37. Abstract 12857: Cardiology Curbside: A Novel Telephone Model of Efficient and Cost-Effective Delivery of Care

Abstract

Introduction:Readily available specialty physicians can assist referring providers in appropriate testing, treatment and triage of complex patients. The Cardiology Curbside (CC) program was created in 2014 to support Allina Health?s work on the Triple Aim by improving the health of populations while improving patient experience and lowering the cost of care. The utilization of the program was quantified and the cost savings attributable to the care provided were assessed. Physician productivity was also evaluated.Methods:120 hours were spent observing the CC program. The number of calls, sources of calls (10 categories), and care suggestion provided (11 categories) were recorded. These results were used to assess potential cost savings. The work relative value unit (wRVU) per FTE day was assessed for each CC cardiologist and used to compare to the 50thpercentile wRVU productivity of a non-invasive cardiologist.Results:Over the 120 hours, cardiologists took 208 calls from 8 different types of locations within and outside of the Twin Cities metropolitan area, with outpatient Allina Health affiliated physicians calling the most frequently (n=49). Annualized, this service showed a financial impact in savings of over $3.5 million to Allina Health, $5.5 million to the payer, and $980,000 to the patient. We also evaluated the productivity of a CC physician, as measured by wRVUs, who participated in this service. These cardiologists showed an average 38.98 wRVU per FTE day, placing them between the 50thand 75thpercentile for national benchmarks of a cardiologist performing echocardiography interpretation.Conclusions:Introducing a telephone-based cardiology consulting service increased savings for the healthcare system, the payer, and the patient. The service provided positive financial impacts to these groups while maintaining above average wRVU per physician per FTE day than the 50thpercentile national benchmark.
