Abstract

The article discusses a study conducted in Germany that examined the use of dupilumab, a biologic treatment, for pediatric patients with moderate-to-severe atopic dermatitis (AD). The study found that dupilumab significantly improved disease severity and quality of life in these patients. The treatment was generally well-tolerated, with only a few reported adverse events. Abstract

Background: The persistently high prevalence of allergic diseases in Western industrial nations and the limited possibilities of causal therapy make evidence-based recommendations for primary prevention necessary. Methods: The recommendations of the S3 guideline Allergy Prevention, published in its last version in 2014, were revised and consulted on the basis of a current systematic literature search. The evidence search was conducted for the period 06/2013 - 11/2020 in the electronic databases Cochrane and MEDLINE, as well as in the reference lists of current reviews and through references from experts. The literature found was screened in two filtering processes, first by title and abstract, and the remaining papers were screened in the full text for relevance. The studies included after this were sorted by level of evidence, and the study quality was indicated in terms of potential bias (low/high). The revised recommendations were formally agreed and consented upon with the participation of representatives of the relevant professional societies and (self-help) organizations (nominal group process). Of 5,681 hits, 286 studies were included and assessed. Results: Recommendations on maternal nutrition during pregnancy and breastfeeding as well as on infant nutrition in the first months of life again play an important role in the updated guideline: Many of the previous recommendations were confirmed by the current data. It was specified that breastfeeding should be exclusive for the first 4 - 6 months after birth, if possible, and that breastfeeding should continue with the introduction of complementary foods. A new recommendation is that supplementary feeding of cow's milk-based formula should be avoided in the first days of life if the mother wishes to breastfeed. Abstract

Hintergrund: Die anhaltend hohe Prävalenz allergischer Erkrankungen in westlichen Industrienationen und die eingeschränkten Möglichkeiten einer kausalen Therapie machen eine evidenzbasierte Primärprävention notwendig. Methoden: Die Empfehlungen der in letzter Fassung 2014 veröffentlichten S3-Leitlinie Allergieprävention wurden auf der Basis einer aktuellen systematischen Literatursuche überarbeitet und konsentiert. Die Evidenzsuche erfolgte für den Zeitraum 06/2013 – 11/2020 in den elektronischen Datenbanken Cochrane und MEDLINE, sowie in den Referenzlisten von aktuellen Übersichtsarbeiten und durch Hinweise von Experten. Die aufgefundene Literatur wurde in zwei Filterprozessen zunächst nach Titel und Zusammenfassung und die verbliebenen Arbeiten im Volltext auf Relevanz hin überprüft. Die danach eingeschlossenen Studien wurden nach Evidenzgrade geordnet und die Studienqualität i.S. des Verzerrungspotenzials (niedrig/hoch) angegeben. Die überarbeiteten Empfehlungen wurden unter Beteiligung von Vertretern der relevanten Fachgesellschaften und (Selbsthilfe-)Organisationen formal abgestimmt und konsentiert (nominaler Gruppenprozess). Von 5.681 Treffern wurden 286 Studien eingeschlossen und bewertet. Ergebnisse: Einen wichtigen Stellenwert in der aktualisierten Leitlinie nehmen wieder Empfehlungen zur Ernährung der Mutter während der Schwangerschaft und Stillzeit sowie des Säuglings in den ersten Lebensmonaten ein: Dabei wurden viele der bisherigen Empfehlungen durch die aktuelle Datenlage bestätigt. Präzisiert wurde, dass für den Zeitraum der ersten 4 – 6 Monate nach Geburt nach Möglichkeit ausschließlich gestillt werden soll und auch mit Einführung von Beikost weitergestillt werden soll. Neu ist die Empfehlung, dass eine Zufütterung von kuhmilchbasierter Formulanahrung in den ersten Lebenstagen bei Stillwunsch der Mutter vermieden werden sollte. Ferner wurde festgestellt, dass die Evidenz für eine klare positive Empfehlung für hydrolysierte Säuglingsnahrung bei nicht gestillten Risikokindern aktuell nicht mehr ausreicht. Aktuell wird daher empfohlen zu prüfen, ob bis zur Einführung von Beikost eine Säuglingsanfangsnahrung mit in Studien zur Allergieprävention nachgewiesener Wirksamkeit verfügbar ist. Schließlich wurden auf Basis der EAACI-Leitlinie zur Prävention der Hühnereiallergie die Empfehlung ausgesprochen, durcherhitztes (zum Beispiel verbackenes oder hartgekochtes), aber nicht „rohes“ Hühnerei (auch kein Rührei) mit der Beikost einzuführen und regelmäßig weiter zu geben. Die Empfehlung zur Einführung von Erdnuss in der Beikost wurde für den deutschsprachigen Raum zurückhaltend formuliert: In Familien mit bereits bestehendem regelmäßigem Erdnusskonsum kann zur Prävention einer Erdnussallergie bei Säuglingen mit atopischer Dermatitis die regelmäßige Gabe von erdnusshaltigen Nahrungsmitteln in altersgerechter Form (zum Beispiel Erdnussbutter) mit der Beikost erwogen werden. Vor der Einführung muss insbesondere bei Säuglingen mit moderater bis schwerer AD eine klinisch relevante Erdnussallergie ausgeschlossen werden. Für eine allergiepräventive Wirksamkeit von Präbiotika oder Probiotika, Vitamin D oder anderen Vitaminen in Form von Präparaten fehlt weiterhin ausreichende Evidenz, sodass in der aktuellen Leitlinie erstmals Empfehlungen gegen deren Supplementierung verabschiedet wurden. Die Biodiversität spielt für die Entwicklung einer immunologischen Toleranz gegenüber Umwelt- und Nahrungsmittelallergenen eine wichtige Rolle: So gibt es deutliche Hinweise darauf, dass das Aufwachsen auf dem Bauernhof mit einem geringeren Risiko für die Entwicklung von Asthma und allergischen Erkrankungen assoziiert ist. Dies wird mit einer frühzeitigen unspezifischen Immunstimulation unter anderem durch die größere mikrobielle Biodiversität des Hausstaubs in diesem Lebensraum in Verbindung gebracht. Dieser Aspekt spiegelt sich auch in den Empfehlungen zur Tierhaltung wider, zu der eine differenzierte Stellungnahme erfolgte: In Familien ohne erkennbares erhöhtes Allergierisiko soll die Haustierhaltung mit Katzen oder Hunden nicht generell eingeschränkt werden. Familien mit erhöhtem Allergierisiko (d. h. Vater, Mutter oder Geschwisterkind sind von einer atopischen Erkrankung betroffen) oder bei Kindern mit bereits bestehender atopischer Dermatitis sollten eine Katze nicht neu anschaffen – im Gegensatz dazu sollte von einer Hundehaltung jedoch nicht abgeraten werden. Interventionen zur Reduktion der Exposition gegenüber Hausstaubmilbenallergenen im Haushalt, zum Beispiel die Verwendung milbenallergendichter Matratzenüberzüge („encasings“), sollten nicht mit dem Ziel einer primären Allergieprävention erfolgen. Kinder, die durch einen Kaiserschnitt geboren wurden, haben ein geringfügig erhöhtes Asthmarisiko – dies soll bei der Beratung zum Geburtsmodus außerhalb von Notfallsituationen berücksichtigt werden. Auch aktuelle Arbeiten stützen die Empfehlungen zu Luftschadstoffen: Die aktive und passive Exposition gegenüber Tabakrauch erhöhen das Allergierisiko und sind deshalb zu vermeiden. Die Exposition gegenüber Stickoxiden, Ozon und Feinstaub der Partikelgröße < 2,5 Mikrometer (PM 2,5) ist mit einem erhöhten Risiko, besonders für Asthma, verbunden. Daher sollte die Exposition gegenüber Emissionen gegenüber Stickoxiden, Ozon und Feinstaub (PM 2,5) gering gehalten werden. Die Autor/-innen dieser Leitlinie sprechen sich einhellig für den Erlass entsprechender Verordnungen zur Minimierung dieser Luftschadstoffe aus. Es gibt keine Belege, dass Impfungen das Allergierisiko erhöhen, umgekehrt aber Hinweise, dass Impfungen das Allergierisiko senken können. Alle Kinder, auch Risikokinder, sollen auch aus Gründen der Allergieprävention nach den aktuellen Empfehlungen der ­STIKO geimpft werden. Schlussfolgerung: Die Konsentierung von Empfehlungen in dieser Leitlinie beruht auf einer umfangreichen Evidenzgrundlage. Die Aktualisierung der Leitlinie ermöglicht evidenzbasierte und aktuelle Empfehlungen zur Prävention allergischer Erkrankungen.

13. Transient binocular vision loss and pain insensitivity in Klippel–Feil syndrome: a case report

Abstract

Abstract Background Klippel–Feil syndrome is a rare congenital bone disorder characterized by an abnormal fusion of two or more cervical spine vertebrae. Individuals with Klippel–Feil syndrome exhibit diverse clinical manifestations, including skeletal irregularities, visual and hearing impairments, orofacial anomalies, and anomalies in various internal organs, such as the heart, kidneys, genitourinary system, and nervous system. Case presentation This case report describes a 12-year-old Pashtun female patient who presented with acute bilateral visual loss. The patient had Klippel–Feil syndrome, with the typical clinical triad symptoms of Klippel–Feil syndrome, along with Sprengel’s deformity. She also exhibited generalized hypoalgesia, which had previously resulted in widespread burn-related injuries. Upon examination, bilateral optic disc swelling was observed, but intracranial pressure was found to be normal. Extensive investigations yielded normal results, except for hypocalcemia and low vitamin D levels, while parathyroid function remained within the normal range. Visual acuity improved following 2 months of calcium and vitamin D supplementation, suggesting that the visual loss and optic nerve swelling were attributed to hypocalcemia. Given the normal parathyroid function, it is possible that hypocalcemia resulted from low vitamin D levels, which can occur after severe burn scarring. Furthermore, the patient received a provisional diagnosis of congenital insensitivity to pain on the basis of the detailed medical history and the findings of severe and widespread loss of the ability to perceive painful stimuli, as well as impaired temperature sensation. However, due to limitations in genetic testing, confirmation of the congenital insensitivity to pain diagnosis could not be obtained. Conclusion This case highlights a rare presentation of transient binocular vision loss and pain insensitivity in a patient with Klippel–Feil syndrome, emphasizing the importance of considering unusual associations in symptom interpretation.

15. Peanut-induced anaphylaxis in children and adolescents: Data from the European Anaphylaxis Registry

Abstract

Background: Peanut allergy has a rising prevalence in high-income countries, affecting 0.5%–1.4% of children. This study aimed to better understand peanut anaphylaxis in comparison to anaphylaxis to other food triggers in European children and adolescents. Methods: Data was sourced from the European Anaphylaxis Registry via an online questionnaire, after in-depth review of food-induced anaphylaxis cases in a tertiary paediatric allergy centre. Results: 3514 cases of food anaphylaxis were reported between July 2007-March 2018, 56% in patients younger than 18 years. Peanut anaphylaxis was recorded in 459 children and adolescents (85% of all peanut anaphylaxis cases). Previous reactions (42% vs. 38%; p = .001), asthma comorbidity (47% vs. 35%; p < .001), relevant cofactors (29% vs. 22%; p = .004) and biphasic reactions (10% vs. 4%; p = .001) were more commonly reported in peanut anaphylaxis. Most cases were labelled as severe anaphylaxis (Ring&Messmer grade III 65% vs. 56% and grade IV 1.1% vs. 0.9%; p = .001). Self-administration of intramuscular adrenaline was low (17% vs. 15%), professional adrenaline administration was higher in non-peanut food anaphylaxis (34% vs. 26%; p = .003). Hospitalization was higher for peanut anaphylaxis (67% vs. 54%; p = .004). Conclusions: The European Anaphylaxis Registry data confirmed peanut as one of the major causes of severe, potentially life-threatening allergic reactions in European children, with some characteristic features e.g., presence of asthma comorbidity and increased rate of biphasic reactions. Usage of intramuscular adrenaline as first-line treatment is low and needs to be improved. The Registry, designed as the largest database on anaphylaxis, allows continuous assessment of this condition. © 2021 The Authors. Abstract

BACKGROUND: Since the beginning of the COVID-19 pandemic, the treatment of patients with allergic and atopy-associated diseases has faced major challenges. Recommendations for "social distancing" and the fear of patients becoming infected during a visit to a medical facility have led to a drastic decrease in personal doctor-patient contacts. This affects both acute care and treatment of the chronically ill. The immune response after SARS-CoV-2 infection is so far only insufficiently understood and could be altered in a favorable or unfavorable way by therapy with monoclonal antibodies. There is currently no evidence for an increased risk of a severe COVID-19 course in allergic patients. Many patients are under ongoing therapy with biologicals that inhibit type 2 immune responses via various mechanisms. There is uncertainty about possible immunological interactions and potential risks of these biologicals in the case of an infection with SARS-CoV-2. MATERIALS AND METHODS: A selective literature search was carried out in PubMed, Livivo, and the internet to cover the past 10 years (May 2010 - April 2020). Additionally, the current German-language publications were analyzed. Based on these data, the present position paper provides recommendations for the biological treatment of patients with allergic and atopy-associated diseases during the COVID-19 pandemic. RESULTS: In order to maintain in-office consultation services, a safe treatment environment must be created that is adapted to the pandemic situation. To date, there is a lack of reliable study data on the care for patients with complex respiratory, atopic, and allergic diseases in times of an imminent infection risk from SARS-CoV-2. Type-2-dominant immune reactions, as they are frequently seen in allergic patients, could influence various phases of COVID-19, e.g., by slowing down the immune reactions. Theoretically, this could have an unfavorable effect in the early phase of a SARS-Cov-2 infection, but also a positive effect during a cytokine storm in the later phase of severe courses. However, since there is currently no evidence for this, all data from patients treated with a biological directed against type 2 immune reactions who develop COVID-19 should be collected in registries, and their disease courses documented in order to be able to provide experience-based instructions in the future. CONCLUSION: The use of biologicals for the treatment of bronchial asthma, atopic dermatitis, chronic rhinosinusitis with nasal polyps, and spontaneous urticaria should be continued as usual in patients without suspected infection or proven SARS-CoV-2 infection. If available, it is recommended to prefer a formulation for self-application and to offer telemedical monitoring. Treatment should aim at the best possible control of difficult-to-control allergic and atopic diseases using adequate rescue and add-on therapy and should avoid the need for systemic glucocorticosteroids. If SARS-CoV-2 infection is proven or reasonably suspected, the therapy should be determined by weighing the benefits and risks individually for the patient in question, and the patient should be involved in the decision-making. It should be kept in mind that the potential effects of biologicals on the immune response in COVID-19 are currently not known. Telemedical offers are particularly desirable for the acute consultation needs of suitable patients.

Abstract

Food allergens are frequent causes of anaphylaxis. In particular in children and adolescents they are the most frequent elicitors of severe allergic reactions, and in adults food allergens rank third behind insect venom and drugs. Since July 2006 severe allergic reactions from Germany, Austria, and Switzerland are collected in the anaphylaxis registry. Currently 78 hospitals and private practises are connected. From July 2006 until February 2009 1,156 severe allergic reactions were registered. Among children and adolescents (n = 187, age range from 3 months to 17 years) food allergens were the most frequent triggers, comprising 58% of cases. In the adult group (n = 968, 18 - 85 years) food allergens were in the third position (16.3%) behind insect venom and drugs. In children legumes (31%) and in particular peanuts were frequently responsible food allergens, followed by tree nuts (25%) with hazelnut being the most frequent elicitor. In adults fruits (13.4%) most often induced severe food-dependent anaphylaxis, but also animal products (12.2%); among these most frequently crustaceans and molluscs. Cofactors were often suspected in food-dependent anaphylaxis, namely in 39% of the adult group and in 14% of the pediatric group. In adults drugs (22%) and physical activity (10%) were reported to be the most frequent cofactors, in children physical activity was suspected in 8.7% and drugs in 2.6%. Concomitant diseases like atopic dermatitis, allergic asthma, or allergic rhinoconjunctivitis were reported in 78% of children and adolescents and in 67% of the adults. In conclusion, food-induced anaphylaxis, its cofactors and concomitant diseases are age-dependent. The data offers to identify risk factors of anaphylaxis.

Abstract

Background: The number of patients affected by allergies is increasing worldwide. The resulting allergic diseases are leading to significant costs for health care and social systems. Integrated care pathways are needed to enable comprehensive care within the national health systems. The ARIA (Allergic Rhinitis and its Impact on Asthma) initiative develops internationally applicable guidelines for allergic respiratory diseases. Methods: ARIA serves to improve the care of patients with allergies and chronic respiratory diseases. In collaboration with other international initiatives, national associations and patient organizations in the field of allergies and respiratory diseases, real-life integrated care pathways have been developed for a digitally assisted, integrative, individualized treatment of allergic rhinitis (AR) with comorbid asthma. In the present work, these integrated care pathways have been adapted to the German situation and health system. Results: The present ICP (integrated care pathways) guideline covers key areas of the care of AR patients with and without asthma. It includes the views of patients and other healthcare providers. Discussion: A comprehensive ICP guideline can reflect real-life care better than traditional guideline models. © 2019, The Author(s).

Abstract

ABSTRACT The broad host range fungal insect pathogen, Beauveria bassiana, has been commercialized as an alternative to chemical insecticides for pest control worldwide. B. bassiana represents a unique model system with which to examine host-pathogen interactions, and a wide range of genes and processes have been studied. However, significant aspects of virulence, particularly on the genomic scale, remain poorly studied. Here, we have combined available transcriptomes with three newly generated data sets for a combined total analysis of 76 deep-sequenced samples covering growth, development, stress responses, and infection during the life cycle of B. bassiana. Co-expression network analyses resulted in the identification of gene modules enriched during two critical stages of the infection process, namely (i) cuticle penetration and (ii) in vivo hyphal body (dimorphic transition) growth capable of avoiding innate and humoral immune defenses. These analyses identify unique signatures of metabolism, signaling, secondary metabolite production, host defense suppression, membrane reorganization, effector production, and secretion for each stage, including genetic regulators and epigenetic patterns. These data provide a comprehensive framework for understanding and probing fungal adaptations to its pathogenic life cycle and expand the candidate repertoire for continued dissection of the host-pathogen interaction. IMPORTANCE Insect fungal pathogens have evolved unique strategies for overcoming host structural and immunological defenses that span from the sclerotized cuticle to innate and humoral cellular responses. Two critical stages of the infection process involve (i) cuticle penetration and (ii) immune evasion within the insect hemocoel. A set of 76 global transcriptomic data for B. bassiana that include the cuticle penetration and hemocoel growth stages were analyzed for patterns (gene modules) of expression, yielding unique insights into these different life stages. These analyses integrate gene networks involved in fungal development, stress response and pathogenesis to further the systematic understanding of the global processes integral to the unique adaptation employed by fungal pathogens of insects.

Abstract

Colon cancer is the third most diagnosed cancer worldwide, followed by lung and breast cancer. Conventional treatment methods are associated with numerous side effects and compliance issues. Thus, colon targeted drug delivery has gained much attention due to its evident advantages. Although many technologies have been explored, the use of pH-sensitive polymers, especially biodegradable polymers, holds exceptional promise. This review aims to collate research articles concerning recent advances in this area. A systematic search using multiple databases (Google Scholar, EMBASE, PubMed, MEDLINE and Scopus) was carried out following the preferred reported items for systematic reviews and meta-analyses (PRISMA) guidelines with an aim to explore the use of pH-sensitive carbohydrate polymers in developing colon targeted pharmaceutical formulations. Following screening and quality assessment for eligibility, 42 studies were included, exploring either single or a combination of carbohydrate polymers to develop targeted formulations for colon cancer therapy. Pectin (11) is the most widely used of these biopolymers, followed by chitosan (09), alginate (09) and guar gum (08). This systematic review has successfully gathered experimental evidence highlighting the importance of employing carbohydrate polymers in developing targeting formulations to manage colon cancer.

Abstract

Background Anaphylaxis in children and adolescents is a potentially life-threatening condition. Its heterogeneous clinical presentation and sudden occurrence in virtually any setting without warning have impeded a comprehensive description. Objective We sought to characterize severe allergic reactions in terms of elicitors, symptoms, emergency treatment, and long-term management in European children and adolescents. Methods The European Anaphylaxis Registry recorded details of anaphylaxis after referral for in-depth diagnosis and counseling to 1 of 90 tertiary allergy centers in 10 European countries, aiming to oversample the most severe reactions. Data were retrieved from medical records by using a multilanguage online form. Results Between July 2007 and March 2015, anaphylaxis was identified in 1970 patients younger than 18 years. Most incidents occurred in private homes (46%) and outdoors (19%). One third of the patients had experienced anaphylaxis previously. Food items were the most frequent trigger (66%), followed by insect venom (19%). Cow's milk and hen's egg were prevalent elicitors in the first 2 years, hazelnut and cashew in preschool-aged children, and peanut at all ages. There was a continuous shift from food- to insect venom- and drug-induced anaphylaxis up to age 10 years, and there were few changes thereafter. Vomiting and cough were prevalent symptoms in the first decade of life, and subjective symptoms (nausea, throat tightness, and dizziness) were prevalent later in life. Thirty percent of cases were lay treated, of which 10% were treated with an epinephrine autoinjector. The fraction of intramuscular epinephrine in professional emergency treatment increased from 12% in 2011 to 25% in 2014. Twenty-six (1.3%) patients were either admitted to the intensive care unit or had grade IV/fatal reactions. Conclusions The European Anaphylaxis Registry confirmed food as the major elicitor of anaphylaxis in children, specifically hen's egg, cow's milk, and nuts. Reactions to insect venom were seen more in young adulthood. Intensive care unit admissions and grade IV/fatal reactions were rare. The registry will serve as a systematic foundation for a continuous description of this multiform condition. © 2016 American Academy of Allergy, Asthma & Immunology.

Abstract

Dermatophytes are common and cause important human fungal infections in many parts of the world, including Lebanon. The prevalence of these fungi, however, tends to vary with time and geographic location. In our region, studies on the prevalence of dermatophytoses and the distribution of the various dermatophyte species involved are rare. In Lebanon, only one study was published on this subject over the last forty years.This study was undertaken to shed light on the types and prevalence of dermatophytes recovered at the American University of Beirut Medical Center (AUBMC), and to compare the findings with those of the only study from the same hospital published in the early 1960s.Records from the clinical microbiology section were reviewed for the patients on whom dermatophyte cultures were requested between 1996 and 2002. All positive cultures were identified and analyzed.Among 1631 submitted specimens for culture (one per patient), 208 (12.7%) yielded 11 species of dermatophytes, dominated by Trichophyton spp. (89.9%), followed by Microsporum spp. (9.1%), Epidermophyton floccosum (0.4%) and Trichosporum beigelii (0.4%). The male to female ratio was almost 1:1, and the age range was 1 to 77 yrs for both sexes; 14% were children (or = 15 yrs). The most commonly recovered species were: T. tonsurans (54.8%), T. mentagrophytes (24.5%), M. canis (7.7%), T. rubrum (5.3%) and T. verrucosum (4%). The distribution of infection according to body sites was : tinea unguium (44.2%), tinea corporis (43.2%), tinea capitis (7.7%) and tinea pedis (4.8%). Compared to the previous study from AUBMC in 1962, the current study showed an overall lower prevalence of dermatophytoses (12.7 vs 18.5%, p = 0.001) and variations in the prevalence rates of the dermatophyte species involved (dominated by T. tonsurans and T. mentagrophytes in the current study compared to E. floccosum and T. rubrum recovered in 1962). Based on the source of infection the current study showed higher transmission from human to human (anthropophylic) (86.5 vs 66.3%) and animal to human (zoophylic) (12.5 vs 1.1 %), but lower transmission from soil to human (geophylic) (1% vs 32.6%).The prevalence of dermatophytoses remains high in this country. The recognition of the changing prevalence in the causative dermatophyte species should help with the treatment approach and for potential implementation of control measures.

Background: Coronavirus disease-2019 (COVID-19), caused by the severe acute respiratory distress syndrome–coronavirus-2 (SARS-CoV-2), is primarily a respiratory infection but has been recently associated with a variety of neurological symptoms. We present herewith a COVID-19 case manifesting as opsoclonus-myoclonus syndrome (OMS), a rare neurological disorder.Case Presentation: A 63-year-old male diagnosed with COVID-19 infection developed behavioral changes, confusion, and insomnia followed by reduced mobility and abnormal eye movements within 48 h of recovery from respiratory symptoms associated with COVID-19. On examination, he had rapid, chaotic, involuntary saccadic, multidirectional eye movements (opsoclonus), and limb myoclonus together with truncal ataxia. CSF analysis, MRI of the brain, and screening for anti-neuronal and encephalitis related antibodies were negative. Extensive testing revealed no underlying malignancy. The patient was successfully treated with intravenous immunoglobulin (IVIG) with complete resolution of symptoms within 4 weeks of treatment.Conclusion: COVID-19 infection can be associated with the manifestation of opsoclonus myoclonus syndrome, a rare neurological disorder that can be treated with IVIG if not responsive to corticosteroids.

Abstract

Ducted wind turbines are a kind of small wind turbine having a diffuser or any other shape around the rotor which increases the air flow through the blades and absorbs more power. In the present study, a small wind turbine was ducted with a relatively simple ring and its performance was investigated in a wind tunnel. The duct is shaped using rolling steel sheets on a sloping surface and finally fabricated in double-glazed surfaces. The turbine utilizes polyester resin glass fiber-armed composite hollow blades. Bare turbine produces 165 watts in its highest power generation mode which can reach 282 watts when it is ducted. The evaluation of the system in the wind tunnel showed that the power generation of the ducted system compared to a conventional turbine was 14 % higher on average. Furthermore, the rotor speed of the ducted turbine was 45 % higher than the bare one which increases the tip speed ratio (TSR). In this study, TSR increment raised the absorbed power in the developed wind turbine.

Abstract

Chronic low back pain (LBP) ranks among the most common reasons for patient visits to healthcare providers. Drug treatments often provide only partial pain relief and are associated with considerable side-effects. J-2156 [(1’S,2S)-4amino-N-(1’-carbamoyl-2’-phenylethyl)-2-(4”-methyl-1”-naphthalenesulfonylamino)butanamide] is an agonist that binds with nanomolar affinity to the rat and human somatostatin receptor type 4 (SST4 receptor). Hence, our aim was to assess the efficacy of J-2156 for relief of chronic mechanical LBP in a rat model. Male Sprague Dawley rats were anaesthetised and their lumbar L4/L5 and L5/L6 intervertebral discs (IVDs) were punctured (0.5 mm outer diameter, 2 mm-deep) 10 times per disc. Sham-rats underwent similar surgery, but without disc puncture. For LBP-rats, noxious pressure hyperalgesia developed in the lumbar axial deep tissues from day 7 to day 21 post-surgery, which was maintained until study completion. Importantly, mechanical hyperalgesia did not develop in the lumbar axial deep tissues of sham-rats. In LBP-rats, single intraperitoneal (i.p.) injection of J-2156 (3, 10, 30 mg kg-1) alleviated primary and secondary hyperalgesia in the lumbar axial deep tissues at L4/L5 and L1, respectively. This was accompanied by a reduction in the otherwise augmented lumbar (L4–L6) dorsal root ganglia expression levels of the pro-nociceptive mediators: phosphorylated p38 (pp38) mitogen-activated protein kinase (MAPK) and phosphorylated p44/p42 MAPK and a reduction in pp38 MAPK in the lumbar enlargement of the spinal cord. The SST4 receptor is worthy of further investigation as a target for discovery of novel analgesics for the relief of chronic LBP.
