PurposeStereotactic radiosurgery (SRS) may be an alternative to anterior temporal lobectomy (ATL) for mesial temporal lobe epilepsy (MTLE). Visual field defects (VFD) occur in 9-100% of patients following open surgery for MTLE. Postoperative VFD after minimally invasive versus open surgery may differ.MethodsThis prospective trial randomized patients with unilateral hippocampal sclerosis and concordant video-EEG findings to SRS versus ATL. Humphries perimetry was obtained at 24 m after surgery. VFD ratios (VFDR = proportion of missing homonymous hemifield with 0 = no VFD, 0.5 = complete superior quadrantanopsia) quantified VFD. Regressions of VFDR were evaluated against treatment arm and covariates. MRI evaluated effects of volume changes on VFDR. The relationships of VFDR with seizure remission and driving status 3 years after surgery were evaluated.ResultsNo patients reported visual changes or had abnormal bedside examinations, but 49 of 54 (91%) of patients experienced VFD on formal perimetry. Neither incidence nor severity of VFDR differed significantly by treatment arm. VFDR severity was not associated with seizure remission or driving status.ConclusionThe nature of VFD was consistent with lesions of the optic radiations. Effective surgery (defined by seizure remission) of the mesial temporal lobe results in about a 90% incidence of typical VFD regardless of method.

Previous studies have explored use of smart glasses in telemedicine, but no study has investigated its use in teleradiography. The purpose of this study was to implement a six-month pilot program for Western Australian X-ray operators (XROs) to use smart glasses to obtain assisted reality support in their radiography practice from their supervising radiographers, and evaluate its effectiveness in terms of XROs' competence improvement and equipment usability. Pretest–posttest design with evaluation of the XROs' competence (including their X-ray image quality) and smart glasses usability by XROs in two remote centers and their supervising radiographers from two sites before and after the program using four questionnaire sets and X-ray image quality review was employed in this experimental study. Paired t-test was used for comparing mean values of the pre- and post-intervention pairs of 11-point scale questionnaire and image quality review items to determine any XROs' radiography competence improvements. Content analysis was used to analyze open questions about the equipment usability. Our study's findings based on 13 participants (11 XROs and 2 supervising radiographers) and 2053 X-ray images show that the assisted reality support helped to improve the XROs' radiography competence (specifically X-ray image quality), with mean post-intervention competence values of 6.16–7.39 (out of 10) and statistical significances (p < 0.001–0.05), and the equipment was considered effective for this purpose but not easy to use. The specific focus of this study is how a team of four preservice teachers experienced a collaborative practicum model to support the development of cogenerative dialogue and foster professional growth. Data sources included individual video club annotations and the associated group discussions facilitated by comparison of groups members selected annotations. The analysis found that participation in peer collaboration provided multiple viewpoints of shared teaching experiences that enabled preservice teachers' different ways to notice student thinking. Providing a structured framework for reflection, namely the individual video club annotations, served as the genesis for cogenerative dialogues centered on instructional change for the preservice teachers. This work's implications showcase the importance of allowing for the iterative enactment and reflection on pedagogical choices by preservice teachers early in their professional development. PurposeGamma knife radiosurgery (RS) may be an alternative to open surgery for mesial temporal lobe epilepsy (MTLE), but morbidities and the anticonvulsant mechanisms of RS are unclear. Examination of visual field defects (VFDs) after RS may provide evidence of the extent of a postoperative fixed lesion. VFDs occur in 52-100% of patients following open surgery for MTLE.MethodsThis multicenter prospective trial of RS enrolled patients with unilateral hippocampal sclerosis and concordant video-electroencephalography (EEG) findings. Patients were randomized to low (20 Gy) or high (24 Gy) doses delivered to the amygdala, hippocampal head, and parahippocampal gyrus. Postoperative perimetry were obtained at 24 months after RS. Visual field defect ratios (VFDRs) were calculated to quantify the degree of VFDs. Results were contrasted with age, RS dose and 50% isodose volume, peak volume of radiation-induced change at the surgical target, quality of life measurements, and seizure remission.Key findingsNo patients reported visual changes and no patients had abnormal bedside visual field examinations. Fifteen (62.5%) of 24 patients had postoperative VFDs, all homonymous superior quadrantanopsias. None of the VFDs were consistent with injury to the optic nerve or chiasm. Clinical diagnosis of VFDs correlated significantly with VFDRs (p = 0.0005). Patients with seizure remission had smaller (more severe) VFDRs (p = 0.04). No other variables had significant correlations.SignificanceVFDs appeared after RS in proportions similar to historical comparisons from open surgery for MTLE. The nature of VFDs was consistent with lesions of the optic radiations. The findings support the hypothesis that the mechanism of RS involves some degree of tissue damage and is not confined entirely to functional changes in neuromodulation.

Objective: Fish population surveys in north‐temperate lakes are often conducted in the fall or spring when individuals are easy to capture with traditional fisheries techniques. Because some fishes are preparing to spawn or are spawning during these seasons, there is a critical need to better understand the potential influences of these surveys on decisions that are related to fish reproduction. Methods: We tested whether spring mark–recapture surveys using fyke nets followed by electrofishing affect the reproductive behaviors of male Smallmouth Bass Micropterus dolomieu in a northern Wisconsin lake. Fyke netting, electrofishing, and whole‐lake nest snorkeling surveys were conducted during 2001–2008, and Floy‐tagged males were tracked across years to test whether capture in the fyke nets only or capture in the electrofishing survey influenced interyear nest site fidelity and reproductive timing. Result: The mark–recapture surveys were conducted preceding the spawning of Smallmouth Bass, and returning males that were caught in the electrofishing survey nested ~50 m farther from their prior year's nest than both males that were captured only in fyke nets and males that were captured by neither method. Average interyear nest distances were ~200 m, and median interyear nest distances were ~90 m for males that were not captured in the electrofishing survey. Electrofishing and fyke netting did not influence the timing of reproduction. Conclusion: Spring electrofishing surveys for Smallmouth Bass have the potential to displace breeding males from preferred nesting habitats. If displacement negatively influences fitness (i.e., age‐0 survivorship to maturation), spring electrofishing surveys would not be recommended for assessing Smallmouth Bass populations. However, spring population surveys often occur soon after ice off, and surveys that are conducted at these colder temperatures are typically less stressful and less likely to result in mortality. Future research should test for fitness implications of reduced nest site fidelity following electrofishing in Smallmouth Bass while considering potential fitness trade‐offs if surveys are moved later in the year. Impact statementFish populations are often surveyed in the spring because many species move to the shoreline to spawn at this time. Being near the shoreline makes fish easier to catch, but we do not understand well how electrofishing surveys impact reproduction. The proportional cover of rubble on reefs is predicted to increase as disturbances increase in intensity and frequency. Unstable rubble can kill coral recruits and impair binding processes that transform rubble into a stable substrate for coral recruitment. A clearer understanding of the mechanisms of inhibited coral recovery on rubble requires characterisation of the hydrodynamic conditions that trigger rubble mobilisation. Here, we investigated rubble mobilisation under regular wave conditions in a wave flume and irregular wave conditions in situ on a coral reef in the Maldives. We examined how changes in near-bed wave orbital velocity influenced the likelihood of rubble motion (e.g. rocking) and transport (by walking, sliding or flipping). Rubble mobilisation was considered as a function of rubble length, branchiness (branched vs. unbranched) and underlying substrate (rubble vs. sand). The effect of near-bed wave orbital velocity on rubble mobilisation was comparable between flume and reef observations. As near-bed wave orbital velocity increased, rubble was more likely to rock, be transported and travel greater distances. Averaged across length, branchiness and substrate, loose rubble had a 50 % chance of transport when near-bed wave orbital velocities reached 0.30 m s -1 in both the wave flume and on the reef. However, small and/or unbranched rubble pieces were generally mobilised more and at lower velocities than larger, branched rubble. Rubble also travelled further distances per day (∼2 cm) on substrates composed of sand than rubble. Importantly, if rubble was interlocked, it was very unlikely to move (< 7 % chance) even at the highest velocity tested (0.4 m s -1). Furthermore, the probability of rubble transport declined over 3 d deployments in the field, suggesting rubble had snagged or settled into more hydrodynamically stable positions within the first days of deployment. We expect that snagged or settled rubble is transported more commonly in locations with higher-energy events and more variable wave environments. At our field site in the Maldives, we expect recovery windows for binding (when rubble is stable) to predominantly occur during the calmer north-eastern monsoon when wave energy impacting the atoll is less and wave heights are smaller. Our results show that rubble beds comprised of small rubble pieces and/or pieces with fewer branches are more likely to have shorter windows of recovery (stability) between mobilisation events, and thus be good candidates for rubble stabilisation interventions to enhance coral recruitment and binding. There is widespread agreement that local and global drivers need to be addressed to reduce the degradation of coral reef ecosystems worldwide (1, 2). Numerous reef fisheries are so severely [...], Continuing degradation of coral reef ecosystems has generated substantial interest in how management can support reef resilience (1-2). Fishing is the primary source of diminished reef function globally (3-5) leading to widespread calls for additional marine reserves to recover fish biomass and restore key ecosystem functions (6). Yet there are no established baselines for determining when these conservation objectives have been met or whether alternative management strategies provide similar ecosystem benefits. Here we establish empirical conservation benchmarks and fish biomass recovery timelines against which coral reefs can be assessed and managed by studying the recovery potential of more than 800 coral reefs along an exploitation gradient. We show that resident reef fish biomass in the absence of fishing ([B.sub.0]) averages ~1,000 kg [ha.sup.-1], and that the vast majority (83%) of fished reefs are missing more than half their expected biomass, with severe consequences for key ecosystem functions such as predation. Given protection from fishing, reef fish biomass has the potential to recover within 35 years on average and less than 60 years when heavily depleted. Notably, alternative fisheries restrictions are largely (64%) successful at maintaining biomass above 50% of [B.sub.0], sustaining key functions such as herbivory. Our results demonstrate that crucial ecosystem functions can be maintained through a range of fisheries restrictions, allowing coral reef managers to develop recovery plans that meet conservation and livelihood objectives in areas where marine reserves are not socially or politically feasible solutions.
