Abstract

Introduction: Age has historically been considered the main criterion to determine eligibility for intensive chemotherapy in patients with acute myeloid leukemia (AML), but age alone can no longer be considered an absolute indicator in determining which patients should be defined as unfit. Assessment of fitness for a given treatment today serves an important role in tailoring therapeutic options. Areas covered: This review examines the main options used in real life to define eligibility for intensive and nonintensive chemotherapy in patients with AML, with a main focus on the Italian SIE/SIES/GITMO Consensus Criteria. Other published real-life experiences are also reviewed, analyzing the correlation between these criteria and short-term mortality, and thus expected outcomes. Expert opinion: Assessment of fitness is mandatory at diagnosis to tailor treatment to the greatest degree possible, evaluating the patient’s individual profile. This is especially relevant when considering the availability of newer, less toxic therapeutic regimens, which have shown promising results in patients with AML who are older or considered unfit for intensive treatment. Fitness assessment is now a fundamental part of AML management and a critical step that can potentially influence outcomes and not just predict them.

31. Comparison of the Outcomes of Salter and Pemberton Osteotomies in the Treatment of Developmental Hip Dysplasia

Abstract

Aim The aim of this study is to compare two common procedures, Salter and Pemberton osteotomies, applied in developmental hip dysplasia (DDH) treatment. Materials and Methods The patients, treated with Salter and Pemberton osteotomies in our clinic due to DDH and applied for the last follow-up upon call, were included in this study. Preoperative acetabular index (AI) as well as AI, center-edge angle, and femoral head avascular necrosis type regarding Bucholz-Ogden classification in the radiographs, obtained at the last follow-up, were assessed. Clinical outcomes were evaluated considering modified McKay criteria. Results Ten and 35 hips of 36 patients, included in this study, were treated with Pemberton and Salter osteotomies, respectively. Salter group comprised 7 (20.0%) excellent, 20 (57.1%) good, 5 (14.3%) fair, and 3 (8.6%) poor results whereas Pemberton group comprised 7 (70.0%) excellent, 1 (10.0%) good, 1 (10.0%) fair, and 1 (10.0%) poor results, according to modified McKay criteria. AI improvement demonstrated insignificant difference between Salter (26.71±6.1 degrees) and Pemberton (27.8±6.26 degrees) groups (p=0.87). Salter (26.76±13.35 degrees) and Pemberton (24.5±13.96 degrees) groups demonstrated no significant difference considering the center-edge angle (p=0.728). The osteotomy groups also demonstrated insignificant difference regarding the presence (p=0.546) and distribution of the types of avascular necrosis (p=0.516). Conclusion Salter and Pemberton osteotomies are efficient procedures in DDH treatment over 18 months of age. One of them can be applied in patients between the ages of 18 months and 6 years, depending on the surgeon’s experience and preference in case no contraindication is present. Abstract

[EN] Fault diagnosis is important for industrial processes because it permits to determine the necessity of emergency stops in a process and/or to propose a maintenance plan. Two strategies for fault diagnosis are compared in this work. On the one hand, the data are preprocessed using the independent components analysis for dimension reduction, then the wavelet transform is used in order to highlight the faulty signals, with this information an artificial neural network was fed. On the other hand, the second strategy, the main contribution of this work, is the implementation of a long short term memory. This memory is fed with the most representative variables selected by an elastic net to use both, the L1 and L2 norms. These strategies are applied in the Tennessee Eastman process, a benchmark widely used for fault diagnosis. The fault isolation had better results than those reported in the literature., [ES] El diagnóstico de fallas es importante en los procesos industriales, ya que permite determinar si es necesario detener el proceso en operación y/o proponer un plan de mantenimiento. En el presente trabajo se comparan dos estrategias para diagnosticar fallas. La primera realiza un preprocesamiento de datos usando el análisis de componentes independientes para reducir la dimensión de los datos, posteriormente, se emplea la transformada wavelet para resaltar las señales de falla, con esta información se alimenta una red neuronal artificial. Por su parte, la segunda estrategia, principal contribución de este trabajo, usa una memoria de corto y largo plazo. Esta memoria es alimentada por las variables más significativas seleccionadas mediante una red elástica para usar tanto la norma $L_1$ como la $L_2$. Como ejemplo de aplicación se utilizó el proceso químico Tennessee Eastman, un proceso ampliamente usado en el diagnóstico de fallas. El aislamiento de fallas mostró mejores resultados con respecto a los reportados en la literatura.

Abstract

Acute myeloid leukemia (AML) withFLT3-ITD mutations (FLT3-ITDmut) remains a therapeutic challenge, with a still high relapse rate, despite targeted treatment with tyrosine kinase inhibitors. In this disease, the CD34/CD123/CD25/CD99+ leukemic precursor cells (LPCs) phenotype predicts forFLT3-ITD-positivity. The aim of this study was to characterize the distribution ofFLT3-ITD mutation in different progenitor cell subsets to shed light on the subclonal architecture ofFLT3-ITD(mut)AML. Using high-speed cell sorting, we sequentially purified LPCs and CD34+ progenitors in samples from patients withFLT3-ITD(mut)AML (n = 12). A higherFLT3-ITD(mut)load was observed within CD34/CD123/CD25/CD99+ LPCs, as compared to CD34+ progenitors (CD123+/-,CD25-,CD99low/-) (p = 0.0005) and mononuclear cells (MNCs) (p < 0.0001). This was associated with significantly increased CD99 mean fluorescence intensity in LPCs. Significantly higherFLT3-ITD(mut)burden was also observed in LPCs of AML patients with a smallFLT3-ITD(mut)clones at diagnosis. On the contrary, the mutation burden of other myeloid genes was similar in MNCs, highly purified LPCs and/or CD34+ progenitors. Treatment with an anti-CD99 mAb was cytotoxic on LPCs in two patients, whereas there was no effect on CD34+ cells from healthy donors. Our study shows thatFLT3-ITD mutations occur early in LPCs, which represent the leukemic reservoir. CD99 may represent a new therapeutic target inFLT3-ITD(mut)AML.

Abstract

Background: In a 6-year, multicenter, prospective nested case-control study, we aimed to evaluate risk factors for incident cancer in inflammatory bowel disease (IBD), when considering clinical characteristics of IBD and immunomodulator use. The secondary end point was to provide characterization of incident cancer types. Methods: All incident cases of cancer occurring in IBD patients from December 2011-2017 were prospectively recorded in 16 Italian Group for the Study of Inflammatory Bowel Disease units. Each of the IBD patients with a new diagnosis of cancer was matched with 2 IBD patients without cancer, according to IBD phenotype (ulcerative colitis [UC] vs Crohn's disease [CD]), age (±5 years), sex. Risk factors were assessed by multivariate logistic regression analysis. Results: Cancer occurred in 403 IBD patients: 204 CD (CD cases), 199 UC (UC cases). The study population included 1209 patients (403 IBD cases, 806 IBD controls). Cancer (n = 403) more frequently involved the digestive system (DS; 32%), followed by skin (14.9%), urinary tract (9.7%), lung (6.9%), genital tract (6.5%), breast (5.5%), thyroid (1.9%), lymphoma (2.7%, only in CD), adenocarcinoma of the small bowel (SBA; 3.9%, 15 CD, 1 pouch in UC), other cancers (15.9%). Among cancers of the DS, colorectal cancer (CRC) more frequently occurred in UC (29% vs 17%; P < 0.005), whereas SBA more frequently occurred in CD (13% vs 6.3% P = 0.039). In CD, perforating (B3) vs non-stricturing non-perforating (B1) behavior represented the only risk factor for any cancer (odds ratio [OR], 2.33; 95% confidence interval [CI], 1.33-4.11). In CD, risk factors for extracolonic cancer (ECC) were a B3 vs B1 and a stricturing (B2) vs B1 behavior (OR, 2.95; 95% CI, 1.62-5.43; OR, 1.79; 95% CI, 1.09-2.98). In UC, risk factors for ECC and for overall cancer were abdominal surgery for UC (OR, 4.63; 95% CI, 2.62-8.42; OR, 3.34; 95% CI, 1.88-5.92) and extensive vs distal UC (OR, 1.73; 95% CI, 1.10-2.75; OR, 1.99; 95% CI
