Monogenic high bone mass (HBM) disorders are characterized by an increased amount of bone in general, or at specific sites in the skeleton. Here, we describe 59 HBM disorders with 50 known disease-causing genes from the literature, and we provide an overview of the signaling pathways and mechanisms involved in the pathogenesis of these disorders. Based on this, we classify the known HBM genes into HBM (sub)groups according to uniform Gene Ontology (GO) terminology. This classification system may aid in hypothesis generation, for both wet lab experimental design and clinical genetic screening strategies. We discuss how functional genomics can shape discovery of novel HBM genes and/or mechanisms in the future, through implementation of omics assessments in existing and future model systems. Finally, we address strategies to improve gene identification in unsolved HBM cases and highlight the importance for cross-laboratory collaborations encompassing multidisciplinary efforts to transfer knowledge generated at the bench to the clinic., peer-reviewed

ObjectivesEvidence on the current status of gender equity in academic rheumatology in Europe and potential for its improvement is limited. The EULAR convened a task force to obtain empirical evidence on the potential unmet need for support of female rheumatologists, health professionals and non-clinical scientists in academic rheumatology.MethodsThis cross-sectional study comprised three web-based surveys conducted in 2020 among: (1) EULAR scientific member society leaders, (2) EULAR and Emerging EULAR Network (EMEUNET) members and (3) EULAR Council members. Statistics were descriptive with significance testing for male/female responses assessed by χ2 test and t-test.ResultsData from EULAR scientific member societies in 13 countries indicated that there were disproportionately fewer women in academic rheumatology than in clinical rheumatology, and they tended to be under-represented in senior academic roles. From 324 responses of EULAR and EMEUNET members (24 countries), we detected no gender differences in leadership aspirations, self-efficacy in career advancement and work–life integration as well as the share of time spent on research, but there were gender differences in working hours and the levels of perceived gender discrimination and sexual harassment. There were gender differences in the ranking of 7 of 26 factors impacting career advancement and of 8 of 24 potential interventions to aid career advancement.ConclusionsThere are gender differences in career advancement in academic rheumatology. The study informs a EULAR task force developing a framework of potential interventions to accelerate gender-equitable career advancement in academic rheumatology.

Interleukin 6 (IL-6) is a pleiotropic cytokine which has diverse biological activity, with wide-ranging effects on the immune system, homeostasis, and metabolism. It is the most abundant pro-inflammatory cytokine in the synovial joints and sera of patients with active rheumatoid arthritis (RA) and has been found to play a central role in the pathogenesis of the disease, causing joint inflammation and destruction, as well as systemic manifestations. Inhibition of IL-6 signalling has been investigated and developed as a potential treatment strategy for RA. Clinical trials have demonstrated the efficacy and safety of tocilizumab, a humanized monoclonal antibody directed against the IL-6 receptor, showing therapeutic benefit both in early RA and those considered to have refractory disease. The success of tocilizumab has spurred the development of an array of new biologic agents targeting the IL-6 pathway in RA.

Introduction The anti-neutrophil cytoplasmic antibody (ANCA) - associated vasculitides are a heterogeneous group of rare multisystem disorders characterised by inflammation and necrosis of small and medium blood vessels. Early diagnosis and prompt initiation of immunosuppressive treatment are essential to induce and maintain remission of disease, and thereby prevent severe life- or organ-threatening complications. We present a challenging case of a patient presenting with progressive lung lesions in the context of known ANCA-associated vasculitis (AAV), despite escalating immunosuppressive treatment. Case description A 66-year old-female was diagnosed in 2010 with cANCA (PR3) positive vasculitis with lung involvement. Remission was induced with high dose steroids and cyclophosphamide. Methotrexate was given as maintenance treatment until 2013 when she had a significant renal relapse treated with plasma exchange, steroids and cyclophosphamide. Her maintenance treatment was changed to mycophenolate. She had a further lung relapse in 2014, treated successfully with high dose steroids and rituximab. She was eventually weaned off all treatment in 2017. She was admitted to hospital in 2019 with dry cough, dyspnoea and sinus congestion. She was noted to have several purpuric lesions on her limbs. Her CRP was elevated at 123 with a stable eGFR of 77. ANCA screen was negative. Chest Xray revealed bilateral consolidation. She was initially treated with six pulses of 500mg of IV methylprednisolone and one pulse of rituximab as an inpatient over the course of 2 weeks, with broad spectrum antibiotic cover. Despite this, she continued to deteriorate with a worsening purpuric rash and increasing oxygen requirement. She underwent plasma exchange with little improvement. CT of her chest revealed multiple progressive bilateral cavitating lesions. It was noted that her IgG levels were low (3.72) following rituximab, increasing her risk of infection. An atypical infection screen was performed and she was discovered to have cytomegalovirus (CMV) reactivation with high viral titres of 56889. She underwent bronchoscopic alveolar lavage which was positive for CMV on PCR. CT guided lung biopsy revealed necrotising granulomatous inflammation with occasional CMV positive nuclei. She was given valganciclovir and IV immunoglobulin, and her prednisolone dose was reduced to 10mg. This resulted in her viral load reducing to undetectable levels, her CRP improving to 23 and immunoglobulins normalising. This correlated with a clinical improvement allowing the patient to be discharged home. Discussion The patient in this case appeared in the first instance to have another flare of her AAV. However the flare was atypical in that it was not associated with an increase in her cANCA titre and it did not respond quickly to steroids as had been the case on each previous occasion. This prompted a reassessment of her case and investigation into other potential causes for her deterioration. CMV is a persistent herpesvirus which can affect up to 75% of healthy individuals. The primary infection is rarely symptomatic but can be associated with significant morbidity and mortality in immunocompromised patients. Pulmonary CMV disease can mimic pulmonary disease associated with vasculitis. CMV can also directly damage endothelial cells and cause an occlusive vasculitis itself. The patient was immunosuppressed, increasing her risk of acquiring primary CMV infection or developing CMV reactivation. She was given further potent immunosuppression during her admission. Serology is often unreliable in immunosuppressed patients and in this case her CMV IgG was positive with a negative IgM, indicating past CMV infection only. It is important to send samples for viral PCR to detect and diagnose active infection, and allow treatment to be initiated promptly. The lung biopsy was felt to be more in keeping with active AAV, although occasional CMV positive nuclei were present. The patient was therefore managed as having CMV reactivation in the context of active AAV. Treatment of viral infection can pose further challenges when trying to manage coexistent active vasculitis as highlighted in this case. Close monitoring of the patient was required, together with a multidisciplinary approach involving input from colleagues in infectious diseases and immunology, to allow the patient to be managed safely and effectively. Prophylactic antiviral therapy may need to be considered if further immunosuppression is required in future. Key learning points CMV reactivation can complicate treatment of ANCA associated vasculitis. Clinicians should have a low threshold to test for CMV in immunocompromised patients with vasculitis, particularly in cases which are atypical or refractory to standard treatment. Viral PCR should be tested, as serology can often be unreliable in immunosuppressed patients. Conflict of interest The authors declare no conflicts of interest.

Background:A growing number of professional societies in clinical and medically related disciplines investigate evidence, make recommendations, and take action to advance gender equity. Evidence on women’s advancement and leadership in the context of the European Alliance of Associations for Rheumatology, EULAR, is limited [1].Objectives:The objective of the EULAR Task Force on Gender Equity in Academic Rheumatology was to establish the extent of the unmet need for support of female rheumatologists, health professionals and non-clinical scientists in academic rheumatology and develop a framework to address this through EULAR and Emerging EULAR Network (EMEUNET).Methods:Potential interventions to accelerate gender-equitable career advancement in academic rheumatology were gathered from a narrative review of the relevant literature, expert opinion of a multi-disciplinary Task Force (comprised of 23 members from 11 countries), data from the surveys of EULAR scientific member society leaders, EULAR and EMEUNET members, and EULAR Executive Committee members. These interventions were rated by Task Force members, who ranked each according to perceived priority on a five-point numeric scale from 1 = very low to 5 = very high.Results:A framework of 29 potential interventions was formulated, which covers six thematic areas, namely, EULAR policies, advocacy and communication, EULAR Congress and associated symposia, training courses, mentoring/peer support, and EULAR funding (Figure 1).Figure 1.A framework of potential interventions with the levels of priority, mean and standard deviation (SD)Conclusion:The framework provides structured interventions for accelerating gender-equitable career advancement in academic rheumatology.References:[1]Andreoli L, Ovseiko PV, Hassan N, et al. Background:Women represent an increasing proportion of the overall rheumatology workforce, but are underrepresented in academic rheumatology, especially in leadership roles [1].Objectives:The EULAR Task Force on Gender Equity in Academic Rheumatology has been convened to establish the extent of the unmet need for support of female rheumatologists, health professionals and non-clinical scientists in academic rheumatology and develop a framework to address this through EULAR and EMEUNET.Methods:To investigate gender equity in academic rheumatology, an anonymous web-based survey was targeted at the membership of EULAR and Emerging EULAR Network (EMEUNET) and their wider networks. The survey was developed based on a narrative literature review [1], best practice from The Association of Women in Rheumatology, a survey of task force members and face-to-face task force discussions. Personal experiences were explored and 24 potential interventions to aid career advancement were ranked. Statistics were descriptive with significance testing for male/female responses compared using chi-squared/t-tests. The level of significance was set at pResults:A total of 301 respondents from 24 countries fully completed the survey. By profession, 290 (86.4%) were rheumatologists, 19 (6.3%) health professionals, and 22 (7.3%) non-clinical scientists. By gender, 217 (72.1%) were women, 83 (27.6%) men, and 1 (0.3%) third gender. By age, 203 (67.5%) were 40 or under. By ethnicity, 30 (10.0%) identified themselves as ethnic minority. A high proportion of respondents reported having experienced gender discrimination (47.2% total: 58.1% for women and 18.1% for men) and sexual harassment (26.2%: 31.8% and 10.8% respectively) (Figure 1). Background Definitive diagnosis of pleural disease (particularly malignancy) depends upon histologic proof obtained via pleural biopsy or positive pleural fluid cytology. Image-guided sampling is now standard practice. Local anesthetic thoracoscopy has a high diagnostic yield for malignant and nonmalignant disease, but is not always possible in frail patients, if pleural fluid is heavily loculated, or where the lung is adherent to the chest wall. Such cases can be converted during the same procedure as attempted thoracoscopy to cutting-needle biopsy. This study aimed to determine the diagnostic yield of a physician-led service in both planned biopsies and cases of failed thoracoscopy. Methods This study was a retrospective review of all ultrasound-guided, cutting-needle biopsies performed at the Oxford Centre for Respiratory Medicine between January 2010 and July 2013. Histologic results were assessed for the yield of pleural tissue, final diagnosis, and clinical follow-up in nonmalignant cases. Results Fifty ultrasound-guided biopsies were undertaken. Overall, 47 (94.0%) successfully obtained sufficient tissue for histologic diagnosis. Of the 50 biopsy procedures, 13 were conducted after failed thoracoscopy (5.2% of 252 attempted thoracoscopies over the same time period); of these 13, 11 (84.6%) obtained sufficient tissue. Thirteen of 50 biopsy specimens (26.0%) demonstrated pleural malignancy on histology (despite previous negative pleural fluid cytology), while 34 specimens (68.0%) were diagnosed as benign. Of the benign cases, 10 were pleural TB, two were sarcoidosis, and 22 were benign pleural thickening. There was one "false negative" of mesothelioma (median follow-up, 16 months). Conclusions Within this population, physician-based, ultrasound-guided, cutting-needle pleural biopsy obtained pleural tissue successfully in a high proportion of cases, including those of failed thoracoscopy.

AIMS: We sought to determine the prevalence of asymmetric hypertensive heart disease (HHD) overlapping morphologically with hypertrophic cardiomyopathy (HCM) and to determine predictors of this pattern of hypertensive remodelling.METHODS AND RESULTS: One hundred and fifty hypertensive patients underwent 1.5 T cardiovascular magnetic resonance imaging. Twenty-one patients were excluded due to concomitant cardiac pathology that may confound the hypertrophic response, e.g. myocardial infarction, moderate-severe valvular disease, or other cardiomyopathy. Asymmetric HHD was defined as a segmental wall thickness of ≥15 mm and >1.5-fold the opposing wall in ≥1 myocardial segments, measured from short-axis cine stack at end-diastole. Ambulatory blood pressure, myocardial replacement fibrosis, aortic distensibility and aortoseptal angle were investigated as predictors of asymmetric HHD by multivariate logistic regression. Out of 129 hypertensive subjects (age: 51 ± 15 years, 50% male, systolic blood pressure: 170 ± 30 mmHg, diastolic blood pressure: 97 ± 16 mmHg), asymmetric HHD occurred in 21%. Where present, maximal end-diastolic wall thickness (EDWT) was 17.8 ± 1.9 mm and located exclusively in the basal or mid septum. In asymmetric HHD, aortoseptal angle (114 ± 10° vs. 125 ± 9° vs. 123 ± 12°, P < 0.05, respectively) was significantly reduced compared to concentric left ventricular hypertrophy (LVH) and compared to no LVH, respectively. Aortic distensibility in asymmetric HHD (1.01 ± 0.60 vs. 1.83 ± 1.65 mm(2)/mmHg × 10(3), P < 0.05, respectively) was significantly reduced compared to subjects with no LVH. Age (odds ratio [95th confidence interval]: 1.10 [1.02-1.18], P < 0.05) and indexed LV mass (1.09 [0.98-1.28], P < 0.0001) were significant, independent predictors of asymmetric HDD.CONCLUSIONS: Asymmetric HHD morphologically overlapping with HCM, according to the current ESC guidelines, is common. Postulating a diagnosis of HCM on the basis of EDWT of ≥15 mm should be made with caution in the presence of arterial hypertension particular in male subjects with elevated LV mass.

Background IAs are detected in 2.3% of adults. Systemic hypertension is a risk factor. Screening for IA in certain highrisk groups, such as patients with coarctation of the aorta, has been discussed in international guidelines. Patients with coarctation often have concomitant systemic hypertension. Moreover, the prevalence of hypertension in patients with coarctation and IA is significantly higher than those without IA. It is uncertain whether hypertension alone represents a sufficient risk factor to prompt screening for IAs. Methods

Arterial blood gas sampling forms a vital part of bedside investigation but is invasive and physician dependent. This study aimed to determine whether capillary blood gas sampling could provide an accurate and less invasive substitute for the measurement of blood gases and pH.

Background Brainstem hypoperfusion may evoke systemic hypertension as a mechanism to boost cerebral blood flow: the selfish brain hypothesis. This notion is supported by evidence of vertebral artery hypoplasia (VAH) in hypertensive rat models. We determined the prevalence of anatomical variations of posterior cerebral circulation which could predispose to brainstem hypoperfusion in a hypertensive human cohort. Methods The study was conducted in accordance with The Governance Arrangements for Research Ethics Committees. Patients referred from our tertiary hypertension clinic underwent comprehensive MR assessment including CMR and 3D time-of-flight MR angiography (MRA) imaging at 1.5T. Antiresorptive agents are generally recommended as first-line treatment for osteoporosis in postmenopausal women. These drugs suppress bone resorption but do not rebuild bone, limiting their efficacy. Antiresorptive use is further hampered by concerns over rare side effects, including atypical femoral fractures and osteonecrosis of the jaw. Anabolic treatments overcome limitations of antiresorptive treatment by stimulating new bone formation, reducing the risk of fracture with greater efficacy. This review summarises the latest trial data for the three anabolic agents currently available for the treatment of osteoporosis in postmenopausal women: teriparatide, abaloparatide, and romosozumab. Data from head-to-head studies comparing anabolic and antiresorptive treatments are reviewed. At present, anabolic treatments are generally reserved for use in patients with severe osteoporosis at very high fracture risk; the factors limiting their more widespread use are discussed together with how this may change in the future.

Methods Consecutive patients referred from our tertiary hypertension clinic, who underwent CMR at 1.5T, were included. Exclusion criteria included patients with clinical or CMR evidence of concomitant pathology (e.g. moderate-severe aortic stenosis) which may confound remodeling/hypertrophy pattern. Indexed LVM (iLVM), including papillary muscle mass by blood pool thresholding, indexed LV enddiastolic volume (iEDV) and ejection fraction (EF) were calculated using established CMR methods and normalized to body surface area. Values out-with the 95 th confidence intervals of established CMR normal reference values were considered abnormal. Mass : volume ratio (M/ V) >1.12 for men and >1.14 for women was defined as abnormal, in accordance with previous literature. The phenotypes of ventricular remodeling and hypertrophy were defined as either normal, concentric remodeling, asymmetric remodeling, concentric hypertrophy, asymmetric hypertrophy, eccentric hypertrophy or decompensation depending on the constellation of iLVM, iEDV, M/V, asymmetric thickness (>13mm and >1.5 fold opposing wall) and EF Results One hundred and twenty three (n=123) patients were analysed. The prevalence of different phenotypical responses were as follows: normal (42.3%), concentric remodeling (6.5%), asymmetric remodeling (5.7%), concentric hypertrophy (12.2%), asymmetric hypertrophy (17.9%) eccentric hypertrophy (8.9%) and decompensation (6.5%). The demographic and CMR characteristics of the different types of remodeling and hypertrophy are described in Figure 1. There was no predilection of remodeling/hypertrophic pattern according to hypertension type. 12.2% of our cohort had normal iLVM but demonstrated concentric/asymmetric remodeling. Subgroup analysis by remodeling (n=15) versus hypertrophy (n=22) revealed no significant difference in age (62±9.4 vs 55.1±12.4 years, p=0.0598), gender (% male 74.4% vs 68.2%, p=0.999), BMI (30.9±3.0 vs 30.1±4.9 kg/m 2 , p=0.5836), degree of hypertension (SBP 179.9±31.3 vs 176.8±24.7 mmHg, p=0.7407 and DBP 98.4±11.4 vs 95.7±14.8 mmHg, p=0.5557) or prevalence of potentially remodeling modifying medication (ACEi/ARB 80.0% vs 77.3%, p=0.999). Conclusions Varied CMR patterns of LV remodeling/hypertrophy occur in hypertensive patients with no predilection demonstrated in subgroup analysis. CMR-derived iLVM is increasingly used an end-point for clinical trials in hypertension. Our data suggest that patterns of LV remodeling/ hypertrophy should also be taken into account to avoid misclassifying patients with normal iLVM (but abnormal ventricles due to remodeling) together with patients with normal iLVM and truly normal ventricles.
