Abstract

Objectives: Although many people who are incarcerated have risk factors for hepatitis A virus (HAV) infection, the proportion of hepatitis A cases among people with a recent incarceration is unknown. We examined the relationship between recent incarceration and HAV infection during community-based, person-to-person outbreaks to inform public health recommendations. Methods: The Centers for Disease Control and Prevention surveyed health departments in 33 jurisdictions reporting person-to-person HAV outbreaks during 2016-2020 on the number of outbreak-associated cases, HAV-infected people recently incarcerated, and HAV-associated hospitalizations and deaths. Results: Twenty-five health departments reported 18 327 outbreak-associated hepatitis A cases during January 11, 2016–January 24, 2020. In total, 2093 (11.4%) HAV-infected people had been recently incarcerated. Of those with complete data, 1402 of 1462 (95.9%) had been held in a local jail, and 1513 of 1896 (79.8.%) disclosed hepatitis A risk factors. Eighteen jurisdictions reported incarceration timing relative to the exposure period. Of 9707 cases in these jurisdictions, 991 (10.2%) were among recently incarcerated people; 451 of 688 (65.6%) people with complete data had been incarcerated during all (n = 55) or part (n = 396) of their exposure period. Conclusions: Correctional facilities are important settings for reaching people with risk factors for HAV infection and can also be venues where transmission occurs. Providing HAV vaccination to incarcerated people, particularly people housed in jails, can be an effective component of community-wide outbreak response. [ABSTRACT FROM AUTHOR]

36. North Carolina Water Utility Builds Resilience with Distributed Energy Resources

Abstract

As the frequency and duration of grid outages increase, backup power systems are becoming more important for ensuring that critical infrastructure continues to provide essential services. Most facilities rely on diesel generators, which may be ineffective during long outages owing to limited fuel supplies and high generator failure rates. Distributed energy resources such as solar, storage, and combined-heat-and-power systems, coupled with on-site biofuel production, offer an alternative source of on-site generation that can provide both cost savings and resilience (i.e., the ability to respond to catastrophic events with longer-term consequences). A mixed-integer linear program minimizes costs and maximizes resilience at a wastewater treatment plant in Wilmington, North Carolina. We find that the plant can reduce life-cycle energy costs by 3.1% through the installation of a hybrid combined-heat-and-power, photovoltaic, and storage system. When paired with existing diesel generators, this system can sustain full load for seven days while saving $664,000 over 25 years and reducing diesel fuel use by 48% compared with the diesel-only solution. This analysis informed a decision by the Cape Fear Public Utility Authority to allocate funds for the implementation of a combined-heat-and-power system at the wastewater treatment plant in fiscal year 2023. The benefits of deploying hybrid combined-heat-and-power technologies and the utilization of on-site biofuel production extend, on a national scale, to thousands of wastewater treatment facilities and other types of critical infrastructure.Funding:This work was supported by the U.S. Department of Energy, Advanced Manufacturing Office [Grant DE-AC36-08GO28308].

Abstract

Monkeypox, a zoonotic infection caused by an orthopoxvirus, is endemic in parts of Africa. On August 4, 2022, the U.S. Department of Health and Human Services declared the U.S. monkeypox outbreak, which began on May 17, to be a public health emergency (1,2). After detection of the first U.S. monkeypox case), CDC and health departments implemented enhanced monkeypox case detection and reporting. Among 2,891 cases reported in the United States through July 22 by 43 states, Puerto Rico, and the District of Columbia (DC), CDC received case report forms for 1,195 (41%) cases by July 27. Among these, 99% of cases were among men; among men with available information, 94% reported male-to-male sexual or close intimate contact during the 3 weeks before symptom onset. Among the 88% of cases with available data, 41% were among non-Hispanic White (White) persons, 28% among Hispanic or Latino (Hispanic) persons, and 26% among non-Hispanic Black or African American (Black) persons. Forty-two percent of persons with monkeypox with available data did not report the typical prodrome as their first symptom, and 46% reported one or more genital lesions during their illness; 41% had HIV infection. Data suggest that widespread community transmission of monkeypox has disproportionately affected gay, bisexual, and other men who have sex with men and racial and ethnic minority groups. Compared with historical reports of monkeypox in areas with endemic disease, currently reported outbreak-associated cases are less likely to have a prodrome and more likely to have genital involvement. CDC and other federal, state, and local agencies have implemented response efforts to expand testing, treatment, and vaccination. Public health efforts should prioritize gay, bisexual, and other men who have sex with men, who are currently disproportionately affected, for prevention and testing, while addressing equity, minimizing stigma, and maintaining vigilance for transmission in other populations. Clinicians should test patients with rash consistent with monkeypox,† regardless of whether the rash is disseminated or was preceded by prodrome. Likewise, although most cases to date have occurred among gay, bisexual, and other men who have sex with men, any patient with rash consistent with monkeypox should be considered for testing. Abstract

At the time of this writing, the U.S. Geological Survey estimates that the average American-born human will need millions of pounds of fuels, minerals, and other extracted resources in his or her lifetime. Mining is a critical global industry, spanning all but one continent (Antarctica), with the highest-producing countries being China, the United States, Russia, Australia, and India. Increasingly, this demand is driving mining companies to explore and pursue deeper mineral deposits as near-surface deposits deplete. Correspondingly, there has been a significant rise in industry interest in applying operations research techniques to improve underground mine planning. Newman et al. [Newman AM, Rubio E, Caro R, Weintraub A, Eurek K (2010) A review of operations research in mine planning. Interfaces40(3):222–245] present a review of such techniques, applied to both open pit and underground mining operations. We focus here on the advancements since that publication and concentrate on underground applications in metalliferous deposits, such as copper, iron, and gold.

47. Barrick’s Turquoise Ridge Gold Mine Optimizes Underground Production Scheduling Operations

Abstract

Mining operations determine a long-term production schedule, often to maximize net present value. For a time horizon of between years and decades, optimization models seek the extraction times—with monthly or yearly fidelity—of three-dimensional, notional blocks of ore and waste within a deposit to satisfy spatial precedence constraints, as well as resource constraints on the amount of material extracted and sent to the mill. With algorithmic advances, as well as those in mine planning software and in hardware, we are able to solve instances with a decade-long horizon at daily fidelity. The resulting objective, repeatable, and defensible schedules inform production and maintenance supervisory decisions based on resource availability, that is, loaders, shovels, haul trucks, and mineral processors. We implement our solutions at the Turquoise Ridge underground gold mine in Nevada, United States. These solutions indicate more than a 2% increase in total ounces extracted over a decade while decreasing development footage by as much as 11% over the same time horizon. Furthermore, we are able to incorporate rules governing a shared resource and to evaluate binding versus nonbinding capacity constraints.

48. Improving Sports Media’s Crystal Ball for National Basketball Association Playoff Elimination

Abstract

The National Basketball Association (NBA) is divided into two conferences, each of which comprises 15 teams. At the end of the regular season, the top eight teams from each conference, based on winning percentage, compete in the playoffs. Mixed-integer-programming (MIP) models determine when a team has guaranteed its position in the playoffs (clinched) or, conversely, when it has been eliminated before the completion of the regular season. Our models incorporate a series of complex two-way tiebreaking criteria used by the NBA to determine how many more games are needed either to clinch or to avoid elimination. We compare the time at which a given team has clinched or been eliminated, in terms of the number of games played in the season to date, as posted in the NBA official standings, against results from our mixed-integer program. For the 2017–2018 season, when our models outperform those of the NBA, they do so by an average of 4.1 games. We also describe a scenario in which the NBA erroneously reported that the Boston Celtics had clinched a playoff spot and, conversely, show that the Golden State Warriors had clinched a playoff spot before the official announcement by the NBA.

Abstract

Tranexamic acid reduces surgical bleeding and reduces death due to bleeding in patients with trauma. Meta-analyses of small trials show that tranexamic acid might decrease deaths from gastrointestinal bleeding. We aimed to assess the effects of tranexamic acid in patients with gastrointestinal bleeding.
