Introduction Les complications les plus fréquentes et les mieux connues de l’embolie pulmonaire sont la récidive et l’hypertension artérielle pulmonaire. L’embolie pulmonaire a également un impact psychologique, et plusieurs études ont montré que les patients ayant été atteints d’une embolie pulmonaire avaient des scores de dépression, d’anxiété et de qualité de vie moins bons que les groupes témoins. Aucune étude à notre connaissance ne s’est attachée à analyser le vécu des patients de moins de 50 ans, tranche d’âge particulièrement active et porteuse de projets. L’objectif de cette étude qualitative transversale est d’analyser le vécu des patients ayant été atteints d’une embolie pulmonaire alors qu’ils avaient moins de 50 ans. Patients et méthodes Les patients étaient inclus s’ils avaient moins de 50 ans au moment du diagnostic d’embolie pulmonaire, et en l’absence de néoplasie sous jacente. Un entretien semidirigé basé sur un guide de 11 questions a été réalisé pour chaque patient au CHU de Rennes. Ces entretiens ont été enregistrés, puis retranscrits sur ordinateur. Une analyse phénoménologique interprétative a été réalisée pour l’ensemble des entretiens afin de dégager les thèmes principaux. Résultats Dix-neuf patients ont été inclus, 1 mois à 19 ans après le diagnostic d’ embolie pulmonaire. L’analyse phénoménologique interprétative permet de dégager 3 thèmes principaux. Premièrement, l’embolie pulmonaire est une maladie peu connue, les symptômes initiaux sont souvent banalisés par les patients et la gravité sous estimée. La diversité des symptômes peut entraîner des erreurs diagnostiques et un retard au diagnostic, source d’anxiété pour le patient. La recherche étiologique est parfois négative, aggravant l’anxiété ressentie par les patients. Les anticoagulants, malgré leurs contraintes et leurs risques apparaissent essentiels. Deuxièmement, l’embolie pulmonaire est un évènement traumatisant, de survenue brutale chez des patients en bonne santé, qui prennent souvent conscience de la gravité de la situation à la vue de leurs proches ou suite à l’annonce diagnostique par les médecins. Cette peur rétrospective est associée à l’évocation de la mort chez de nombreux patients. Une rupture identitaire peut survenir au décours avec une accumulation de pertes : perte de l’image d’invincibilité, des capacités physiques, de l’image des autres. Les proches sont également touchés par la survenue de l’embolie pulmonaire. Enfin, les réactions au décours de l’embolie pulmonaire sont variables, un sentiment d’injustice est fréquent chez ces patients jeunes qui sont parfois initialement dans le déni. Le temps favorise l’acceptation de la maladie, bien que certains patients développent une symptomatologie dépressive. Néanmoins, cet évènement aigu permet une prise de conscience et de nombreux patients déclarent profiter plus de la vie qu’auparavant. Les stratégies d’adaptation déployées varient selon les patients, mais passent souvent par la recherche de soutien. Leur vie peut être bouleversée par cet épisode, entraînant l’abandon de certaines activités ou le renoncement à certains projets. Certains patients sont obligés de s’adapter aux limitations physiques imposées par la maladie, alors que d’autres ne changent ni leurs habitudes ni leurs projets de vie, et considèrent l’embolie comme une maladie aiguë. Quelle que soit l’appréciation qu’ont les patients de cette maladie, ils vivent tous avec la peur de la récidive. Conclusion L’embolie pulmonaire survient brutalement chez des patients jeunes, il s’agit d’un évènement traumatisant pouvant entraîner une rupture identitaire et des modifications des projets de vie. Il est essentiel de s’intéresser au vécu des patients ayant été atteints d’une embolie pulmonaire, afin de leur proposer une prise en charge optimale et pluridisciplinaire. [ABSTRACT FROM AUTHOR]

Background: Methodological limitations affect a significant number of oncology and haematology trials, raising concerns about the applicability of their results. For example, a suboptimal control arm or limited access to best care upon progression may skew the trial results toward a benefit in the experimental arm. Beyond the fact that such limitations do not prevent drugs reaching the market, other assessment tools, such as those developed by professional societies-ESMO-MCBS and ASCO Value Framework-do not integrate these important shortcomings., Methods: We propose creating a novel framework with the scope of assessing registration cancer clinical trials in haematology and oncology (randomized or single arm)-that is trials leading to a marketing authorization. The main steps of the methods are (1) assembling a scientific board; (2) defining the scope, goal and methods through pre-specified, pre-registered and protocolized methodology; (3) preregistration of the protocol; (4) conducting a scoping review of limitations and biases affecting oncology trials and assessing existing scores or methods; (5) developing a list of features to be included and assessed within the framework; (6) assessing each feature through a questionnaire sent to highly cited haematologists and oncologists involved in clinical trials; and (7) finalizing the first version of framework., Results: Not applicable., Conclusions: Our proposal emerged in response to the lack of consideration for key limitations in current trial assessments. Objectives: To evaluate whether there is evidence of efficacy of the most commonly used medications in their primary indications., Study Design and Setting: This scoping review was executed using the Cochrane Library and MEDLINE databases up to May 2023. The 10 most prescribed medications in England, France, and the United States were identified using country-specific public databases. Up to 3 common indications in primary care were defined for each medication, based on a survey of general practitioners. The outcomes were determined by the authors to be patient-important outcomes, with placebo as the comparator. Two investigators independently conducted searches, following a predefined algorithm, to identify randomized controlled trials or meta-analyses of randomized controlled trials assessing the efficacy of these medications for each indication. The risk of bias was assessed using the ROBIS or ROB 2.0 tools., Results: We identified 21 drugs, covering 56 indications and 114 outcomes. Sixty-seven percent of the evaluated medications demonstrated efficacy for at least one outcome in at least one of the sought indications. Overall, evidence of efficacy was found for 48% of the indications. There was no study evaluating the efficacy of amoxicillin and salbutamol. For other drugs such as phloroglucinol or cholecalciferol, available studies suggested an absence of efficacy in the most common indications., Conclusion: This study underscores the lack of data regarding the level of evidence for the most prescribed medications.
