Günümüzdeantibiyotik direnci, insan sağlığını tehdit eden zorlu mesele olaraknitelendirilmektedir. Bu çalışmada akılcı antibiyotik kullanımında, ailehekimlerinin uyum hedeflerinin gelişimine etkisini araştırdık. 18 yaş üzerirastgele seçilen hastalarda yüz yüze anket kullanılarak gerçekleştirildi.Demografik özellikler, akılcı antibiyotik kullanımı (AAK), aile hekimlerinin veeczacıların hastaları bilgilendirmesiyle ilgili kapalı uçlu sorular soruldu.Elde edilen veriler istatistiksel olarak analiz edildi. AAK uyumsuzluğu %50,3(67) idi. Üst solunum yolu enfeksiyonu nedeniyle antibiyotik reçete edilen%51,1 (68) hastanın AAK uyumsuzluk oranı %63,2 (43) en fazla idi. Ailehekimleri %38,3 (51) oranında antibiyotik hakkında hastaları bilgilendirmişti.Aile hekimleri tarafından bilgilendirilen hasta grubunda antibiyotikkullanımına uyum ve hastanın yeterli bilgiye sahip olması bilgilendirilmeyengruba göre istatistiksel olarak anlamlı derecede daha iyiydi (p=0,034). Yeni bir antibiyotiğin geliştirilip kullanıma girmesi,yüksek maliyetler ve uzun zamanları gerektirdiğinden dolayı en önemli yaklaşımmevcut antibiyotiklerin korunmasıdır. Çalışmamızda bilgilendirmenin aile hekimitarafından verilmesinin AAK başarısına ve uyumuna doğrudan etkisinin olduğugörüldü., Nowadays, antibiotic resistance isdescribed as the most powerful matter that threatens human health. We searchfor the effects of family physicians to rational use of antibiotics (RUA) andenhancing adaptation goals. The study isperformed randomly in outpatients olderthan 18 whom were prescribed antibiotics by face-to-face questionnaire. Closedended questions are asked about demographic features, RUA adaptation, familyphysician and chemist notification. Statistical analysis of acquired data isdone. RUA non-adherence is 50.3%(67). Antibiotic is prescribed 51.1% (68) by family physicians because of upper respiratory tractinfection, this group is themost non-adherence one with 63.2% (43). Family physicians informed 38.3% (51)prescribed patients about antibiotics. In the group informed by familyphysicians, it makes a meaningful difference in terms of adherence to the useof antibiotics and patient's having enough knowledge. Because it takes a long time to develop new antibiotics, the mostsignificant action to be taken should be protection of antibiotics in stock. Inour study, it was seen that the information given by family physicians haddirect impact on patients being capable of RUA and adherence.

Objective: Brucellosis is a zoonotic infectious disease of domestic and wild animals caused by Brucella strains and is transmitted to humans by various routes. Brucellosis is an important problem leading to economic losses due to high morbidity rates. Although the disease has been eradicated in most developed countries it remains a public problem especially in the Mediterranean countries of the Middle East and Africa. The objective of this study was to investigate hematological changes in patients with brucellosis from Kahramanmaraş in light of the literature.Methods: A total of 197 patients diagnosed with brucellosis and followed-up in the infectious diseases clinics of Kahramanmaraş Sütçü İmam University Hospital between 1 January 2012 and 31 December 2017 were retrospectively evaluated for hematological changes. Demographic and epidemiologic data of the patients including age, gender, profession, the habit of consuming fresh cheese, and contact with animals were recorded. Objective Acute gastroenteritis is a disease characterized by diarrhea. In this disease, virus, bacteria and parasite are the most common factors. In this study, we aimed to evaluate the diversity of intestinal parasites in patients with gastroenteritis who applied to the Emergency Department (ED).Material and MethodsIn this descriptive and cross-sectional study, a total of 4177 records of patients with acute gastroenteritis (ICD: K52.9) who were admitted to the ED of our hospital between November 2014 and November 2017 were evaluated retrospectively. A total 410 stool samples for intestinal parasites using the standard parasitological methods were examined.Results 45.9% of the cases (n=188) were male and 54.1% (n=222) were female. The most common cause of parasitic diarrhea was E. histolytica (85.2%). Of the 410 patients included in the study, 34.6% (n = 142) had leukocytes in the microscopic examination. The microscopic examination of 62.7% (n = 89) of patients with leukocytes also showed parasites. Amaç: Diabetes mellitus kronik bir hastalıktır ve diyabetik ayak enfeksiyonu gibi önemli bir komplikasyona neden olur. Buna bağlı olarak hastaların mobilizasyonu azalacak ve öz bakımının düşeceğini düşünmekteyiz. Bu çalışmanın amacı diyabetik ayak enfeksiyonu olan hastaların öz bakım gücünün değerlendirmektir.Yöntemler: Bu çalışma diyabetik ayak enfeksiyonu olan 97 hastanın öz bakım gücünü tanımlamak için yapılan anket temelli kesitsel tanımlayıcı bir araştırmadır. Veriler 35 sorudan oluşan öz bakım gücü ölçeği kullanılarak yüz yüze görüşme yöntemi ile toplanmıştır.Bulgular: Katılımcıların öz bakım gücü değerlendirildiğinde olguların %30.9’u (n=30) düşük seviyede, %58.8’inin (n=57) orta seviyede, %10.3’ünün (n=10) ise yüksek seviyede olduğu gözlendi. Amputasyon süresi ile öz bakım gücü toplam skoru arasında pozitif yönde istatistiksel olarak anlamlı ilişki olduğu saptanmıştır (r=0.514, p=0.002).Sonuç: Sonuç olarak diyabet ve diyabete bağlı komplikasyonlar sıkça karşılaştığımız önemli bir hastalık grubudur. Hastalara ve hasta yakınlarına verilecek eğitim ile öz bakımın daha iyi olacağı düşüncesindeyiz., Aim: Diabetes mellitus is a chronic disease and causes a major complication such as diabetic foot infection. Accordingly, we think that the mobilization of patients will decrease and self-care will decrease. The aim of this study was to evaluate the self-care agency of patients with diabetic foot infection.Methods: This is a questionnaire-based cross-sectional study to identify the self-care agency of 97 patients with diabetic foot infection. Data were collected by face to face interview technique, using a questionnaire the Self-care Agency Scale which is consists of a total of 35 questions. Results: When the self-care agency of the participants was evaluated, it was observed that 30.9% (n = 30) of the cases were low, 58.8% (n = 57) were moderate, and 10.3% (n = 10) were high. There was a statistically significant positive correlation between amputation time and self-care agency total score (r = 0.514, p = 0.002).Conclusion: As a result, diabetes and its complications are an important group of diseases that we frequently encounter. We think that self-care will be better with education to be given to patients and their relatives.

Objective: Theaim of this study was to evaluate the preoperative and postoperative managementof patients with surgical field infection and to evaluate the factors relatedto the host.Material and Methods: Thestudy was planned as prospective, cross-sectional and descriptive. Patients whowere operated by different departments in the Faculty of Medicine of KahramanmaraşSütçü İmam University and who developed infection were included in the study.Results: Thestudy included 67 patients (59.7% male, 40.3% female). While19.4% (n = 13) of the cases had blood transfusion in the preoperative period,80.6% (n = 54) had no blood transfusions. 97% (n = 65) of the patients hadantiseptic shower in the preoperative period and 53.7% (n = 36) of the caseshad hair cleansing preoperatively. When the distributionof the patients who developed infection at the wound site after the operationis examined, 61 % Gram (-) bacteria, 25.5 % Gram (+) bacteria and 4.5 % fungus werefound.Conclusion: Surgicalsite infections increase the length of hospital stay and cause loss of laborand economic loss. Surgical site infections are preventablecauses. Therefore, maximum attention should be given to taking the necessarymeasures., Amaç: Buçalışmada operasyon sonrası cerrahi alan enfeksiyonu gelişen hastalarınoperasyon öncesi ve sonrası yaptığı uygulamalar ve konağa ait faktörlerindeğerlendirilmesi amaçlanmıştır.Gereç ve Yöntemler: Çalışmaprospektif, kesitsel ve tanımlayıcı olarak planlandı. Kahramanmaraş Sütçü imamÜniversitesi Tıp Fakültesi Hastanesinde farklı bölümler tarafından opere edilenve enfeksiyon gelişen hastalar çalışmaya dahil edildi.Bulgular: Çalışmaya67 hasta dahil edildi (% 59.7’si erkek, %40.3’ü kadın). Olguların % 19.4’üne(n=13) preoperatif dönemde kan transfüzyonu yapılmış iken, % 80.6’sına (n=54)kan transfüzyonu yapılmamıştır. Olguların % 97’si (n=65) preoperatif dönemdeantiseptikli duş almış ve olguların % 53.7’si (n=36) preoperatif dönemde kıltemizliği yapmıştır. Operasyon sonrası yara yerinde enfeksiyon gelişenhastaların dağılımı incelendiğinde hastaların % 61’inde Gram (-) bakteri, % 25.5’indaGram (+) bakteri ve % 4.5’inde mantar üremesi saptanmıştır.Sonuç: Cerrahialan enfeksiyonları hastaların hastanede yatış süresini arttırarak, iş gücükaybına ve ekonomik kayba neden olmaktadır. Cerrahi alan enfeksiyonlarınınönlenebilir nedenlerdir. Bu nedenle gerekli önlemleri almak konusunda azamidikkat gösterilmelidir.

Amaç: Burkholderia cepacia, glikozu fermente edemeyen aerobik, Gram-negatif ve çok ilaca dirençli bir bakteridir. İmmunsüprese hastalarda önemli bir fırsatçı bakteri olan Burkholderia cepacia, ciddi pulmoner enfeksiyonlara neden olur. Bu çalışmada, son beş yılda tespit edilen Burkholderia cepacia vakalarını değerlendirmeyi amaçladık.Yöntemler: Çalışma retrospektif olarak planlandı. Çalışmaya 2013-2018 yılları arasında üçüncü basamak hastanedeki 46 B. cepacia olgusu dahil edildi. Yaş, cinsiyet, klinik öykü, alınan örnek tipi, hastaların son durumu (yaşıyor veya ölü) ve hastanede yatış süresi kaydedildi. Bulgular: Örneklerin dağılımı incelendiğinde, kan kültüründe %32,6 (n=15), idrar kültüründe %32,6 (n=15), trakeal aspiratta %17.4 (n=8) bulundu. Hastaların son durumları yaşıyor veya ölü olarak değerlendirildi. Buna göre, olguların %65,2’sinin (n=30) yaşadığı, %34,8’inin (n=16) ise öldüğü tespit edildi. Olguların kliniklere göre dağılımı incelendiğinde, anestezi %19,6 ile (n=9) ilk sıradaydı. Hastanede kalış süresi ortalama 24,6 ± 25,3 gündü (minimum-maksimum: 3-122 gün).Sonuç: Burkholderia cepacia önemli bir nozokomiyal fırsatçı enfeksiyondur ve sıklıkla çok ilaca dirençlidir. Bu sebeple hastalık etkin bir şekilde tedavi edilmelidir aksi halde hastalığın mortalite ile sonuçlanacağı unutulmamalıdır., Aim: Burkholderia cepacia is an aerobic, Gram-negative and multi-drug resistance bacteria that cannot ferment glucose. Burkholderia cepacia, important opportunistic bacteria in immunosuppressed patients, causes severe pulmonary infections. In this study, we aimed to evaluate Burkholderia cepacia cases detected in last five years.Methods: The study designed as retrospectively. Forty-six cases with B. cepacia in the tertiary hospital between 2013 and 2018 were included in the study. Age, gender, clinical history of the patient, type of sample taken, and patients’ final conditions (alive or dead) and duration of hospitalization were recorded.Results: When the distribution of the samples were examined, it was found that 32.6% (n=15) in the blood culture, 32.6% (n=15) in the urine culture, 17.4% (n=8) in the tracheal aspirate culture and 17.4% (n=8). Patients’ final conditions were evaluated as alive or dead. Accordingly, 65.2% (n=30) were alive and 34.8% (n=16) of the patients were dead. When the distribution of the cases according to the clinics were examined, Anesthesia with 19.6% (n=9) was the first place. The average length of stay in hospital was 24.6 ± 25.3 days (minimum-maximum: 3-122 days). Conclusion: Burkholderia cepacia is an important nosocomial opportunistic infection and is often multi drug resistant. For this reason, the disease should be effectively treated otherwise it should not be forgotten that the disease will result in mortality.

Lungabscess is a serious infectious disease characterized by necrosis andcavitation in the lung parenchyma. If rapid and accurate antibiotherapy is notprovided, mortality of the disease is high. Case 18 years old female patient.Hospitalization and immunosuppressive disease are absent. She applied to theclinic for complaints of fever, cough, night sweats and shortness of breath,which lasted for about a month. The patient was hospitalized because of airfluid level in the cavity on chest X-ray. Enterobacter cloacae was detected inlung abscess culture. Ceftriaxone therapy was given for twenty-one days. Thefindings of the chest X-ray improved and her complaints declined significantly,and the patient was discharged healthily.Inthis article, it is aimed to present a case of pulmonary abscess withEnterobacter cloacae associated cavitary lesion., Akciğerabsesi, akciğer parankiminde nekroz ve kavite oluşumuyla karakterize ciddi birenfeksiyondur. Hızlı ve doğru antibiyoterapi sağlanmadığı takdirde hastalığınmortalitesi oldukça yüksektir. Olgu 18 yaşında kadın hasta. Hastane yatışöyküsü ve immun sistemi baskılayan hastalığı yok. Yaklaşık bir aydır devam edenateş, öksürük, gece terlemesi, nefes darlığı şikayetiyle polikliniğe başvurdu.Akciğer grafisinde kavite içinde hava sıvı seviyesi veren görüntü olmasıüzerine yatışı yapıldı. Akciğer abse kültüründe Enterobacter cloacae tespitedildi. Yirmi bir gün seftriakson tedavisi verildi. Akciğer grafi bulgularıdüzelen ve şikayetleri anlamlı derecede gerileyen hasta sağlıklı bir şekildetaburcu edildi. Bu yazıda Enterobacter cloacae’ye bağlı kaviter lezyonlaseyreden bir akciğer absesi olgusunun sunulması amaçlanmıştır.

Objective:Patient safety culture is a patient-centered concept whichincludes leadership, team work, communication, and learning. Patient safetyculture should be formed in hospitals with a view to protecting patients frompotential harms and providing patient safety. The study was conducted to determine andcompare patient safety culture perceptions of health workers working in the maternityand children’s hospital in two different regions. Material and Methods: Thisdescriptive study was conducted with 216 health professionals working in twoMaternity and Children Hospitals connected to the Ministry of Health Departmentin November 2015 and located in the South and East parts of Turkey. The data werecollected through Personal Information Form and Hospital Survey on PatientSafety Culture (HSPSC). Ethical permissions were received from the institutionswhere the study was conducted. The data were analyzed in SPSS 16. Results: Ofall the health professionals participating in the study, 71,8% were in HospitalA and 28,2% were in Hospital B; 56,8% were nurses, 41,2% were midwives, and 12%were doctors. 80,6% of the health professionals received training on patientsafety. Hospital A was found to receive higher mean scores in the HSPSCsubscales of Error Evaluation (2,71±0,883), Hospital Management Support(3,12±0,814), Duty Shift (2,37 ±0,673), Error Reporting (3,12± 1,088), andCorporate Communication (3,27 ±0,950).On the other hand, Hospital B was foundto have higher mean scores in the subscales of Overall Safety (2,66±0,59), Organizational Learning (2,44±0,788), Team Work (3,07± 1,026), Staffing (3,04 ±0,723), and Relations acrossUnits (3,30±0,857) . Conclusion: This study found that total HSPSC mean score of HospitalB was higher and health professionals had positive attitudes about patientsafety culture, but this score was not at a desired level., Amaç: Hastalarıoluşabilecek zararlardan korumak ve hasta güvenliğini sağlamak için sağlıkkurumlarında hasta güvenliği kültürünün oluşturulması gerekmektedir. Araştırma,iki farklı bölgedeki kadın doğum ve çocuk hastalıkları hastanesinde çalışansağlık çalışanlarının hasta güvenliği kültürü algılarını belirlemek vekarşılaştırmak amacıyla yapılmıştır. Gereç ve Yöntem: Tanımlayıcıtipteki araştırma Kasım 2015’ de Sağlık Bakanlığı’na bağlı Türkiye’ningüneyinde ve doğusundaki 2 ilde bulunan Kadın, Doğum ve Çocuk HastalıklarıHastanesi’nde görev yapan 216 sağlık çalışanı ile yapılmıştır. Verilerintoplanmasında kişisel bilgi formu ve Hasta Güvenliği Kültürü Ölçeği (HGKÖ) kullanılmıştır.Araştırmada ilgili kurumlardan ve etik kuruldan izin alınmıştır. Verilerindeğerlendirilmesinde SPSS 16.0 kullanılmıştır.Bulgular: Araştırmayakatılan sağlık çalışanlarının %71,8’i A Hastanesinde görev yapan sağlıkçalışanlarının, %28,2’si B Hastanesinde görev yapan sağlık çalışanlarının,%46,8’i hemşire, %41,2’si ebe, %12’si doktor olarak görev yapmaktadır. Sağlıkçalışanlarının %80,6’sı hasta güvenliği konusunda eğitim almıştır. HGKÖ’nün hatadeğerlendirmesi (2,71±0,883), hastane yönetimi desteği (3,12±0,814), görevdeğişimi (2,37 ±0,673), hata raporlama (3,12± 1,088), kurumsal iletişim (3,27±0,950) alt boyutlarında a hastanesinde görev yapan sağlık çalışanlarının dahayüksek puan ortalamasına sahip olduğu bulunmuştur. Genel Güvenlik (2,66 ±0,59), örgütsel öğrenme (2,44 ±0,788), takımçalışması (3,07± 1,026), istihdam (3,04 ±0,723) ve birimler arası ilişki (3,30±0,857) alt boyutlarında ise B Hastanesinde görevyapan sağlık çalışanlarının daha yüksek puan ortalamasına sahip olduğugörülmektedir. Sonuç: ÇalışmadaB Hastanesinde görev yapan sağlık çalışanlarının HGKÖ toplam puan ortalamasınındaha yüksek olduğu, sağlık çalışanlarının hasta güvenliği kültürüne ilişkin algılarınınolumlu olduğu ancak istendik düzeyde olmadığı belirlenmiştir.

AMAÇ: Fetüsün Toksoplazma ve Rubella virus enfeksiyonları ciddikonjenital doğum defektlerine veya fetal ölüme neden olabilir. Bu çalışmada,hastanemize rutin muayene için başvuran gebe kadınlarda yaş gruplarına göreToksoplazma ve Rubella virus IgM ve IgG tipi antikorların seropozitifliğinindeğerlendirilmesi amaçlandı.HASTALAR VEYÖNTEM: Aralık 2011- Aralık 2016 tarihleri arasındaki beşyıllık periyotta hastanemize muayene için başvuran 10178 gebe kadın yaşgruplarına göre 15-24 yaş, 25-34 yaş, 35-44 yaş ve ≥45 yaş olarak dört grubaayrıldı. Bu hastaların Toksoplazma ve Rubella virus IgM ve IgG tipi antikorlarınınlaboratuvar sonuçları retrospektif olarak incelendi.SONUÇLAR: Toksoplazma ve Rubella IgM antikorları seropozitiflikyüzdeleri sırasıyla %2,0 ve %0,8 olarak, IgG antikorları seropozitiflikyüzdeleri ise sırasıyla %63,0 ve %91,1 olarak saptanmıştır.TARTIŞMA: Bu çalışmada hastanemize başvuran gebe kadınlardaToksoplazma ve Rubella virus seropozitiflikleri belirlendi. Çalışmamızdasaptanmış olan seropozitiflik sonuçlarının Türkiye’nin farklı bölgelerinde yapılmışdiğer çalışmalarla benzer olduğu görülmüştür. In this study, we aimed to evaluate at the seropositivityof IgM and IgG type antibodies against Toxoplasmosis and Rubella virusaccording to the age groups of pregnant women who applied for routineexamination in our hospital. PATIENTSAND METHOD: 10178 pregnant women who applied forexamination in our hospital between December 2011 - December 2016 during fiveyears period were seperated in to four age groups of 15-24 years, 25-34 years,35-44 years and ≥45 years. These patient’s laboratory results of IgG and IgMantibodies against Toxoplasmosis and Rubella virus were analysedretrospectively.RESULTS: Seropositivitiesof IgM for Toxoplasmosis and Rubella were found as 2,0% and 0,8% respectively,seropositivities of IgG for Toxoplasmosis and Rubella were found as 63,0% and91,1% respectively. DISCUSSION: In this study, Toxoplasmosis and Rubella virusseropositivities were determined in pregnant women who admitted to ourhospital. It was seen that the seropositivity results determined in our studywere the same other studies which were taken in Turkeys' different regions. Asa result, we believe that this study will contribute to the literature.

Altun, Eren (Balikesir Author), Amaç: Çocukluk çağındaki en sık acil cerrahi operasyon nedeni olan apandisitin tanısını koymak çoğu zaman zordur. Çalışmamızda preoperatif Tam Kan Sayımı ile elde edilen değerler ve bunlardan türetilen Nötrofil Lenfosit Oranı ve Trombosit Lenfosit Oranı ile patolojik olarak tanı almış apandisit altgrupları, komplike olmayan akut apandisit (AA), süpüratif apandisit (SA) ve gangrenöz apandisit (GA), arasındaki ilişkinin ortaya konulması amaçlanmıştır. Gereç ve Yöntem: Çalışmaya hastanemize 2013-2015 yıllarında başvuran 18 yaşından küçük patolojik olarak apandisit tanısı doğrulanmış 291 hasta ve 101'i sağlıklı kontrol toplam 392 olgu dahil edildi. Tam Kan Sayımı verileri ve bundan türetilen oranlar kaydedildi. Bulgular: Yaş ve cinsiyet yönünden benzer olan hasta ve kontrol grupları arasında Ortalama Trombosit Hacmi için istatistiksel olarak sınırda bir anlamlı artış (p=0,042) bulunurken; ileri düzeyde anlamlı azalış bulunan Lenfosit sayısı ve artış bulunan Lökosit, Nötrofil sayıları, Nötrofil yüzdesi ile Nötrofil Lenfosit Oranı ve Trombosit Lenfosit Oranı (tümü için p9,89); 0,813 (>6,57); 0,760 (68); 0,857 (>2,35); 0,706 (>111,62) olarak bulundu. Sonuç: Çocukluk çağının en sık acil cerrahi operasyon nedeni olan apandisitin erken tanısı hayati önem arz etmektedir. Muayene bulguları, radyodiagnostik tetkikler ve prediktif değeri düşük olan çoğu laboratuvar testine ek olarak Nötrofil Lenfosit Oranı ve Trombosit Lenfosit Oranlarının da bu amaçla kullanılabilecek değerli veriler olduğu görülmüştür., Objective: It is generally a gruelling job to diagnose the appendicitis, the most common cause of a surgical operation in childhood. In this study, we aimed to reveal the relationship between the values obtained by the preoperative Complete Blood Count, and the Neutrophile Lymphocyte - Platelets Lymphocyte Ratio which are derived from the Complete Blood Count, and pathologically diagnosed appendicitis subgroups which are uncomplicated acute appendicitis (AA), suppurative appendicitis (SA) and gangrenous appendicitis (GA). Materials and Methods: The Complete Blood Counts and the ratio derived from the Complete Blood Count of a total of 392 cases, under 18 years old, are studied in this work. All these cases had been the patients of our hospital between 2013 and 2015. 291 of them were diagnosed for appendicitis while 101 were healthy. Results: For patients and control groups with similar ages and same gender, an on-the-edge statistically significant increase for the Mean Platelet Volume is observed (p=0,042). For the Leucocyte, Neutrophile, Lymphocyte count, Neutrophile percentage, Neutrophile Lymphocyte Ratio and Platelets Lymphocyte Ratio ( p9,89); 0,813 (>6,57); 0,760 (68); 0,857 (>2,35) and 0,706 (>111,62), respectively. Conclusion: Early diagnosis of the cause of most common childhood medical operation, the appendicitis, is vital for patients. Along with various well-known tests and examinations, such as physical examinations, radiodiagnosis and low-predictive laboratory tests, it has been realized that the Neutrophile Lymphocyte - Platelets Lymphocyte Ratio can be used for early diagnosis of appendicitis as well.
