2. Complicaciones a largo plazo en pacientes geriátricos con diabetes mellitus tipo 2

Objetivo: determinar las principales complicaciones asociados a la Diabetes Mellitus tipo 2 en pacientes geriátricos de una casa de salud de la ciudad de Riobamba. Método: investigación de tipo descriptiva, transversal y retrospectiva, se recolectaron los datos de las historias clínicas a una población de adultos geriátricos mayores a 60 años de una casa de salud en la ciudad de Riobamba. Resultados: 56% de los participantes fueron mujeres con diagnóstico de diabetes mellitus tipo 2. La media de edad fue de 70.8 años, aunque se consideró una edad mayor de 80 años para el desarrollo de complicaciones como cardiopatías, retinopatías y nefropatías. La nefropatía fue la complicación crónica más común, con una prevalencia del 26.7% en adultos mayores con diabetes. Además, se encontró que el 46.7% de los pacientes requerían insulinoterapia como parte de su tratamiento, mientras que solo el 0.8% dependía de intervenciones no farmacológicas, como dieta y ejercicio. Conclusiones: existió una alta prevalencia de diabetes mellitus tipo 2 en mujeres mayores de 70 años, con la nefropatía como la complicación crónica más común. El elevado porcentaje de pacientes que requieren insulinoterapia resalta la importancia de un enfoque farmacológico en el manejo de la enfermedad. Estos hallazgos subrayan la necesidad de intervenciones específicas y protocolos de tratamiento adaptados a las características de la población de adultos mayores con diabetes para prevenir y controlar eficazmente las complicaciones asociadas.

Urinary epidermal growth factor (uEGF) is primarily produced by the kidney, and alterations of it have been associated with several kidney diseases. The aim of this review was to describe uEGF levels in presence or progression of kidney diseases. We conducted a systematic review of observational studies with uEGF determination, patients with acute kidney injury, chronic kidney disease, primary or secondary nephropathy, or renal cancer were included. Studies were searched in Medline, Google Scholar, Science Direct, and EBSCO up to August 2, 2021. Participants and measurements characteristics from which uEGF were determined as the specificity, sensitivity, and the area under the ROC curve, whenever available, were gathered. 53 studies were included, the most frequent kidney diseases studied were acute kidney injury, chronic kidney disease, and diabetic nephropathy. In most studies, uEGF levels were lower in cases than in controls. Studies showed that uEGF levels can predict presence or progression of acute kidney injury, chronic kidney disease, and nephropathy. Heterogeneity in the reported uEGF values can be attributed to the different techniques, sampling, and ways of reporting uEGF values.Although uEGF values are lower in patients with almost all kidney diseases and their progression, uEGF evaluation methods should be standardised to be used as a biomarker in clinical practice. Resumen: El factor de crecimiento epidérmico urinario (uEGF) es producido principalmente por el riñón, y sus alteraciones se han asociado con varias enfermedades renales. El objetivo de esta revisión fue describir los niveles de uEGF en presencia o progresión de enfermedades renales. Realizamos una revisión sistemática de estudios observacionales con determinación de uEGF en la que se incluyeron pacientes con insuficiencia renal aguda, enfermedad renal crónica, nefropatía primaria o secundaria, o cáncer renal. Se realizaron búsquedas de estudios en Medline, Google Scholar, Science Direct y EBSCO hasta el 2 de agosto de 2021. Se extrajeron las características de los participantes y de las mediciones del uEGF, así como la especificidad, la sensibilidad y el área bajo la curva ROC, siempre que estuvieran disponibles. Se incluyeron 53 estudios, y las enfermedades renales más frecuentes estudiadas fueron la insuficiencia renal aguda, la enfermedad renal crónica y la nefropatía diabética.En la mayoría de los estudios los niveles de uEGF fueron más bajos en los casos que en los controles. Los estudios demostraron que los niveles de uEGF pueden predecir la presencia o la progresión de la lesión renal aguda, la enfermedad renal crónica y la nefropatía. La heterogeneidad en los valores de uEGF informados se puede atribuir a las diferentes técnicas, muestreo y formas de informar los valores de uEGF.Aunque los valores de uEGF son más bajos en pacientes con casi todas las enfermedades renales y su progresión, los métodos de evaluación de uEGF deben estandarizarse para ser utilizados como biomarcadores en la práctica clínica.

The protein concentration in urine is a valuable parameter in assessing kidney function. Usually, blood proteins do not pass through the renal filter in healthy subjects; proteinuria, which can sometimes be detected as a simple, occasional laboratory finding, is a relatively common condition of which those affected are unaware. Epidemiologically, it is a condition that involves almost 10% of the population worldwide. The mechanisms underlying the release of proteins by the kidneys are different: a newly detected proteinuria may precede the onset of renal disease or systemic diseases, and the physician must appropriately evaluate it. This paper provides a hint of epidemiology, some notes of pathophysiology, an overview of the types of proteinuria, a rundown on laboratory techniques and a brief, certainly not exhaustive, review of some non-oncological pathologies associated with proteinuria. The aim is to show the extent to which proteinuria can play a decisive role in the patient's overall assessment. The urine protein test, a basic, non-invasive test, easy to perform by any laboratory, even the most poorly equipped, is a fundamental indicator of a subject's health status in combination with other markers. Thus, it confirms once again the crucial role of the laboratory in providing the physician with necessary diagnostic and prognostic data that is easy to perform, inexpensive and yet of great benefit for the patient management. 10. Identificación de genes asociados con Nefropatía Diabética regulados por miRNAS: Análisis in sílico.

- Abstract
Introduction. Albuminuria greater than 300mg/dL/24h (A1-300mg/g) is a characteristic of diabetic nephropathy (DN), which can trigger the development of advanced chronic kidney disease (ACKD). Objective. Identify genes regulated by miRNAs that are associated with DN through an in-silico analysis. Material and methods. Through the use of microarrays and bioinformatic analysis, potential miRNA target genes were identified: hsa-miR-126-3p, miR-320a-3p, and miR-1288-3p. These genes were subjected to signaling pathway analysis to identify processes associated with DN pathogenesis. Results. 57 target genes were identified from the analyzed miRNAs, which were associated with 14 genetic ontologies and 7 KEGG signaling pathways. These results allowed the generation of an in-silico model showing an interaction network between target genes regulated by miRNAs whose alteration can lead to the development of ND. Conclusions. In the in-silico model, the network of interactions found between target genes regulated by miRNAs could contribute to the understanding of the DN mechanism, opening the panorama for new research on genes and miRNAs that could be evaluated as markers in the early detection of DN. El diagnóstico precoz de las complicaciones microvasculares de la diabetes mellitus (DM) nos permite realizar un tratamiento temprano y prevenir su progresión. Existen evidencia que la neuropatía diabética presenta un comienzo insidioso, que es previo al incremento de la glucemia, pero además concurren otros factores del síndrome metabólico como la dislipidemia, la obesidad y la hipertensión, las cuales impactan sobre el trofismo y la irrigación del nervio1,2. En relación a la nefropatía, sabemos la importancia de la hiperglucemia en el desarrollo de esta complicación, inclusive en la disglucemia considerándola a partir de 100 mg/dl3, todo esto sin dejar de tener en cuenta los factores genéticos, epigenéticos e inflamatorios que influyen en la progresión de la nefropatía. En conclusión, es imprescindible resaltar la importancia de la evaluación exhaustiva de nuestros pacientes con síndrome metabólico, con y sin disglucemia, para detectar precozmente las complicaciones microvasculares.

COVID-19 most related glomerular disease to date seems to be collapsing glomerulopathy, mostly in young Afroamerican patients with APOL1 gene risk alleles. However, in our population, predominant in elderly Caucasian patients, most biopsied pathology since the beginning of the pandemic has been IgA nephritis or Schönlein-Henoch purpura.Since the description of the first case of this entity after SARS-CoV-2 infection by our research group, three more cases have arisen, which are described in the following article. In contrast to the rest of IgA vasculitis cases reported, our patients presented more renal function deterioration and all of them required immunosupresive therapy. Moreover, some showed incomplete recovery of renal function.This case series strengthens the hypothesis that SARS-CoV-2 infection may be another trigger of this pathology. Resumen: La patología glomerular más relacionada con enfermedad COVID-19 hasta la fecha parece ser la glomerulopatía colapsante, principalmente en pacientes de raza afroamericana y con alelos de riesgo para el gen APOL1. No obstante, en nuestra población, conformada por pacientes añosos de raza caucásica, la patología más biopsiada desde el inicio de la pandemia ha sido la nefritis IgA o púrpura de Schönlein-Henoch.Desde la descripción del primer caso de esta entidad tras infección por SARS-CoV-2 por nuestro grupo de investigación hemos objetivado otros tres, los cuales se describen a continuación. En contraste con el resto de los casos publicados de vasculitis IgA, nuestros pacientes presentaban mayor deterioro de función renal y todos requirieron tratamiento inmunosupresor. Además, algunos presentaron recuperación incompleta de función renal. Esta serie de casos afianza la posibilidad de que la infección por SARS-CoV-2 sea un desencadenante más de esta patología.

A dictofimose é uma doença causada pelo nematódeo Dioctophyma renale e caracteriza-se por um achado relativamente comum em cães oriundos de zona rural, na maioria das vezes, como um quadro assintomático. O parasitismo normalmente acomete o rim direito, por conta de sua proximidade ao duodeno, porém, há relatos literários indicando o acometimento do rim contralateral e de localizações ectópicas. A epidemiologia do parasitismo pelo nematódeo envolve um ciclo evolutivo complexo, podendo inclusive ter o homem como hospedeiro acidental. O tratamento mais eficaz é através da remoção cirúrgica do parasita e retirada do rim afetado. Populações caninas que habitam beiras de rios, lagos e outros habitats aquáticos, possuem maior susceptibilidade a infecção, devido ao maior contato e possibilidade de ingestão de hospedeiros paratêmicos como peixes e rãs. Desta forma, o conhecimento desta enfermidade por Médicos Veterinários, deve ser incluído no diagnóstico diferencial de causas de nefropatias.

17. Prevalencia de daño renal previo al diagnóstico de enfermedad de Fabry en Argentina.

Kidney damage is one of the major complications of Fabry disease and its clinical presentation is proteinuria and a progressive decrease in glomerular filtration rate. The frequency of renal damage prior to the diagnosis of Fabry disease has been reported in the order of 8.7% for men with "classic" Fabry disease, 3% for men with "non-classic" Fabry disease, 1.4% for women with "classic" Fabry disease and 0.7% in women with "non-classic" Fabry disease, in patients from Germany, the United Kingdom and the Netherlands, with no information on this in our country to date. Objective: To evaluate the frequency of kidney damage prior to the diagnosis of Fabry disease in Argentina. Material and methods: Cross-sectional epidemiological study conducted in three centers in Argentina. Results: Data from the first consultation of 72 patients with Fabry disease were included; 44 (61.1%) patients had kidney damage prior to diagnosis. The independent variables that correlated with statistical significance to "previous kidney damage" were age (p=<0.001), genotype (p=0.009) and being diagnosed in adulthood (p=<0.001). No correlation was found between previous kidney damage and the following independent variables: gender (p=0.421), being an "index case" (p=0.139), type of variant (classic/late) of Fabry disease (p=0.107) and the enzymatic activity of the lysosomal enzyme a-galactosidase-A (p=0.916). Conclusions: Kidney damage frequently occurs prior to diagnosis in patients affected by Fabry disease. In the presence of manifest nephropathy due to a drop in the estimated glomerular filtration rate or proteinuria of unknown cause, Fabry disease should be considered as a differential diagnosis in patients of both genders, both pediatric and adult. OBJETIVOS: Vários estudos associam a infecção crônica pelo vírus da hepatite C a manifestações extra-hepáticas, incluindo glomerulopatias. O objetivo deste artigo é fazer uma breve revisão da literatura sobre as manifestações renais da hepatite C e apresentar os resultados sumarizados de estudo que buscou avaliar a associação de alterações urinárias assintomáticas em doadores de sangue com anticorpos antiVHCHC positivo. MATERIAIS E MÉTODOS: Realizou-se um estudo transversal, contemporâneo, controlado e aberto no período de julho de 1997 a março de 1998 no Serviço de Nefrologia do HCPA para avaliar a prevalência de alterações urinárias assintomáticas em portadores do vírus da hepatite C. Foram incluídos no estudo os 58 doadores de sangue VHC+ e 128 doadores VHC- (grupo controle) que não apresentavam história de doença hepática ou renal ou outros marcadores virais positivos. Avaliou-se a prevalência de hematúria (> 5 hemácias/campo de grande aumento no E.Q.U), proteinúria [uma (+) no exame por fita do E.Q.U], microalbuminúria (índice microalbuminúria/creatininúria em amostra isolada > 15,6 mg/g), N-acetil-beta-D-glucosaminidase (> 5,6U/g creatinina). RESULTADOS: Não foi constatada anormalidade urinária significativa no grupo VHC+, comparativamente com o grupo controle. A prevalência de hematúria e proteinúria nos grupos VHC+ e VHC- foi respectivamente 12,0% x 9,4% e 5,4% x 5,5% , sem diferença estatística entre os 2 grupos. CONCLUSÃO: A prevalência de hematúria foi semelhante à da população geral, que oscila entre 3-20%.

Diabetic nephropathy is a chronic complication of diabetes mellitus, highly prevalent worldwide and nationally, it is characterized by decreased glomerular filtration and is the product of pathophysiological mechanisms that generate changes at the micro and macroscopic level at the kidney, causing irreparable damage in advanced stages. The objective of this review is to describe the risk factors for diabetic nephropathy in adults, based on updated literature, referring to evidence-based medicine. A bibliographic review was carried out, using data bases like Pubmed, Scielo, Google Scholar, Elsevier, Medigraphic. The results of this research showed that the main risk factors are poorly controlled hyperglycemia and arterial hypertension, evolving to chronic renal failure, with significant morbidity and mortality, so its early diagnosis is of vital importance to avoid its appearance. The most used biomarkers for its detection are serum creatinine, microalbuminuria and glomerular filtration rate. Um achado hematológico comumente importante em pacientes veterinários com doença renal crônica (DRC) em estágios mais avançados é a anemia normocítica, normocrômica e não regenerativa, que acarreta inúmeros sinais clínicos, dentre eles: fraqueza muscular, letargia, anorexia e perda de peso. Sabe-se que vários fatores da DRC levam a anemia nos animais, dentre eles estão a baixa síntese do hormônio eritropoietina pelos rins, redução da meia vida das hemácias em consequência de um quadro urêmico e aumento do estresse oxidativo, deficiências nutricionais de substâncias envolvidas na eritropoiese (devido anorexia) e ulcerações de mucosas que levam a perda de sangue. O objetivo deste trabalho é relatar o caso de uma paciente canina, diagnosticada com doença renal crônica a qual apresentava um quadro de anemia normocítica, normocrômica e não regenerativa, onde optou-se pela instituição do tratamento com eritropoietina recombinante humana obtendo-se efeitos hematológicos benéficos, bem como melhora na qualidade de vida

Objetivo: Determinar a prevalência da nefropatia induzida por contraste em pacientes cardiopatas submetidos a procedimentos angiográficos de diagnóstico e/ou tratamento. Método: Estudo prospectivo, quantitativo, realizado no setor de hemodinâmica de um hospital de grande porte, situado na região norte do Rio Grande do Sul, Brasil. A amostra foi constituída por 79 participantes através do cálculo de tamanho amostral. Resultados: A amostra foi formada por 52 (65,8%) homens e 27 (34,2%) mulheres. A idade média foi de 65,9 ± 9,52 anos. A incidência de nefropatia induzida por contraste foi de 30,38%, totalizando 24 pacientes. Conclusão: Foi evidenciada uma alta prevalência de nefropatia por contraste, apesar dos pacientes apresentarem poucos fatores de risco, o que ressalta a necessidade de medidas preventivas e redução do volume de contraste.

Background and Objective: Extracellular vesicles (EV) reflect the pathophysiological state of their cells of origin and are a reservoir of renal information accessible in urine. When biopsy is not an option, EV present themselves as sentinels of function and damage, providing a non-invasive approach. However, the analysis of EV in urine requires prior isolation, which slows down and hinders transition into clinical practice. The aim of this study is to show the applicability of the "single particle interferometric reflectance imaging sensor" (SP-IRIS) technology through the ExoView® platform for the direct analysis of urine EV and proteins involved in renal function., Materials and Methods: The ExoView® technology enables the quantification and phenotyping of EV present in urine and the quantification of their membrane and internal proteins. We have applied this technology to the quantification of urinary EV and their proteins with renal tubular expression, amnionless (AMN) and secreted frizzled-related protein 1 (SFRP1), using only 5 μl of urine. Tubular expression was confirmed by immunohistochemistry., Results: The mean size of the EV analysed was 59 ± 16 nm for those captured by tetraspanin CD63, 61 ± 16 nm for those captured by tetraspanin CD81, and 59 ± 10 for tetraspanin CD9, with CD63 being the majority EV subpopulation in urine (48.92%). The distribution of AMN and SFRP1 in the three capture tetraspanins turned out to be similar for both proteins, being expressed mainly in CD63 (48.23% for AMN and 52.1% for SFRP1)., Conclusions: This work demonstrates the applicability and advantages of the ExoView® technique for the direct analysis of urine EV and their protein content in relation to the renal tubule. Objective: To analyze the association between the polymorphism rs2291166 of TJP1 gene, with the urine-excreted levels of GAGS as a marker of early stages of nephropathy. Methods: A 600 urine samples from newly diagnosed subjects with type 2 diabetes were analyzed, of which 203 were included. The GAGS detection in direct urine (corresponding to the first urine of the morning), was performed by albumin turbidity test and precipitation with cetylpyridinium (CPC). Results: The present study shows that 26.64% of diabetic patients are in early stages of nephropathy, corresponding to patients with a positive GAG test, being those with the highest GAGS excretion, heterozygous for the polymorphism. Conclusion: We suggest that the TJP1 polymorphism rs2291166 influences mucopolysacchariduria in type 2 diabetic patients of the Mexican population, which could be used as a valid genetic/biochemical marker for the early stages of diabetic nephropathy. A distocia em psitacideos é comum, sendo definida como uma obstrução do trato reprodutivo por um ou mais ovos, devido a problemas em sua formação, como também ser influenciada fatores como os físicos, nutricionais, e genéticos dentre outros. O presente trabalho objetivou relatar o caso clínico de uma calopsita (Nymphicus hollandicus) fêmea com aproximadamente dois anos de idade apresentando complicações isquêmicas renais secundárias à prolongada retenção de ovo, atendida no Hospital Veterinário Metropolitano do município de Caucaia - Ceará, diagnosticada com distocia após anamnese, exame físico e radiografia abdominal. Após confirmação da afecção, foi realizado tratamento de suporte seguido de ovocentese, no entanto, o paciente vindo a óbito 48h. Na necropsia, observo, e quadro de comprometimento metabólico em virtude do acúmulo dos ovos na cavidade, favorecendo a ruptura de oviduto, aderência e isquemia de outros órgãos.

Background: Renal dysplasia is a congenital disorder that occurs during differentiation of the renal parenchyma or as a consequence of a functional and/or structural obstruction of the lower urinary tract. In wild animals, this pathology has been reported in cynomolgus monkeys (Macaca fascicularis), golden hamsters (Mesocricetus auratus) and African elephants (Loxodanta africana). However, there are no reports of the disease in the giant anteater (Myrmecophaga tridactyla). Thus, this paper describes a case of renal dysplasia in a free-living giant anteater cub, which was sent to the wild animal clinic of the Federal University of Mato Grosso (UFMT) in Cuiabá, MT, Brazil. Case: The rescued animal had a good body condition score and clinical parameters within the normal range for the species. After a routine clinical evaluation, the anteater cub was subjected to radiography and ultrasound tests. Blood tests, serum tests for hepatic and renal profiles, urinalysis, urinary protein creatinine ratio, and chest X-rays did not reveal significant changes. However, the abdominal ultrasound examination revealed a volumetric loss of about 1.17 cm in length in the left kidney, and a renal length to aortic artery diameter ratio of approximately 2.8. This kidney showed irregular contours, loss of corticomedullary demarcation, with preserved echogenicity and cortical echotexture. The right kidney showed the standard size of the species, with a length of approximately 3.08 cm. In view of the suspicion of renal dysplasia, a contrast-enhanced CT scan was performed in order to assess the dynamics of uptake and excretion of the contrast medium in the affected kidney and in the ipsilateral collecting system. An examination of the tomographic images indicated that the volume of the left kidney was reduced, isodense in relation to the right kidney, with discrete and homogeneous uptake in all phases after administration of the contrast medium, no occurrence of nephrogram and pyelogram phases, or any detection of contrast in the corresponding ureter. Discussion: Giant anteaters (Myrmecophaga tridactyla) are animals classified as a species vulnerable to extinction. Today, most research involving this species focuses on its ecology, behavior, diet, morphology and parasitology, but little is known about the imaging aspects of the species or about congenital changes such as renal dysplasia. Although the definitive diagnosis of this pathology depends on a histopathological examination, the same diagnosis can be made with a wide margin of safety by assessing the epidemiological aspects and the dynamics of renal uptake of the contrast medium through computed tomography. In this analysis, both vascularization and renal filtration capacity can be assessed. Thus, based on CT imaging, it was concluded that this was a case of renal dysplasia, since the left kidney showed a discrete homogeneous uptake stable in both the arterial and venous phases, without producing any accumulation of contrast medium in the pelvic region or the collecting system, proving to be completely nonfunctional. These findings differ from cases of renal hypoplasia, which, although they reduce renal volume, do not cause structural changes in the renal parenchyma or disturbances in the filtration dynamics of contrast media. They also differ from cases of acquired chronic nephropathy, since, albeit associated with reduced renal volume and changes in renal filtration dynamics, they produce different parenchymal ultrasound changes that usually occur in elderly animals and generally produce bilateral lesions. Numerosas condições patológicas associadas à gestação, constituem fatores de risco que podem resultar em possíveis complicações materno-fetais. Nesse contexto, o presente estudo observacional descritivo, apresenta o caso de uma paciente de 24 anos G1P1C1A0, portadora de tumor pancreático, nefropatia membranosa, hipertensão arterial crônica, diabetes mellitus 2, dislipidemia, esteatose hepática e infecção urinária de repetição. Os dados relacionados ao presente estudo foram coletados por meio de análise de prontuários e dos exames laboratoriais e de imagem, após autorização do Comitê de Ética (CAAE: 59909122.4.0000.5076). Com ajuste da terapêutica farmacológica e orientações sobre a alimentação adequada e atividades físicas, a gestação cursou sem complicações até a 36ª semana e 1 dia, quando a paciente deu entrada na emergência obstétrica da Santa Casa de Misericórdia de Anápolis-GO em iminência de eclâmpsia. Na ocasião fora realizado o parto cesáreo sem intercorrências. O recém-nascido do sexo feminino, classificado como pré-termo tardio, apresentou peso de adequado à idade gestacional, valores de APGAR de 7 (1º minuto) e 8 (5 º minuto), sem malformações aparentes. Portanto, nota-se que a implementação de práticas oportunas e adequadas, baseadas em evidências, permite a redução da mortalidade materna e fetal.

Patients on hemodialysis often have more urinary tract infections (UTIs) due to immunosuppression, lack of germ removal, presence of lithiasis, neurogenic bladder, abnormalities in the urinary tract, use of intravenous catheters and urinary catheters. The antibiotics used in the treatment of UTIs in the general population can be used in hemodialysis patients with the corresponding adjustment of dose. UTI is the most common bacterial infection in kidney transplant recipients due to factors such as immunosuppressive therapy, UTIs prior to transplantation, vesicoureteral reflux, polycystic kidney disease, diabetes, prolonged use of urinary catheters, delay in graft function, episodes of acute rejection, recurrent asymptomatic bacteriuria and lack of antibiotic prophylaxis. They occur mainly in the immediate post-surgery period and the first months after transplant. The recurrence of UTIs or AB episodes can trigger acute rejection.

This study was set to investigate the effect of gum Arabic (G.A.) on diabetic kidney disease. We divided sixty male Sprague rats randomly into six groups. Normal control, normal rats treated with G.A., untreated diabetic rats, diabetic rats treated with insulin, diabetic rats treated with G.A., and diabetic rats treated with both insulin and G.A. Diabetes was induced by a single intraperitoneal injection of STZ. Forty eight hr post injections. Insulin was injected subcutaneously (1.6/IU/100g/day). We provided G.A. in drinking water (10 %w/v).). At the end of the twelve weeks, blood was drawn for measurement of blood glucose, glycosylated hemoglobin (HbA1C), serum lipids, serum creatinine, and blood urea. Renal tissue oxidative stress (O.S.) was assessed by measuring the activities of superoxide dismutase (SOD) and catalase (CAT), and the concentrations of reduced glutathione (GSH) and malondialdehyde (MDA). For histological assessments, sections from segments of kidneys were processed and stained with hematoxylin and eosin (H&E) for assessment under the light microscope. STZinduced diabetes caused an elevation of blood glucose, HbA1c, urea and creatinine, triglycerides LDL and cholesterol, MDA with reduction of HDL, GSH level, and CAT and SOD activities. Histologically, kidneys from diabetic rats showed marked glomerular and tubular changes. Administration of G.A. alone to diabetic rats had a significant hypoglycemic, hypolipidemic, and antioxidant effect, although the levels achieved remained significantly abnormal compared with the untreated group with no effect on urea and creatinine levels. Co-administration of G.A. with insulin reversed the impact of D.M. on all parameters evaluated including the histological changes and led to normal urea and creatinine levels. We concluded that G.A., in combination with insulin, improves chemically-induced diabetes and its renal complications, possibly by modulation of oxidative stress. [ABSTRACT FROM AUTHOR]

Objetivo: Caracterizar la nefropatía inducida por medios de contraste en pacientes hospitalizados. Me-todología: Investigación transversal, cuyos datos se obtuvieron de fuente secundaria; realizada en 534 pacientes mayores de 18 años que requirieron procedimientos o estudios diagnósticos con medios de contraste en una Clínica de 4° nivel (Clínica General del Norte. Colombia), durante el periodo enero a junio de 2017; se tuvo en cuenta el valor de creatinina tanto inicial como el posterior a aplicación del contraste, cantidad y osmolalidad del medio de contraste, estadío de la Insuficiencia Renal Crónica antes y después del procedimiento, tipo de procedimiento y comorbilidades. Resultados: Predominó el género masculino (55%); la mayoría fueron adultos mayores (45,5%); el NIC estuvo presente en 2,8 % de los pacientes y su frecuencia fue mas alta en mujeres (66,6%); los casos de NIC en procedimientos intervencionistas duplicaron a los imagenológicos (10 vs 5) y el volumen de medio de contraste utilizado fue < 100 mL; el 86,66% de los NIC se presentó en pacientes bajo nefroprotección con solución salina; la mayoría de los pacientes (66,6%), incrementaron su estadío de Enfermedad Renal Crónica inmediatamente después de NIC; en cuanto a las comorbilidades, 10, presentaron entre tres y cuatro, y uno tuvo 5, las más frecuentes fueron las neoplasias y la anemia. Estos resultados son similares a los de otros autores. Conclusión: Tanto los procedimientos diagnósticos como los intervencionistas con medios de contraste, son relativamente seguros; la edad avanzada y el género femenino aumentan la probabilidad de desarrollar NIC

[ES] La actividad de innovación docente que se presenta se enmarca dentro del proyecto NutClinROLE con financiación del "Vicerrectorado de Formación Permanente, Transformació Docent i Ocupació" de la Universitat de València, (UV-SFPIE_PID-2075858). Se basa en la realización de entrenamientos de simulación en nutrición, como estrategia metodológica innovadora para la enseñanza de las ciencias de la nutrición y la dietética en la educación superior. El objetivo de este trabajo fue realizar unos vídeos de actividades de simulación para aprender y practicar habilidades en entrevistas nutricionales mediante el trabajo de casos clínicos. Los casos clínicos que se elaboraron en la simulación fueron sobre pacientes con enfermedad renal. Los alumnos trabajaron tras el visionado de los vídeos, estableciendo roles de paciente y nutricionista para la resolución de otros casos clínicos en nutrición. Durante la simulación y mediante unas fichas que incorporan información sobre el paciente y su patología, incluyendo datos antropométricos y analíticos, el estudiante-nutricionista debe desarrollar la anamnesis clínico-dietética, planificar la intervención nutricional y realizar la pauta dietética al estudiante-paciente. Durante la capacitación, los estudiantes adquirieron conocimientos sobre insuficiencia renal crónica, y pusieron en práctica entrevistas nutricionales y recomendaciones dietéticas. Se evaluó el impacto de la metodología de juego de roles en base a sus opiniones antes y después de la actividad, reportando una experiencia positiva y mayor conocimiento en nutrición. En cuanto a la resolución del caso clínico, se ha observado que el alumno tiene los conocimientos suficientes para resolverlo, pero presenta deficiencias en la organización de la información y habilidades comunicativas., [EN] The teaching innovation activity that is presented is part of the NutClinROLE project with financial support from the "Vicerectorado de Formación Permanente, Transformació Docent i Ocupació" of the University of Valencia, (UV-SFPIE_PID-2075858). It is based on carrying out simulation training in nutrition, as an innovative methodological strategy for teaching nutrition sciences and dietetics in higher education. The objective of this work was to make videos of simulation in clinical nutrition to learn and practice skills in nutritional interviews. The clinical cases that were developed in the simulation were about patients with kidney disease. The students worked after viewing the videos, establishing the roles of patient and nutritionist for the resolution of other clinical cases in nutrition. During the activity, it was incorporated information about the patient and his pathology, including anthropometric and analytical data, however the nutritionist does not have this information and must solve and guide the patient nutritionally during the simulation. During the training, the students acquired knowledge about chronic renal failure, and put into practice nutritional interviews and dietary recommendations. The impact of the role-play methodology was evaluated based on their opinions before and after the activity. It was reported a positive experience and greater knowledge in nutrition. Regarding the resolution of the clinical case, it has been observed that the student has sufficient knowledge to solve it, but presents deficiencies in the organization of information and communication skills.

40. Identificación de genes asociados con Nefropatía Diabética regulados por miRNAS: Análisis in sílico.

Introduction. Albuminuria greater than 300mg/dL/24h (A1-300mg/g) is a characteristic of diabetic nephropathy (DN), which can trigger the development of advanced chronic kidney disease (ACKD). Objective. Identify genes regulated by miRNAs that are associated with DN through an in-silico analysis. Material and methods. Through the use of microarrays and bioinformatic analysis, potential miRNA target genes were identified: hsa-miR-126-3p, miR-320a-3p, and miR-1288-3p. These genes were subjected to signaling pathway analysis to identify processes associated with DN pathogenesis. Results. 57 target genes were identified from the analyzed miRNAs, which were associated with 14 genetic ontologies and 7 KEGG signaling pathways. These results allowed the generation of an in-silico model showing an interaction network between target genes regulated by miRNAs whose alteration can lead to the development of ND. Conclusions. In the in-silico model, the network of interactions found between target genes regulated by miRNAs could contribute to the understanding of the DN mechanism, opening the panorama for new research on genes and miRNAs that could be evaluated as markers in the early detection of DN., Introducción. Albuminuria superior a 300 mg/dL/24h (A1-300mg/g) es una característica de la nefropatía diabética (ND), lo que puede desencadenar en el desarrollo de enfermedad renal crónica avanzada (ERCA). Objetivo. Identificar genes regulados por miRNAs asociados con ND a través de un análisis in sílico. Material y Métodos. A través del uso de microarreglos y análisis bioinformáticos se identificaron potenciales genes blancos de los miRNAs; hsa-miR-126-3p, miR-320a-3p y miR-1288-3p. Estos genes fueron sometidos a un análisis de vías de señalización para identificar procesos asociados con la patogénesis de la ND. Resultados. Se identificaron 57 genes blanco de los miRNAs analizados, los cuales fueron asociados con 14 ontologías genéticas y 7 vías de señalización KEGG. Estos resultados permitieron generar un modelo in sílico en el que se muestra una red de interacción entre genes blanco regulados por miRNAs cuya alteración puede conducir al desarrollo de la ND. Conclusiones. En el modelo in sílico la red de interacciones encontradas entre genes blancoregulados por miRNAs podrían contribuir a la comprensión del mecanismo de la ND abriendo el panorama para realizar nuevas investigaciones sobre genes y miRNAs que podrían ser evaluados como marcadores en la detección temprana de la ND

Presentamos un caso de coroidopatía lúpica bilateral en paciente femenino de 22 años de edad con diagnóstico de lupus eritematoso sistémico, diagnosticado desde hace 6 años, manejada con micofenolato mofetilo y prednisona que suspendió 4 meses previos a la presentación de síntomas oculares. La paciente a su ingreso hospitalario mostró datos de actividad lúpica, refiriendo visión borrosa. Se diagnosticó coroidopatía lúpica en base a hallazgos fluorangiográficos y de tomografía de coherencia óptica de mácula, encontrando retardo en llenado de fase coroidea con zonas de fuga tardía en vasculatura coroidea, así como despegamiento del epitelio pigmentario de la retina. La paciente reinició manejo sistémico a base de prednisona y ciclofosfamida, con mejoría en agudeza visual. Alrededor de un tercio de pacientes con lupus eritematoso sistémico tienen afectación a nivel oftálmico que va desde la manifestación más frecuente que es el síndrome de ojo seco, hasta aquellas que afectan severamente la visión, entre ellas, la coroidopatía lúpica, la cual se presenta raramente en el lupus eritematoso sistémico y se ha reportado en únicamente alrededor de 40 pacientes en la literatura.

Resumo Objetivo Identificar os diagnósticos de enfermagem em crianças com doenças renais e analisar a associação entre esses diagnósticos, suas características definidoras e os fatores relacionados ou de risco. Métodos Estudo transversal, quantitativo, realizado com 68 crianças internadas em um Hospital Universitário no Nordeste do Brasil. Para a coleta de dados foi utilizado um roteiro de entrevista e exame físico. A elaboração dos diagnósticos foi processual e os dados foram analisados por meio da estatística descritiva e inferencial. Resultados Identificaram-se 13 diagnósticos. Os mais frequentes tiveram relação estatisticamente significativa com seus componentes e estavam inseridos nos domínios promoção da saúde, eliminação/troca, nutrição, segurança/proteção e atividade/repouso. Conclusão Os diagnósticos de enfermagem mais prevalentes foram volume de líquidos excessivo, risco de infecção, eliminação urinária prejudicada, fadiga e proteção ineficaz. Em geral, as características definidoras e os fatores dos diagnósticos apresentaram associação significante.

As infecções do sistema geniturinário estão entre as mais frequentes na rotina clínica de pequenos animais, sendo fundamental a instituição de terapia antimicrobiana de acordo com o teste de susceptibilidade do uropatógeno, portanto, evitando a disseminação de resistência bacteriana aos antibióticos. Realizou-se um estudo clínico com 32 cães doentes renais crônicos, de ambos os sexos, com idade variada, atendidos no Hospital Veterinário Universitário, submetidos à urocultura com o objetivo de avaliar a infecção urinária, identificar os patógenos bacterianos e verificar sua sensibilidade antibiótica in vitro. A urocultura foi positiva em 10 cães, em sua maioria machos, sem predisposição racial e com idade média de 8,28 anos. A maioria das ITUs foi monobacteriana, sendo os microrganismos mais encontrados: Pseudomonas sp. e Staphylococcus sp. O Imipenem e Meropenem foram os antibióticos com melhor perfil de sensibilidade global e a Ampicilina o de maior resistência. A variação do perfil epidemiológico e da susceptibilidade ao uropatógeno no presente estudo reforça a importância da escolha correta do antibiótico orientada pelo antibiograma, além da necessidade de mais pesquisas em busca de novos agentes antimicrobianos.

La infección por VIH es altamente prevalente a nivel mundial. Las alteraciones renales, tanto agudas como crónicas, son más comunes en enfermos con VIH que en la población general, estando presentes hasta en un 30 % de dichos pacientes. Los pacientes con infección por el VIH pueden desarrollar diversas nefropatías glomerulares, vasculares, tubulointersticiales y obstructivas relacionadas en algunos casos con el propio virus, con los fármacos administrados o con las coinfecciones que presentan. Dado esto es importante la determinación de la prevalencia de la nefropatía en los pacientes portadores de HIV en el centro de salud. Objetivo: determinar la prevalencia de los pacientes con nefropatía en la población portadora de VIH atendidos en el Hospital RACG. Población: pacientes en el servicio de infectología Hospital Rafael Ángel Calderón Guardia Métodos: estudio observacional, retrospectivo. Se documenta una prevalencia de 20% pacientes con enfermedad renal crónica de estadios KDIGO 2 o más; el 2.5 % de la población presento proteinuria significativa sin relación con la carga viral o el conteo de CD4. Se necesita mejorar el control para la prevención y/o el diagnóstico de la nefropatía en el paciente con VIH en el servicio de infectología del H. R. A. C. G.
