Background: Findings from the RESTART trial suggest that starting antiplatelet therapy might reduce the risk of recurrent symptomatic intracerebral haemorrhage compared with avoiding antiplatelet therapy. Brain imaging features of intracerebral haemorrhage and cerebral small vessel diseases (such as cerebral microbleeds) are associated with greater risks of recurrent intracerebral haemorrhage. We did subgroup analyses of the RESTART trial to explore whether these brain imaging features modify the effects of antiplatelet therapy.Methods: RESTART was a prospective, randomised, open-label, blinded-endpoint, parallel-group trial at 122 hospitals in the UK that assessed whether starting antiplatelet therapy might reduce the risk of recurrent symptomatic intracerebral haemorrhage compared with avoiding antiplatelet therapy. For this prespecified subgroup analysis, consultant neuroradiologists masked to treatment allocation reviewed brain CT or MRI scans performed before randomisation to confirm participant eligibility and rate features of the intracerebral haemorrhage and surrounding brain. We followed participants for primary (recurrent symptomatic intracerebral haemorrhage) and secondary (ischaemic stroke) outcomes for up to 5 years (reported elsewhere). For this report, we analysed eligible participants with intracerebral haemorrhage according to their treatment allocation in primary subgroup analyses of cerebral microbleeds on MRI and in exploratory subgroup analyses of other features on CT or MRI. The trial is registered with the ISRCTN registry, number ISRCTN71907627.Findings: Between May 22, 2013, and May 31, 2018, 537 participants were enrolled, of whom 525 (98%) had intracerebral haemorrhage: 507 (97%) were diagnosed on CT (252 assigned to start antiplatelet therapy and 255 assigned to avoid antiplatelet therapy, of whom one withdrew and was not analysed) and 254 (48%) underwent the required brain MRI protocol (122 in the start antiplatelet therapy group and 132 in the avoid antiplatelet therapy group). There were no clinically or statistically significant hazards of antiplatelet therapy on recurrent intracerebral haemorrhage in primary subgroup analyses of cerebral microbleed presence (2 or more) versus absence (0 or 1) (adjusted hazard ratio [HR] 0·30 [95% CI 0·08–1·13] vs 0·77 [0·13–4·61]; pinteraction=0·41), cerebral microbleed number 0–1 versus 2–4 versus 5 or more (HR 0·77 [0·13–4·62] vs 0·32 [0·03–3·66] vs 0·33 [0·07–1·60]; pinteraction=0·75), or cerebral microbleed strictly lobar versus other location (HR 0·52 [0·004–6·79] vs 0·37 [0·09–1·28]; pinteraction=0·85). There was no evidence of heterogeneity in the effects of antiplatelet therapy in any exploratory subgroup analyses (all pinteraction>0·05).Interpretation: Our findings exclude all but a very modest harmful effect of antiplatelet therapy on recurrent intracerebral haemorrhage in the presence of cerebral microbleeds. Further randomised trials are needed to replicate these findings and investigate them with greater precision.Funding: British Heart Foundation.

We aimed to determine the association of evidence-based processes of care for acute ischemic stroke (AIS) and clinical outcome of patients who participated in the HEADPOST (Head Positioning in Acute Stroke Trial), a multicenter cluster crossover trial of lying flat versus sitting up, head positioning in acute stroke. Methods and Results-Use of 8 AIS processes of care were considered: reperfusion therapy in eligible patients; acute stroke unit care; antihypertensive, antiplatelet, statin, and anticoagulation for atrial fibrillation; dysphagia assessment; and physiotherapist review. Hierarchical, mixed, logistic regression models were performed to determine associations with good outcome (modified Rankin Scale scores 0-2) at 90 days, adjusted for patient and hospital variables. Among 9485 patients with AIS, implementation of all processes of care in eligible patients, or defect-free care, was associated with improved outcome (odds ratio, 1.40; 95% CI, 1.18-1.65) and better survival (odds ratio, 2.23; 95% CI, 1.62-3.09). Defect-free stroke care was also significantly associated with excellent outcome (modified Rankin Scale score 0-1) (odds ratio, 1.22; 95% CI, 1.04-1.43). No hospital characteristic was independently predictive of outcome. Only 1445 (15%) of eligible patients with AIS received all processes of care, with significant regional variations in overall and individual rates. Conclusions-Use of evidence-based care is associated with improved clinical outcome in AIS. Strategies are required to address regional variation in the use of proven AIS treatments. Background: Results of small trials indicate that fluoxetine might improve functional outcomes after stroke. The FOCUS trial aimed to provide a precise estimate of these effects.Methods: FOCUS was a pragmatic, multicentre, parallel group, double-blind, randomised, placebo-controlled trial done at 103 hospitals in the UK. Patients were eligible if they were aged 18 years or older, had a clinical stroke diagnosis, were enrolled and randomly assigned between 2 days and 15 days after onset, and had focal neurological deficits. Patients were randomly allocated fluoxetine 20 mg or matching placebo orally once daily for 6 months via a web-based system by use of a minimisation algorithm. The primary outcome was functional status, measured with the modified Rankin Scale (mRS), at 6 months. Patients, carers, health-care staff, and the trial team were masked to treatment allocation. Functional status was assessed at 6 months and 12 months after randomisation. Patients were analysed according to their treatment allocation. This trial is registered with the ISRCTN registry, number ISRCTN83290762.Findings: Between Sept 10, 2012, and March 31, 2017, 3127 patients were recruited. 1564 patients were allocated fluoxetine and 1563 allocated placebo. mRS data at 6 months were available for 1553 (99·3%) patients in each treatment group. The distribution across mRS categories at 6 months was similar in the fluoxetine and placebo groups (common odds ratio adjusted for minimisation variables 0·951 [95% CI 0·839–1·079]; p=0·439). Patients allocated fluoxetine were less likely than those allocated placebo to develop new depression by 6 months (210 [13·43%] patients vs 269 [17·21%]; difference 3·78% [95% CI 1·26–6·30]; p=0·0033), but they had more bone fractures (45 [2·88%] vs 23 [1·47%]; difference 1·41% [95% CI 0·38–2·43]; p=0·0070). There were no significant differences in any other event at 6 or 12 months.Interpretation: Fluoxetine 20 mg given daily for 6 months after acute stroke does not seem to improve functional outcomes. Although the treatment reduced the occurrence of depression, it increased the frequency of bone fractures. These results do not support the routine use of fluoxetine either for the prevention of post-stroke depression or to promote recovery of function.Funding: UK Stroke Association and NIHR Health Technology Assessment Programme.

7. Effect of fat-modified and fruit- and vegetable-enriched diets on blood lipids in the Indian Diet Heart study

This study was designed to test the efficacy of the administration of fruits and vegetables for 12 weeks as an adjunct to a prudent diet in decreasing blood lipids in 310 (intervention; group A) and 311 (control; group B) patients with risk factors of coronary artery disease (CAD) in a parallel, single-blind fashion. At entry to the study, sex, mean age, body weight, body mass index, systolic and diastolic blood pressures, and blood lipoproteins were comparable between both groups. Tasty fruits and vegetables were given to patients to eat before major meals for better nutrient adherence and adequacy. Dietary intakes were determined by questionnaires and by weighing of fruit and vegetable intake. Fruits and vegetables decreased total cholesterol level by 6.5% and low-density lipoprotein cholesterol level by 7.3% in group A, whereas the levels were unchanged in group B. The high-density lipoprotein cholesterol levels that decreased during the diet stabilization period in both groups, increased by 5.6% in group A after 12 weeks. Serum triglycerides also decreased (7%) more in group A than B. Fasting blood glucose decreased by 6.9% in group A and by 2.6% in group B. The combined effect of a fat-modified diet plus fruits and vegetables was greater than these changes. Because tasty fruits were taken by the patients before meals (when they are hungry) and are easily available at reasonable cost in our marketing and buying capacity, the compliance was excellent. It is possible that fruits and vegetables may be a safe adjunct to a prudent diet in free-living subjects with risk factors of CAD for the modulation of blood lipoproteins and for the prevention of CAD. A factorialexperiment has been conducted to study three factors,groundwater velocity, inlet BTX concentration andhydrogen peroxide dose. Observed concentration datacollected from the sand tank model have been used forestimating the transport parameters. Three differentbiodegrdation kinetics, namely first-order/zero-order,Monod and Michaelis Menten (a modification of Monodkinetics considering no microbial growth) kineticshave been used to model the biodegration. The datahave been found to fit all three models withacceptable coefficient of regression. Groundwatervelocity has been found to be the most significantfactor governing the biodegradation rate constants(determined from the first-order rate constant) of BTXcompounds. Hydrogen peroxide dose and BTX concentration have also been found to be significant factors.

Numerical models have been developed using finite difference and orthogonal collocation methods to simulate one dimensional transport with time-dependent pore water velocity. The modeling process includes sorption given by linear isotherm and biodegradation given by a variety of kinetics such as first-order, zero-order, Monod, non-growth associated Monod (Michaelis-Menten), Haldane and many other inhibitory and non-inhibitory kinetics. A number of initial and boundary conditions such as Dirichlet's, Neuman's, mixed, decaying, etc. have been modeled. The method of finite difference (for first order/zero order model only) and the method of orthogonal collocation (for all kinetics models) have been used to solve the governing transport equation. Numerical solutions have been verified with existing analytical solutions for special cases. Three models (first-order and/or zero-order, non growth associated Michaelis Menten, and Monod) have been inverted using a Gauss-Marquardt-Levenberg algorithm to assess the transport parameters. The models have been used to simulate one dimensional transport of BTX compounds in a pilot scale sand tank model. The data have been found to fit to all three kinetic models with acceptable coefficient of determination (R2) and parameter values. The high concentration data have been found to fit better to the Michaelis Menten and the Monod models than the first order/zero order model.

The state of the art bioremediation has been rapidly expanding during the last decade. Numerous articles are being published in a remarkable number of journals. Biodegradability of all possible organic pollutants is being tested in field and lab. New technologies are being discovered to biodegrade even the most recalcitrant organic chemicals. Many new books on bioremediation studies arc also being written and edited. Information is scattered in books, journals, conferences and many other sources. The objective of this paper is to summarize the available existing literature on bioremediation studies. Issues covered are bioremediation in context with other remediation technologies, biodegradability of different organic compounds under different condition as reported in lab and fields studies, factors affecting bioremediation, and review of existing literature on modeling and case studies.

The effect of denitrification on the behavior of nitrate in saturated sandy soil was studied in the laboratory and the results were compared with the temporal and spatial concentration of nitrate in nonreactive (without denitrification) and reactive (with denitrification) cases. A laboratory model was fabricated to study steady one-dimensional flow and to transport nitrate with or without denitrification. Denitrification at various rates has been simulated with varied C∶N ratios and detention time. Retardation constant, dispersivity, and degradation constants have been computed using existing analytical models supporting adsorption and zero and/or first order production or decay. It was found that such analytical models can be used to fit the concentration of nitrate in saturated porous media for a C∶N ratio between 2 and 5.

A decision support system for produced water management (DISSPROWM) in offshore operations is presented. The system determines the risk and hazards to human and marine species from non-carcinogenic and carcinogenic pollutants present in produced water including radionuclides. The DISSPROWM also evaluates best available treatment technology for treating the produced water whose properties are in the database. The system consists of a Windows based Graphical User Interface (GUI) developed with Microsoft Visual Basic that integrates a SQL Server database, risk assessment model and dilution model for produced water contaminants. The database contains produced water pollutants and existing treatment technologies.

20. Benzo(a)pyrene-induced Inflammatory Cytokine Production Contributes to Suppressed E2Synthesis and Secretion at Proestrus.

- Abstract
The toxic effects of cigarette smoke on ovarian function are difficult to assess in as much as cigarette smoke contains more than 4000 chemicals. Benzo(a)pyrene (BaP), an endocrine disruptor that perturbs ovarian E2synthesis is one of the major components of cigarette smoke. The objective of this study was to study the mechanism by which BaP perturbs ovarian E2synthesis. Female F-344 rats with regular four-day estrous cycle were administered BaP (5mg BaP/kg) or vehicle (tricaprilyn; control) by oral gavage for 30 days. A week after the onset gavaging, daily vaginal smears were conducted to determine the ability of BaP-exposed rat to cycle normally. At the end of the treatment regimen, blood samples were collected from rats at proestrus for the determination of serum E2, LH and FSH concentrations. Subsequently, ovaries were harvested post CO2asphyxiation for determining the expression of pro-inflammatory bio-markers. BaP significantly increased estrous cycle length by approximately 3 days. Serum concentrations of E2 and LH were decreased (E2 [BaP, 49.1 ± 5.0 vs Control, 65.3 ± 3.0 pg/ml, P<0.05]; LH [BaP, 37.3 ± 5.0 vs Control, 63.1 ± 4.0 ng/ml]; P<0.05). Serum concentrations of FSH were increased (BaP, 27.3 ± 5.1 vs Control, 10.6 ± 5.6; P<0.01) at the same stage of the estrous cycle compared with those of their control counterparts. Furthermore, some pro-inflammatory biomarkers were significantly elevated in BaP-exposed versus control rats. We conclude that the reduced response of the ovarian follicles to pituitary FSH stimulation, based on reduced serum E2concentrations at proestrus, is contributed to by pro-inflammatory cytokines. As a consequence, increases in E2-stimulated LH secretion at proestrus and normal E2-regulated estrous cycle length could not be realized.(poster)
