Abstract

Introduction: The risk of mortality in people with a history of injection drug use (PHID) is high, as is the prevalence of hepatitis C virus (HCV) infection. Although direct‐acting antivirals (DAA) are effective in this population in terms of sustained virological response, it is not known whether PHID benefit as much as people with no history of injection drug use from DAA‐related HCV cure in terms of reduced all‐cause mortality. Methods: Using Cox proportional hazards models based on the ANRS CO22 Hepather cohort data (n = 9735), we identified factors associated with all‐cause mortality among HCV‐infected people. We tested for interaction effects between drug injection status, HCV cure and other explanatory variables. Results: DAA‐related HCV cure was associated with a 66% (adjusted hazard ratio [95% confidence interval]: 0.34 [0.29–0.39]) lower risk of all‐cause mortality, irrespective of drug injection status. Detrimental effects of unhealthy alcohol use on mortality were identified in PHID only. Discussion and Conclusions.: DAA‐related HCV cure led to comparable benefits in terms of reduced mortality in PHID and people with no history of injection drug use. Abstract

Background Patients with chronic hepatitis C virus (HCV) infection are at greater risk of developing metabolic disorders. Obesity is a major risk factor for these disorders, and therefore, managing body weight is crucial. Cannabis use, which is common in these patients, has been associated with lower corpulence in various populations. However, this relationship has not yet been studied in persons with chronic HCV infection. Methods Using baseline data from the French ANRS CO22 Hepather cohort, we used binary logistic and multinomial logistic regression models to test for an inverse relationship between cannabis use (former/current) and (i) central obesity (i.e., large waist circumference) and (ii) overweight and obesity (i.e., elevated body mass index (BMI)) in patients from the cohort who had chronic HCV infection. We also tested for relationships between cannabis use and both waist circumference and BMI as continuous variables, using linear regression models. Results Among the 6348 participants in the study population, 55% had central obesity, 13.7% had obesity according to their BMI, and 12.4% were current cannabis users. After multivariable adjustment, current cannabis use was associated with lower risk of central obesity (adjusted odds ratio, aOR [95% confidence interval, CI]: 0.45 [0.37–0.55]), BMI-based obesity (adjusted relative risk ratio (aRRR) [95% CI]: 0.27 [0.19–0.39]), and overweight (aRRR [95% CI]: 0.47 [0.38–0.59]). This was also true for former use, but to a lesser extent. Former and current cannabis use were inversely associated with waist circumference and BMI. Conclusions We found that former and, to a greater extent, current cannabis use were consistently associated with smaller waist circumference, lower BMI, and lower risks of overweight, obesity, and central obesity in patients with chronic HCV infection. Longitudinal studies are needed to confirm these relationships and to assess the effect of cannabis use on corpulence and liver outcomes after HCV cure. 8. Performance of algorithms for identifying patients with chronic hepatitis B or C infection in the french health insurance claims databases using the ANRS CO22 HEPATHER cohort.

Abstract

The validity of algorithms for identifying patients with chronic hepatitis B or C virus (HBV or HCV) infection in claims databases has been little explored. The performance of 15 algorithms was evaluated. Data from HBV‐ or HCV‐infected patients enrolled between August 2012 and December 2015 in French hepatology centres (ANRS CO22 HEPATHER cohort) were individually linked to the French national health insurance system (SNDS). The SNDS covers 99% of the French population and contains healthcare reimbursement data. Performance metrics were calculated by comparing the viral status established by clinicians with those obtained with the algorithms identifying chronic HBV‐ and HCV‐infected patients. A total of 14 751 patients (29% with chronic HBV and 63% with chronic HCV infection) followed‐up until December 2018 were selected. Despite good specificity, the algorithms relying on ICD‐10 codes performed poorly. By contrast, the multi‐criteria algorithms combining ICD‐10 codes, antiviral dispensing, laboratory diagnostic tests (HBV DNA or HCV RNA detection and quantification, HCV genotyping), examinations for the assessment of liver fibrosis and long‐term disease registrations were the most effective (sensitivity 0.92, 95% CI, 0.91–0.93 and specificity 0.96, 95% CI, 0.95–0.96 for identifying chronic HBV‐infected patients; sensitivity 0.94, 95% CI, 0.94–0.94 and specificity 0.85, 95% CI, 0.84–0.86 for identifying chronic HCV‐infected patients). In conclusion, the multi‐criteria algorithms perform well in identifying patients with chronic hepatitis B or C infection and can be used to estimate the magnitude of the public health burden associated with hepatitis B and C in France. Abstract

Background: We aimed to estimate the seropositivity to anti-SARS-CoV-2 antibodies in May-June 2020 after the first lockdown period in adults living in three regions in France and to identify the associated risk factors.Methods: Between 4 May 2020 and 23 June 2020, 16 000 participants in a survey on COVID-19 from an existing consortium of three general adult population cohorts living in the Ile-de-France (IDF) or Grand Est (GE) (two regions with high rate of COVID-19) or in the Nouvelle-Aquitaine (NA) (with a low rate) were randomly selected to take a dried-blood spot for anti-SARS-CoV-2 antibodies assessment with three different serological methods (ClinicalTrial Identifier #NCT04392388). The primary outcome was a positive anti-SARS-CoV-2 ELISA IgG result against the spike protein of the virus (ELISA-S). Estimates were adjusted using sampling weights and post-stratification methods. Multiple imputation was used to infer the cumulative incidence of SARS-CoV-2 infection with adjustments for imperfect tests accuracies.Results: The analysis included 14 628 participants, 983 with a positive ELISA-S. The weighted estimates of seropositivity and cumulative incidence were 10.0% [95% confidence interval (CI): 9.1%, 10.9%] and 11.4% (95% CI: 10.1%, 12.8%) in IDF, 9.0% (95% CI: 7.7%, 10.2%) and 9.8% (95% CI: 8.1%, 11.8%) in GE and 3.1% (95% CI: 2.4%, 3.7%) and 2.9% (95% CI: 2.1%, 3.8%) in NA, respectively. Seropositivity was higher in younger participants [odds ratio (OR) = 1.84 (95% CI: 1.79, 6.09) in <40 vs 50-60 years old and OR = 0.56 (95% CI: 0.42, 0.74) in ≥70 vs 50-60 years old)] and when at least one child or adolescent lived in the same household [OR = 1.30 (95% CI: 1.11, 1.53)] and was lower in smokers compared with non-smokers [OR = 0.71 (95% CI: 0.57, 0.49)].Conclusions: Seropositivity to anti-SARS-CoV-2 antibodies in the French adult population was ≤10% after the first wave. Abstract

Les polyomavirus sont des virus très prévalents dans la population générale. Chez les sujets immunodéprimés, quatre polyomavirus sont associés à des pathologies. Parmi ces virus, le MCPyV est quant à lui responsable du carcinome à cellules de Merkel. Son implication dans un cancer humain a conduit à un regain d’intérêt pour la famille des Polyomaviridae. Au cours de ma thèse, nous nous somme intéressés à l’épidémiologie de six nouveaux polyomavirus humains (MCPyV, HPyV6, HPyV7, TSPyV, HPyV9 et MWPyV). Ces études ont montré que ces virus sont très répandus dans la population générale et que les infections surviennent dès l’enfance. Par ailleurs, nous nous sommes intéressés aux réactivités croisées entre polyomavirus humains et simiens phylogénétiquement proches. Nous avons montré qu’il existe des cross réactions entre le LPyV et l’HPyV9, entre le MCPyV et deux virus de chimpanzé (PtvPyV1 et PtvPyV2). Ces polyomavirus simiens ne circulent donc pas chez l’Homme. D’autre part, afin de mieux comprendre le cycle du MCPyV, nous avons initié l’identification des partenaires cellulaires de l’ensemble de ses protéines. Ce travail a tout d’abord été réalisé sur les protéines mineures de capside VP2/VP3. Des cribles en double hybride en levure ont permis d’identifier les partenaires cellulaires des VP2/3. L’interaction effective entre les protéines virales et cellulaires a ensuite été validée en cellules de mammifères par une approche de complémentation de la luciférase Gaussia princeps. Les partenaires cellulaires des protéines VP2/VP3 identifiés sont impliqués dans les voies de prolifération cellulaire, d’apoptose, NFκB et dans le transport intracellulaire du virus.

Abstract

ABSTRACT Although Hepatitis C virus (HCV) infection can be cured with direct‐acting antivirals (DAA), some cured patients face a serious risk of advanced liver damage and early mortality. In order to avoid these two negative health outcomes, it is important to identify and assess related risk factors. Little is currently known about socioeconomic and behavioural factors in this context. Using data from the ANRS CO22 Hepather cohort, we tested for associations between socioeconomic and behavioural factors and (i) advanced liver fibrosis (defined as an FIB‐4 > 3.25) assessed longitudinally using a mixed‐effects logistic regression model (both the whole population and stratified on advanced liver fibrosis status at the time of HCV cure) and (ii) all‐cause mortality (Cox proportional hazards model), during post‐HCV cure follow‐up. Among 5833 participants cured of HCV, living in poverty was associated with postcure advanced liver fibrosis in participants without this diagnosis at the time of HCV cure (population attributable fraction—PAF—of 8.6%) and with mortality in the whole study population (PAF of 10.6%). The detrimental effects of unhealthy alcohol use and heavy tobacco smoking, as well as the beneficial effect of living with a stable partner were also highlighted. We highlighted the major role of poverty and behavioural factors in advanced liver fibrosis and all‐cause mortality in patients cured of HCV. Encouraging linkage to social support services and healthy behaviours after successful DAA treatment could limit morbidity and increase survival in this population.Clinical Trial Registration: ClinicalTrials.gov: NCT01953458 [ABSTRACT FROM AUTHOR]
