This chapter delivers an overview of child protection services in Australia and New Zealand. Both nations’ main concern primarily revolves around numerous reports of children at risk and the overrepresentation of Aboriginal and Torres Strait Islander (ATSI) children in Australia and Māori children in New Zealand. In Australia, child protection legislation, policy and practice processes are mostly similar across states and territories and differ in terms of reporting, investigation, and interventions for a child in either risk or need of protection and care. On the other hand, New Zealand’s whangai system was eroded due to state intervention to protect children and westernized laws governing adoption and welfare. The over-representation of Indigenous children living away from their whanau sparked major reform of the country’s child protection system.

BACKGROUND & AIMS: The Primary Biliary Cholangitis (PBC) Obeticholic Acid (OCA) International Study of Efficacy (POISE) randomized, double-blind, placebo-controlled trial demonstrated that OCA reduced biomarkers associated with adverse clinical outcomes (ie, alkaline phosphatase, bilirubin, aspartate aminotransferase, and alanine aminotransferase) in patients with PBC. The objective of this study was to evaluate time to first occurrence of liver transplantation or death in patients with OCA in the POISE trial and open-label extension vs comparable non-OCA-treated external controls. METHODS: Propensity scores were generated for external control patients meeting POISE eligibility criteria from 2 registry studies (Global PBC and UK-PBC) using an index date selected randomly between the first and last date (inclusive) on which eligibility criteria were met. Cox proportional hazards models weighted by inverse probability of treatment assessed time to death or liver transplantation. Additional analyses (Global PBC only) added hepatic decompensation to the composite end point and assessed efficacy in patients with or without cirrhosis. RESULTS: During the 6-year follow-up, there were 5 deaths or liver transplantations in 209 subjects in the POISE cohort (2.4%), 135 of 1381 patients in the Global PBC control (10.0%), and 281 of 2135 patients in the UK-PBC control (13.2%). The hazard ratios (HRs) for the primary outcome were 0.29 (95% CI, 0.10-0.83) for POISE vs Global PBC and 0.30 (95% CI, 0.12-0.75) for POISE vs UK-PBC. In the Global PBC study, HR was 0.20 (95% CI, 0.03-1.22) for patients with cirrhosis and 0.31 (95% CI, 0.09-1.04) for those without cirrhosis; HR was 0.42 (95% CI, 0.21-0.85) including hepatic decompensation. Background Annually in England, over 1.5 million children and young people (CYP) are admitted to hospital. However, a proportion of these CYP will experience failure to rescue (FtR), a failure to recognize, respond and escalate clinical deterioration, which can result in significant harm or death. Aim To identify and quantify FtR episodes from emergency events at a 110-bedded tertiary children's hospital located within a University Teaching Hospital and evaluate the impact of targeted interventions on reducing FtR. Methods A quality improvement approach was adopted. From 170 446 patients admitted between 2011 and 2019, all emergency event calls were systematically reviewed to identify FtR episodes. Root-cause analysis was performed to identify practice deficiencies. The Plan-Do-Study-Act fundamentals were used. Results A total of 520 emergency events were reviewed over the 9-year period. One hundred and thirty-two (n = 132; 25%) were cardiac arrest events, with the majority occurring within the PCCU setting. Three hundred and twelve (60%) of the events were in children who had been inpatient for more than 48 hours. FtR trend declined over the study period from 23.6% in 2011 when the project commenced to 2.5% or less over the following 8 years. Conclusions Identifying rates of FtR events from routinely collected emergency events data can be used as a patient safety measure to identify emergency concerns. This enables dynamic problem solving through delivery of strategic and targeted interventions. The proposed interventions outlined in this quality improvement study have application to critical care nursing as mechanisms for reducing unplanned admissions to paediatric critical care unit (PCCU), patient mortality, and PCCU and non-PCCU cardiac arrests.

Anaesthesia combined with functional neuroimaging provides a powerful approach for understanding the brain mechanisms that change as consciousness fades. Although propofol is used ubiquitously in clinical interventions that reversibly suppress consciousness, its effect varies substantially between individuals, and the brain bases of this variability remain poorly understood. We asked whether three networks that are primary sites of propofol-induced sedation and key to conscious cognition — the dorsal attention (DAN), executive control (ECN), and default mode (DMN) network — underlie responsiveness variability under anaesthesia. Healthy participants (N=17) underwent propofol sedation inside the fMRI scanner at dosages of ‘moderate’ anaesthesia, and behavioural responsiveness was measured with a target detection task. To assess information processing, participants were scanned during an active engagement condition comprised of a suspenseful auditory narrative, in addition to the resting state. A behavioural investigation in a second group of non-anesthetized participants (N=25) qualified the attention demands of narrative understanding, which we then related to the brain activity of participants who underwent sedation. 30% of participants showed no delay in reaction times relative to wakefulness, whereas the others, showed significantly delayed and fragmented responses, or full omission of responses. These responsiveness differences did not relate to information processing differences. Rather, only the functional connectivity within the ECN during wakefulness differentiated the participants’ responsiveness level, with significantly stronger connectivity in the fast relative to slow responders. Consistent with this finding, fast responders had significantly higher grey matter volume in the frontal cortex aspect of the ECN. For the first time, these results show that responsiveness variability during propofol anaesthesia relates to inherent differences in brain function and structure within the executive control network, which can be predicted prior to sedation. These results shed light on the brain bases of responsiveness differences and highlight novel markers that may help to improve the accuracy of awareness monitoring during clinical anaesthesia.

Although ‘child safety’ is now a national policy priority in Australia, there is little research exploring the practices in schools that contribute to children and young people’s felt sense of safety and wellbeing. Drawing on a mixed-method Australian Research Council (ARC) Discovery project, this article presents findings from interviews with school staff (N = 10), leaders (N = 5) and nine focus groups with students (N = 58), in primary and secondary schools in three Australian states (New South Wales, Victoria and South Australia). We employ relational ethics, recognition theory and the theory of practice architectures to explore practices at school that support student wellbeing and safety. The findings contribute significantly to understanding the ‘bundled’ nature of current practices and the conditions that enable and constrain these. Close attention to these findings is critical as schools seek to operationalise the National Child Safe Principles and refine ongoing safeguarding procedures. The findings have informed the development of an online survey that is currently testing, on a much larger scale, which elements of ethical practice are most positively associated with students’ safety, wellbeing and recognition at school.

Failure to recognise deterioration early which results in patient death, is considered failure to rescue and it is identified as one of the leading causes of harm to patients. It is recognised that patients and their families can often recognise changes within the child's condition before healthcare professionals. To mitigate the risk of failure to rescue and promote early intervention, family-activated rapid response systems are becoming widely acknowledged and accepted as part of family integrated care.To identify current family-activated rapid response interventions in hospitalised paediatric patients and understand mechanisms by which family activation works.A narrative systematic review of published studies was conducted. Seven online databases; AMED, CINHAL, EMBASE, EMCARE, HMIC, JBI, and Medline were searched for potentially relevant papers. The critical appraisal skills programme tool was used to assess methodological rigor and validity of included studies.Six studies met the predefined inclusion criteria. Five telephone family activation interventions were identified; Call for Help, medical emergency-teams, Condition HELP, rapid response teams, and family initiated rapid response. Principles underpinning all interventions were founded on a principal of granting families access to a process to escalate concerns to hospital emergency teams. Identified interventions outcomes and mechanisms include; patient safety, empowerment of families, partnership working/ family centred care, effective communication and better patient outcomes. Interventions lacked multi-lingual options.Family activation rapid response system are fundamental to family integrated care and enhancing patient safety. Underlying principles and concepts in delivering interventions are transferable across global healthcare system.

Orientation: Disconnected scholarly work on the theoretical and empirical structure of individual work performance negatively impacts predictive studies in human resource management. Greater standardisation in the conceptualisation and measurement of performance is required to enhance the scientific rigour with which research is conducted in human resource management in South Africa.Research purpose: The present study aimed to conceptualise and empirically validate the structural validity of five broad generic dimensions of individual work performance, based on 20 narrow dimensions of performance.Motivation for the study: A generic model and standardised measurement of individual work performance, measuring performance at the appropriate level of breadth and depth, may help human resource professionals to make accurate decisions about important work-based criteria and their related predictors. A validated generic model of performance could further increase the replicability of science around performance measurement in South Africa.Research approach/design and method: A cross-sectional design was implemented by asking 448 managers across several organisations to rate the performance of their subordinates on the Individual Work Performance Review (IWPR). The quantitative data were analysed by means of hierarchical confirmatory factor analyses.Main findings: An inspection of the discriminant validity of the 20 narrow performance dimensions supported the multidimensionality of performance to a fair degree. The bifactor statistical indices, in turn, suggested that the five broad factors explained a significant amount of common variance amongst the manifest variables and could therefore be interpreted as more unidimensional.Practical/managerial implications: Practitioners can interpret the broader performance dimensions in the IWPR as total scores, especially when high-stakes decisions are made about promoting or rewarding employees. The interpretation of the narrow performance dimensions might be more useful in low-stakes development situations. Cross-scale interpretations are encouraged to enable a holistic understanding of employees’ performance, as the narrow performance dimensions covary.

The importance of psychological support for orthognathic patients has taken an increasing precedence over recent years and is embedded in orthognathic commissioning guidelines. Furthermore, attention towards mental health-related conditions and their management is of prime importance and continues to be a key area of focus within healthcare settings. With this in mind, this paper aims to outline our experience of establishing a need for and subsequently securing funding to establish a clinical psychology service within an existing orthognathic service in the NHS. The information outlined may be of benefit to orthognathic teams seeking to secure such psychological support within their respective units.

Child safety is now a national policy priority in Australia. Extensive enquiries and reviews have escalated legislative and policy responses focused on developing, maintaining and monitoring “child safe” organisations. The recommendations of the Royal Commission into Institutional Responses to Child Sexual Abuse point to the importance of cultural conditions within organisations in supporting child safety and the need for responsive change in some organisations. Drawing on a recent policy analysis, undertaken as part of a larger Australian Research Council Discovery Project, this article examines how children and safety are constructed, within and across relevant state and federal government policies in Australia, and the implications of this. Distinctions are drawn between conceptualisations of children within the broader education policy context and two specific policy contexts in which children are considered particularly vulnerable to abuse – out‐of‐home care and disability. The findings indicate that policy discourses of “child safe” potentially foster different emphases and approaches in organisations. These have implications for the way children are positioned in relation to their safety, how their rights are recognised and implemented, and what is required to foster cultural conditions within organisations to best support children’s safety and wellbeing. A growing number of studies show that a significant proportion of patients, who meet the clinical criteria for the diagnosis of the vegetative state (VS), demonstrate evidence of covert awareness through successful performance of neuroimaging tasks. Despite these important advances, the day-to-day life experiences of any such patient remain unknown. This presents a major challenge for optimizing the patient’s standard of care and quality of life (QoL). We describe a patient who, following emergence from a state of complete behavioral unresponsiveness and a clinical diagnosis of VS, reported rich memories of his experience during this time. This case demonstrates the potential for a sophisticated mental life enabled by preserved memory in a proportion of patients who, similarly, are thought to be unconscious. Therefore, it presents an important opportunity to examine the implications for patient QoL and standard of care, both during the period of presumed unconsciousness and after recovery.

Keeping children safe from harm is a national policy priority in Australia. Extensive inquiries and reviews have highlighted institutions' persistent failures to respond ethically and appropriately to child abuse and its life‐long impacts on survivors. Policy efforts now reflect considerable emphasis on safeguarding children, including through the development of ‘child safe’ organisations. The realisation of these policy aspirations requires close attention to how ‘child safe’ is conceptualised and operationalised in different organisational contexts. Drawing on an analysis of policy in Australia and other international jurisdictions, namely New Zealand, the United Kingdom and Ireland, this article engages critically with the notion of ‘child safe’ in policy, to explore who is to be safe, from what and how, in organisational settings. The findings suggest emergent discourses of ‘child safe’ are bound up with particular constructions of both children and safety, reflect current social and political understandings and agendas, and have implications for organisations' approaches to safety. Ensuring the safety of children in Australia and other jurisdictions requires continuing scrutiny of policy implementation to make sure current policy efforts are not reduced to compliance‐based imperatives that protect organisations, but fail to create the cultural conditions that enhance children's wellbeing and safety. Refereed/Peer-reviewed In recent years there has been increased focus on supporting the safety and wellbeing of children and young people with disability. This paper reports on a study that asked children and young people with disability and adults who work with them about practices that support their wellbeing and safety, including barriers and enablers to ethical practice. We used the theory of practice architectures to unpack the practices. Findings point to a range of practices that both young people and adults regarded as important in creating the cultural conditions for young people to feel happy, safe and well, albeit placing different emphases on particular practices. Several tensions within and between these practices and the evolving requirements of disability and safeguarding policy environments were also apparent. While often not well articulated, the inherent tensions have implications when considering what children and young people need, particularly those with higher support needs. Findings suggest closer attention needs to be given to the intersection between individual and systemic factors in shaping ethical practice aimed at supporting the wellbeing and safety of children and young people with disability.

Objectives Characterise telehealth use in MS clinics during the COVID–19 pandemic. Assess patient and clinician attitudes towards telehealth. Compare telehealth–based and physical EDSS obtained during period of telehealth implementation. Methods Clinic records from Mar-Dec 2020 were reviewed. Patients and clinicians completed questionnaires about experiences using Telehealth. The iMed database was searched for EDSS recorded via face-to-face and telehealth appointments during and compared to face-to-face EDSS preceding and following the study period. T-test and Chi-square test were used for between-group comparisons. Results 2023 appointments (27% face-to-face, 35% video, 37% telephone) were conducted. New referrals were predominantly face-to-face (66%). 89% of patients were satisfied with telehealth. 58% felt they were as good as face-to-face visits, whilst only 11% of clinicians agreed. Many patients favoured a hybrid model. Safety during the COVID-19 pandemic was important to both groups. EDSS increase from the preceding visit was recorded in a significantly higher proportion of face-to-face than telehealth appointments (p=0.027), with the increase driven by patients with baseline EDSS≤4.0. Amongst patients with EDSS increases, similar numbers of suspected relapses were seen via both modalities. Absolute increase in EDSS was also significantly greater amongst patients seen face-to-face (p Conclusion Patient satisfaction with telehealth was high, whilst clinicians preferred face-to-face consultations. EDSS increase was more frequently recorded via face-to-face than telehealth appointments, which may underestimate lower EDSS. Future clinics could combine both modalities.

Objective To examine factors determining risk of self-reported infections and antimicrobial use in patients receiving Ocrelizumab for MS. Methods Retrospective, observational cohort study conducted in Ocrelizumab-treated patients at the Royal Melbourne Hospital. The association of clinical and laboratory factors with self-reported infection rate and antimicrobial use were estimated using univariate and multivariable logistic regression models. Results 185 patients were included in the study, and 176 infections were reported in 89 patients (46.1%), and in 47 patients (25.3%) antimicrobial use was identified. In univariate analyses, a higher serum IgA was associated with reduced odds of infection (OR 0.44, 95% CI 0.25 - 0.76). In multivariable analyses, older age (OR 0.94, 95% CI 0.88 - 0.99), higher serum IgA (OR 0.37, 95% CI 0.17 - 0.80) and higher serum IgG (OR 0.81, 95% CI 0.67 - 0.99) were associated with reduced odds of infection. Older age (OR 0.85, 95% CI 0.75 - 0.96) and higher serum IgA (OR 0.23, 95% CI 0.07 - 0.79) were associated with reduced odds of antimicrobial use, whilst longer MS disease duration (OR 1.22, 95% CI 1.06 - 1.41) and higher EDSS (OR 1.99, 95% CI 1.02 - 3.86) were associated with increased odds of antimicrobial use. Conclusions Higher serum IgA, IgG and older age were associated with reduced odds of infection. Our findings highlight non-uniformity of infection risk in Ocrelizumab-treated MS patients, and substantiate the need to monitor immunoglobulin levels pre-treatment and whilst on therapy.

Purpose: The fatigue resistance of the finger flexors is known to be a key determinant of climbing performance. This study set out to establish the association between the single all-out assessment of finger flexor critical force (ff-CF) and the impulse above CF (W') on climbing performance (self-reported sport and boulder climbing ability). Methods: A total of 129 subjects completed an assessment of dominant arm ff-CF, comprised of a series of rhythmic isometric maximum voluntary contractions (CF defined as mean end-test force [in kilograms]; W' impulse above CF [in kilogram second]). Results: The ff-CF protocol resulted in the same force decay to a plateau seen in previous isometric critical torque and critical force tests. Linear regression analysis, adjusting for sex, revealed that CF percentage of body mass explained 61% of sport and 26% of bouldering performance and W' per kilogram body mass explained 7% sport and 34% bouldering performance. A combined model of CF as a percentage of body mass and W' per kilogram body mass, after adjustment for sex differences, was able to explain 66% of sport climbing and 44% of bouldering performance. Conclusions: The results illustrate the relevance of the CF threshold in describing the fatigue resistance of the finger flexors of rock climbers. Given ff-CF ability to describe a considerable proportion of variance in sport climbing and bouldering ability, the authors expect it to become a common test used by coaches for understanding exercise tolerance and for determining optimal training prescription.

The use of do-it-yourself artificial pancreas systems (DIYAPS) among people with type 1 diabetes is increasing. At present, it is unclear how DIYAPS compares with other technologies such as FreeStyle Libre (FSL) and continuous subcutaneous insulin infusion (CSII). The aim of this analysis is to compare safety, effectiveness and quality-of-life outcomes of DIYAPS use with the addition of FSL to CSII.Data from two large UK hospitals were extracted from the Association of British Clinical Diabetologists (ABCD) DIYAPS and FSL audits. Background Ocrelizumab safety outcomes have been well evaluated in clinical trials and open-label extension (OLE) studies. However, risk factors for infection in patients with multiple sclerosis (MS) receiving ocrelizumab have not been extensively studied in the real-world setting. Objective The aim of this study was to examine factors determining risk of self-reported infections and antimicrobial use in patients receiving ocrelizumab for MS. Methods A retrospective, observational cohort study was conducted in patients receiving ocrelizumab at the Royal Melbourne Hospital. Infection type and number were reported by patients, and the associations of potential clinical and laboratory risk factors with self-reported infection and antimicrobial use were estimated using univariate and multivariable logistic regression models. Results A total of 185 patients were included in the study; a total of 176 infections were reported in 89 patients (46.1%), and antimicrobial use was identified in 47 patients (25.3%). In univariate analyses, a higher serum IgA was associated with reduced odds of infection (OR 0.44, 95% CI 0.25–0.76). Highlights • Covid-19 pandemic can have implications for MS and neuroimmunology patients and potentially their adherence to disease modifying therapies (DMTs). • A large proportion of patients receiving DMT's for their MS expressed some degree of concern regarding the COVID-19 pandemic and their therapy in one MS centre in Australia. • The level of concern in most patients was at most mild however. • Patients ascribed similar concern to the risk of a relapse of their disease compared to the risk of contracting COVID-19, perhaps underscoring a willingness to continue their DMT despite the pandemic., Objective: Patients with Multiple Sclerosis (MS) and on disease modifying therapies (DMTs) that can be immunosuppressive or immunomodulatory form a special group where risk of continuation of DMT needs to be taken into account with risk of contracting Covid-19. This concept can pose a degree of anxiety for patients as well as neurologists. We aimed to evaluate patient perspectives regarding the use of Natalizumab and anti-CD20 therapies (Rituximab and Ocrelizumab) in the context of the COVID-19 pandemic. Methods: cross-sectional study conducted via voluntary survey filled in by patients with MS and related disorders receiving their infusional treatment in one MS centre in Australia, exploring their concerns regarding their therapy, their therapy and COVID-19, precautions undertaken in response to the pandemic, and factors impacting their decision-making. Results: 170 patients completed the survey. Of patients on Natalizumab, the majority had either no or mild concern regarding their DMT and COVID-19, and of patients on B-cell depleting therapies, again, the majority had no or mild concern, though a slightly higher proportion had a moderate level of concern. Asked to delineate their concerns, an increased risk of contracting COVID-19 was more commonly conveyed than MS-specific factors or poor outcomes pertaining to COVID-19 if contracted, by patients in both groups. Conversely, being invited to specifically consider the possibility of contracting COVID-19 or experience a relapse of MS, almost half of the cohort rated both of equal of concern. More than half of the cohort were self-isolating more stringently than general government advice and government-related resources followed by information provided by patient's neurologist where the commonest means of information to guide decision making. Conclusions: Whilst a large proportion of patients had some concern regarding the impact of their DMT on COVID-19, whether on their risk of contracting COVID-19 or a theoretical risk for more severe disease, the overall level of concern in most cases was at most mild. Patients on B-cell depleting therapies were more inclined to express a higher level of concern. A similar concern was ascribed to a risk of a relapse or worsening MS symptoms compared to the risk of contracting COVID-19. Such attitudes may underscore a willingness of patients to continue their DMT where benefits outweigh risks during future phases of the COVID-19 pandemic.

Increasing the numbers of Pacific Island students who graduate from New Zealand universities is important for the students’ futures, their families, communities and society in general. In this study, Pacific graduates ( N = 365) who are participants in the Graduate Longitudinal Study New Zealand were asked to describe what factors helped or hindered their qualification completion. Graduates from all eight New Zealand universities were surveyed between July and December 2011. Pacific graduates identified external (e.g. family), institutional (e.g. university staff) and student or personal (e.g. motivation) factors that both helped and hindered the successful completion of their studies. Understanding the hindrance factors faced by Pacific students and the factors that have contributed to their educational success can help to inform policy and practice to achieve national priorities of equity and successful outcomes for Pacific learners.

This article considers children’s right to participate in the context of private law disputes concerning their post-separation, day-to-day care and contact arrangements. In New Zealand the approach to ascertaining children’s views has been both long-standing and systematic for contested proceedings within the Family Court (via children’s legal representatives and judicial meetings with children). However, major reform of the family justice system in 2014 shifted the emphasis to new out-of-court processes for resolving post-separation parenting arrangements. The reforms were disappointingly silent on the issue of children’s participation in the new Family Dispute Resolution services, particularly mediation. A disparity has thus arisen between opportunities for children’s engagement in New Zealand’s in-court and out-of-court dispute resolution processes. Research evidence and international developments in Australia and England and Wales are reviewed for the guidance they can offer in remedying this in New Zealand and elsewhere.

Companion animals play an important role in many human’s lives, including many Australian social workers and clients. Yet Australian social work has been slow to address the burgeoning area of human-animal studies. In this embryonic research, we focus on women’s close relationships with companion animals and some of the broad implications this has for social work practice. We analyze some of the themes expressed by women who participated in three focus groups we conducted: two on a university campus and another in a community welfare agency setting. We also examine how the women interacted with each other as they spoke of “their pets,” as these dynamics point to a potentially important source of inspiration and energy that social workers may wish to harness in their day-to-day work with women.

Māori (Indigenous New Zealand) university graduates are role models for educational success and are critical for the social and economic wellbeing of Māori whānau (extended family) and communities. In this study, Māori graduates ( N = 626) from all eight New Zealand universities participated in the Graduate Longitudinal Study New Zealand between July and December 2011. They were asked to describe factors that they found helpful or challenging to the completion of their qualifications. Graduates described a range of external (e.g. family), institutional (e.g. academic support), and student or personal (e.g. persistence) factors. Describing the experiences of Māori graduates can provide a blueprint for future indigenous success by building an evidence base of the factors that may promote indigenous higher educational achievement in New Zealand and internationally.

This study examined the potential usefulness of a life design-related career development intervention to enhance the career adaptability of working adults in a business sector most likely to be disrupted by technology and robotics. Participants were employed at a manufacturing workshop in a South African correctional centre. A mixed-methods intervention study design was implemented. The participants completed a number of career-based assessments during the intervention. The results indicated that the participants were well prepared to face the career challenges presented by Industry 4.0 and that the intervention shows promise in improving career adaptability in working adults. More longitudinal research with diverse participants is needed.
