Introduction: Unexplained distressing bodily complaints like localised heaviness in the body, tingling, heat, pain and crawling sensations, unattributable to physical pathology and psychiatric morbidity, are common among patients that attend Family Medicine Clinic. Objectives: The study assessed patterns of psychiatric morbidity and somatisation symptoms Family Medicine Clinic of a University Teaching Hospital in Enugu, Southeast Nigeria. Methods: A cross-sectional survey of 81 somatising patients were part of a case-control study, selected by a consecutive sampling of 89 patients at the Family Medicine Clinic of the University of Nigeria Teaching Hospital, Enugu. Data was collected using the PHQ-15 and MINI plus English Version 6.0 and analysed with SPSS 21. Results: Seventy-six (93.3%) of the participants had psychiatric diagnoses. The most prevalent symptoms were heat sensations (75.3%), pain sensations (61.7%), crawling sensations (51.9%), heaviness (46.9%) and tingling/paresthesia (29.6%). The mean age at onset was 32.99 years. The mean duration was 6.07 years (±7.58). The study revealed that 76 (93.3%) participants had psychiatric diagnoses, and somatisation disorder was the most prevalent psychiatric disorder 71(87.7%). Conclusion: Knowledge of the patterns of somatisation symptoms and comorbid psychiatric conditions is vital for the effective management of these patients.

Symptomatic large pineal cyst (PC) remains a rare entity. The stable natural course of asymptomatic PCs is well established. However, large cysts may cause pressure-related symptoms necessitating surgical intervention. The surgical strategy for symptomatic PCs is still controversial. Regardless of the approach, total resection of the cyst is not mandatory. The endoscopic approach allows cyst fenestration in patients with associated obstructive hydrocephalus. On the other hand, the necessity of simultaneous endoscopic third ventriculostomy (ETV) is still debatable. Here, we report a case of a woman who underwent endoscopic cyst fenestration, biopsy, and third ventriculostomy for a large symptomatic PC and discuss the surgical strategy. A 30-year-old woman presented with headache and diplopia, MRI showed a large PC and ac- companying obstructive hydrocephalus. Simultaneous cyst fenestration, biopsy and ETV with endoscopy was successfully completed. She had an uneventful recovery period with immediate relief of symptoms. Although, the aqueduct was communicated due to cyst shrinkage, the patency of the third ventricular stoma was demonstrated in long-term follow-up scans. Based on clinical course of the present case, we concluded that ETV in addition to cyst fenestration should be considered necessary and beneficial in cases of large symptomatic PC with associated hydrocephalus whenever an endoscopic intraventricular approach is considered., This is an Open Access article distributed under the Creative Commons Attribution-NonCommercial-NoDerivatives 4.0 International License. Objectives This study aimed to determine the pattern of aggression among senior secondary school students in Calabar and the psychosocial factors that are associated with it. Methods A descriptive cross-sectional study of 453 randomly selected senior secondary school students utilizing a self-administered Socio-demographic questionnaire obtain data regarding age, gender etc. and a brief interview using the Modified Overt Aggression Scale (MOAS) to determine aggression. Many caregivers experience significant psychological burden which may impact on the management of a sick child.To determine the prevalence and associated factors of psychological distress among caregivers of children admitted at the Children Emergency Room.This was a descriptive cross-sectional study among caregivers of children who were hospitalized for at least 24 hours. The 28-item General Health Questionnaire (GHQ 28) was used to assess the psychological distress among the caregivers. GHQ scores were stated as means ± standard deviation (SD). Chi-square or Fisher's exact test was used to test for association between sociodemographic variables and psychological distress. Mean GHQ scores in the various domains of psychological dysfunction were compared among groups using the independent sample t-test; at p0.05.Of the 97 caregivers who participated in the study, 96 had their data analyzed. The caregivers were aged 19 to 63 (mean 34.25 (8.46)) years; 86 (89.7%) were females and 48 (50%) had tertiary education. The prevalence of psychological distress among the care givers was 69.8%. Caregivers had high levels of anxiety but low levels of depression. Those with lower educational attainment had higher scores on severe depression domain (p = 0.001). Unemployed caregivers had higher mean scores on the anxiety/insomnia (p = 0.039) and social dysfunction domains (p = 0.031). This study assessed dependent personality, marital satisfaction, and mindful awareness and their association with postpartum depression and anxiety. It was a cross-sectional study of nursing mothers, within 6-14 weeks postpartum, at the postnatal and children's welfare clinics of two tertiary hospitals in Enugu, southeastern Nigeria. The nursing mothers responded to different questionnaires that measure each of these variables. Correlations and multiple regression analyses were used to measure the strength of association between the variables in the study and to identify predictors for the outcome variables of interest, respectively. Low mindful attention awareness, poor marital satisfaction, and low dependency predicted an increased risk of postpartum depression and anxiety, with low mindful attention awareness as the strongest predictor. The findings from the study carry significant implications for the roles of dependent personality, marital satisfaction, and mindful awareness in the etiology of postpartum depression and anxiety in Southeast Nigeria.

Background: Postpartum depression and anxiety could cause poor mother-infant relationship that could impair infant growth and cognitive development. Psychiatric assessment has not been part of periodic evaluations in postnatal clinics. Some apparently well, but high-risk mothers continue to live with psychological distress, without treatment to relieve their burden. Objectives: The study assessed the prevalence of postpartum anxiety and depression, their co-morbidity, and socio-demographic predictors, within 6 - 14 weeks postpartum among nursing mothers in two tertiary hospitals in Enugu, South-East Nigeria. Methods: A cross-sectional study that was carried out at the postnatal and children’s welfare clinics of two tertiary hospitals in Enugu, South-East Nigeria. Hospital Anxiety and Depression Scale, Social Support Scale and Socio-demographic Questionnaire were used for the study. Correlations and multiple regressions were used to test for associations and risk factors. Results: Occurrence rate for anxiety and depression were 30.1% and 33.3% respectively with a co-morbidity rate of 22%. Low social support and multigravida predicted risk factors for postpartum psychological distress, while a higher number of children alive predicted a decrease in the risk for postpartum psychological distress. Conclusion: The study supports service planning and the development of strategies to reliably identify women at high-risk, for effective treatment. Background Adolescents do present with somatization disorder which is often neglected by pediatricians. This could have serious consequences if not curbed early. Objectives This study is aimed at determining the pattern and types of Somatization disorder among adolescents attending secondary schools in south east Nigeria. Methods Somatization disorder was investigated among 485 adolescents from mixed schools using a stratified random sampling of adolescents from four secondary schools in southeast Nigeria. The Enugu somatization scale was used to evaluate for presence of somatization in the participants. Statistical analysis was with statistical package for social sciences (SPPS) version 19 (Chicago IL). Results A total of 485 adolescents aged 10–19 years were included in this study. The mean age of the respondents was 16.36 with standard deviation (SD) of 3.14 years. Two hundred and fifty-one (51.8%) had head features, 262 (54.0%) had body features, 303 (62.5%) had either head or body features while 210 (43.3%) had both head and body features. One hundred and thirty-four males (51.3%) compared to 117 females (52.2%) reported symptoms consistent with head symptoms (p = 0.038). One hundred and eleven males (42.5%) compared to 99 females (44.2) reported symptoms related to the head and body (p = 0.137) while 135 males (51.7%) compared to 127 females (56.7%) reported symptoms related to the body (p = 0.925). The efficacy of the endoscopic transcortical transventricular approach (ETTA) for craniopharyngioma in the third ventricle with hydrocephalus has been reported focusing on its reduced invasiveness. On the other hand, suprasellar craniopharyngioma without ventriculomegaly is generally surgically managed by craniotomy or the endoscopic endonasal approach (EEA). Here, we report an elderly patient who received cyst fenestration and Ommaya reservoir placement in ETTA for recurrent suprasellar cystic craniopharyngioma without ventriculomegaly. The ETTA as a less invasive procedure is feasible in patients not only with intraventricular craniopharyngioma but also with suprasellar craniopharyngioma without hydrocephalus provided a navigational system is applied and the surgeon has ample experience with transcranial endoscopic procedures.

Introduction: Spontaneous intracerebral haemorrhage (SICH) is a major cause of stroke worldwide. SICH management is still challenging, especially in developing countries. This study highlights certain factors affecting outcome of SICH managed in a Nigerian Neurosurgical centre, on a background of the modernisation of the patient care facilities. Materials and Methods: Retrospective analysis of patients managed for SICH at Memfys Hospital for Neurosurgery and Neurology in Enugu from years 2009–2016. All patients had computed tomography or magnetic resonance imaging for diagnosis. Treatment included medical, surgical and intensive care unit (ICU) care. Patients with aneurysmal subarachnoid haemorrhage and trauma were excluded. Factors analysed include age, admission Glasgow Coma Score (GCS), haematoma location, complications encountered during admission, duration of hospital stay and 6-month Glasgow Outcome Score (GOS). Results: There were 66 cases, age range of 21–85 years (mean 57 years). A total of 30 (45.5%) patients were admitted with GCS ≤ 8/15; 63.3% of these died within 6 months. The proportion of mortalities was 63.6% (>70 years) and 35.6% (41–70 years). Most commonly associated complication was chest infection 27 (40.9%), with 56.6% mortality. Common haematoma locations were basal ganglia (43.9%) and lobar haemorrhage (40.9%) with a similar effect on outcome (P = 0.098). Outcomes were GOS 1: (43.9%), GOS 5: (30.3%) and GOS 4: (13.6%). Among 42 (63.3%) admitted to ICU, 25 (59.5%) died, while 11 (26.2%) achieved GOS of ≥ 4 at 6 months. Those discharged between days 11 and 20 had 26.7% mortality while 53.3% were independent. Conclusion: Good admission GCS, absence of chest infections, younger age group are predictors of good outcome following SICH. Anatomical location of haemorrhage alone does not have a significant impact on 6 months' mortality.

A 13-year old right handed Nigerian girl presented with eleven weeks history of progressive visual loss, three weeks right hemi-body weakness which progressed to quadriparesis, dysphasia, severe headache, tonic-clonic seizures, neck pain, vomiting and fever.Examination revealed Glasgow Coma Score (E4M6V2), expressive dysphasia and depressed mood. Visual Acuity was counting finger bilaterally. Neck was supple. She had global hypotonia, grade 3 hyper-reflexia and extensor Babinski bilaterally. Power was 0/5 in the right upper limb and right lower limb, 2/5 in the left lower limb and 4/5 in the left upper limb. Other examinations were unremarkable.Contrast enhanced brain MRI revealed three cystic ring enhancing masses at left fronto-parieto-occipital region, associated oedema, midline shift and C2-C4 intramedullary mass with cord oedema. The ring enhancement was incomplete towards the cortex.Following initial suspicions of cystic brain tumour with spinal cord metastasis, she had decompression biopsy. Histology was inflammatory lesion, clinically assumed to be from brain abscess. Failure of antibiotic treatment and the deteriorating neurology prompted mini-craniectomy and biopsy of the cyst wall. Final diagnosis was tumefactive multiple sclerosis. She was successfully managed with methylprednisolone. A significant proportion of patients treated for schizophrenia respond poorly to medication. Few studies have systematically examined the impact of clinical characteristics of patients to antipsychotic response in our environment. The study aimed to identify clinical variables associated with response in patients with schizophrenia. A total of 172 participants diagnosed with schizophrenia, placed on antipsychotics, who presented for the first time to the hospital or have stopped medication in the preceding six months, whose responses were assessed after four to six weeks were studied. Improvement was defined as 20% or greater reduction in PANSS scores. Good response was associated with good medication adherence, good pre-morbid functioning, fewer negative symptoms, less cognitive impairment, absence of co-morbid personality disorder, and short duration of untreated psychosis. Due consideration of the impact of clinical variables could help detect poor responders early, hence avoiding unnecessary exposure to ineffective treatments and their side effects while effective interventions are delayed.

Presence of psychological distress and poor quality of life (QoL) may affect the outcome of HIV/AIDS. One hundred and seventeen consecutive and consenting participants were interviewed using a Socio-demographic questionnaire, Kessler Psychological Distress Scale (K10) and World Health Organization Quality of Life-HIV Bref. Fifty-six (47.9%) participants scored 20 and above on K10 Scale. QoL was significantly higher in married participants, those who perceived their state of health as being good, asymptomatic participants and those who scored well–mild on K10. The older age group scored higher on QoL Scale than the younger age group in the physical health domain while the married participants scored higher in the domains of physical health, psychological health, environment and spiritual/religion. The study aims to compare the coping styles of patients with diabetes with those with depression. A total of 224 patients, 112 with depression and an equal number with diabetes were recruited. Sociodemographic variables were determined with the sociodemographic questionnaire, and coping styles with the Mental Adjustment to Cancer (MAC) adapted and Brief Religious Coping (Brief RCOPE) Scales. Fighting Spirit and MAC Summary Positive Adjustment were used more by participants with depression. Both groups of participants used the Brief RCOPE positive method to the same extent. Brief RCOPE negative was used more by participants with depression. Positive coping skills were used more by participants with depression than those with diabetes. This underscores the need for clinicians to explore the coping resources available to their patients, both religious and non-religious, and projecting them to their patients for enhancement and application as an adjunct for the purpose of a better clinical outcome.

Socioculturally influenced interpretations of mental illness play decisive roles on the time and choice of treatment. These often result in significant delays before patients present at the psychiatric services, with consequent worsening of the prognosis. The aim was to assess the association between sociocultural factors, choice of treatment and treatment delay among patients with schizophrenia. A cross-sectional study of 360 patients with schizophrenia, without previous contact with any statutory psychiatric services, was done. Background: With the poor health indices in most developing countries, the consequences of substance abuse by healthcare professionals are grave. While studies from western countries observed that a significant proportion of healthcare professionals abuse substances; and are less likely to seek help, the prevalence is largely unknown in Nigeria. The healthcare professionals in this region poorly perceive the problem and thus, are unwilling to seek help.Objective: This study aimed to assess the prevalence and patterns of abuse of substances by healthcare professionals in a Nigerian tertiary health institution; their perception of substance abuse and willingness to seek help. Methodology: This was a cross-sectional study of 149 healthcare workers who were screened for substance abuse using the alcohol, smoking and substance involvement screening test (ASSIST) version 3. Their perception and willingness to seek help were assessed using questions developed by the researchers based on literature.Results: Excess workload/stress of work was perceived as the most common reason for abusing drugs. Overall, substance abuse was perceived as a mental health problem. However, those that abuse substances more readily perceived it as a habit problem rather than a mental health challenge. Moreover, there was lower readiness to seek and pay for treatment among them.Conclusion: The study has brought to the fore the need to provide mental health education especially to healthcare professionals and to expose them to healthier methods of coping with stress.Keywords: Addiction, medical personnel, screening, treatment

Early detection and treatment of psychological morbidity have been associated with reduction in long-term disability and increase in total well-being of amputees. However, much attention have been focused on the management of the physical disability with little interest on differences in distress on account of socio-demographic factors such as gender. This cross-sectional study examined psychological distress in 86 orthopaedic patients with limb amputation participated in the study. They were assessed with the Symptom Distress Checklist-90 (SCL-90) and socio-demographic questionnaire. Results showed that about sixty-four percent (64.4%) of the participants manifested distress in at least one domain of the Symptom Distress Checklist. None of the socio-demographic variables (e.g., gender) differentiated the participants in their self-reported psychological distress. Considering the high prevalence of psychological distress among amputees, mental health professionals should be involved in the management of these patients prior to and after amputation in order to enhance and promote prevention, early detection and treatment of psychological morbidity among amputees.Keywords: Amputation, Gender, Mental Health, Psychological distress, Psychotherapy
