The synthesis during the present decade of different arts and sciences to form oncology as a mUltidisciplinary subject is of profound importance for coordinating the clinical and research efforts to control a number of diseases that cause high mortality and morbidity in the human race and to elucidate their causation. These diseases traditionally have been grouped together under the general term of cancer, without any scientific reason and irrespective of many differences existing between them. The word cancer, because it is synonymous with diseases that cause suffering and death, naturally generates fear in people throughout the world. Cancer fortunately has a changing face caused by the realization that these diseases are different diseases with variable etiology and prognosis; they need different kinds of treatment, and even prevention is a practical proposition. The time has therefore come to delete the term cancer from our terminol­ ogy as unscientific and unhelpful and to substitute oncological diseases governed by the system of knowledge designated oncology. This conforms also with the designations used for other groups of diseases. In the divisions of oncology an important place is held by surgical oncology because many oncological diseases require surgical treatment alone, or in combination with radiotherapy and chemotherapy. Combina­ tion therapy is being used effectively for an increasing number of these diseases, and this trend will become more pronounced as chemotherapy develops. The results achieved with available chemicals and present dosage schedules are impressive.

The development of the capacity to use electron beams at various energies stimulated the development of the new technology of intraoperative radiation therapy. The visual­ ization of the primary tumor, definition of the site to be irradiated, and knowledge of the size of tumor to be irradiated, including adequate margins for microscopic extent along with debulking of the tumor, have allowed intraoperative radiation therapy to be pur­ sued in a wide variety of disease processes. Not only is it possible to treat tumors that were highly resistant, but it is also possible to specifically localize the tumor with a mar­ gin adequate to encompass potential microscopic extensions and to choose the appro­ priate electron beam energy to treat the potential residual tumor and spare the adjacent normal structures. It is, furthermore, possible to treat patients with unresectable tumors. Central to this entire development is the fact that normal structures can be removed from the field of treatment. In the beginning, the emphasis was on single fractions of electron beam radiation without subsequent supplementation by external photon beam radiation therapy. How­ ever, more recently it has become necessary to use not only a single electron beam frac­ tion during the course of the operation, but to supplement it with external beam treat­ ment. The encouraging results obtained with intraoperative radiation therapy have led to clinical trials at a large number of institutions throughout the world.

During the past 9 years, reports of'port-site'deposits following laparoscopic surgery for malignancy, especially laparoscopic resection of colonic cancer, have cast a shadow on the wisdom of the laparoscopic approach in the surgical man­ agement in patients with cancer. Those reports of port-site deposits, some 90 cases reported in the literature up to 1999, have opened a'can of worms'and highlighted the scarcity of our knowledge on cancer cell migration from solid tu­ mors and the factors that underlie their successful implantation in surgical wounds both in the presence and absence of a positive pressure pneumoperito­ neum. The jury is out even in relation to the effect of the healing surgical access wound - do the biochemical and cellular repair processes and the associated growth factors enhance or prevent implantation of exfoliated viable tumor cells? Whatever the answer to this question, it is clear that tumor cells do implant in healing surgical wounds and the key question is whether this is facilitated by lap­ aroscopic surgery with CO pneumoperitoneum compared to the traditional 2 surgical exposure. It is known that tumors shed malignant cells into the blood stream, the peritoneal cavity and in the case of hollow organs, intraluminally. Equally there is good evidence that surgical and instrumental manipulation of tumors induce exfoliation of viable tumor cells.
