15. The adolescent HIV executive function and drumming (AHEAD) study, a feasibility trial of a group drumming intervention amongst adolescents with HIV

Abstract

AHEAD feasibility trial assessed the feasibility and acceptability of an 8-session group drumming programme aiming to improve executive function, depression and anxiety symptoms, and perceived social support in adolescents living with HIV in a rural low-income South African setting. Sixty-eight 12- to 19-year-old adolescents participated. They were individually randomised. The intervention arm (n = 34) received weekly hour-long group drumming sessions. Controls (n = 34) received no intervention. Feasibility and acceptability were assessed using rates of: enrolment; retention; attendance; logistical problems; adolescent-reported acceptability. Secondary measures included: five Oxford Cognitive Screen-Executive Function (OCS-EF) tasks; two Rapid Assessment of Cognitive and Emotional Regulation (RACER) tasks; the Self-Reporting Questionnaire-20 (SRQ-20) measuring depression and anxiety symptoms; the Multidimensional Scale of Perceived Social Support (MSPSS). All feasibility criteria were within green progression limits. Enrolment, retention, and acceptability were high. There was a positive effect on adolescent depressed mood with signal for a working memory effect. There were no significant effects on executive function or socio-emotional scales. Qualitative findings suggested socio-emotional benefits including: group belonging; decreased internalised stigma; improved mood; decreased anxiety. Group drumming is a feasible and acceptable intervention amongst adolescents living with HIV in rural South Africa. A full-scale trial is recommended.

Abstract

Optimal design of a tall tower observation network for greenhouse gases (CO2, CH4 and N2O) over Africa. Incremental optimisation was used to select sites from a candidate list to achieve maximum uncertainty reduction in the total CO2, CH4 and N2O fluxes over Africa. The Lagrangian particle dispersion model FLEXPART was used to determine the sensitivity of each candidate site to the fluxes. The code is provided to calculate the uncertainty reduction achieved by adding a site. This includes the sensitivity matrices for each candidate site and the uncertainties in the prior CO2, CH4 and N2O fluxes. The biomass burning observation errors assigned to each candidate site are also provided.

Abstract

Objective: Nightmares are relatively common in patients experiencing psychosis but rarely assessed or treated. Nightmares may maintain persecutory delusions by portraying fears in sensory-rich detail. We tested the potential benefits of imagery-focused cognitive behavioural therapy (CBT) for nightmares on nightmare severity and persecutory delusions. Method: This assessor-blind parallel-group pilot trial randomized 24 participants with nightmares and persecutory delusions to receive CBT for nightmares delivered over 4 weeks in addition to treatment as usual (TAU) or TAU alone. Assessments were at 0, 4 (end of treatment), and 8 weeks (follow-up). Feasibility outcomes assessed therapy uptake, techniques used, satisfaction, and attrition. The primary efficacy outcome assessed nightmare severity at week 4. Analyses were intention to treat, estimating treatment effect with 95% confidence intervals (CIs). Results: All participants offered CBT completed therapy (mean [SD], 4.8 [0.6] sessions) with high satisfaction, and 20 (83%) participants completed all assessments. Compared with TAU, CBT led to large improvements in nightmares (adjusted mean difference = −7.0; 95% CI, –12.6 to –1.3; d = –1.1) and insomnia (6.3; 95% CI, 2.6 to 10.0; d = 1.4) at week 4. Gains were maintained at follow-up. Suicidal ideation was not exacerbated by CBT but remained stable to follow-up, compared with TAU, which reduced at follow-up (6.8; 95% CI, 0.3 to 3.3; d = 0.7). CBT led to reductions in paranoia (–20.8; 95% CI, –43.2 to 1.7; d = –0.6), although CIs were wide. Three serious adverse events were deemed unrelated to participation (CBT = 2, TAU = 1). Conclusions: CBT for nightmares is feasible and may be efficacious for treating nightmares and comorbid insomnia for patients with persecutory delusions. It shows promise on paranoia but potentially not on suicidal ideation.

30. Regional CO₂ flux estimates for South Africa through inverse modelling

Abstract

Bayesian inverse modelling provides a top-down technique of verifying emissions and uptake of carbon dioxide (CO₂) from both natural and anthropogenic sources. It relies on accurate measurements of CO₂ concentrations at appropriately placed sites and "best-guess" initial estimates of the biogenic and anthropogenic emissions, together with uncertainty estimates. The Bayesian framework improves current estimates of CO₂ fluxes based on independent measurements of CO₂ concentrations while being constrained by the initial estimates of these fluxes. Monitoring, reporting and verification (MRV) is critical for establishing whether emission reducing activities to mitigate the effects of climate change are being effective, and the Bayesian inverse modelling approach of correcting CO₂ flux estimates provides one of the tools regulators and researchers can use to refine these emission estimates. South Africa is known to be the largest emitter of CO₂ on the African continent. The first major objective of this research project was to carry out such an optimal network design for South Africa. This study used fossil fuel emission estimates from a satellite product based on observations of night-time lights and locations of power stations (Fossil Fuel Data Assimilations System (FFDAS)), and biogenic productivity estimates from a carbon assessment carried out for South Africa to provide the initial CO₂ flux estimates and their uncertainties. Sensitivity analyses considered changes to the covariance matrix and spatial scale of the inversion, as well as different optimisation algorithms, to assess the impact of these specifications on the optimal network solution. This question was addressed in Chapters 2 and 3. The second major objective of this project was to use the Bayesian inverse modelling approach to obtain estimates of CO₂ fluxes over Cape Town and surrounding area. I collected measurements of atmospheric CO₂ concentrations from March 2012 until July 2013 at Robben Island and Hangklip lighthouses. CABLE (Community Atmosphere Biosphere Land Exchange), a land-atmosphere exchange model, provided the biogenic estimates of CO₂ fluxes and their uncertainties. Fossil fuel estimates and uncertainties were obtained by means of an inventory analysis for Cape Town. As an inventory analysis was not available for Cape Town, this exercise formed an additional objective of the project, presented in Chapter 4. A spatially and temporally explicit, high resolution surface of fossil fuel emission estimates was derived from road vehicle, aviation and shipping vessel count data, population census data, and industrial fuel use statistics, making use of well-established emission factors. The city-scale inversion for Cape Town solved for weekly fluxes of CO₂ emissions on a 1 km × 1 km grid, keeping fossil fuel and biogenic emissions as separate sources. I present these results for the Cape Town inversion under the proposed best available configuration of the Bayesian inversion framework in Chapter 5. Due to the large number of CO₂ sources at this spatial and temporal resolution, the reference inversion solved for weekly fluxes in blocks of four weeks at a time. As the uncertainties around the biogenic flux estimates were large, the inversion corrected the prior fluxes predominantly through changes to the biogenic fluxes. I demonstrated the benefit of using a control vector with separate terms for fossil fuel and biogenic flux components. Sensitivity analyses, solving for average weekly fluxes within a monthly inversion, as well as solving for separate weekly fluxes (i.e. solving in one week blocks) were considered. Sensitivity analyses were performed which focused on how changes to the prior information and prior uncertainty estimates and the error correlations of the fluxes would impact on the Bayesian inversion solution. The sensitivity tests are presented in Chapter 6. These sensitivity analyses indicated that refining the estimates of biogenic fluxes and reducing their uncertainties, as well as taking advantage of spatial correlation between areas of homogeneous biota would lead to the greatest improvement in the accuracy and precision of the posterior fluxes from the Cape Town metropolitan area.

Background: Encephalitis is a serious neurological disorder, yet data on admission rates for all-cause childhood encephalitis in England are lacking. We aimed to estimate admission rates for childhood encephalitis in England over 33 years, to describe trends in admission rates and to observe how these have varied with the introduction of vaccines and improved diagnostics. Methods: A retrospective analysis of hospital admission statistics for encephalitis for individuals aged 0-19 years was conducted using the English national Hospital Inpatient Enquiry (HIPE, 1979 -1985) and Hospital Episode Statistics (HES, 1990 -2011). Annual age-specific and age standardised admission rates in single calendar years and admission rate trends for specified aetiologies in relation to introduction of polymerase chain reaction (PCR) testing and measles-mumps-rubella (MMR) vaccination. Abstract

Background: Guidelines recommend that clinicians identify individuals at high cardiometabolic risk and support weight loss in those with overweight or obesity. However, we lack individual level data quantifying the benefits of weight change for individuals to guide consultations in primary care.Aim: To examine how weight change affects cardiometabolic risk factors, and to facilitate shared decision making between patients and clinicians regarding weight loss.Design and Setting: Observational analysis using data from two trials of referral of individuals with overweight or obesity in primary care to community weight-loss groups.Method: Linear mixed effects regression modelling examining the association between weight change and change in systolic blood pressure (SBP), diastolic blood pressure (DBP), fasting glucose, glycated haemoglobin (HbA1c), and lipid profile across multiple timepoints (baseline to 24 months). Subgroup analyses examined changes in individuals with hypertension, diabetes, and hyperlipidaemia.Results: In total, 2041 participants had a mean (standard deviation) age of 50 (SD 13.5) years, mean baseline weight of 90.6 (14.8) kg and mean body mass index (BMI) of 32.7 (SD 4.1) kg/m2. Mean (SD) weight change was -4.3 (SD 6.0) kg. All outcome measures showed statistically significant improvements. Each 1 kg weight loss was associated with 0.4 mmHg reduction in SBP and 0.3 mmHg reduction in DBP, or 0.5 mmHg and 0.4 mmHg/kg respectively in people with hypertension. Each 1 kg weight loss was associated with 0.2 mmol/mol reduction in HbA1c, or 0.6 mmol/mol in people with diabetes. Effects on plasma lipids were negligible.Conclusion: Weight loss achieved through referral to community weight-loss programmes, which are commonly accessible in primary care, can lead to clinically relevant reductions in BP and glucose regulation, especially in those at highest risk. Abstract

We present a city-scale inversion over Cape Town, South Africa. Measurement sites for atmospheric CO2 concentrations were installed at Robben Island and Hangklip lighthouses, located downwind and upwind of the metropolis. Prior estimates of the fossil fuel fluxes were obtained from a bespoke inventory analysis where emissions were spatially and temporally disaggregated and uncertainty estimates determined by means of error propagation techniques. Net ecosystem exchange (NEE) fluxes from biogenic processes were obtained from the land atmosphere exchange model CABLE (Community Atmosphere Biosphere Land Exchange). Uncertainty estimates were based on the estimates of net primary productivity. CABLE was dynamically coupled to the regional climate model CCAM (Conformal Cubic Atmospheric Model), which provided the climate inputs required to drive the Lagrangian particle dispersion model. The Bayesian inversion framework included a control vector where fossil fuel and NEE fluxes were solved for separately. Due to the large prior uncertainty prescribed to the NEE fluxes, the current inversion framework was unable to adequately distinguish between the fossil fuel and NEE fluxes, but the inversion was able to obtain improved estimates of the total fluxes within pixels and across the domain. The median of the uncertainty reductions of the total weekly flux estimates for the inversion domain of Cape Town was 28 %, but reach as high as 50 %. At the pixel level, uncertainty reductions of the total weekly flux reached up to 98 %, but these large uncertainty reductions were for NEE-dominated pixels. Improved corrections to the fossil fuel fluxes would be possible if the uncertainty around the prior NEE fluxes could be reduced. In order for this inversion framework to be operationalised for monitoring, reporting, and verification (MRV) of emissions from Cape Town, the NEE component of the CO2 budget needs to be better understood. Abstract

Background: Sleep difficulties might be a contributory causal factor in the occurrence of mental health problems. If this is true, improving sleep should benefit psychological health. We aimed to determine whether treating insomnia leads to a reduction in paranoia and hallucinations.Methods: We did this single-blind, randomised controlled trial (OASIS) at 26 UK universities. University students with insomnia were randomly assigned (1:1) with simple randomisation to receive digital cognitive behavioural therapy (CBT) for insomnia or usual care, and the research team were masked to the treatment. Online assessments took place at weeks 0, 3, 10 (end of therapy), and 22. The primary outcome measures were for insomnia, paranoia, and hallucinatory experiences. We did intention-to-treat analyses. The trial is registered with the ISRCTN registry, number ISRCTN61272251. Findings: Between March 5, 2015, and Feb 17, 2016, we randomly assigned 3755 participants to receive digital CBT for insomnia (n=1891) or usual practice (n=1864). Compared with usual practice, the sleep intervention at 10 weeks reduced insomnia (adjusted difference 4·78, 95% CI 4·29 to 5·26, Cohen’s d=1·11; pInterpretation: To our knowledge, this is the largest randomised controlled trial of a psychological intervention for a mental health problem. It provides strong evidence that insomnia is a causal factor in the occurrence of psychotic experiences and other mental health problems. Whether the results generalise beyond a student population requires testing. The treatment of disrupted sleep might require a higher priority in mental health provision.Funding: Wellcome Trust
