S. obtusifolium leaves in models of inflammation and to clarify its action mechanisms. Methods Male Swiss mice were distributed intocontrols and groups treated with NMP (25, 50 and 100 mg/kg, p.o.), indomethacin or morphine (reference drugs). The animals were subjected to the formalin, carrageenan-induced edema and peritonitis tests. Furthermore, peritoneal lavage and slices from edematous paws were used for histological and immunohistochemical (iNOS, TNF-alpha, COX-2 and NF-kB) assays. Results Decreases in licking time, in the 1st and mainly in the 2nd phases of the formalin test, were shown after NMP treatments. In addition, decreases (around 50%) in paw edema were noticed at the 3rd h. The HE staining of paw slices demonstrated a complete reversion of the increased PMN cell numberafter NMP treatment. Similarly, decreases higher than 70% were also demonstrated in PMN cells, in the peritoneal fluid. Furthermore, NMP significantly decreased iNOS, TNF-alpha, COX-2 and NF-kB immunoreactivities. Conclusions We showed that S. obtusifolium presents a potent anti-inflammatory activity, due to the presence of the N-methyl-(2S,4R)-trans-4-hydroxy-l-proline(NMP) in the plant extract. This action is related to the inhibition by NMP of TNF-alpha and inflammatory enzymes.

Our objective was to investigate the protective effect of Lawesson’s reagent, an H2S donor, against alendronate (ALD)-induced gastric damage in rats. Rats were pretreated with saline or Lawesson’s reagent (3, 9, or 27 mmol/kg, po) once daily for 4 days. After 30 min, gastric damage was induced by ALD (30 mg/kg) administration by gavage. On the last day of treatment, the animals were killed 4 h after ALD administration. Gastric lesions were measured using a computer planimetry program, and gastric corpus pieces were assayed for malondialdehyde (MDA), glutathione (GSH), proinflammatory cytokines [tumor necrosis factor (TNF)-a and interleukin (IL)-1b], and myeloperoxidase (MPO). Other groups were pretreated with glibenclamide (5 mg/kg, ip) or with glibenclamide (5 mg/kg, ip)+diazoxide (3 mg/kg, ip). After 1 h, 27 mmol/kg Lawesson’s reagent was administered. After 30 min, 30 mg/kg ALD was administered. ALD caused gastric damage (63.35±9.8 mm2); increased levels of TNF-a, IL-1b, and MDA (2311±302.3 pg/mL, 901.9±106.2 pg/mL, 121.1±4.3 nmol/g, respectively); increased MPO activity (26.1±3.8 U/mg); and reduced GSH levels (180.3±21.9 mg/g). ALD also increased cystathionine-c-lyase immunoreactivity in the gastric mucosa. Pretreatment with Lawesson’s reagent (27 mmol/kg) attenuated ALD-mediated gastric damage (15.77±5.3 mm2); reduced TNFa, IL-1b, and MDA formation (1502±150.2 pg/mL, 632.3±43.4 pg/mL, 78.4±7.6 nmol/g, respectively); lowered MPO activity (11.7±2.8 U/mg); and increased the level of GSH in the gastric tissue (397.9±40.2 mg/g). Glibenclamide alone reversed the gastric protective effect of Lawesson’s reagent. However, glibenclamide plus diazoxide did not alter the effects of Lawesson’s reagent. Our results suggest that Lawesson’s reagent plays a protective role against ALD-induced gastric damage through mechanisms that depend at least in part on activation of ATP-sensitive potassium (KATP) channels.

Many algal species contain relatively high concentrations of polysaccharide substances, a number of which have been shown to have anti-inflammatory and/or immunomodulatory activity. In this study, we evaluated the anti-inflammatory and antinociceptive effects in mice of a sulfated polysaccharide fraction (PLS) extracted from the algae Gracilaria caudata. The antiinflammatory activity of PLS was evaluated using several inflammatory agents (carrageenan, dextran, bradykinin, and histamine) to induce paw edema and peritonitis in Swiss mice. Samples of the paw tissue and peritoneal fluid were removed to determine myeloperoxidase (MPO) activity or TNF-α and IL-1β levels, respectively. Mechanical hypernociception was induced by subcutaneous injection of carrageenan into the plantar surface of the paw. Pretreatment of mice by intraperitoneal administration of PLS (2.5, 5, and 10 mg/kg) significantly and dose-dependently reduced carrageenan-induced paw edema ( p < 0.05) compared to vehicle-treated mice. Similarly, PLS 10 mg/kg effectively inhibited edema induced by dextran and histamine; however, edema induced by bradykinin was unaffected by PLS. PLS 10 mg/kg inhibited total and differential peritoneal leukocyte counts following carrageenan-induced peritonitis. Furthermore, PLS reduced carrageenan-increased MPO activity in paws and reduced cytokine levels in the peritoneal cavity. Finally PLS pretreatment also reduced hypernociception 3-4 h after carrageenan. We conclude that PLS reduces the inflammatory response and hypernociception in mice by reducing neutrophil migration and cytokines concentration. [ABSTRACT FROM AUTHOR]

Omeprazole, the first proton pump inhibitors developed, is still widely used in clinical practice and has its widespread use in the population. Despite its great efficacy and considerable safety profile, recently clinical studies have been linking its chronic use to central effects. Currently, the increase in the occurrence of neuropsychiatric disorders is a global concern, and it is essential to identify preventable factors, such as adverse effects to the use of medications. This study aimed at identifying the chronic administration effects of Omeprazole in the CNS of adult mice in a dose equivalent to that used in the clinic, investigating alterations in behavioral, inflammatory and oxidative parameters. For this, C57BL/6 adult male mice, weighing between 20-30g, were used and divided into two groups: OME and Sham, each received either solutions of Omeprazole (8 mg/kg) or vehicle (sodium bicarbonate pH 8.4) respectively, orally by gavage for 28 days. 24 hours after the last administration, the animals were submitted to the following behavioral tests: Open Field (OF) and Rotarod, Forced Swimming Test (FST), elevated plus maze, Y maze (YM), passive avoidance, object recognition (OR) and pre-pulse inhibition. The weight of the animals was registered in 48-72h intervals during the protocol. Moreover, the following brain areas - hippocampus (HP), prefrontal cortex (PFC) and striatum - were dissected, weighted, and the serum was collected. Oxidative stress parameters, malondialdehyde (MDA), nitrite/nitrate and reduced glutathione (GSH), were quantified in brain areas. The inflammatory markers, IL-1β and TNF-α, were measured in peripheral blood and in HP and PFC. The project was approved by the Ethics Committee on the Use of Animals of the Federal University of Ceará filed under no. 1977300718. The following results were observed: a decrease in the crossing parameter in the OF, no change in the motor performance verified by the Rotarod test, a reduction in the immobility time in the FST, an improvement in the percentage of correct alternances in the YM and a reduced in the exploration time in the novel object in the OR. Furthermore, a reduced weight gain and hippocampal weight were observed in Omeprazole-treated mice. Regarding the inflammatory and oxidative stress parameters, there was an increase in the cytokine IL1-β levels in the PFC and serum, whereas TNF-α only in the PFC. Nitrate/nitrite levels increased in HP and PFC, while MDA and GSH levels decreased. These findings suggest that Omeprazole improves depressive-like behavior and working memory, likely through the increase in Nitrate/Nitrite and reduction in MDA levels in PFC and HP. Whereas, the finding regarding the impairment of the recognition memory is more likely to be related to the reduced hippocampal weight. The diminished weight gain might be associated with the IL-1β increased levels in the peripheral blood. Lastly, the results of the present study alongside with the evidence already published, reinforces the cautions and attention concerning the long-term use of Omeprazole should be considered as well as the necessity of doing more studies to better understand the underlying mechanisms behind these effects. O Omeprazol, primeiro representante da classe dos inibidores da bomba de prótons, é ainda o mais utilizado na prática clínica e tem seu uso bastante difundido na população. Apesar de sua ótima eficácia e considerável perfil de segurança, estudos de natureza clínica têm relacionado seu uso crônico a efeitos centrais. O aumento da ocorrência de transtornos neuropsiquiátricos é uma preocupação global, sendo fundamental a identificação de fatores evitáveis, como efeitos adversos ao uso de medicamentos. O objetivo deste trabalho foi, então, compreender os efeitos da administração crônica de Omeprazol no sistema nervoso central de camundongos em dose equivalente à utilizada na clínica, investigando alterações comportamentais, inflamatórias e oxidativas. Para isto, camundongos C57BL/6 adultos machos, com peso entre 20-30 g foram utilizados e divididos em dois grupos: OME e Sham, os quais receberam as soluções de Omeprazol (8 mg/kg) e do veículo (bicarbonato de sódio pH 8.4), respectivamente, por gavagem, via oral, durante 28 dias. 24h após a última administração, os animais foram submetidos aos testes comportamentais: Campo Aberto (CA), Rotarod, Nado Forçado (NF), Labirinto em Cruz Elevado, Inibição Pré-Pulso, Esquiva Passiva, Labirinto em Y (LY) e Reconhecimento de Objetos (RO). Ademais, foi registrado o peso dos animais com intervalos de 48-72h durante o protocolo. Após a eutanásia, as áreas do hipocampo (HP), córtex pré-frontal (CPF) e corpo estriado (CE) foram dissecadas, pesadas, e o soro, coletado. Os parâmetros do estresse oxidativo, malondialdeído (MDA), nitrato/nitrito e glutationa reduzida (GSH) foram dosados nas áreas cerebrais. Os marcadores inflamatórios, IL-1β e TNF-α, foram dosados no soro, HP e CPF. O estudo foi aprovado no Comitê de Ética no Uso de Animais da Universidade Federal do Ceará, protocolado sob o nº 1977300718. Os resultados mostram diminuição do parâmetro de crossing no CA, sem nenhuma alteração no desempenho motor no Rotarod, redução do tempo de imobilidade no NF, aumento no número de alternâncias corretas no LY e redução do tempo de exploração ao objeto novo no RO. Houve redução do peso do HP e do ganho de peso corporal no grupo OME. A avaliação neuroquímica revelou aumento dos níveis de IL-1β no CPF e no soro, e de TNF-α apenas no CPF. Os níveis de nitrato/nitrito aumentaram no HP e CPF, enquanto os de MDA e GSH diminuíram em todas as áreas cerebrais. Diante dos resultados, sugere-se que Omeprazol melhorou o comportamento tipo depressivo e a memória de trabalho por meio do aumento dos níveis de nitrato/nitrito e redução de MDA no CPF e HP. Em relação ao prejuízo na memória de reconhecimento pode estar associado à diminuição do peso do hipocampo. A redução do ganho de peso corporal pode estar relacionada ao aumento dos níveis de IL-1β no sangue periférico. A partir dos resultados encontrados junto às evidências já publicadas, reforça-se a importância de maior cautela no uso crônico de Omeprazol e a necessidade de realizar mais estudos visando à melhor compreensão dos mecanismos por trás dos efeitos desse fármaco.

A doenca do refluxo gastroesofagico (DRGE) tem uma alta prevalencia mundial e impacto na qualidade de vida, assim podendo ter complicacoes esofagicas e extraexofagicas. Alem disso, a terapia padrao com IBPs e o tratamento de escolha, cerca de um terco dos pacientes nao responde adequadamente ao tratamento, principalmente aqueles com doenca nao erosiva. O presente estudo teve por objetivo avaliar o efeito protetor na mucosa laringea e esofagica, do biopolimero da Goma do Angico (GA), obtido da casca da Anadenanthera colubrina, em um modelo de refluxo gastroesofagico em camundongos. Inicialmente, os camundongos foram submetidos a uma inducao cirurgica do refluxo gastroesofagico, na qual realizou-se uma sub-estenose pilorica seguida de ligadura de fundo gastrico. Durante 3 dias (laringe) e 7 dias (esofago), os animais foram tratados com salina ou GA diariamente em diversas concentracoes. Entao nos respectivos dias, os animais foram eutanasiados e amostras de laringe e esofago distal foram retiradas para avaliar parametros inflamatorios, edema e atividade da MPO. No estudo da funcao de barreira, amostras de laringe e esofago distal foram montadas em camaras Ussing para avaliar a Resistencia Eletrica Transepitelial (RETE) e, em seguida, a permeabilidade a fluoresceina. - Abstract
Purpose: Shock, cardiovascular problems, and respiratory failure constitute the main causes of death in patients cared in medical emergency rooms. Patients commonly require orotracheal intubation (OTI), a fact that has been intensified by diseases that generate important and fatal hemodynamic and respiratory problems in the affected patient., Methods: Although etomidate (ETO) is a highly used anesthetic for OTI, its use remains controversial in several scenarios. Some studies refer to an increase in mortality with its use in critically patients, while others do not refer to a difference. Therefore, we evaluated the mortality of patients submitted to OTI in the public hospital of a public federal university, with the use of ETO and other sedative-hypnotic drugs used in the induction of the performance of OTI, with the in-hospital mortality of patients cared in hospital., Results: The results demonstrate that the use of ETO as a hypnotic for OTI in the emergency room is not associated with a significant difference in morbidity or early mortality, within 30 days of hospitalization, compared with other hypnotics., Conclusions: There was no difference in mortality between patients intubated in the emergency department who used ETO and those who used non-ETO hypnotic within 72 hours and 30 days.
