Abstract

X-linked lissencephaly with abnormal genitalia is a rare and devastating syndrome. The authors present an infant with a multisystem phenotype where the intestinal manifestations were as life limiting as the central nervous system features. Severe chronic diarrhea resulted in failure to thrive, dehydration, electrolyte derangements, long-term hospitalization, and prompted transition to palliative care. Other multisystem manifestations included megacolon, colitis, pancreatic insufficiency hypothalamic dysfunction, hypothyroidism, and hypophosphatasia. A novel aristaless-related homeobox gene mutation, c.1136G>T/p.R379L, was identified. This case contributes to the clinical, histological, and molecular understanding of the multisystem nature of this disorder, especially the role of ARX in the development of the enteroendocrine system.

28. In situmantle cell lymphoma: case report and literature review

Abstract

We present a rare case of in situmantle cell lymphoma in an 87-year-old female who presented with enlarged left inguinal and left axillary lymph node and no B symptoms. The morphology on routine H&E stains revealed reactive appearing follicles, with well defined mantle zone. Immunohistochemical analysis showed a subtle increase in CD5 and cyclin D1 staining in the mantle zone. Flow cytometry results were suggestive of mantle cell lymphoma (MCL). The results of morphology, immunohistochemistry and flow cytometry were correlated and the lesion was best regarded as in situmantle cell lymphoma.

We report two cases of Helicobacter heilmanniigastritis, in a 59-year-old woman and in a 61-year-old man. Helicobacter heilmanniiis a spiral organism first observed in the gastric mucosa of dogs in 1881. 30. A review of diagnostic utility of P16INK4Aimmunostaining as a marker of high-risk HPV and high-grade cervical intraepithelial neoplasia

Abstract

The most frequent type of cervical cancer is squamous-cell carcinoma which develops from cervical intraepithelial neoplasia (CIN) / squamous intraepithelial lesion (SIL). Its tumorigenesis is related to human papillomavirus (HPV). High-risk HPV is integrated into the genome and leads to tumour progression. Cytological screening leads to detection of precursors and their mimics. P16INK4aand Ki-67 immunohistochemistry assists in the histological differential diagnosis of precursors to reactive and metaplastic epithelium. P16INK4ahas emerged as a valuable surrogate marker for high-risk HPV infection and shows increased immunoexpression with worsening grades of CIN. Numerous studies have supported its role in the detection of high-grade dysplasia and have lead to the use of p16INK4aimmunohistochem-istry in many laboratories. However, only a few studies have examined the possible predictive or prognostic value of p16INK4ain CIN or cervical cancer. This review addresses some of the practical issues in the application of p16INK4ain everyday practice, including the problems in the interpretation of the patterns of immunostaining and standardisation of the p16INK4ascoring.

Abstract

Collagen types I and III can be characterized at the molecular level (at the tens to hundreds of nanometers scale) using small angle x-ray scattering (SAXS). Although collagen fibril structural parameters at this length scale have shown differences between diseased and nondiseased breast tissues, a comprehensive analysis involving a multitude of features with a large (>50) patient cohort has not previously been investigated. Breast tissue samples were excised from 80 patients presenting with either a breast lump or reduction mammoplasty. From these, invasive carcinoma, benign tissue, and normal parenchyma were analyzed. Parameters related to collagen structure, including longitudinal (axial) and lateral (equatorial) features, polar angle features, total scattering intensity, and tissue heterogeneity effects, were extracted from the SAXS patterns and examined. The amplitude of the third-order axial peak and the total scattering intensity (amorphous scatter) showed the most separation between tissue groups and a classification model using these two parameters demonstrated an accuracy of over 95% between invasive carcinoma and mammoplasty patients. Normal tissue taken from disease-free patients (mammoplasty) and normal tissue taken from patients with presence of disease showed significant differences, suggesting that SAXS may provide different diagnostic information from that of conventional histopathology.

Abstract

Mycobacterium ulcerans (MU) is the third common mycobacterial infection after tuberculosis and leprosy. In endemic areas, MU ulcers should be considered in the differential diagnosis of any unusual or nonhealing lesion or ulcer. Diagnosis and treatment should be instigated promptly. Delay may lead to disfiguring or disabling scars. Surgical management, therefore, should aim towards early excision, with clear margins of the ulcer. We present 4 consecutive patients treated by our department within a 6-month period for MU ulcers. The presentation, diagnosis and surgical management are described. Based on our experience and after reviewing the literature, we have developed a surgical algorithm for the management of MU ulcers.
