Background Multiple Sclerosis (MS) patients might represent a vulnerable population to SARS-CoV-2 infection due to several factors, including the immunosuppressive or immunomodulating disease-modifying therapies (DMT) they use and the young age that expose them to a high number of contacts. This study aimed to estimate incidence of SARS-Cov-2 infection in a cohort of MS patients living in the Lazio region and to identify the risk factors for the infection. Methods A retrospective cohort study was performed based on regional health administrative databases linked to the new integrated surveillance system of SARS-CoV-2 infection. MS patients residing in Lazio region and aged ≥18 years on 31 December 2019 were enrolled and followed-up to 31 March 2021. Age- and gender-standardised incidence ratios (SIR) was calculated to compare the infection rate of MS cases with that of the regional general population. A large number of socio-demographic and clinical variables were explored as potential risk factors using a multivariable logistic regression model. Results In the Lazio region lived 10,248 individuals affected by MS (mean age 49.2±12.9; 32.2% males; 67.8% females). During the follow-up period the overall incidence of SARS-CoV-2 infection was 5.0% (n = 510), 5.2% and 4.8% among males and females, respectively. An overall SIR of 0.92 (95%CI 0.84-1.00) was estimated, and 0.97 (95%CI 0.83-1.12) and 0.89 (95%CI 0.80-0.99) for males and females, respectively. The multivariable logistic regression model identified the use of antidiabetic drugs as unique predictor of SARS-CoV-2 infection (OR = 1.69; 95%CI 1.19-2.40). No association with any DMT was observed. Conclusions No difference was observed in the incidence of SARS CoV-2 infection in MS patients compared with the general population. DMT does not seem to affect the risk of infection, while the higher incidence in individuals using antidiabetics could reflect the higher prevalence of diabetic population in infected people. Key messages This study supports previous research suggesting that the incidence of SARS-CoV-2 infection in MS population does not seem to be higher than that in the general population. Disease Modifying Therapy at the moment of infection does not appear to affect the risk of became infected from SARS-CoV-2.

Taking into account gender, age, marital status, and real estate prices, education level was strongly associated with all-cause mortality; compared to highly educated the higher mortality, risk was 35% (95%CI 32%-37%) for low education level and 16% (95%CI 14%-19%) for medium education level. Taking into account the same factors and education level, each increase of 1,000 euros in price/m2 was inversely associated with mortality (HR 0.96, 95%CI 0.96-0.97).there is an independent association between the two indicators and mortality in Rome. A simple indicator such as real estate prices can be used to tackle inequalities.

Background Most dementia patients experience one or more behavioral and psychological symptoms (BPSD) at some time during their illness, in particular in the middle and later stages. Antipsychotic drugs (AP) are often prescribed off-label for treating BPSD, despite concerns about safety and limited efficacy. The impact of AP on short- and long-term mortality has not been fully assessed. Objective To assess the association between the use of APs and short- and long-term mortality in a cohort of dementia patients aged ≥ 65 years residing in the Lazio region. Methods We use data from regional health information systems to conduct a retrospective age, gender and comorbidity matched cohort study with a 1:3 matching ratio to pair dementia patients new AP users with non-users. For the exposed participants, the index date was defined as the date of the first drug prescription; the same date was used for non-users matched subjects. Patients were enrolled on 31/12/2016 and followed-up from the index date through 2018. Four cohorts were enrolled to analyze mortality at 30, 60, 180 and 65 days from the index date. Adjusted estimates were obtained with propensity score matching using the Greedy Nearest Neighbor Matching algorithm. Results We enrolled 34,625 individuals (67% females, mean age 82 years) distributed as follows: 30 days cohort, 4321 users vs 12,960 non-users; 60 days cohort, 4202 users vs 12,606 non-users; 180 days cohort, 3641 users vs 19,923 non-users; 365 days cohort, 2618 users vs 7854 non-users. Multivariate analyses showed a statistically significant excess mortality at 60, 180, and 365 days since the first prescription (HR: 1.83, HR: 1.63, and HR: 1.62, respectively). Conclusions This study showed that antipsychotics increase short- and long-term mortality risk in older adults with dementia. The prescription of AP should be carefully evaluated and, if it is the best option for patient and caregiver, stringently monitored. Key messages Antipsychotic medication use is associated with an increased all-cause mortality in elderly with dementia. The mortality risk increases in 60 days after starting their use and remains elevated at 1 year.

Background Several indicators at individual and area level have been used to stratify the population. The aim of this work was to investigate the association between real estate prices, education and health status in Rome. Methods We used the administrative cohort of residents in Rome at the 2011 census. We followed subjects aged 18+ from 2011 to 2016 using anonymous record linkage procedures with administrative databases. The census includes several individual information, comprising sex, age, education, and residential urbanistic zone. From mortality register we collected date and cause of death. Real estate prices (€/m2) were available for each urbanistic zone. We classified the population in five categories according to the quintile of the distribution (1=highest prices, 5=lowest prices). We used adjusted Cox regression models to estimate the hazard ratios (HRs) and 95%CIs. Results We selected 2,053,668 subjects, not living in institutions at the time of the census survey. Women were 54% of the population; the mean age was 52 years (sd 18), 22.5% had a high education (university degree) and 37% a low education (≤ junior high school). During the follow-up, 126,548 subjects died. As expected educational level was strongly associated with all-cause mortality (39% higher risk of low vs. high educated). The mortality risk increased with decreasing real estate prices, taking account of age and sex. Compared to subjects living in highest prices areas, those living in level 2 areas had a HR = 1.08 (95%CI: 1.06-1.10), those in level 3 had a HR = 1.14 (95%CI: 1.12-1.16), those in level 4 a HR = 1.16 (95%CI 1.14-1.18), and finally those living in the cheapest areas a HR = 1.22 (95%CI: 1.20-1.25). When we considered both education and real estate prizes, the HRs marginally decreased. Conclusions There is an independent association of both individual education and socioeconomic characteristics of the area of residence and all-cause mortality. Key messages There are strong socioeconomic inequalities in Rome. A simple indicator such as real estate prices can be used to tackle inequalities.

BACKGROUND: the Covid-19 pandemic has provoked a huge of clinical and epidemiological research initiatives, especially in the most involved countries. However, this very large effort was characterized by several methodological weaknesses, both in the field of discovering effective treatments (with too many small and uncontrolled trials) and in the field of identifying preventable risks and prognostic factors (with too few large, representative and well-designed cohorts orcase-control studies). OBJECTIVES: in response to the fragmented and uncoordinatedresearch production on Covid-19, the italian Association of Epidemiology (AIE) stimulated the formation of a working group (WG) with the aims of identifying the most important gaps in knowledge and to propose a structured research agenda of clinical and epidemiological studies considered at high priority on Covid-19, including recommendations on the preferable methodology. METHODS: the WG was composed by 25 subjects, mainly epidemiologists, statisticians, and other experts in specific fields, who have voluntarily agreed to the proposal. The agreement on a list of main research questions and on the structure of the specific documents to be produced were defined through few meetings and cycles of document exchanges. RESULTS: twelve main research questions on Covid-19 were identified, covering aetiology, prognosis,interventions, follow-up and impact on general and specific populations (children, pregnant women). For each of them, a two-page form was developed, structured in: background, main topics, methods (with recommendations on preferred study design and warnings for bias prevention) and an essential bibliography. CONCLUSIONS: this research agenda represents an initial contribution to direct clinical and epidemiological research efforts on high priority topics with a focus on methodological aspects. Further development and refinements of this agenda by Public Health Authorities are encouraged.

Background Polypharmacy in the elderly is a matter of concern, especially regarding adverse events and drug–drug interactions. Little is known about real world drug use in the elderly in Italy. The present study aimed at describing drug use in the elderly with respect to individual characteristics, performing a cross-sectional population study in Lazio, Italy. Methods From the regional health care assistance file, residents aged 65 years and older (mean age = 75.9 years, SD = 7.4) and alive at the end of 2010 were enrolled. Drug consumption was linked from the drug claims registry for 2010, and hospital admissions were retrieved from the hospital admission registry during 2009–2010. Enrolees were characterised for socio-demographic variables, presence of chronic diseases, and drug consumption, considering large groups, and specific sub-groups. Polypharmacy was defined as use of 6+ drugs (ATC 4th level). Results Of the 1,122,864 elderly residents, 57.9% were women, and 48.8% aged 75+. In the previous two years, 28.3% had been admitted to hospital; most common causes were cerebro- and cardiovascular diseases, diabetes and tumours; 78.9% of the elderly population of the city of Rome lived in low-level socio-economic areas. Only 6.8% had not been prescribed drugs in the 12-months observation period, and 58.8% were exposed to polypharmacy. With respect to the overall population, polypharmacy users were more often females (59.7%), older (54.5% aged 75 + ), had poorer health conditions (37.2% hospitalised), and more often lived in poorer areas (19.9%). Conclusion In Lazio, polypharmacy is an important phenomenon in the elderly population. Female gender, older ages and presence of chronic diseases are related to multiple drug use. The present results offer a useful tool for future initiatives aiming at reducing inappropriate polypharmacy and target interventions to subpopulations at higher risk of being exposed to polypharmacy.

The phenomenon of dementia among immigrants and ethnic minorities represents an emerging challenge for Western countries. The aim of the present study was to estimate the number of dementia cases among immigrant subjects residing in Italy and in each Italian region to provide pivotal information on the magnitude of such public health issue. The number of immigrant individuals, aged 65 years or older, living in Italy and in the 20 Italian regions was derived by the 2017 data of the National Institute for Statistics. The dementia prevalence rates were taken from the European data provided by the Neurologic Diseases in the Elderly Research Group. The estimated dementia cases were calculated by multiplying the number of immigrants with the age- and sex-specific prevalence rates. Overall, 186,373 older immigrant subjects lived in Italy in January 2017. Nearly 7700 dementia cases were estimated in this population (5022 among women, 2725 among men). When considering each specific Italian region, the number of estimated cases ranged from 19 (Basilicata) to 1500 (Lombardia) with a marked inter-regional variability. Our findings indicate that the occurrence of dementia among immigrants and ethnic minorities constitutes a novel but already relevant issue for our healthcare systems. A non-negligible number of immigrant individuals is probably already seeking or might seek help for cognitive disturbances, thus potentially referring to general practitioners and/or to the Italian dementia services. The forecasted increasing magnitude of this phenomenon reinforces the need for tailored and locally oriented initiatives and policies.

Objectives This study investigated the role of walking outdoors on longevity, controlling for individual and other life-style factors as possible confounders. Methods A 10-year cohort study was conducted with 152 self-caring and mobile, mean age 80 years, were enrolled in the study. Information on socio-demographic characteristics, clinical and biochemical data, diet, physical activity, smoking, depression status, cognitive status and anthropometrics measurements, were obtained for all participants. Cox proportional-hazards models were used to determine independent predictors of longevity. Results During the 10-years of follow-up, 96 (63%) died. Old age, chronic diseases, smoking, depression, CD4/CD8 ratio and coffee consumption were significantly predictors of mortality. Over-all survival was highest for subjects walking at open air for 4 times weekly for at least 15 min in comparison to subjects walking less than 4 times weekly (40% versus 22%). After adjusting for sex, age, education, chronic diseases, smoking, Body Mass Index and CD4/CD8 ratio, elderly people walking at open air for four times weekly had 40% decreased risk of mortality that individuals who walked less than four times weekly [relative risk (RR) = 0.53; 95% confidence interval (CI) = 0.32–0.88, p = 0.01]. Conclusions Findings suggest an independent and protective effect of walking on mortality and supports the encouragement of physical activity in advanced age for increasing longevity.

Summary Aim To study the relationship between respiratory/allergic disorders and chronic environmental tobacco smoke (ETS) exposure to husband or at workplace among non-smoking women of a general population in Italy. Methods Analyses regard 2195 married or employed women. Information was collected through a self-administered questionnaire. ETS exposure was validated by salivary cotinine. Results Exposure both to husband and at work resulted a significant risk factor for current dyspnoea (odds ratio (OR) 1.61, 95% confidence interval (CI) 1.20–2.16), any shortness of breath at rest (OR 2.81, 95% CI 1.83–4.30), recent wheeze (OR 1.71, 95% CI 1.04–2.82), recent attacks of shortness of breath with wheeze (OR 1.85, 95% CI 1.05–3.26), asthma diagnosis/symptoms (OR 1.50, 95% CI 1.09–2.08), diagnosis of asthma or bronchitis/emphysema (obstructive lung diseases (OLD)) (OR 2.24, 95% CI 1.40–3.58), current cough/phlegm (OR 1.52, 95% CI 1.07–2.15), and rhino-conjunctivitis (OR 1.48, 95% CI 1.13–1.94). Exposure only at work yielded higher adjusted odds ratios for all health conditions, except for rhino-conjunctivitis. Overall, about 24% of shortness of breath at rest, 16% of dyspnoea, 17% of rhino-conjunctivitis, 12% of OLD, and 10% of asthma diagnosis/symptoms are attributable to the effect of exposures to both husband and at work. Twelve percent of shortness of breath at rest and 10% of rhino-conjunctivitis cases might be avoided by eliminating exposure only at work and only to husband, respectively. Conclusions Lifetime ETS exposure, especially at work, is associated with respiratory symptoms/diseases, and it accounts for a sizeable proportion of such disorders. The combined effect of both exposures is higher than the separate effects.

Idiopathic pulmonary fibrosis (IPF) is a non-neoplastic pulmonary disease that is characterised by the formation of scar tissue within the lungs in the absence of any known cause. IPF is the most common of the idiopathic interstitial pneumonias and is an important cause of respiratory mortality. IPF is a relatively rare disease with an estimated prevalence ranging from two to 29 cases per 100,000 and slightly higher in men (20.2/100,000) than in women (13.2/100,000). The mean age at presentation is 66 years. Little recent epidemiological data on the prevalence, incidence, risk factors, and mortality related to the disease are available or are limited by methodological weaknesses. Outstanding questions remain, including the causes of IPF, why the incidence is on the rise, and how best to manage this disease. New comparable epidemiological data on IPF are needed.

Background The aim of this study was to compare cotinine determinations in three biological fluids for assessing environmental tobacco smoke (ETS) exposure in female non-smokers (n=1605) in Italy. Methods Information about ETS exposure at home, in the workplace, and in other places within the previous week was collected via questionnaire. Plasma, salivary and urinary cotinine levels were measured. Cotinine levels of > or =0.1 ng/mL for plasma, > or =0.2 ng/mL for saliva, and > or =0.5 ng/mL for urine were used to determine biochemical exposure. Results Median cotinine levels were significantly higher in exposed than in unexposed women (0.21 vs. 0.05 ng/mL in plasma, 0.80 vs. 0.41 ng/mL in saliva, and 9.74 vs. 5.30 ng/mL in urine). Self-reported ETS exposure was significantly related to biochemical exposure [odds ratio 2.99, (95% CI 2.40-3.72) for plasma; 1.90 (1.51-2.39) for saliva; and 2.67 (2.14-3.34) for urine]. Cotinine significantly increased with increasing exposure level, regardless of the exposure source. Among self-reported exposed subjects, higher percentages of cotinine level above the cut-off, i.e., indicating exposure, were found in saliva (76%) and urine (75%) than in plasma (52%). Conclusions In general, women correctly reported their ETS exposure status. Both non-invasive salivary and urinary cotinine determinations seem preferable in epidemiological studies, in view of their higher sensitivity, when compared to plasma cotinine.

OBJECTIVE: To determine if either supplemental vitamin A, zinc, or both increases cell-mediated immune response in an older population. DESIGN: A double-blind, randomized, controlled trial of supplementation with vitamin A and zinc. SETTING: Casa Di Riposo Roma III, a public home for older people in Rome, Italy. SUBJECTS: The health and nutritional status of 178 residents were evaluated. One hundred thirty-six residents agreed to participate in the trial and were randomized into four treatment groups, and 118 of these residents completed the trial. INTERVENTION: The four treatments consisted of: (1) Vitamin A (800 μg retinol palmitate); (2) Zinc (25 mg as zinc sulfate); (3) Vitamin A and Zinc (800 μg retinol palmitate and 25 mg as zinc sulfate); (4) Placebo capsules containing starch. MAIN OUTCOME MEASUREMENTS: Immune tests – counts of leucocytes, lymphocytes, T-cell subsets, and lymphocyte proliferative response to mitogens – were measured before and after supplementation. RESULTS: Zinc increased the number of CD4+DR+ T-cells (P = .016) and cytotoxic T-lymphocytes (P = .005). Subjects treated with vitamin A experienced a reduction in the number of CD3+ T-cells (P = .012) and CD4+ T-cells (P = .012). CONCLUSIONS: These data indicate that zinc supplementation improved cell-mediated immune response, whereas vitamin A had a deleterious effect in this older population. Further research is needed to clarify the clinical significance of these findings.

The strength of association between urinary cotinine and questionnaire data on passive smoking among 542 adolescents was evaluated. There were 103 individuals with urinary cotinine higher than 30 ng/ml; they were compared with all other subjects. The single variable that best predicted the urinary cotinine level was maternal smoking. A strong effect was made by house size and, consequently, house crowding. Maternal smoking and house crowding had a synergic effect. The subject's perception of passive smoking at home also was an independent indication of a high cotinine level. The final model had a fairly good sensitivity, whereas the specificity was somewhat lower. The results suggest that maternal smoking, house crowding, and subject's perception of a smoky environment could be surrogate indices of high passive smoking exposure in this age group and could be useful for epidemiologic studies.
