Abstract

Emerging research indicates an immense burden on children and families related to the COVID-19 pandemic. This study uses data from semi-structured interviews and focus groups with early childhood service providers (n=19) to demonstrate the pandemic's impact on families with very young children and early childhood services in two high-need communities in Massachusetts, USA. We found that although the pandemic has worsened existing inequities and severely limited resources for young children and families, community mobilization in response to the crisis and innovative strategies stemming from resilience were developed quickly. Findings highlight the usefulness of early childhood systems of care in crisis responses and leveraging public-private cooperation to serve the needs of diverse families with young children. Lessons learned are applicable to global settings with high pre-pandemic inequities and can be used to develop stronger models of crisis response within the early childhood sector in preparation for future crises.

PURPOSE: To evaluate the surgical outcomes in combined hamartoma of retina and retinal pigment epithelium (CHRRPE) using optical coherence tomography (OCT). METHODS: This was a retrospective interventional study, in which medical records of 12 eyes with CHRRPE that underwent vitrectomy and membrane peeling at three tertiary vitreoretinal institutes were reviewed. Abstract

Purpose: To analyze the impact of a revised care plan for retinopathy of prematurity (ROP) during SARS-CoV-2 pandemic in a tertiary eye care facility in eastern India. Methods: In a retrospective study, we analyzed the medical records of babies managed for ROP during the peak of the SARS-CoV-2 pandemic, with particular reference to the challenges, and the revised strategies addressing travel restrictions for five months, from April to August 2020. The strategy included selective referral (babies with higher treatment probability), longer follow-up intervals (babies with non-alarming findings), use of locally available workforce, and teleconsultation whenever feasible. Results: In the given period, 222 babies were examined versus 624 in the preceding year (P = 0.001). The average gestational age, birth weight, and postmenstrual age at presentation were 30.4 weeks, 1.31 kg, and 37.7 weeks, respectively. The first examination was on time in 40.1% of babies but was delayed by a median of 23 days in the remaining babies. In the cohort, 56.7% of babies had any ROP, and 27.9% required treatment (versus 8.8% in the previous year; P < 0.001). The intravitreal anti–vascular growth factor (anti-VEGF) injection was more often used than in the previous year (n = 72 vs 36; P < 0.0001). The treatment outcome was comparable before and after the SARS-CoV-2 lockdown period. There was no report of health issues among the care providers attributable to ROP care. Conclusion: The revised strategy resulted in a smaller pool of babies screened but a larger proportion of babies treated for ROP. This strategy could be used more profitably in future ROP care. Abstract

Tens of thousands of children were orphaned or separated from their parents by the 1994 Rwandan Genocide. Following an all too familiar practice in post-conflict societies, these children were placed in unaccompanied children's centres (UCCs) referred to as orphanages. Staff in a proportion of these centres received training in simple trauma alleviation methods as part of a program instituted by the United Nations Children's Fund (UNICEF) with Rwandan governmental ministries. This study examines whether children in UCCs with staff trained in these methods had lower levels of post-traumatic stress symptoms (PTSS) than children in UCCs staffed by individuals who, to the best of our knowledge, lacked any formal mental health training. Data for these analyses derived from a National Trauma Survey conducted by UNICEF (1995) that included sampling of children from UCCs. Ordinary least squares multiple regression analysis was used to evaluate the effect of staff training on levels of PTSS among children, controlling for relevant covariates. Overall PTSS scores did not differ between children in UCCs with and without staff training. However, avoidance/numbing and hypervigilance symptoms were significantly elevated among females in UCCs with trained staff as compared with UCCs with staff lacking this training. Whereas these findings might result from unmeasured confounding variables, they nonetheless underscore the importance of formal assessment of treatment safety and effectiveness before implementing interventions. Methods: A noncomparative, consecutive analysis of case series from two tertiary care campuses of LV Prasad Eye Institute, India, between January 2011 and April 2014 was performed. Medical records of the patients presenting with DUSN (early or late stage) were reviewed. Results: The current study included 13 patients. The majority (10/13, 76.92%) of the patients were aged 20 years or less. All patients had unilateral eye involvement. Visual acuity at presentation was 20/200 or worse in 9/13 (69.23%) patients. A delay in diagnosis occurred in 6/13 patients, and initial diagnosis in these patients included retinitis pigmentosa (4 patients) and posterior uveitis (2 patients). Clinical features included early presentation (prominent vitritis, localized retinitis, and vasculitis) in 7/13 (53.85%) patients and late presentation (attenuation of vessels, retinal pigment epithelium atrophic changes, and optic atrophy) in 6/13 (46.15%) patients. Worm could not be identified in any of the cases. All the patients received laser photocoagulation of retina and oral albendazole treatment for a period of 30 days. With treatment, visual acuity improved in seven patients (six early stage, one late stage) and remained unchanged in six patients. Mean follow-up period was 8.69 months (range, 1–21 months).
