Abstract

The South Asia region is facing a high burden of chronic kidney disease (CKD) with limited health resources and low expenditure on health care. In addition to the burden of CKD and kidney failure from traditional risk factors, CKD of unknown etiologies from India and Sri Lanka compounds the challenges of optimal management of CKD in the region. From the third edition of the International Society of Nephrology Global Kidney Health Atlas (ISN-GKHA), we present the status of CKD burden, infrastructure, funding, resources, and health care personnel using the World Health Organization’s building blocks for health systems in the ISN South Asia region. The poor status of the public health care system and low health care expenditure resulted in high out-of-pocket expenditures for people with kidney disease, which further compounded the situation. There is insufficient country capacity across the region to provide kidney replacement therapies to cover the burden. The infrastructure was also not uniformly distributed among the countries in the region. There were no chronic hemodialysis centers in Afghanistan, and peritoneal dialysis services were only available in Bangladesh, India, Nepal, Pakistan, and Sri Lanka. Kidney transplantation was not available in Afghanistan, Bhutan, and Maldives. Conservative kidney management was reported as available in 63% (n = 5) of the countries, yet no country reported availability of the core CKM care components. There was a high hospitalization rate and early mortality because of inadequate kidney care. The lack of national registries and actual disease burden estimates reported in the region prevent policymakers’ attention to CKD as an important cause of morbidity and mortality. Data from the 2023 ISN-GKHA, although with some limitations, may be used for advocacy and improving CKD care in the region.

Abstract

This review article provides an overview of the current status of velocity-selective arterial spin labeling (VSASL) perfusion MRI and is part of a wider effort arising from the International Society for Magnetic Resonance in Medicine (ISMRM) Perfusion Study Group. Since publication of the 2015 consensus paper on arterial spin labeling (ASL) for cerebral perfusion imaging, important advancements have been made in the field. The ASL community has, therefore, decided to provide an extended perspective on various aspects of technical development and application. Because VSASL has the potential to become a principal ASL method because of its unique advantages over traditional approaches, an in-depth discussion was warranted. VSASL labels blood based on its velocity and creates a magnetic bolus immediately proximal to the microvasculature within the imaging volume. VSASL is, therefore, insensitive to transit delay effects, in contrast to spatially selective pulsed and (pseudo-) continuous ASL approaches. Recent technical developments have improved the robustness and the labeling efficiency of VSASL, making it a potentially more favorable ASL approach in a wide range of applications where transit delay effects are of concern. In this review article, we (1) describe the concepts and theoretical basis of VSASL; (2) describe different variants of VSASL and their implementation; (3) provide recommended parameters and practices for clinical adoption; (4) describe challenges in developing and implementing VSASL; and (5) describe its current applications. As VSASL continues to undergo rapid development, the focus of this review is to summarize the fundamental concepts of VSASL, describe existing VSASL techniques and applications, and provide recommendations to help the clinical community adopt VSASL.

Abstract

Purpose To determine the accuracy and test‐retest repeatability of fast radiofrequency (RF) transmit measurement approaches used in Dynamic Contrast Enhanced Magnetic Resonance Imaging (DCE‐MRI). Spatial variation in the transmitted RF field introduces bias and increased variance in quantitative DCE‐MRI metrics including tracer kinetic parameter maps. If unaccounted for, these errors can dominate all other sources of bias and variance. The amount and pattern of variation depend on scanner‐specific hardware and software. Methods Human tissue mimicking torso and brain phantoms were constructed. RF transmit maps were measured and compared across eight different commercial scanners, from three major vendors, and three clinical sites. Vendor‐recommended rapid methods for RF mapping were compared to a slower reference method. Imaging was repeated at all sites after 2 months. Ranges and magnitude of RF inhomogeneity were compared scanner‐wise at two time points. Limits of Agreement of vendor‐recommended methods and double‐angle reference method were assessed. Results At 3 T, B1+ inhomogeneity spans across 35% in the head and 120% in the torso. Fast vendor provided methods are within 30% agreement with the reference double angle method for both the head and the torso phantom. Conclusions If unaccounted for, B1+ inhomogeneity can severely impact tracer‐kinetic parameter estimation. Depending on the scanner, fast vendor provided B1+ mapping sequences allow unbiased and reproducible measurements of B1+ inhomogeneity to correct for this source of bias.

Abstract

Peritoneal Dialysis (PD) has been an underutilized modality in the treatment of Acute Kidney Injury (AKI). Concerns regarding clearance, fluid removal, infection, complications of therapy, and the hypercatabolic state of AKI has led to PD falling into disrepute. Recent studies have challenged this notion of ineffectiveness. The lower cost, and simplicity of the procedure makes it a particularly attractive option for the developing world which may lack even basic HD facilities, and patients continue to die for want of Renal Replacement Therapy (RRT). We present a review of the available literature about PD in the AKI setting with special reference to the developing world, including the procedure, costs, and effectiveness of the treatment. We also describe the procedure in detail to help ‘hand hold’ physicians interested in performing this lifesaving procedure.
